Abstract

Posterior capsular opacity (PCO), also known as after cataract, is a complication of cataract surgery that manifests as an opacity that persists or develops after extracapsular lens extraction. 1 The incidence of PCO varies widely in the literature, influenced by the type of intraocular lens (IOL) and patient risk factors. The standard treatment for PCO-related visual impairment is posterior capsulotomy using a neodymium-doped yttrium-aluminum-garnet (Nd: YAG) laser. Although PCO recurrence after initial treatment with YAG capsulotomy is more common in children, it is rare in adults. The underlying pathophysiologic mechanism is analogous to primary PCO, involving proliferation, migration, and/or clustering of lens epithelial cells (LECs), resulting in posterior aperture re-closure. A comprehensive review of recent literature has identified several potential risk factors for PCO recurrence, including younger age, female gender, high myopia, diabetes, vitrectomy status, uveitis, low diopter IOLs, and certain types of IOLs with higher water content.2A 69-year A 69-year-old male with a history of cataract surgery four years ago, a physical examination showing results within normal limits, and an ophthalmologic physical examination showing ODS virus 0.2, PH - NC, arcus senilis +/+, fibrosis type posterior lens opacity +/+, and IOL +/+ was identified. Funduscopic examination revealed a fundus reflex (+/+), diminished due to posterior lens opacities, with a macular reflex that was difficult to assess (+/+) and retinal degeneration (+/+). The diagnosis of ODS pseudophakia + PCO was established based on the patient's history, physical examination results, and supporting examinations of the mother and baby that had been carried out. According to the diagnosis, the patient was administered the primary pharmacological therapy of P Pred ED 6x1 drops in both eyes and the non-pharmacological therapy of Nd: YAG laser treatment in both eyes.

Abstract

Introduction : Adolescence is a phase where individuals experience emotional, psychosocial development and move towards a certain maturity. According to Media Sains Indonesia (2022), the consequences of premarital sexual relations, such as pregnancy at a young age, can bring physical, psychological and social risks for teenagers, including the possibility of abortion. Objectives: Research shows that 86% of students have held hands and hugged their partners, with 20% of these activities done voluntarily, and 34% engaging in sexual activities every time they meet. As many as 42% of respondents revealed that the most common places to engage in sexual activities are in boarding houses or hotels. Methods : Research design is a framework that provides clear guidance and direction for researchers during the conduct of research. The aim is to provide detailed instructions so that researchers can follow the research stages correctly. This research uses a descriptive quantitative approach with a design that includes observational analysis and applies a cross sectional approach. Results : The research results show that there is no relationship between the level of knowledge regarding premarital sexual behavior among students and female students at SMA PGRI 10 Glenmore. This result can be seen from the p value of 0.848, so (H0) in this study is accepted. Thus it can be concluded that the level of knowledge has no relationship with premarital sexual behavior. Conclusions : Analysis shows that out of 60 respondents with good knowledge, 5 respondents (8.2%) have a positive risk towards premarital sexual behavior, while 34 respondents (55.7%) have a negative risk. On the other hand, out of 10 respondents (16.4%) with less knowledge, 9 respondents (14.8%) showed a negative risk and 1 respondent (1.6%) showed a positive risk towards premarital sexual behavior. The statistical test using chi-square yielded a significant value of ρ = 0.848. Because the ρ-value is greater than 0.05, it can be concluded that there is no relationship between the level of knowledge and premarital sexual behavior.

Abstract

Unintended pregnancy (UP) among adolescents is a public health issue that affects physical, psychological, and social aspects. This study aims to explore the role of parents in the incidence of UP among adolescent girls in the working area of Puskesmas Sokaraja II, Banyumas Regency. The study employed a qualitative approach with a holistic case study design. The primary informants consisted of six adolescent girls who experienced UP, while the triangulation informants were healthcare professionals handling UP cases at Puskesmas Sokaraja II. Informants were selected using purposive sampling and snowball sampling, including adolescents who experienced UP and relevant healthcare professionals. Data were collected through in-depth interviews and analyzed using thematic analysis. The findings revealed four main themes: Permissive parenting contributes to risky behaviors due to a lack of parental supervision, limited reproductive health and sexuality education results in adolescents’ poor understanding of UP risks, low economic status restricts adolescents’ access to reproductive health services, and low self-confidence and susceptibility to environmental influences increase the risk of UP among adolescents.

Abstract

A stroke is defined as a neurological deficit attributed to an acute focal injury of the CNS (i.e., brain, retina, or spinal cord) by a vascular cause. Ischemic stroke is caused by disrupted cerebral blood flow due to thrombotic or embolic events. Stroke risk factors are classified into modifiable factors, such as hypertension, diabetes mellitus, hyperlipidemia, and smoking habits, and non-modifiable factors, such as age, gender, race/ethnicity, and genetic predisposition. This case report discusses a 60-year-old female patient presenting to the emergency department with right-sided body weakness, difficulty swallowing, slurred speech, and a history of uncontrolled hypertension. Physical examination revealed severe hypertension (216/106 mmHg), underweight nutritional status (BMI 17.24 kg/m²), motor weakness, and a positive Babinski reflex. A head CT scan showed infarction in the pons and left external capsule, consistent with a diagnosis of ischemic stroke due to small vessel occlusion. The patient was managed using the Code Stroke protocol, which included fluid administration, antihypertensives, neuroprotectors, and thrombolytic rTPA. Following therapy, clinical symptoms improved, with a reduction in the NIHSS score from 8 to 5, indicating a positive response. Patient education emphasized the importance of controlling risk factors such as hypertension and hypercholesterolemia and adherence to pharmacological treatments and lifestyle modifications. In conclusion, early diagnosis and the appropriate application of the Code Stroke protocol can enhance clinical outcomes, reduce morbidity and mortality, and prevent long-term complications in ischemic stroke patients. Ischemic stroke highlights the significance of an integrated multidisciplinary approach in stroke management.

Abstract

This study describes a case of Ischialgia caused by hernia nucleus pulposus (HNP) in a 44-year-old male patient. The patient presented with complaints of pain in the buttocks radiating to the right leg, accompanied by tingling and numbness, particularly after heavy physical activity. The diagnosis was established through anamnesis, physical examinations such as the Lasegue, Bragard and Socard tests, and MRI imaging, which revealed bulging discs at L1-2, L2-3, L3-4, L4-5, and L5-S1. Management was conducted using a holistic approach, including pharmacological therapy (NSAIDs, muscle relaxants, and vitamin B12), physiotherapy, and a lumbar corset to support spinal stability. After four days of treatment, the patient showed significant improvement, with the Visual Analog Scale (VAS) pain score decreasing from 8 to 3, normal motor strength recovery, and improved walking ability. Lifestyle modification education was provided to prevent recurrence, such as reducing heavy physical activities and engaging in controlled physical exercises. These results emphasize the importance of early diagnosis and multidisciplinary therapy in managing HNP cases to enhance the patient’s quality of life. This case contributes significantly to clinical practice by demonstrating the effectiveness of an integrated approach in optimally managing HNP.

Abstract

Introduction: Stunting is a global challenges and issues faced by societies around the world. The prevalence of stunting in the world is 148.1 million or 22.3%. The highest incidence of stunting is in Asian countries (52%) and African countries (43%). Stunting causes morbidity and mortality. Objective: to analyze the effectiveness of video and Leaflet media on mothers' knowledge, attitudes, and behavior to prevent stunting. Method: This quasi-experiment study used a non-equivalent control group design. It involved 32 mothers who have low birth weight babies (LBW) selected using purposive sampling techniques. Prior to its use, the questionnaire was subjected to tests of validity and reliability. The collected data were subsequently analyzed using statistical techniques, specifically paired t-tests and independent samples t-tests. Results: video and leaflet media effectively increased mothers' knowledge, attitudes, and behavior with a sig value of p = 0.000. The intervention group got the highest score with an average value of 15.62, 12.00, and 13.44 for knowledge, attitude, and behavior respectively. There was no significant difference in mothers’ knowledge (0.522), attitude (0.052), and behavior (0.476). Multiple Regression Analysis with a dummy on age, education and occupation did not influence mothers’ knowledge, attitude, and behavior to prevent stunting. Conclusions: Videos and leaflets effectively improve mothers' knowledge, attitudes, and behaviors towards stunting prevention efforts at Puskesmas Karangmojo II. however, no significant difference was found between the two grous. Abstract

Performance is a measure of individual or group success in carrying out tasks and responsibilities, as well as the ability to achieve predetermined goals and standards. Of all the regions in NTT province, Kupang City took the top spot with the highest percentage of reporting, at 95.27%. Southwest Sumba Regency ranked second with a reporting achievement of 69.83%, and followed by the NTT Provincial government which ranked third with a reporting percentage of 66.38%. This study aims to determine the relationship of job stress to employee performance at the Regional Inspectorate of Sikka Regency. The type of research used is analytic observational using Cross Sectional design. The sample in this study were 31 respondents. Samples in this study were taken with simple random sampling technique. The data analysis used was univariate analysis and bivariate analysis, using the Chi-square statistical test. The results of this study indicate that the variables associated with employee performance are workload (p = 0.047), working hours (p = 0.017), work pressure (p = 0.042) and work motivation (p = 0.020). This study highlights the importance of effective human resource management in improving the performance of an agency or organization.

Abstract

Introduction: Stunting is a chronic growth disorder caused by a lack of nutritional intake or malnutrition for a long time, especially in the first 1000 days of life. This study aims to determine the Relationship between the Use of MKJP Contraceptives and the incidence of Stunting in the Work Area of ​​the Kramatwatu Health Center UPT in 2023. The method used in this study is quantitative using a cross-sectional research design. The results of this study using statistical tests obtained a p value = 0.000. The value (p <0.05) so that the conclusion is that there is a Relationship between the Use of MKJP Contraceptives and the incidence of Stunting in the Work Area of ​​the Kramatwatu Health Center UPT in 2023. It is recommended for the Kramatwatu District Health Center UPT to further promote the benefits of MKJP contraceptives in preventing stunting.

Abstract

Pain assessment is a critical aspect of delivering quality care, particularly in intensive care units (ICU). Undetected and poorly managed pain can negatively impact patient recovery. One of the commonly used pain assessment methods for non-verbal patients is the FLACC scale (Face, Legs, Activity, Cry, Consolability). This study aims to evaluate the effectiveness of FLACC pain assessment education through Zoom media on the assessment skills of ICU nurses at Kapuas B RSUD Dr. Saiful Anwar, East Java Province. The research employs a quantitative approach with a pre-experimental design using the one-group pretest-posttest design. The sample consists of 27 ICU nurses selected through total sampling. The results showed that before the education intervention, most nurses (74.1%) were deemed incompetent in pain assessment skills. After the intervention, the majority of nurses (85.2%) were assessed as competent. Statistical analysis using the Wilcoxon test indicated a significant difference in assessment skills before and after the education intervention (Z = -4.000; Asymp. Sig. = 0.000). These findings demonstrate that providing education through Zoom is effective in improving the assessment skills of nurses in using the FLACC scale to evaluate patient pain in the ICU. Continuous education for healthcare professionals, particularly ICU nurses, is crucial to enhance pain assessment skills and support optimal patient care. Online education methods, such as via Zoom, prove to be an efficient alternative in certain circumstances.

Abstract

Introduction: A Headache is a common complaint among individuals with hypertension and is often associated with elevated blood pressure. One form of complementary therapy that can be applied to alleviate this symptom is Slow Stroke Back Massage (SSBM). Objectives: This study aimed to examine the effect of Slow Stroke Back Massage on headache intensity in older adults with hypertension. Methods: This quantitative research employed a quasi-experimental design with a non-equivalent control group pretest-posttest approach. Thirty-four hypertensive older adults participated in the study and were selected through purposive sampling. The intervention group received SSBM therapy three times per week for 10 minutes over four weeks, while the control group received only standard antihypertensive medication. Headache intensity was measured using the Numeric Rating Scale (NRS). Results: The mean pre-test headache intensity score was 6.12 ± 1.62 in the intervention group and 5.82 ± 1.55 in the control group. After the intervention, the mean scores decreased to 2.71 ± 1.53 and 3.82 ± 1.55, respectively. The reduction in headache intensity was 3.41 points in the intervention group and 2.00 points in the control group. The Wilcoxon test showed a significant difference in the intervention group (p = 0.000), while the paired sample t-test confirmed significance in the control group (p = 0.000). The pre-test comparison between groups using the independent t-test indicated no significant difference (p = 0.592), whereas the post-test comparison using the Mann-Whitney U test showed a significant difference (p = 0.041). These findings suggest that SSBM significantly reduces headache intensity among hypertensive older adults. Conclusions: Slow Stroke Back Massage is an effective, safe, and non-pharmacological intervention for reducing headache intensity in hypertensive patients. Its application is recommended as a complementary therapy in elderly care.
