Abstract

Delirium is an acute neurocognitive syndrome commonly occurring in older adults and is characterized by disturbances in consciousness, attention, cognition, and behavior that develop rapidly and fluctuate throughout the day. This condition is frequently observed among hospitalized elderly patients, particularly those with chronic illnesses, neurological disorders, infections, or those recovering from surgery. Delirium is associated with increased morbidity, mortality, prolonged hospital stays, functional decline, and a higher risk of developing dementia in later life. This review aims to examine the risk factors, pathophysiology, diagnosis, and management of delirium in older adults based on current scientific literature. The method used was a literature review of relevant scientific journals. The findings indicate that the major risk factors for delirium include advanced age, pre-existing cognitive impairment, infection, dehydration, metabolic disturbances, the use of certain medications, and immobility. Diagnosis is established through clinical assessment focusing on acute changes in mental status and impaired attention. Management primarily involves identifying and treating the underlying causes, along with non-pharmacological interventions such as reorientation strategies, early mobilization, sleep optimization, and family support. Pharmacological treatment is reserved for selected cases with severe symptoms that threaten the safety of the patient or others. Early detection and comprehensive management are essential to reduce complications and improve the quality of life of older adults experiencing delirium.

Abstract

Osteosarcoma is the most common primary malignant bone tumor in children and adolescents, particularly during periods of rapid growth. This tumor originates from mesenchymal cells that produce osteoid or immature bone matrix and commonly affects the metaphyseal regions of long bones, such as the distal femur, proximal tibia, and proximal humerus. The exact etiology of osteosarcoma remains unclear; however, genetic factors, radiation exposure, and certain bone disorders are known to increase the risk of its development. Clinical manifestations typically include progressive bone pain, localized swelling, limited joint movement, and, in some cases, pathological fractures. Diagnosis is established through clinical evaluation, physical examination, radiological imaging, and histopathological confirmation. The management of osteosarcoma requires a multidisciplinary approach involving neoadjuvant chemotherapy, surgical resection of the tumor, and adjuvant chemotherapy. Advances in surgical techniques and chemotherapy regimens have significantly improved patient survival rates. Prognosis is influenced by tumor size, location, response to chemotherapy, and the presence of metastases, particularly to the lungs. Early detection and appropriate treatment are essential to improve patient outcomes and quality of life.

Abstract

Sleep disorders are common health problems that can have significant effects on brain health. Conditions such as insomnia, obstructive sleep apnea, and circadian rhythm disorders are known to be associated with various neurological disorders. This article aims to further examine the relationship between sleep disorders and brain health, including the underlying biological mechanisms and their impact on cognitive function and neurological diseases. The method used in this study is a literature review by examining various relevant scientific sources. The results of the review indicate that sleep disturbances can affect brain function through several mechanisms, including disruption of memory consolidation processes, increased oxidative stress, inflammation, and impairment of metabolic waste clearance through the glymphatic system. In addition, sleep disorders are also associated with an increased risk of neurological diseases such as stroke, cognitive decline, and neurodegenerative disorders. In conclusion, poor sleep quality and the presence of sleep disorders can negatively affect brain health; therefore, maintaining healthy sleep patterns is important as a preventive effort against various neurological disorders.

Abstract

This study aims to analyze the effect of credit risk management based on the 5C principles (Character, Capacity, Capital, Collateral, Condition) and the 3R principles (Return, Repayment, Risk Bearing Ability) on Non-Performing Loans (NPL) at BPR Abdi Warga Mulia. The research method used is a quantitative approach with multiple linear regression analysis. Data were obtained through questionnaires distributed to 100 respondents of working capital loan customers as well as documentation of non-performing loan reports. The results show that partially, the variables Character (X1), Capacity (X2), Capital (X3), Return (X6), Repayment (X7), and Risk Bearing Ability (X8) have a significant effect on NPL with significance values < 0.05. Meanwhile, Collateral (X4) and Condition (X5) do not significantly affect NPL (p > 0.05). Simultaneously, the 5C and 3R principles significantly influence NPL with an F-statistic = 41.27 > F-table = 2.12 and significance = 0.000. The coefficient of determination (R²) is 0.715, indicating that 71.5% of the variation in NPL can be explained by the 5C and 3R variables, while the remaining 28.5% is explained by other factors outside the research model. It can be concluded that the application of credit risk management based on the 5C and 3R principles plays an important role in reducing the level of NPL at BPR Abdi Warga Mulia. Therefore, the bank needs to strengthen the assessment of character, business capacity, capital, and repayment discipline to maintain loan quality and minimize the potential occurrence of non-performing loans.

Abstract

Cesarean section (CS) is an obstetric surgical procedure that has seen a significant global increase. Post-CS patients commonly experience moderate-to-severe acute pain at the abdominal wall and uterine incision sites. A multimodal analgesia strategy effectively utilized to provide long-lasting pain relief is the administration of intrathecal morphine as an adjuvant to spinal anesthesia. Although proven effective in extending the pain-free interval, the use of intrathecal morphine carries risks of specific side effects that necessitate routine monitoring. To determine the side effect profile of intrathecal morphine adjuvant among patients undergoing Cesarean section (CS) under spinal anesthesia at Hermina Hospital Padang in 2026. This was a quantitative descriptive study utilizing a cross-sectional design. The sample comprised patients undergoing CS under spinal anesthesia with intrathecal morphine adjuvant at Hermina Hospital Padang in 2026 who met the inclusion and exclusion criteria. Data collection instruments included observation sheets and medical records to evaluate demographic characteristics and the incidence of postoperative side effects. The minimum required sample size was 30 samples. Regarding patient characteristics, the majority fell within the healthy reproductive age range of 20–35 years, accounting for 28 individuals (84.84%), most were primiparous (54.53%), and a majority of 19 patients (57.57%) had no prior history of CS. The most predominant side effect observed among respondents was nausea and vomiting, occurring in 8 cases (24.24%), followed by pruritus in 7 cases (21.21%). Additionally, a concurrent combination of nausea-vomiting and pruritus was observed in 4 cases (12.12%). Severe side effects, such as somnolence, urinary retention, and respiratory depression, were entirely absent among all respondents in this study. Conclusion: The administration of intrathecal morphine as an adjuvant to spinal anesthesia in CS patients at Hermina Hospital Padang in 2026 was shown to be relatively safe. The most frequently occurring side effects were mild-to-moderate, namely nausea-vomiting and pruritus, with no severe adverse effects reported.

Abstract

Subarachnoid hemorrhage (SAH) is a neurological emergency characterized by bleeding into the subarachnoid space, most commonly caused by rupture of an intracranial aneurysm. This condition is associated with high morbidity and mortality rates and requires prompt diagnosis and management. Patients typically present with sudden severe headache, often described as “the worst headache of life,” accompanied by nausea, vomiting, decreased level of consciousness, neck stiffness, and focal neurological deficits. Diagnosis is established through clinical evaluation supported by neuroimaging, particularly non-contrast computed tomography (CT) scan, and may be followed by lumbar puncture or cerebral angiography if necessary. Early treatment focuses on stabilizing the patient, preventing rebleeding, managing intracranial pressure, and addressing complications such as vasospasm and hydrocephalus. Definitive management may involve surgical clipping or endovascular coiling of the aneurysm. Comprehensive and timely intervention is essential to improve patient outcomes and reduce the risk of long-term neurological disability.

Abstract

Endophthalmitis is a severe intraocular inflammatory condition caused by microbial infection involving the vitreous and aqueous humor, which may lead to significant visual impairment or blindness if not treated promptly. This condition can occur as a result of postoperative complications, ocular trauma, endogenous hematogenous spread, or extension of adjacent ocular infections. The most common causative organisms include Gram-positive bacteria, Gram-negative bacteria, and fungi. Clinical manifestations typically include decreased visual acuity, ocular pain, redness, photophobia, hypopyon, and vitreous inflammation. Diagnosis is established through clinical examination supported by microbiological analysis of vitreous or aqueous samples. Early recognition and prompt management are essential to preserve visual function. Treatment commonly involves intravitreal antibiotic or antifungal injections, systemic antimicrobial therapy, and pars plana vitrectomy in severe cases. Prognosis depends on the causative organism, severity at presentation, and timeliness of intervention. Comprehensive management and early diagnosis significantly improve visual outcomes and reduce complications associated with endophthalmitis.

Abstract

Laryngitis is an inflammation of the larynx that can be acute or chronic and is commonly caused by viral or bacterial infections, irritation, excessive voice use, as well as exposure to cigarette smoke or air pollution. This condition is frequently characterized by hoarseness, sore throat, cough, throat discomfort, and difficulty speaking. In some cases, laryngitis may also be accompanied by fever and respiratory distress, especially in children. Diagnosis is established through history taking, physical examination, and laryngoscopic evaluation when necessary to assess the condition of the vocal cords and laryngeal structures. Management of laryngitis depends on the underlying cause and includes voice rest, adequate hydration, symptomatic therapy, antibiotic administration in bacterial infections, and avoidance of irritant factors. The prognosis of laryngitis is generally good when treated appropriately; however, chronic laryngitis may lead to persistent voice disorders and requires further evaluation.

Abstract

Pleural Effusion and Pneumothorax are pleural cavity disorders that can cause respiratory impairment and decreased lung function. Pleural effusion occurs due to the accumulation of fluid within the pleural cavity and may be caused by heart failure, infection, malignancy, or other systemic diseases. Meanwhile, pneumothorax is a condition characterized by the presence of air in the pleural space leading to partial or complete lung collapse, and it may occur spontaneously, traumatically, or iatrogenically. Clinical manifestations of both conditions include shortness of breath, chest pain, cough, and decreased breath sounds on the affected side. Diagnosis is established through history taking, physical examination, and supporting investigations such as chest radiography, ultrasonography, or thoracic CT scan. Management depends on the underlying cause and severity of the disease, ranging from observation and medical therapy to thoracentesis, chest tube insertion, and surgical intervention. Early diagnosis and appropriate management are essential to prevent complications and improve patient outcomes.

Abstract

Epiglottitis is an acute and potentially life-threatening inflammation of the epiglottis and surrounding supraglottic structures that can lead to rapid airway obstruction. The condition is commonly caused by bacterial infection, particularly Haemophilus influenzae type b, although viral and fungal infections may also contribute. Epiglottitis can occur in both children and adults, with clinical manifestations including high fever, severe sore throat, dysphagia, drooling, muffled voice, stridor, and respiratory distress. Diagnosis is established through clinical evaluation supported by laryngoscopic examination and radiologic findings, while careful airway assessment remains the primary priority. Prompt management is essential and includes airway stabilization, oxygen therapy, intravenous antibiotics, corticosteroids, and supportive care. Early recognition and rapid intervention are crucial to prevent complications such as complete airway obstruction and respiratory failure.
