Jurnal Klinik dan Riset Kesehatan (JK-RISK) – RSUD dr. Saiful Anwar Malang adalah jurnal open-access resmi yang dikelola oleh tim Penelitian dan Pendidikan Rumah Sakit Umum Daerah dr. Saiful Anwar Malang, Jawa Timur, Indonesia. JK-RISK menerbitkan satu volume yang terdiri atas tiga nomor setiap tahunnya yang diterbitkan pada bulan Oktober, Februari, dan Juni. JK-RISK hanya akan menerbitkan jurnal berbahasa Indonesia. Jurnal Klinik dan Riset Kesehatan – RSUD dr. Saiful Anwar Malang menerbitkan tulisan-tulisan ilmiah hasil penelitian, laporan kasus, tinjauan pustaka dalam bidang kedokteran dan kesehatan yang belum pernah diterbikan di jurnal manapun. Pernyataan kode etik publikasi JK-RISK merupakan pernyataan kode etik yang melibatkan pengelola jurnal, editor, mitra bestari serta penulis dalam proses publikasi jurnal ilmiah. Abstract

Cardiomyopathies are uncommon, although they are significant contributors to serious cardiovascular problems during pregnancy. Prior to pregnancy, it is crucial to have a thorough understanding of the potential dangers linked to cardiomyopathies and how to effectively treat them in pregnant women with significant pre-existing diseases. This knowledge is essential for providing appropriate guidance to patients. Given that all interventions relate to both the mother and the fetus, it is crucial to focus on providing the most efficient care for both. Maternal illness complicates pregnancy in approximately 1-4% of instances. There is a lack of comprehensive data on the frequency and occurrence of heart disease connected to pregnancy in most regions of the world including peripartum cardiomyopathy (PPCM) which was the predominant causes of maternal mortality in the UK in the mid of 2000’s. PPCM may result in persistent systolic dysfunction over an extended period of time. Although heart disease is a prominent, if not the primary, cause of death among pregnant or postpartum women across the country. PPCM is frequently misdiagnosed as a result of insufficient awareness among both medical professionals and the general public. The absence of an early and precise diagnosis of this ailment can have life-threatening implications for women affected by PPCM. Patients who had an early diagnosis saw a more expedited recovery compared to patients who received a late diagnosis. Improving the early monitoring, detection and diagnosis is linked to better recovery.

Abstract

Background: Dengue fever has become a global and national health concern due to its increasing incidence rate over the past six years. Dengue infection may present with variable sign and symptoms. Major organ involvement may occur, including liver, which is characterized by an elevated transaminase serum level. Aim: This study aims to identify factors contributing to increased transaminase serum levels in patients with dengue fever at Ngudi Waluyo Wlingi General Hospital between 2023-2024. Methods: One hundred and sixty-nine patients with positive dengue serology admitted to Ngudi Waluyo Wlingi General Hospital from January 1st, 2023 to May 19th, 2024, were reviewed retrospectively. Only patients aged 15 years and over with complete laboratory data were included in this retrospective cross-sectional study, using total sampling approach. We collected demographic and laboratory data from electronic medical records, then analyzed and correlated the data with liver function values. Results: Increased AST levels was observed in 70 patients (41.4%), while ALT values was elevated in 32 patients (24.9%). This increase was significantly linked to decreased platelet counts (p=0). Additionally, age was statistically associated with ALT levels (p=0.022), while gender was significantly related to AST levels (p=0.04). However, leukocyte levels and dengue serology showed no significant correlation with serum transaminase levels (p>0.05). Together, dengue serology, platelet counts, leukocyte levels, gender, and age collectively have a significant effect on AST (14.3%) and ALT (13.6%) values. Among these factors, platelet counts exerted the greatest influence on AST and ALT levels. Conclusion: Elevated transaminase serum levels may occur in patients with dengue fever. This is associated with platelet counts, age, and gender. Therefore, diagnosing dengue infection should prompt measurement of liver function to ensure appropriate management according to each patient’s condition.

Abstract

Latar Belakang: Sindroma lisis tumor adalah sekumpulan gejala metabolik akibat terlepasnya subtansi intraselular kedalam sirkulasi darah. Kriteria Cairo dan Bishop digunakan untuk menegakkan sindroma lisis tumor. Pasien dengan keganasan hematologis seperti leukemia akut sering mengalami sindroma lisis tumor, begitu juga dengan komplikasi infeksi. Evaluasi sindroma lisis tumor, khususnya mengetahui parameter laboratorium yang dapat digunakan serta komplikasi yang dapat muncul seperti infeksi merupakan hal yang penting untuk dipahami. Laporan Kasus: Anak laki-laki berusia 2 tahun menjalani pengobatan kemoterapi rumatan untuk LLA L2 yang di diagnosa sejak kurang lebih 1 tahun lalu. Pasien datang tanpa keluhan, namun mengalami perburukan kondisi seiring berjalannya kemoterapi. Diapatkan imbalans elektrolit serta gejala klinis yang sesuai dengan gambaran sindroma lisis tumor. Pada saat perawatan pasien juga menderita pneumonia, infeksi saluran cerna serta sepsis (skor PELOD 15). Kesimpulan dan Saran: Pasien mengalami sindroma lisis tumor saat sedang menjalani proses kemoterapi rumatan. Meskipun pasien datang tanpa keluhan, namun terdapat hiperurisemia dan peningkatan kreatinin yang mungkin dapat dipertimbangkan sebelum inisiasi kemoterapi. Sepsis pada pasien ini mungkin disebabkan oleh bakteri pada saluran cerna mengingat neutropenia berat yang dialami.

Abstract

Pendahuluan: Keluhan individu yang harus bangun untuk berkemih satu kali atau lebih pada malam hari yang didahului dan diikuti oleh tidur didefinisikan sebagai nokturia. Hal ini sangat rentan terjadi pada orang dewasa muda yang memiliki volume beban kerja tinggi. Mahasiswa profesi merupakan salah satu kelompok dengan tingkat stres dan kelelahan fisik yang tinggi yang berakibat pada terjadinya gangguan tidur yang dapat menurunkan kualitas tidur. Terlebih saat masa pandemi Covid-19 yang berdampak pada berbagai aspek kehidupan masyarakat termasuk dalam penerapan sistem perkuliahan dan pembatasan jam kerja untuk mahasiswa profesi kedokteran di seluruh Indonesia. Tujuan: Penelitian ini bertujuan untuk mengetahui angka kejadian nokturia, kualitas tidur dan hubungan keduanya pada mahasiswa profesi Program Studi Pendidikan Dokter Fakultas Kedokteran Universitas Brawijaya selama pandemi Covid-19. Metode: Penelitian ini merupakan penelitian observasional analitik dengan desain Cross-sectional dan teknik Purposive Sampling. Penelitian dilakukan pada bulan April-Mei 2022. Responden terdiri dari 78 mahasiswa profesi yang ditentukan berdasarkan kriteria inklusi dan eksklusi. Data dianalisis dengan analisis univariat dan bivariat menggunakan uji Chi-square. Hasil: Dari 78 responden, 39 responden (50%) mengalami nokturia dan 47 responden (60,30%) memiliki kualitas tidur tidak baik. Pengujian hubungan antara nokturia dengan kualitas tidur diketahui nilai probabilitas chi-square sebesar 0,488 (p>0,05). Kesimpulan: Mahasiswa profesi cenderung mengalami gejala nokturia dan memiliki kualitas tidur yang tidak baik. Namun, tidak didapatkan hubungan antara nokturia dengan kualitas tidur.

Abstract

Background: Acute Limb Ischemia ALI requires urgent intervention to avert irreversible tissue damage, with treatment complexity increasing in patients with nephrotic syndrome. Case Presentation: A 23-year-old female with nephrotic syndrome presented acute right leg pain. Examination showed erythema and a cold sensation in the affected leg. Ultrasound revealed no blood flow in certain leg arteries. Despite multiple thrombectomies and heparin treatment, only minimal improvement was observed. The patient underwent a below-knee amputation after unsuccessful attempts to restore blood flow. Her condition worsened with new vascular occlusions, requiring Catheter Direct Thrombolysis CDT. Post-CDT, she suffered severe bleeding, hemorrhagic shock, cardiac arrest, and multi-organ complications, leading to her death on treatment day 35. Discussion: This case illustrates the complexities of managing ALI in patients with nephrotic syndrome, which increases hypercoagulability and vascular complication risks. It highlights the need for an integrated nephrology and vascular approach, considering the high risks of hemorrhagic and thrombotic complications. A comprehensive and multidisciplinary treatment strategy is crucial, especially with vigilant monitoring for severe complications like infection and sepsis, to enhance treatment outcomes in similar cases. Conclusion: This case of a 23-year-old woman with ALI and nephrotic syndrome presented significant treatment challenges. Despite numerous interventions, including thrombectomies and CDT, severe complications ensued, emphasizing the need for comprehensive management and further research in similar complex conditions.

Abstract

Background: The worldwide incidence of congenital heart disease (CHD) is estimated at 1.2 million cases out of 135 million live births each year. Pulmonaryarterial hypertension (PAH) is a frequent complication of congenital heart disease (CHD), especially in patients with left-to-right shunt. Adult CHD (ACHD) and ACHD- PAH patients present to health facilities in late phase conditions, because the early- stage symptoms are very uncommon, as a result the treatmentbecomes more difficult with a poor prognosis. Until now, there’s no national registry of ACHD and ACHD- PAH in Indonesia. Aim: Describe the demographic characteristics, clinical presentation, supporting examination findings, management, complications, and outcomes of ACHD and ACHD-PAH populations in Malang. Methods: Left to Right Shunt and Pulmonary Hypertention Registry is an observational study of adult patients (aged ≥ 18 years) diagnosed with left to rightshunt CHD and PAH at Saiful Anwar Hospital Malang. This study evaluated patients from November 2022 - October 2023 consecutively. Patients underwent a series of examinations including clinical examination, ECG, chest X-ray, SMWT, laboratory examination, transthoracic echocardiography and right heart catheterization. Results: Total sample of this registry is 124 samples. The majority of sample is young adult woman and ASD is the most common ACHD defect. The majority of sample (83.0%) is with symptoms with NT pro BNP median is 548 (152.1 – 2550.5). The majority of sample was WHO functional class II Conclusion: LET SHINE Registry is the first ACHD and ACHD-PAH in East Java Province which depict demographic, clinical presentation, hemodynamicpresentation pf ACHD and ACHD-PAH patient.

Abstract

The widespread implementation of invasive procedures such as coronary angiography and primary percutaneous coronary intervention (PCI) into the routine management of patients with acute ST-segment elevation myocardial infarction (STEMI) in the last 10 years has led to a significant improvement in patient prognosis. At the same time, this also raises new problems and questions, mostly related to the fact that there are many elderly patients and/or patients with comorbidities who need to undergo this invasive procedure. One of the most important comorbidities is renal insufficiency. STEMI patients with renal insufficiency typically present with more extensive atherosclerotic lesions, including diffuse coronary calcification, which poses a challenge to the interventional cardiologist due to a higher risk of periprocedural complications, higher risk of restenosis, major adverse cardiovascular event (MACE) , and patient mortality. This review discusses in depth the influence of renal insufficiency on MACE and mortality in STEMI patients. Keywords: renal insufficiency, STEMI, MACE, mortality.

Abstract

Hemangioma is a vascular anomaly that is benign and generally occurs in children. ISVVA 2018 classifies vascular anomalies into two categories, namely vascular malformations and vascular tumors (hemangioma). The prevalence of hemangioma is higher in low birth weight babies, premature babies, and girls. Several theories suggest that hemangiomas are caused by vasculogenesis and angiogenesis or an imbalance between angiogenic and antiangiogenic factors. Hemangiomas grow through a proliferation phase, involution phase, and post-involution phase. Hemangioma classification is based on the depth of the lesion, time of appearance of the lesion, distribution of the lesion, and its relationship to syndrome complication.. Diagnosis of hemangioma is based on anamnesis, physical examination, and supporting examinations which include USG, MRI, and CT-Scan. Hemangiomas that lead to complications should require immediate treatment. Hemangioma treatment can be done with topical therapy, systemic therapy (propranolol, corticosteroids, β-blockers, vincristine, rapamycin), laser therapy (PDL, diode, Nd: YAG, argon, KTP, CO2, IPL), and other treatments consisting of from surgical and non-surgical procedures (bleomycin injection).

Abstract

Pulmonary hypertension (PH) is characterized by pulmonary vascular dysfunction, which can lead to right heart failure and death, as well as the accumulation of inflammatory cells in the perivascular area. Chronic inflammation plays an important role in the pathophysiology of pulmonary hypertension. Inflammatory markers such as C-reactive protein (CRP), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-α) are often elevated in patients with pulmonary hypertension. The accumulation of inflammatory cells accelerates the remodeling process of pulmonary blood vessels and pulmonary hypertension. Despite the current therapeutic approaches for pulmonary hypertension (PH), which aim to improve functional capacity and hemodynamics, pulmonary hypertension remains incurable but remains controllable. Inflammatory processes play a critical role in progressive pulmonary hypertension. Physical exercise can reduce the risk of chronic disease, and recent research has shown that it improves inflammatory marker profiles. This literature review aims to combine evidence on the influence of physical exercise on levels of inflammatory markers in patients with pulmonary hypertension.
