Abstract

Introduction: Drug-resistant tuberculosis (DR TB) is a condition where Mycobacterium tuberculosis is resistant to isoniazid, rifampicin or resistant to isoniazid and rifampin as well as other first-line anti-tuberculosis drugs (OAT). Tuberculosis (TB) is a contagious infectious disease, caused by Mycobacterium tuberculosis germs where the source of transmission is through droplets containing TB germs. Case Report: A 10 years 7 months girl, came to the MDR Polyclinic at the Haji Adam Malik Medan Central General Hospital (RSUP HAM Medan) on March 21 2023 bringing the results of the TB Molecular Rapid Test (TCM-TB) examination from the Teladan Barat Health Center with Rifampicin results were resistant and on March 24 2023 the patient was hospitalized. Currently, the patient is complaining of a cough that has been experiencing for ± 3 weeks. Cough with the impression of a dry cough and is continuous so that it interferes with daily activities. The cough does not improve even though the patient has taken cough medicine. There is no fever, there is also no history of previous fever. According to the anamnesis from the patient's parents, there has been a weight loss in the last 2 months, around ± 4 kg. There is no family history of long-term cough, but according to parents, there is a neighbor of the patient who suffers from pulmonary TB and is undergoing treatment. Discussion: After the diagnosis of drug-resistant tuberculosis is confirmed, an evaluation is carried out whether the patient meets the criteria for drug-resistant TB treatment guided by short-term or long-term oral drug-resistant tuberculosis. Patients who meet the evaluation criteria for treatment can immediately be given short-term oral drug-resistant tuberculosis therapy. If the patient does not meet the criteria for short-term treatment, long-term drug-resistant tuberculosis treatment is given. Routine evaluations are carried out during treatment and when the results of drug sensitivity tests are available, treatment is adjusted according to the examination results. Conclusion: A case of drug-resistant tuberculosis in a girl aged 10 years 7 months has been reported which was diagnosed based on anamnesis, physical examination, and supporting examinations, there was also a history of close contact with TB sufferers, and the results of TCM examination of sputum fluid showed rifampicin resistant MTB growth. Patients are treated as drug-resistant TB cases with a short-term regimen with regular monitoring of treatment response and side effects.

Abstract

Introduction: Bone cysts are inclusive of but not limited to the unicameral bone cyst (UBC) and aneurysmal bone cyst (ABC). UBC have a predilection for males (3:1). UBC make up 3% of primary bone lesions. Most UBCs are found in long bones, most common sites are proximal humerus and femurs, rarely found in flat bones (Skull). Cases of UBC observed following trauma are thought to occur due to intraosseous bleeding when bone organization or repair mechanisms fail. Case Report: Twenty-one-year-old male complained of headache, a history of traumatic brain injury. Motoric examinations show diminished motor function on the left side. CT Scan shows an expansile sclerotic lytic lesion on the right parietal bone. We diagnosed this case with a suggested Bone Cyst on the right parietal, and craniotomy tumor removal with a direct approach was performed. Thickened dura mater was identified, cleansed using NaCL and povidone-iodine, and drilled using a high-speed drill until flattened. The yellowish-colored bony lesion was discovered to contain a semi-translucent substance, the lesion was found to be a unicameral bone cyst. Discussion: UBCs are generally asymptomatic, and usually appear as lesions with lucent characteristics and small zones of transition. As a benign bone tumor, treatments are not required unless the lesion is large and symptomatic, due to the patient's symptomatic condition, craniotomy tumor removal was performed. Conclusion:UBC is a rare condition, that’s scarcely located in the skull. Post-operatively, the patient shows normal motor function and no complaint of pain. Craniectomy tumor removal and flattening of the dura mater using a high-speed drill is sufficient in treating this rare disease.

Abstract

Introduction: Low Back Pain (LBP) is defined as pain and discomfort located below the costal margin to the inferior gluteal fold, with or without pain in the legs. LBP is a common condition that can occur in all age groups with acute or chronic episodes. Beauticians are more susceptible to musculoskeletal disorders. This line of work tends to involve limited positions, repetitive movements, standing for more than 8 hours or sitting in a bent position during activities. This study aims to determine the characteristics of low back pain based on work duration among beauticians of the JA Medical Skincare beauty clinic Medan. Method: The method used in this research was descriptive quantitative with a total sampling technique. The research population was 60 beauticians from the JA Medical Skincare Beauty Clinic Medan. The data were collected by having the respondents fill out a questionnaire. Results: The research results revealed that beauticians at the JA Medical Skincare beauty clinic who were at risk of experiencing low back pain were individuals aged under 30 years (51.8%), those with BMI > 27 or categorized as obese (60%), and those with a working duration of more than 8 hours (49,1%). These groups exhibited higher percentages of complaints related to low back pain. In general, almost all ages have the potential to experience low back pain. Beauticians who work long hours and do not pay attention to ergonomic factors would have caused continuous static loads on their back and are at risk of experiencing complaints of low back pain. Abstract

Introduction: Hypertension is a chronic condition which blood pressure increases above normal limits. It can cause a variety of other health issues such as heart, eyes, and kidney problems. Hypertension can occur due to various risk factors such as sleep disturbance. Sleep disturbance often occurs in students that could be affected by demands from college such as attending school community, school tasks, or exams. Objective: Correlation between sleep patterns and blood pressure in students of the Faculty of Medicine, Universitas Prima Indonesia in 2023. Method: Cross-sectional, the samples in this study are students of the Faculty of Medicine, Universitas Prima Indonesia, Class 2020-2022 who had met the selection and exclusion criteria based on the quota sampling technique. The data was then analyzed with the Chi-Square test. Result: The calculation results show that out of 85 respondents, 49 people (57,6%) had poor sleep patterns and 36 people (42,9%) had good sleep patterns. Out of 85 respondents, 11 people (12,9%) had normal blood pressure, 35 people (41,2%) had pre-hypertension blood pressure, 33 people (38,8%) had stage 1 hypertension blood pressure, and 6 people (7,1%) had stage 2 hypertension blood pressure. The Chi-Square test results showed no meaningful relationship between sleep patterns and blood pressure (p value=0,929). Conclusion: There is no meaningful relationship between sleep patterns and blood pressure of Universitas Prima Indonesia students in 2023. Keywords: blood pressure, hypertension, sleep patterns

Abstract

Introduction: Intracerebral hemorrhage (ICH) accounts for 10–15% of all strokes in the USA, Europe, and Australia, and 20–30% of Asian cases, with a30-day mortality rate of 35% to 52%; half of the related deaths occur in the first 2 days. Its overall incidence is 24.6 per 100,000 person-years, indicating that it represents the most fatal type of stroke around the world. Methods: The aim of study of this study is to investigate the comparison of open craniotomy vs conservative treatment in minor spontaneous intracerebral hemorrhage (ICH). This study used the systematic review method by discovering articles using the search engines PubMed, SagePub, and Science Direct, and 5 articles met the inclusion criteria in this study. Results: Five publications were found to be directly related to our ongoing systematic examination after a rigorous three-level screening approach. Subsequently, a comprehensive analysis of the complete text was conducted, and additional scrutiny was given to these articles. Conclusion: Surgical management of intracerebral hemorrhage has unique advantages as it can remove the hematoma effectively and decrease intracranial pressure and the incidence of complications. Abstract

Introduction: The paranasal sinus mucocele is a slow-growing mass that contains mucous. It can grow to vary in size, well-defined border, and fluctuating.   Case description: A 51-year-old male with the chief complaint was a mass in his left eye 9 months before. The complaint began with a small swelling and it became bigger. He had no chronic flu-like syndrome previously. He admitted that he had no complaints with his vision or his hearing. The mass is as big as eight centimeters in diameter. The physical examination revealed that his left eye was protruded with left eye acuity was 6/6. He had paresis with an upward gaze in his left eye. At the palpation examination, it was soft, elastic, and fluctuative, well-defined border with bony in its surroundings, and no pain sensation. Discussion: In this case, the frontal sinus mucocele can make the eyeball protrude because of downward development. It does not impair visual function, because it does not disturb the optic nerve. The impairment was difficult to gaze upwardly. Conclusion: Given that the lesion came from the frontal sinus, it could grow outwardly and make the eyeball protrude. Histologically, mucocele had to spread inflammatory cells and macrophages, debris cells, and necrotic mass. Keywords: Frontal sinus; mucocele; sphenoorbital tumor; paranasal sinus.

Abstract

Introduction: Brain metastases (BM) are the most frequent type of brain tumors in adults and are the leading source of neurological issues related to systemic cancers. Case Description: Female, 49 years old, presented with decreased consciousness that occurred since 1 month before admission. Weakness of the right limbs is found. CT scan abdomen with contrast show hepatic segment VIII infarction. histopathological examination of hepatic tissue biopsy with the results of hepatocellular carcinoma MRI brain shows multiple lesions suggestive of brain metastasis. Discussion: Patient was diagnosed with space occupying lesion on the frontotemporal dd brain metastasis dd orbital roof meningioma + multiple space occupying lesion intracranial on the left parietooccipital ec. brain metastasis + hepatocellular segment V, VI, VII, and VIII. Patients underwent craniotomy tumor removal to remove intracranial masses. The patient's postoperative condition showed improvement in the level of consciousness with GCS E4M5V3. Conclusion. Craniotomy tumor removal provides several advantages for managing patients with brain metastases of hepatocellular carcinoma. Keyword: Craniotomy, Brain Metastases, Hepatocellular Carcinoma

Abstract

Introduction: Stroke is a clinical condition characterized by the sudden onset of focal (or global, in cases of coma) neurological symptoms that persist for over 24 hours or result in death, with no clear cause other than a vascular issue. Case Description: Male, 62 years old, with chief of complaint of mouth dropped to the right since 1 day before admission. Complaints made the patient speak more difficult, slow and unable to form sentences. Physical examination show parese CN. VII UMN dextra. Siriraj score -5 and Head NCCT show cerebral infarction in the internal capsula interna. Patient was diagnosed with ischemic stroke. Discussion: The use of a scoring system can be useful when there are no brain imaging facilities that can clearly distinguish the causative pathology of stroke. criteria for diagnosis of ischemic stroke are symptoms of global neurological deficits or one / several focal neurological deficits that occur suddenly with evidence of brain imaging images (CT scan or MRI). The most common differential diagnosis is hemorrhagic stroke. Conclusion. Early recognition and appropriate treatment is important to cure acute ischemic stroke to achieve good quality of life and prevent further infarct to brain tissue. Abstract

Introduction: Falcotentorial meningiomas are rare, comprising 1%–2% of all intracranial meningiomas, arising at the junction of the falx cerebri and tentorium. These tumors may compress the quadrigeminal plate and/or splenium, leading to peduncular hallucinosis—vivid, colorful visual hallucinations of people and animals, which can persist even after tumor removal. Case Description: A 31-year-old woman with no psychosis history presented with a bilateral visual field deficit and left extremity weakness. Brain MR imaging revealed a lobulated mass attached to the right cerebellar tentorium, posterior falx cerebri, and adjacent venous sinuses. Visual field analysis showed homonym superior quadranopsia. Following craniotomy and tumor removal, histology confirmed a mixed-type meningioma. Post-surgery, the patient experienced vivid visual hallucinations that resolved in two weeks, but bilateral visual field deficits remained. A post-op CT showed reduced structural compression around the lesion with slight narrowing of the quadrigeminal cistern. Discussion : Visual hallucinations in falcotentorial meningioma patients, often as peduncular hallucinosis, occur due to compression of the quadrigeminal plate and/or splenium and may persist post-surgery if lesions are present in the cerebral peduncles or nearby midbrain areas. Conclusion: The patient's temporary hallucinations may have resulted from post-resection tissue edema causing quadrigeminal cistern narrowing or indicate lesions in the cerebral peduncles or adjacent midbrain regions.

Abstract

Introduction: Transsphenoidal approaches to the sellar region, with endoscopic or microscopic techniques, are regularly used to treat a diverse collection of pathologies. Case Description: Three patients with presenting symptoms visual disturbance and headache were evaluated with MRI that showes sellar region masses. All patients were undergo transphenoid tumor removal. Post operation evaluation show improvement in visual disturbance and headache with no adverse event after procedure. Discussion : Transsphenoidal endoscopic surgery (TSS) for functional pituitary adenomas yields better endocrinologic results for noninvasive macroadenomas. However, the rate of postoperative CSF leakage was greater with the endoscopic method. In patients with pituitary adenomas, younger age, dense visual field defect, and the preoperative absence of central or bilateral visual field abnormalities were predictive factors of visual field recovery following transsphenoidal approach-tumor excision. Overall survival rates are generally high, ranging from 91% to 98%. Conclusion: Transphenoid approach was a good surgical method for sellar region masses and show good outcomes after operation.
