Abstract

Assessment and care of diabetic feet should always be provided to patients periodically to optimally prevent complications. This necessitates a practical guide that nurses can use to evaluate the risk of wounds in diabetic patients. Previous research has developed protocols for assessing the risk of diabetic foot ulcers; however, these protocols are manual, requiring nurses to spend time determining the risk level of wounds. The aim of this study is to develop a Web-Based Diabetic Foot Infection Risk Scale Application using the Forward Chaining Method. This research utilizes the Stetler model for evidence-based application development. The development process involves three stages: preparation, validation, and analysis. In the first stage, researchers conduct a literature review using literature review protocols and source scientific articles and books, followed by the development of a web-based application using the forward chaining method. There were 19 research articles recruited from three data base scientific (PubMed, Scopus, and Wiley online Library) from 2014-2024. In the second stage, experts will evaluate the content of the application concerning infection risk assessment. Finally, the application is tested. The results of this study have shown that, after testing, the Web-Based Expert System for Diabetic Foot Infection Risk Determination using the forward chaining method can be used and implemented directly by nurses working in diabetic foot care specialty clinics. This research represents an initial stage focusing on assessing infection risk levels, and it is hoped that future research will build upon this study, not only assessing infection risk but also evaluating the potential for diabetic foot ulcers based on assessed risk factors.

Abstract

Placental malaria is a serious health problem that must be treated immediately. WHO data records that 12 million pregnant women were infected with malaria throughout the world in 2019. Of the pregnant women infected with malaria, it was reported that 40% experienced placental malaria during childbirth. At the national level, it was found that the prevalence of malaria in pregnant women was 525 from the 2018 Basic Health Research results. At the provincial level, the largest contributor was Papua with a total of 2816 cases in 2020. Jayapura Regency was in 4th position with 402 cases of pregnancy malaria. Disturbances in newborn baby oxygenation caused by placental malaria are the main-focus to be resolved immediately so that efforts are made to investigate the level of relationship between the level of placental malaria parasitemia and clinical signs of impaired newborn baby oxygenation (APGAR Score) needs to be developed in order to create a formula for handling and preventing these impacts. Objective: Analyzing the relationship between the level of malaria parasite density in the blood of mothers giving birth and APGAR Score of newborn babies. Method: This research is descriptive correlational research. The total population was 36 mothers with a history of malaria during their pregnancy and research samples were taken with technique Total Sampling. Data were analyzed using a correlation test Rank Spearman with a significance level <0,05. Results: Based on correlation test Rank Spearman sig value is obtained. (2-tailed) as large as 0.00 <0.05 so that there is a significant relationship between the level of malaria parasite density in the blood of the mother giving birth with APGAR Score of newborn babies. The correlation coefficient value between the two variables is -0.658 which is included in the strong correlation category with a negative or inverse correlation direction. The higher the density of malaria parasites in the mother's blood, the lower the APGAR of newborn baby value. Conclusion: Most of the malaria parasite density levels in mothers who gave birth were in the negative category with an average APGAR score for birth defects of 8.47. There is a relationship between the level of density of malaria parasites in the blood of pregnant women and APGAR Score of newborn babies.

Abstract

The 2023 Indonesian Health Survey Report issued by the Ministry of Health revealed that there was an increase in the prevalence of diabetes mellitus (DM) in the population aged over 15 years based on the results of blood sugar level measurements. In the 2018 Basic Health Research, it was recorded that the prevalence of diabetes in Indonesia reached 10.9%. Now, the prevalence has reached 11.7% in 2023. The Aceh Health Office recorded that sufferers of Diabetes Mellitus until early 2023 reached 154,889 cases. The purpose of this study is to find out the difference in blood sugar levels in people with diabetes mellitus who consume rice after cooking with the rice cooker method and with the steaming method. This quantitative analytical research uses a cross-sectional study design. The population of the study was all people with diabetes mellitus at the Calang Health Center, Aceh Jaya Regency, totaling 50 people. Data collection was carried out from January 15 to March 15, 2024 using nonprobability sampling techniques. This study also took direct data by looking at the sugar content of the respondents by eating rice cooked by the rice cooker method and 1 month later by measuring the sugar content of the respondents consuming rice by cooking the rice by steaming. Data were analyzed using univariate, bivariate and multivariate statistical tests. The results of the study showed that factors related to blood sugar levels in DM patients were gender, age, education, occupation, income, physical activity, nutritional status, smoking behavior, and family history of diabetes. The factors most related to increased blood sugar levels in diabetes mellitus patients were steaming with OR=7.47, use of a rice cooker of 11.2 and family history of diabetes mellitus of 7.50. It is expected that respondents will control their blood sugar levels, so that DM sufferers and their families will cook rice using the steaming method, and control other risk factors that can cause high blood sugar levels.

Abstract

Chemotherapy causes the death of some cells that have been formed from the process of DNA, RNA or protein synthesis. The myelosuppressive effects of long-term chemotherapy treatment will continue throughout the therapy cycle such as infection, septicemia and spontaneous bleeding and worsening of anemia. This study aims to determine the factors that influence the occurrence of anemia in cisplatin chemotherapy patients. This study used an analytical observational design with a retrospective cross-sectional approach (medical records). The population was 394 patients with a sample of 136 with a simple random sampling technique. Data were obtained from medical records and supporting examination results, then summarized and tabulated according to the coding. Data analysis used the chi square test. The results of this study obtained 86 patients who experienced anemia and as many as 136 cases of anemia with 35 patients experiencing recurrent anemia. The p value in the chi-square test for age with the occurrence of anemia was (p = 0.026), the p value in the chemotherapy series (p = 0.011), the p value in leukopenia with anemia (0.000), the p value in thrombocytosis (p = 0.004). The use of chemotherapy with cisplating has a significant relationship with anemia in patients with Ca Cervix IIIA - IVA. The use of chemotherapy with cisplatin has a significant relationship between age, leukopenia levels, thrombocytosis levels, and the series of chemotherapy used in the occurrence of anemia. So that chemotherapy can cause damage to the spinal cord, so that it fails to produce red blood cells, white blood cells, and platelets in the blood.
