Abstract

Introduction: The Elderly Posyandu is a vital community-based program that supports elderly health services in Indonesia. Cadres are key actors in delivering these services, and their performance is influenced by multiple factors. Objective: This study aimed to identify the factors influencing cadre performance in implementing the Elderly Posyandu at Way Krui Health Center, Pesisir Barat Regency, in 2024. Method: A quantitative cross-sectional study was conducted in February 2025. The population consisted of 118 cadres in the Way Krui Health Center area. A total of 92 respondents were selected using the Slovin formula. Data were collected through structured questionnaires and analyzed statistically using computer software. Result: The findings revealed significant relationships between cadre performance and individual factors (ρ = 0.007), organizational factors (ρ = 0.018), psychological factors (ρ = 0.014), and leadership factors (ρ = 0.013). Cadres influenced by these factors demonstrated better performance in delivering elderly health services. Conclusion: Cadre performance in the Elderly Posyandu program is significantly affected by individual, organizational, psychological, and leadership factors. Strengthening these aspects through training, support, and effective communication is essential. Collaboration between the government, community, and health institutions is crucial to sustaining and enhancing the quality of elderly health service.

Abstract

Introduction: Human Resource Management (HRM) in hospital settings is a comprehensive process that includes recruitment, placement, and employee development to ensure optimal healthcare service delivery. A prevalent HRM issue in hospitals is nurse work fatigue, which negatively affects performance, patient safety, and service quality. Shift work, a core aspect of hospital operations, is a major contributor to fatigue due to its impact on physical and psychological well-being. Objective: This study aimed to examine the relationship between work shift factors—specifically length of work shifts, shift changes, and shift rotations—and the level of work fatigue among nurses at Dr. H. Abdul Moeloek Regional Hospital in Bandar Lampung. Method: A quantitative, cross-sectional study was conducted with a sample of 142 nurses selected using proportional stratified random sampling. Data were collected through structured questionnaires and analyzed using statistical tests to assess the significance of associations between work shift variables and nurse fatigue levels. Result: The findings revealed significant associations between all three work shift variables and nurse fatigue. The length of work shifts (p = 0.039), shift changes (p = 0.028), and shift rotations (p = 0.049) were all significantly related to fatigue levels. Nurses with sufficient shift arrangements experienced lower fatigue than those with poor shift conditions. Conclusion: Work shift management significantly influences nurse fatigue levels. Irregular rotations and extended work hours contribute to higher fatigue, potentially compromising nurse performance and patient care. Improved shift scheduling, adequate rest, and supportive health and stress management strategies are essential to reduce fatigue and enhance nurse well-being in shift-based work environments.

Abstract

Introduction: Health services play a crucial role in improving public health, particularly through primary healthcare centers. Patient satisfaction serves as a key indicator of service quality and is influenced by several factors, including performance, facilities, and communication. However, dissatisfaction remains an issue in many healthcare settings. Objective: This study aimed to identify the factors associated with patient satisfaction at the Tiuh Tohou Health Center, Tulang Bawang Regency in 2024. Method: A quantitative analytical survey with a cross-sectional design was used. The population included all patients treated at the health center in 2024 (n=310), from which 175 were selected using purposive sampling. Data were analyzed using the chi-square test to determine relationships between variables. Result: The results showed that performance, facilities, and communication were significantly associated with patient satisfaction. Poor performance increased dissatisfaction likelihood by 48 times (p=0.000; OR=48.417), poor facilities by 2.5 times (p=0.004; OR=2.577), and poor communication by 20 times (p=0.000; OR=20.063). Most patients were over 35 years old, male, had a high school education, were self-employed, and reported dissatisfaction with services received. Conclusion: Patient dissatisfaction at the Tiuh Tohou Health Center is significantly influenced by poor staff performance, inadequate facilities, and ineffective communication. These findings highlight the need for targeted improvements in health worker performance, infrastructure, and patient-provider communication to enhance satisfaction levels and service quality.

Abstract

Introduction: Hypertension is a prevalent non-communicable disease that contributes to serious health complications such as stroke, heart disease, and kidney failure. While pharmacological treatments are widely used, they may lead to drug-related problems, making non-pharmacological alternatives, such as Slow Stroke Back Massage (SSBM), increasingly relevant. Objective: This study aimed to determine the effect of Slow Stroke Back Massage therapy on reducing blood pressure in hypertensive patients. Method: A quantitative approach with a quasi-experimental, one-group pretest-posttest design was employed. The study was conducted at Dr. A. Dadi Tjokrodipo Hospital in Bandar Lampung from December 2024 to February 2025. A total of 21 hypertensive patients participated in the intervention, receiving SSBM therapy. Blood pressure measurements were taken before and after the intervention, and statistical analyses were performed to assess significance. Result: The mean systolic blood pressure decreased from 146.71 mmHg to 132.14 mmHg, and diastolic pressure also showed a reduction. The intervention resulted in statistically significant changes, with p-values of 0.000 for systolic and 0.003 for diastolic pressure, indicating the effectiveness of SSBM therapy. Conclusion: SSBM therapy significantly reduces both systolic and diastolic blood pressure in hypertensive patients. This intervention enhances relaxation, promotes vasodilation, and reduces sympathetic nervous activity. It is recommended that healthcare professionals consider integrating SSBM as a complementary therapy alongside pharmacological treatment for holistic hypertension management.

Abstract

Introduction: Nursing services are a key determinant of healthcare quality and significantly influence patient satisfaction, especially among BPJS (Indonesia’s National Health Insurance) users. Inadequate nursing care has been associated with low satisfaction levels, impacting service utilization and hospital performance. Objective: This study aimed to examine the relationship between nursing services and patient satisfaction among BPJS users in the polyclinic of Dr. A. Dadi Tjokrodipo Regional General Hospital, Bandar Lampung. Method: A quantitative approach with a cross-sectional design was employed. The population included all BPJS patients in the polyclinic room in 2024. A purposive sampling technique was used to select 99 respondents who met the inclusion criteria. Data were collected using a validated structured questionnaire and analyzed using univariate and bivariate statistical methods, including the Chi-square test. Result: The majority of respondents rated the nursing services as good and expressed satisfaction with the services received. Among those who perceived nursing services as good, most reported being satisfied. Conversely, patients who perceived the services as not good tended to be dissatisfied. A significant relationship was found between nursing services and patient satisfaction (p-value = 0.000). Conclusion: There is a significant association between the quality of nursing services and the level of patient satisfaction among BPJS users. Improving nursing care is essential to enhance patient experiences and promote service utilization. Future studies should explore other contributing factors such as patient feedback, infrastructure, and facility availability to provide a more comprehensive understanding of healthcare quality determinants.

Abstract

Introduction: Type 2 diabetes mellitus (T2DM) is a chronic metabolic disorder marked by elevated blood glucose levels due to insulin resistance or impaired insulin secretion. The rising prevalence of T2DM in Indonesia, particularly in Bandar Lampung, highlights the need for effective interventions. While programs like Prolanis offer basic support, specific interventions to improve vascular health and prevent complications are still limited. Objective: This study aimed to assess the effect of resistance exercise on blood sugar levels, foot sensitivity, and ankle-brachial index (ABI) values in patients with T2DM at Kedaton Health Center, Bandar Lampung. Method: A quantitative quasi-experimental design with a one-group pretest-posttest model was used. Thirty-two individuals with T2DM were selected through purposive sampling. Participants underwent resistance exercise interventions involving repetitive limb movements. Blood sugar levels and ABI values were measured before and after the intervention. Results: The results demonstrated a significant decrease in blood sugar levels and a notable improvement in ABI values after the intervention, with both outcomes showing a p-value of 0.000. These findings indicate that resistance exercise can enhance glycemic control and peripheral vascular function. Conclusion: Resistance exercise significantly reduces blood sugar levels and improves ABI values in individuals with T2DM. This form of physical activity offers a promising non-pharmacological approach to managing diabetes and preventing complications. It is recommended that resistance exercise be integrated into routine diabetes care programs. Further research with varied methods and larger samples is encouraged to validate and expand these findings.

Abstract

Intorduction: Hospitals operate continuously to provide essential healthcare services, relying heavily on nurses who often work rotating shifts. These shift patterns, particularly night shifts, can lead to fatigue, which negatively impacts nurses’ health, performance, and patient safety. Objective: This study aimed to analyze the relationship between work shifts and nurse fatigue in the inpatient department of Dr. A. Dadi Tjokrodipo Hospital in 2024. Method: A quantitative analytical survey with a cross-sectional design was conducted. The study population included all inpatient nurses, with a total of 57 respondents selected using total sampling. Data were collected using validated questionnaires and analyzed using Chi-Square tests to determine associations. Result: The findings revealed that nurses working under shift schedules were more likely to experience fatigue compared to those on regular schedules. The statistical analysis demonstrated a significant association between shift work and nurse fatigue (p < 0.05). Conclusion: Shift work is significantly associated with increased nurse fatigue. Hospital administrators should consider implementing better shift management strategies to reduce fatigue and improve both nurse well-being and the quality of patient care.

Abstract

Introduction: Elderly gymnastics is a structured physical activity designed to enhance the functional health of older adults. As hypertension remains a leading cause of outpatient visits at Kalirejo Health Center, Negeri Katon District, the effectiveness of elderly gymnastics as a preventive strategy warrants investigation. Objective: This study aimed to examine the relationship between participation in elderly gymnastics and the incidence of hypertension among elderly individuals at the Kalirejo Health Center in 2024. Method: This research used a quantitative approach with a descriptive correlational method and cross-sectional design. The sample consisted of 80 elderly respondents selected using the Slovin formula. study used an observation sheet as a data collection tool where the data collected was a recapitulation of the attendance list of elderly gymnastics participants and the Community Health Center medical records (patient visit records).Data were analyzed using a chi-square statistical test. Result: Findings showed that most respondents who actively participated in elderly gymnastics did not have hypertension. Statistical analysis yielded a p-value of 0.020, indicating a significant relationship between the level of participation and the incidence of hypertension. Conclusion: There is a significant relationship between elderly gymnastics participation and the incidence of hypertension. Active involvement in regular physical activity such as elderly gymnastics may help reduce hypertension risk. Future research should explore factors influencing elderly participation to enhance program effectiveness.

Abstract

Introduction: Riyadh’s healthcare reforms under Vision 2030 emphasize patient outcomes as key quality indicators. This study examines the underexplored interplay between nurse engagement and change management in shaping healthcare performance within Riyadh Health Clusters. Objective: This study aimed to examine the impact of nurse engagement and change management on patient outcomes within Riyadh Health Clusters. Methods: A quantitative research design was employed, involving 384 healthcare professionals from three clusters. Nurse engagement and change management were analyzed as independent variables, while patient outcomes served as the dependent variable. Data were processed using linear regression analysis in SPSS with a 5% significance threshold. Results: The findings demonstrated that nurse engagement strongly predicted patient outcomes (R = 0.72; R² = 0.71; β = 0.64; p < 0.05), showing that higher engagement levels significantly improved care quality. Change management practices also had a positive but more moderate effect on patient outcomes (R = 0.47; R² = 0.22; β = 0.45; p < 0.05). Conclusion: This study demonstrates that nurse engagement is a strong predictor of patient outcomes, while change management also contributes positively, though to a lesser extent. The findings emphasize that investing in strategies to enhance nurse engagement, such as professional development, participatory decision-making, and supportive leadership, is critical to improving the quality of care. At the same time, structured change management remains essential to sustain organizational adaptation and innovation in healthcare delivery. Together, these insights highlight the importance of integrating workforce-centered initiatives with system-level reforms to strengthen patient-centered outcomes within Riyadh Health Clusters and beyond.

Abstract

Introduction: Saudi Arabia’s Vision 2030 has transformed healthcare delivery through the Riyadh Health Clusters, integrating hospitals and primary care centers to improve efficiency and patient outcomes. Within this context, nurse-to-patient ratios and employee empowerment have emerged as critical factors influencing patient satisfaction but remain underexplored in the Saudi healthcare system. Objective: This study examines the impact of nurse-to-patient ratios and employee empowerment on patient satisfaction within Riyadh Health Clusters, a transformative healthcare model under Saudi Arabia’s Vision 2030. Methods: Using a quantitative, cross-sectional design, data were collected from 384 healthcare professionals, predominantly nurses, across three clusters. Results: Linear regression analyses revealed a statistically significant but modest positive relationship between nurse-to-patient ratios and employee empowerment (β = 0.18, R² = 0.04, p < 0.05), underscoring the limited explanatory role of staffing adequacy in isolation. In contrast, employee empowerment demonstrated a robust impact on patient satisfaction (β = 0.45, R² = 0.56, p < 0.001), highlighting its centrality to care quality. The findings align with Kanter’s Structural Empowerment Theory but diverge from Western models due to Riyadh’s unique challenges, including hierarchical organizational cultures, a predominantly expatriate nursing workforce (70%), and urban-rural resource disparities. Conclusion: The study emphasizes the need for context-specific strategies that integrate staffing reforms with empowerment initiatives, such as leadership training and participatory decision-making, to mitigate workforce burnout and standardize care practices. These insights advance global healthcare literature by contextualizing empowerment dynamics in non-Western settings and offer actionable recommendations for policymakers to align Riyadh’s cluster model with Vision 2030’s patient-centered goals.
