Jurnal Klinik dan Riset Kesehatan (JK-RISK) – RSUD dr. Saiful Anwar Malang adalah jurnal open-access resmi yang dikelola oleh tim Penelitian dan Pendidikan Rumah Sakit Umum Daerah dr. Saiful Anwar Malang, Jawa Timur, Indonesia. JK-RISK menerbitkan satu volume yang terdiri atas tiga nomor setiap tahunnya yang diterbitkan pada bulan Oktober, Februari, dan Juni. JK-RISK hanya akan menerbitkan jurnal berbahasa Indonesia. Jurnal Klinik dan Riset Kesehatan – RSUD dr. Saiful Anwar Malang menerbitkan tulisan-tulisan ilmiah hasil penelitian, laporan kasus, tinjauan pustaka dalam bidang kedokteran dan kesehatan yang belum pernah diterbikan di jurnal manapun. Pernyataan kode etik publikasi JK-RISK merupakan pernyataan kode etik yang melibatkan pengelola jurnal, editor, mitra bestari serta penulis dalam proses publikasi jurnal ilmiah. Abstract

Alhamdulillah, atas berkat dan rahmat Tuhan Yang Maha Esa, akhirnya redaksi Jurnal Klinik dan Riset Kesehatan (JK-RISK) dapat menerbitkan edisi perdananya. JK-RISK merupakan jurnal ilmiah online, open access, dan peer-reviewed yang dikelola oleh tim Penelitian dan Pendidikan Rumah Sakit Umum Daerah dr. Saiful Anwar, Malang, Indonesia. JK-RISK bertujuan utama untuk menyebarluaskan informasi dibidang Kesehatan dan Kedokteran, sehingga menambah khazanah ilmu pengetahuan, mengembangkan ilmu pengetahuan dan teknologi serta mendorong semakin meningkatnya kuantitas dan kualitas penulisan artikel ilmiah di Indonesia pada umumnya.

Abstract

Kanker payudara pada wanita menempati urutan pertama kanker primer tersering, dengan tingkat kematian tinggi. Penelitian ini bertujuan memberikan data gambaran dasar profil klinikopatologi dan imunohistokimia karsinoma payudara invasif karena belum ada data profil klinikopatologi kanker payudara di RSUD Dr. Saiful Anwar Malang. Penelitian ini observasional deskriptif, teknik total sampling, desain penelitian Cross Sectional. Menggunakan data dari rekam medis di Instalasi Patologi Anatomi RSUD Dr. Saiful Anwar Malang periode Januari 2018 - Maret 2020. Variabel yang diukur usia, lokasi payudara, jenis spesimen, histopatologi, derajat keganasan histopatolgis (grading), dan profil immunohstokimia. Hasil menunjukkan kejadian karsinoma payudara invasif terbanyak pada tahun 2019 sebanyak 270 kasus (50%), rentang usia tertinggi antara 46 - 55 tahun sebanyak 142 orang (28%). Teknik pengambilan sampel dengan cara mastektomi sebesar 316 (58%) dibandingkan biopsi sebanyak 226 (42%), lokasi tersering terjadinya karsinoma tidak didapatkan perbedaan bermakna antara payudara kanan atau kiri. Jenis histopatologi terbanyak Invasif Carcinoma of No Special Type (NST) sebesar 475 kasus (88%) dari 542 kasus karsinoma payudara invasif, derajat keganasan histopatologi terbanyak adalah grade III sejumlah 240 kasus (47%). Berdasarkan profil imunohistokimia, Luminal A merupakan kategori tebanyak sebesar 44 kasus (31%), dibandingkan Luminal B, Ekspresi HER2 ataupun kanker payudara tripel negatif. Kesimpulannya adalah usia terbanyak penderita karsinoma invasif payudara terjadi pada usia 46-55 tahun, tidak terdapat perbedaan distribusi karsinoma payudara pada payudara kanan atau kiri, jenis spesimen terbanyak diambil dari Teknik mastektomi, jenis histopatologi terbanyak yaitu invasive carcinoma of NST, derajat keganasan histopatologi terbanyak pada grade III, luminal A terbanyak pada pemeriksaan imunohistokimia karsinoma payudara invasif.

Abstract

Background: The spreading of the Severe Acute Respiratory Syndrome Coronavirus (COVID-19) pandemic could be associated with psychiatric implications, including depression, one of the most common mental disorders. It is up-surging worldwide amid the ongoing coronavirus disease 2019 (COVID-19) pandemic, especially among hospitalized elderly COVID-19 survivors. Aim: To determine the impact of the COVID-19 pandemic on the possibility of depression using Geriatric Depression Scale-15 (GDS-15) questionnaire in hospitalized elderly COVID-19 survivors Methods: The study was an observational study with cross-sectional data collecting. We collected the subject of the study from elderly COVID-19 survivors hospitalized in Saiful Anwar General Hospital. Subjects were called by phone, and then they were asked based on the GDS-15 questionnaire. The questionnaires included demographic and socioeconomic data, health status, and GDS-15 score. Results: Among the 33 sample of this research, cross-sectional phoned surveys indicated that the elderly <70 years old were less psychologically distressed (p 0,057). The length of stay had a positive correlation with the GDS-15 score( p 0,157 ) Conclusion: Elderly COVID-19 survivors >70 years old tend to have possibility depression than <70 years old. A longer stay is correlated with the possibility of depression in elderly COVID-19 survivors. Occupation has a significant correlation with age that can affect the likelihood of depression.

Abstract

Coronavirus Disease 2019 (COVID-19) is a new disease caused by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) infection. Common clinical manifestations are respiratory system disorders. However, COVID-19 can also manifest as extrapulmonary symptoms, one of which is dermatology. The exact mechanism of COVID-19 cutaneous manifestations is still not fully understood, but is suspected to be caused by the direct effects of the virus on keratinocyte, excessive immune response, or drugs used to treat COVID-19. Cutaneous manifestations of COVID-19 can be divided into five groups, namely pseudo-chilblains, vesicular, urticaria, erythematous maculopapular, and vascular lesions. Erythematous maculopapular (44.18%) and chilblains (19.72%) are the most common lesions. Vesicular and vascular lesions are less common. Chilblains lesions are usually found before or together with the onset of other COVID-19 symptoms, and often appear in close contact or asymptomatic patients, so early recognition of it can be used to detect the presence of COVID-19.

Abstract

Parapsoriasis is a dermatoses which reflect clinical overlap includes chronicity, recalcitrans, idiopathic causes against the therapy, and the lesions often nonpruritic, which resembles to psoriasis. There are two common forms of parapsoriasis i.e. small plaque parapsoriasis (SPP) and a large plaque parapsoriasis (LPP). Many skin diseases have symptoms that resemble parapsoriasis, so the diagnosis is difficult to establish. Therefore histopathological and immunohistochemical examination needs to be done. Parapsoriasis is characterized by superficial cutaneous lymphoid infiltrates consisting of T cells in histopathological examination both on LPP and SPP. But, LPP infiltrates often contain lymphocytes with cerebriform nuclei (Lutzner cells) that are similar to Sezary cells in Mycosis fungoides (MF). Immunohistochemical examination of CD3 can show that T cells are normal or atypical if they lead to a malignancy. Therefore, evaluation of T cell density on histopathological features and CD3 immunohistochemical examination can help establish the diagnosis of parapsoriasis, as well as to recognize signs of malignancy leading to MF.

Abstract

Pemphigus vulgaris (PV) is an autoimmune disease characterized by blisters on the skin and mucous membranes. The primary lesion of PV are loose blisters, which can occur anywhere on the surface of the skin and mucosal surface. Diagnostic test aims to confirm the diagnosis of PV to other vesiculobullous disease, including skin or mucosal biopsies for histopathological examination and immunofluorescence (IF). Histopathological features intraepithelial blisters and suprabasilar achantolysis with characteristic row of tombstones which were the result of IgG autoantibodies to glycoprotein desmoglein 3 (dsg3) and partly to desmoglein 1 (dsg1). Direct immunofluorescence examination showed deposits of IgG, IgM and C3 in the intercellular spaces in the epithelium. Blood tests with the ELISA test can only be used to check the levels of autoantibodies to Dsg3 and Dsg1.

Abstract

Introduction: Marjolin's ulcer is a malignant process in the skin caused by trauma or chronic inflammation. This malignancy is rare and aggressive. Squamous cell carcinoma (SCC) is the most common manifestation of burns. Case Illustration: A 51-year-old woman came to Saiful Anwar Hospital Malang with a complaint of a cauliflower-like lump on the upper arm of her right hand since 8 months ago. The lump was initially as small as a fingertip (1cm), then it got bigger. The patient suffered burns on his body after being hit by gasoline spray 33 years ago. The patient also complains of a painful sensation and a foul odor in the area of ​​the lump. Conclusion: The inflammatory process has an essential role in the development of Marjolin's ulcer.

Abstract

Introduction: Carpal Tunnel Syndrome (CTS) is a peripheral nerve lesion that is often encountered due to non-traumatic mechanisms. There are various methods of CTS therapy to reduce complaints and increase patient productivity. One of the non-pharmacological methods that can be used is Neuromuscular Tapping (NMT). The purpose of this case report is to determine the effectiveness of using NMT as a non-pharmacological therapy in CTS patients who have previously received pharmacological therapy, measured by the NRS (Numeric Pain Rating Scale) in patients. Case Illustration: A 52-year-old woman came with complaints of tingling in the right and left wrists for four months with a history of working in cigarette factory for ten years. Physical examination revealed pain in the right and left wrists with NRS 6-7, hypesthesia in fingers 1, 2, 3, and ½ 4 of volar manus bilaterally; the patient has taken antiinflammatory drugs, and the pain improves when taking the drug but will relapse when not taking medication. The patient was diagnosed with Carpal Tunnel Syndrome bilateral (Rosenbaum level 2/ persistent symptoms); the patient received pharmacological therapy by administering NSAIDs (Nonsteroidal Anti-Inflammatory Drugs) and corticosteroid injections but did not show any improvement in pain, then the patient underwent NMT as non-pharmacology therapy. Discussion: In the results of the patient's follow-up after undergoing NMT therapy, there was an improvement in the patient's complaints, as seen from the improvement in the patient's NRS score.

Abstract

External genitalia trauma is more common in men than women, especially in the age range of 15 & 40 years. This is due to the anatomical differences and the increase in the frequency of traffic accidents. The number of external genitalia trauma ranged from 33–66% of all urological cases. External genitalia trauma is most often caused by blunt trauma, one of which is testicular rupture. Testicular rupture refers to a tear in the tunica albuginea resulting in extrusion of testicular contents. Determining the appropriate examination procedure requires adequate physical examination and information about the mechanism of trauma to allow a good prognosis. Scrotal ultrasound (SUS) is the first choice of modality in the case of testicular injury with sensitivity in detecting testicular rupture up to 100%. In addition, the SUS examination procedure can be carried out immediately, non-radiatively, and non-invasively. The main ultrasound findings for testicular rupture include testicular heterogeneity, contour abnormalities, and a tear in the tunica albuginea. These findings can be used to determine the degree of testicular injury according to the AAST (American Association for the Surgery of Trauma), which can then be used to determine the definitive treatment management. We present a case of a 19-year-old male patient complaining of pain on his left testicle since 16 hours before hospital admission after a traffic accident. Physical examination revealed a scrotal hematoma, and nausea and vomiting were found. Ultrasound examination indicated extensive tearing of the left testicular structure according to AAST grade IV with complex hydrocele.
