Abstract

Insomnia is one of the most prevalent sleep disorders in the geriatric population and is considered a geriatric syndrome with substantial effects on physical, cognitive, and psychosocial health. Age-related changes in sleep architecture and circadian rhythms, along with an increased burden of chronic diseases and psychosocial factors, contribute to the high prevalence of insomnia among older adults. This condition is associated with an increased risk of depression, cognitive decline, cardiovascular and metabolic diseases, decreased quality of life, and higher morbidity and mortality. Therefore, an in-depth understanding of insomnia in geriatric patients is essential for effective clinical management. This study employed a literature review design, analyzing relevant scientific publications on insomnia in older adults. Sources included textbooks, clinical practice guidelines, and recent peer-reviewed journal articles discussing definitions, epidemiology, risk factors, pathophysiology, diagnostic methods, and management strategies. The findings indicate that insomnia in older adults is multifactorial, influenced by biological, psychological, and social factors, as well as comorbid medical conditions and polypharmacy. Diagnosis is primarily based on comprehensive clinical history taking, supported by validated assessment tools such as the Insomnia Severity Index (ISI) and the Pittsburgh Sleep Quality Index (PSQI), along with evaluation of underlying medical and psychiatric conditions. Nonpharmacological interventions are recommended as first-line therapy, particularly Cognitive Behavioral Therapy for Insomnia (CBT-I), sleep hygiene education, stimulus control, sleep restriction, and bright light therapy. Pharmacological treatment may be considered cautiously, given age-related pharmacokinetic changes and the heightened risk of adverse effects, including falls, cognitive impairment, and drug dependence. In conclusion, insomnia in the geriatric population is a complex condition requiring comprehensive assessment and a strong emphasis on nonpharmacological management to ensure safe and effective outcomes.

Abstract

Sjogren’s syndrome (SS) is a chronic autoimmune disorder characterized by sicca symptoms, particularly xerostomia, resulting from salivary gland dysfunction. Accurate assessment of glandular function and systemic disease activity is essential, as subjective complaints often do not correlate with objective clinical measurements. This study aimed to evaluate diagnostic and therapeutic approaches in patients with primary Sjogren’s syndrome based on objective salivary gland function assessment and standardized measurement of systemic disease activity. Methods: This observational study included patients diagnosed with primary Sjogren’s syndrome. Subjective xerostomia was assessed using the Summated Xerostomia Index–Indonesian version (SXI-ID), while objective evaluation was performed using the unstimulated whole salivary flow rate and the Clinical Oral Dryness Score (CODS). Systemic disease activity was measured using the EULAR Sjogren’s Syndrome Disease Activity Index (ESSDAI). All assessments were conducted prior to treatment initiation and periodically during follow-up. Non-autoimmune etiologies were excluded through comprehensive history taking, physical examination, and relevant investigations. A discrepancy was observed between subjective xerostomia complaints and objective salivary function measurements. ESSDAI scoring demonstrated variability in systemic disease activity, categorized as low, moderate, and high. A multidisciplinary approach contributed to accurate diagnosis and evaluation of organ involvement. Therapeutic decision-making in Sjogren’s syndrome should be guided by objective salivary gland function assessment and standardized systemic activity indices such as ESSDAI, rather than relying solely on subjective patient-reported symptoms. The integration of subjective and objective evaluations through periodic monitoring may enhance therapeutic precision and improve long-term disease management outcomes in patients with Sjogren’s syndrome.

Abstract

Introduction: Patients undergoing hemodialysis often experience physical changes. These changes can affect their perception of their body image. Self-esteem is believed to be a key determinant of body image. Methods: This was a quantitative study with a cross-sectional design. Measurements were made using the Rosenberg Self-Esteem Scale (RSE) and the Body Image Scale (BIS). The sample size was 78 patients with chronic kidney disease undergoing hemodialysis. Results: The Spearman rank test between self-esteem and body image in patients undergoing hemodialysis showed a p-value of 0.000 (p < 0.05) and an r-value of 0.404, indicating a statistically significant positive correlation between the two variables. Conclusion: There is a significant correlation between self-esteem and body image in patients undergoing hemodialysis

Abstract

Background: Antenatal care (ANC) is a core strategy for reducing maternal and neonatal mortality through early detection and management of pregnancy complications. However, ANC coverage in Kualuh Hulu District, North Labuhanbatu Regency, remains suboptimal, with fluctuating maternal and infant death rates signaling persistent service gaps. Identifying the local determinants of ANC compliance is essential for designing targeted interventions. Methods: A cross-sectional study was conducted among 153 second- and third-trimester pregnant women recruited through total sampling from four public health centers in Kualuh Hulu District. Data were collected using a structured, validated questionnaire. Analysis included univariate description, bivariate analysis using Chi-square tests, and multivariate binary logistic regression to identify independent determinants. Results: High ANC compliance was observed in 65.4% of participants. Bivariate analysis showed significant associations with six factors: knowledge (p<0.001; OR=4.97), attitude (p<0.001; OR=7.52), decision-making autonomy (p<0.001; OR=6.01), perceived service quality (p=0.002; OR=3.89), availability of staff and facilities (p=0.014; OR=2.90), and transportation and examination costs (p=0.006; OR=2.36). After multivariate adjustment, only knowledge (AOR=5.25; 95% CI: 2.17–12.68), attitude (AOR=4.38; 95% CI: 1.72–11.12), and decision-making autonomy (AOR=4.57; 95% CI: 1.87–11.16) remained significant independent predictors. Conclusions: Knowledge, attitude, and decision-making autonomy are the dominant determinants of ANC compliance in this rural setting. Interventions should prioritize continuous health education, awareness-building counseling, and empowerment of women in health-related decisions, while service quality improvements and cost support serve as essential enabling factors.

Abstract

Background: The active phase of the first stage of labor often takes a long time and may increase the risk of complications. Non-pharmacological interventions such as the rebozo technique and birth ball are commonly used to help accelerate labor progress. Methods: This study employed a quasi-experimental design with a pretest–posttest control group approach. A total of 36 laboring mothers were selected using purposive sampling and divided into two groups: 18 in the rebozo group and 18 in the birth ball group. Data were collected using observation sheets and partographs to measure the duration of the active phase of the first stage of labor. Data analysis was performed using the Wilcoxon Signed Rank Test and Mann-Whitney U Test. Results: The Wilcoxon test showed significant differences before and after intervention in both the rebozo group (p=0.000) and the birth ball group (p=0.000). The Mann-Whitney test indicated a significant difference between the two groups (p=0.000). The rebozo technique was found to be more effective than the birth ball in accelerating the first stage of labor. Conclusion: Both rebozo and birth ball techniques are effective in accelerating the first stage of labor; however, the rebozo technique is more effective. It is recommended as a non-pharmacological method in maternity care.

Abstract

Background: Pregnant women’s knowledge regarding healthy dietary patterns is often still low, which may affect maternal and fetal health. Health literacy is one of the promotive efforts that can improve pregnant women’s understanding of nutritional needs during pregnancy. Methods: This study used a pre-experimental design with a one-group pretest-posttest approach. The sample consisted of 32 pregnant women at TPMB Bd. Eny Islamiati, S.Tr.Keb Bululawang. Data were collected using a knowledge questionnaire before and after the health literacy intervention. Data analysis was performed using the Paired Sample t-test with a significance level of 0.05. Results: Before the intervention, most respondents had poor knowledge (84.4%). After the intervention, knowledge improved, with most respondents having sufficient (65.6%) and good (34.4%) knowledge, and no respondents remaining in the poor category. Normality test results showed that the data were normally distributed (p>0.05). The Paired Sample t-test showed a significance value of 0.000 (p<0.05), indicating a significant difference between pretest and posttest. Conclusion: Health literacy is effective in improving knowledge of healthy dietary patterns among pregnant women at TPMB Bd. Eny Islamiati Bululawang.

Abstract

This study aims to systematically synthesize the characteristics, components, theoretical framework, determinants, barriers, effectiveness, and research gaps related to family-based health promotion models in improving compliance with the use of ankle foot orthosis (AFO) in children with cerebral palsy (CP). The method used is Systematic Literature Review (SLR) with the PRISMA 2020 approach through a search of the Scopus database using keywords related to Family Health Promotion, Orthosis Use Adherence, and Pediatric Cerebral Palsy. Of the 659 initial articles, 71 main studies and 2 additional sources met the inclusion criteria to be analyzed through thematic synthesis and narrative integrative review. The results show that there is no single model that specifically integrates family-based health promotion to improve adherence to the use of AFO in CP children, but the literature provides a strong foundation through Family Systems Theory, Pender's Health Promotion Model, WHO Adherence Model, caregiver empowerment, personalized orthotic care, and digital health support. Key factors influencing compliance include family support, caregiver burden, self-efficacy, device comfort, health system, and social environment. In conclusion, improving compliance with the use of AFO requires an integrative approach that places the family at the center of health promotion, as well as opening up opportunities for the development of new conceptual models that are more comprehensive, personalized, and sustainable.

Abstract

Introduction: There is no secret that the way a hospital operates day to day has a lot to do with its internal culture. Clinicians, nurses, and administrative staff do not work willingness to collaborate, and even their approach to patient safety are all tied to the norms and expectations embedded in the institution around them. But despite a growing body of research on this topic, the evidence base remains uneven. Method: Most studies originate from North American or European contexts, leaving open the question of whether their conclusions hold up in settings with very different social and institutional fabrics. Indonesian hospitals, for instance, operate within a landscape shaped by communal decision-making, religious values, and resource constraints that rarely feature in mainstream organizational behavior research. This gap is worth closing, because hospital leaders everywhere need practical, context-sensitive guidance on how to build cultures that actually improve how people work. Results: To explore this issue, the authors reviewed fifteen peer-reviewed empirical studies published from 2016 through 2025, sourced from Scopus, PubMed, Google Scholar, and Indonesia's SINTA portal. The search followed a systematic protocol with defined criteria for what to include and what to leave out, focusing specifically on studies linking organizational culture to employee performance in hospital settings. The picture that comes into view is fairly consistent. When hospitals actively encourage experimentation and learning on the job, and when leadership gives people enough breathing room to take initiative, employees tend to respond with stronger creative output and a deeper sense of loyalty to the institution (Mutonyi et al., 2022; Acosta-Prado et al., 2020). A culture oriented around achievement and personal growth seems to help employees feel that their values align with those of the hospital, which in turn feeds into greater job satisfaction (Xiong et al., 2022). Open lines of communication and a genuine team spirit have also been shown to ease the strain of organizational transitions, keeping burnout in check when things get turbulent (Ellis et al., 2023). What the Indonesian literature brings to the table is a reminder that local context is not a footnote it is central. Spiritual values woven into hospital culture were linked to noticeable gains in staff performance (Kusnanda & Kusumapradja, 2020), and the overall cultural environment shaped how well mid-level managers handled their supervisory responsibilities (Askardin et al., 2025). Discussion: Patient safety culture, meanwhile, kept surfacing as the thread tying organizational mindset to what happens at the bedside. The bottom line is straightforward. Hospital culture is not a nice-to-have it is a lever that directly affects whether staff perform well and whether patients receive reliable care. Administrators who treat culture as something worth investing in, rather than something that simply exists in the background, are likely to see real returns in both workforce engagement and clinical quality.

Abstract

Introduction:  adolescent girls are among the age groups most vulnerable to iron-deficiency anemia. During this period, they experience rapid growth and development, requiring a balanced nutritional intake. One of the essential nutrients needed by the body is iron. In addition, adolescent girls undergo menstruation, which may contribute to the incidence of anemia. Objective:  The purpose of this study was to determine the relationship between respondents' age and their anemia status. Methods:  This study employed a quantitative analysis method with a cross-sectional research design, in which variables were measured simultaneously at a specific point in time. The dependent variable was anemia status, and the independent variable is the respondent's age. The sampling technique used was purposive sampling. The study was conducted at the Salafiyah Syafi’iyah Islamic Boarding School in Sukorejo, Situbondo, with a total of 75 respondents. Univariate analysis presents a frequency distribution of each variable, and bivariate analysis uses the chi-square test because the variables are categorical. Results: The study results show that the majority of participants were early adolescents, totaling 70 individuals (93.3%).  The anemia status in this study showed that most respondents did not have anemia, with 58 individuals (77.3%).  The results of the chi-square test indicate that age is not associated with anemia status. Conclusions:    Age does not have a significant association with anemia status. Nevertheless, it is important to conduct research on this topic, given that anemia status in adolescents is key to improving health outcomes throughout a woman’s life cycle.

Abstract

Introduction:  Healthcare workers are a key component in providing healthcare services to the community, contributing to the achievement of health development goals. Hospitals rely heavily on the quality of care provided by the hospital. However, some services still fall short of patient expectations, and patient satisfaction levels remain substandard, impacting hospital development. Nurses also need to demonstrate loyalty in providing patient care. Suboptimal levels of loyalty negatively impact the quality of hospital services, thus impacting patient satisfaction.  This study aims to determine whether there is a relationship between nurse loyalty and the level of satisfaction of inpatients at the Sula Islands Regency Hospital. Methods:  The research design used a quantitative correlational design with a cross-sectional approach. The variables in this study were the independent variable, namely nurse loyalty, while the dependent variable was the level of satisfaction of inpatients at the Sula Islands Regency Hospital. The population of this study was all nurses, namely 45 nurses and 439 patients calculated on average per year, at the Sula Islands Regency Hospital. And the sample in this study was 45 nurses using total sampling and 81 inpatients with a sampling technique using purposive sampling. The instruments used in this study were a nurse loyalty questionnaire and a patient satisfaction questionnaire. The data analysis method used was the Spearman rank test. Results:  The results of the study proved that the loyalty of nurses who were loyal to the level of patient satisfaction was satisfied, namely 10 (32.3%), and 4 (12.9%) with a very satisfied level of patient satisfaction. Conclusions: The results of the Spearman rank test show that there is a relationship between nurse loyalty and the level of patient satisfaction at the Kepulauan Regency Regional Hospital, with a p value = (0.025) < (0.05), meaning that increasing nurse loyalty is very important to improve the quality of nursing services and increase patient satisfaction in the hospital environment.
