Abstract

Background: Neonatal mortality is a death that occurs during the first twenty-eight days of life after a baby is born. Based on Indonesia Demographic and Health Survey (IDHS data) in 2017, there was a decrease in neonatal mortality between 2012 – 2017, but this has not yet reached the SDG's target of 12 deaths per 1,000 live births. Complications during childbirth are one of maternal factors that directly cause pain and death to a mother and her or newborn due to some disorders resulting from childbirth. One of the factors that can influence neonatal mortality is complications during delivery.Objective: This study aims to observe correlation between complications during childbirth and neonatal mortality. Also, this is to reveal the neonatal mortality rate in children born from 2012-2017 in Indonesia after controlling for all confounding.Methods: This was a cross-sectional study with an analytical observational approach using secondary data from the IDHS 2017. Its samples were women of childbearing age (15-49 years) who have been married and gave birth, with the last birth data of 5 years prior to the survey. Its inclusion criteria were children who were born in 2012-2017 and were not twin-birth as big as 15.357. The sample selection used total sampling, and the obtained data were analysed by univariate, bivariate, and multivariate analysis using multiple logistic regression.Results: The final model indicated that there was a significant relationship between the complications during delivery and neonatal mortality after controlling by variables of age, ANC visits, baby's birth weight, and baby's gender on neonatal mortality. PR 3,90 (95% C.I; 2.467 – 6.187).Conclusion: The results revealed that there was an influence of childbirth complications, maternal age, ANC visits, baby's birth weight, and baby's gender on neonatal mortality.

Abstract

Background: Fournier’s gangrene (FG) is a fatal condition that often leads to death if not treated properly. To this date, there are no data regarding FG in Indonesia. Objective: This study aims to investigate the characteristics of patients with FG.Methods: A cross-sectional study design was used to evaluate the medical records of patients admitted to Cipto Mangunkusumo Hospital between January 2012 and December 2017.Results: The median age of the subjects were 51 (2-81) years. All of the subjects were male and presented with scrotal pain. The scrotal abscess was found in 38 (88.4%) cases. Type II diabetes was found in 10 (23.3%) subjects. E. coli was the most dominant causative agent in 28 (65.1%) cases. Meropenem was administered in 16 (37.2%) subjects, and 16 (37.2%) subjects had antibiotics administered for ≤7 days. Conclusion: Although the mortality rate has decreased, those with FG tend to have a long hospital stay, which implies a high risk of complications. All males presenting with scrotal pain should be suspected of FG. Further studies on long-term hospitalisation complications in those with FG are required.

Abstract

Background: The adherence of type 2 diabetes mellitus (DM) patients plays an important role in therapeutic management. In carrying out every activity, a Muslim must make spirituality as a reference. Therefore, it is necessary to study the relationship of spirituality in type 2 DM patients with diabetes mellitus medication adherence. Objective: Estimate the correlation between spirituality and patients’ type 2 DM adherence.Methods: This study was observational with a cross-sectional design at PKU Muhammadiyah Gamping Hospital. Subjects were patients with type 2 diabetes mellitus. The level of spirituality was obtained using the Holistic Health Care (HHC) questionnaire and adherence was using the Summary of Diabetes Self-Care Activities (SDSCA) questionnaire. There were 70 subjects between 35-73 years, 40 women and 30 men. The data were analysed using the Somers’ D test to determine the correlation. Results: From 70 subjects, the spirituality level was 68.6% nirvana, guide 14.3%, sorrow 12.9% and revive 4.3%. The acceptance score obtained was 82.0% positive and 17.1% negative. The obedient score obtained was 72.9% positive and 27.9% negative. The level of patient adherence was very high at 44.2%, high at 12.9%, moderate at 20%, low at 10%, and very low at 12.9%. There was no correlation between acceptance scores and patient adherence level (r = 0.185, p-value = 0.299), on the contrary, there was a correlation between obedient scores and patient adherence levels (r = 0.34, p-value = 0.021). There is a significant correlation between the total spirituality score and patient adherence (r = 0.262, p-value = 0.035). Conclusion: There is a correlation between spirituality and type 2 DM patients’ adherence.

Abstract

Background: Diabetes mellitus still becomes a significant population health problem. In achieving sufficient awareness, face-to-face public education methods may serve as an effective measure. However, in the amid of pandemic, their implementation is restricted, which leads to the use of virtual platforms for learning activities. In addition, the applicability of the online learning method has yet to be explored.Objective: To evaluate its impact on the knowledge, attitude, and lifestyle practices of the productive-age population related to diabetes.Methods: A pre-test and post-test quasi-experimental study involving a single group was conducted among the productive-age population (aged 15 to 64 years) selected using consecutive sampling in two sub-districts in Surabaya, Indonesia. Baseline and endline levels of participants’ knowledge, attitude, and practice regarding diabetes assessed using a self-developed questionnaire were compared after receiving an educational intervention.Results: A total of 19 participants meeting the eligibility criteria were selected as the study subjects. A statistically significant increase in the participants’ knowledge score from pre- to post-test (p=0.038) was observed. There was no significant change between the pre-test and post-test scores of attitude (p=0.938) and practice (p=0.087) after the intervention.Conclusion: Online learning method improved the population’s knowledge regarding diabetes mellitus. However, it appears to have a limited role in providing better modification for attitude and practice. Further research comparing online and traditional face-to-face education methods are required to establish this approach as an alternative.

Abstract

Background: Semarang is one of the cities in Central Java with the highest positive number and mortality rate of Covid-19. It became the largest contributor to daily cases in Central Java. In general, the severity of Covid-19 in Semarang is 5.3% and increased to 24% in the elderly. Mortality Covid-19 was caused by various factors such as demographic and non-demographic factors.Objective: This study aims to determine the impact of several risk factors of COVID-19 mortality in Semarang.Methods: This is an analytical cross-sectional study. Data analysis was carried out analytically with a binary logistic regression test. The sample used was the total population of 32.555 confirmed positive from March 2020 to March 2021 registered in Semarang Health Office.Results: The results demonstrated that mortality due to Covid-19 in Semarang was frequent in the elderly (27.3%), males (9.4%), had comorbidities such as diabetes mellitus (28.6%), kidney disease (43.5%), neuromuscular (36.4%), and cancer (29.3%), and accompanied by symptoms such as Dyspnea (18.1%), weakness/lethargy (19.9%), and fever (14.6%).Conclusion: Age, gender, travel history, comorbidities (kidney, neuromuscular, cancer, and diabetes mellitus), and symptoms (fever, shortness of breath, and weakness/lethargy) are risk factors for mortality Covid-19 in Semarang.

Abstract

Hypokalemia is a common electrolyte abnormality often encountered in daily practice. While mild hypokalemia often asymptomatic, moderate-to-severe hypokalemia usually manifested as significant symptoms such as muscle weakness. In this paper, we presented a case of 23-years-old male coming to our emergency department because of muscle weakness in all his four limbs when he woke up in the morning. Physical examinations were all within normal limit, except that his muscle strength decreased in all four extremities. Laboratory measures showed severe hypokalemia (1.9 mmol/L), hence patient was treated with potassium infusions. During hospitalization, thyroid function tests revealed hyperthyroidism. Therefore, in patient with paralysis, in which laboratory finding showed hypokalemia, hyperthyroidism should be considered as one of the potential diagnosis especially in younger patient, even in patient without previous history of hyperthyroidism. Merely treating hypokalemia in patient with underlying hyperthyroidism could be a dangerous boomerang.

Abstract

Primary hepatic leiomyosarcoma (PHL) is a very rare primary liver tumour. These tumours usually arise from intrahepatic vascular structures, gallbladder, or ligamentum teres. The pathogenesis of this disease is still unknown. We report a 53-year-old man with complaints of intermittent right upper abdominal pain for one month, decreased appetite, nausea, weight loss, and tea-coloured urination. The patient denied any complaints of defecation pattern. On physical examination, jaundice was present in both eyes and the whole-body skin, tenderness in the right hypochondriacal region, and enlarged liver with a lumpy surface. The laboratory examination demonstrated increased transaminase enzymes and bilirubin, while hepatitis B and C were negative. The patient underwent several tests for tumour markers, such as CA 19-9 50 (<37 U/mL), AFP 1.23 (<5.81 IU/mL), and CEA 0.83 (<3 ng/mL). Ultrasound demonstrated an enlarged liver, multiple liver nodules, and cholelithiasis. Meanwhile, the abdomen’s computerised tomography (CT) scan shows a solid heterogeneity image with an irregular border in segments 1 and 4b and a tumoral thrombus in the inferior vena cava. The diagnosis was confirmed by biopsy and immunohistochemistry (IHC); vimentin and smooth muscle actin (SMA) results were positive, while CD 34, CD 117, and cytokeratin were negative. Our patient was diagnosed with PHL stage IVa (T4N2M0). PHL is a particularly rare tumour with a poor prognosis. The patient died after one month of diagnosis. Diagnosis of PHL is challenging. It was based on clinical features, physical examinations, laboratory examinations, and other supporting investigations.

Abstract

Background: Haemorrhagic stroke causes high disability and mortality. Moreover, the prevalence is increasing. There is evidence of the involvement of the inflammatory process in haemorrhagic strokes. The neutrophil-lymphocyte ratio (NLR) is a strong and easily assessed marker of inflammation. Objective: This study aims to determine the NLR during hospital admission as a predictor of neurological deterioration in acute haemorrhagic strokes.Methods: This study was a prospective cohort study. The subjects were acute haemorrhagic stroke patients treated at the Stroke Unit of the RSUP Dr. Sardjito between March and November 2018. The demographic and laboratory test data, including NLR and National Institutes of Health Stroke Scale (NIHSS) measurement upon admission, were investigated. Logistic regression was performed to identify independent risk factors of neurological deterioration.Results: A total of 65 haemorrhagic stroke patients were included in this study. There were 21 subjects experiencing neurological deterioration, and 44 subjects did not. In bivariate analysis, there was a significant association between hyperglycaemia and neurological deterioration (RR=3.073; 95%CI=1.772-5.329; p=0.011). Glasgow Coma Scale (GCS) at admission was significantly associated with neurological deterioration (RR=2.732; 95%CI=1.455-5.131; p=0.006) as well as the NLR (RR=3.750; 95%CI=1.229-9.4441; p=0.005). The logistic regression results demonstrated that the variables that independently influenced neurological deterioration were NLR (RR= 4.424; 95%CI=1.196-16.369; p=0.026) and GCS (RR=7.461; 95%CI=1.711-32.526; p=0.007).Conclusion: High NLR can predict deterioration in acute haemorrhagic stroke.

Abstract

Background: Worldwide, cervical cancer is the second highest cause of death after breast cancer. In Indonesia, cervical cancer is ranked second among all types of cancer. Psychological distress negatively affects the mortality rate of cancer patients, and there is no stress management treatment of standard integrative psychotherapy to be carried out in the process of individual therapy in cervical cancer.Objective: To treat psychological distress in cases of cervical cancer using an integrative psychotherapy approach, create a model of factors that influence psychopathology related to psychological distress, and an integrative psychotherapy model as the basis for making modules and standard operation procedures of stress management in cervical cancer patients.Methods: This is an integrated case study with descriptive, exploratory, and exploratory qualitative approaches in two women with cervical cancer who were given integrative psychotherapy (integrative psychoeducation combined with integrative guided imagery and music), data analysis using the constant comparative method. Presentation of data using tables and graphs.Results: Two models were illustrated, a model of factors that influence psychopathology related to psychological distress and an integrative psychotherapy model in stress management of cervical cancer patients. Evaluation of decreased distress and improved related physical symptoms (pain and sleep quality).Conclusion: A model of factors that influence psychopathology related to psychological distress and an integrative psychotherapy model that can be implemented in stress management of cervical cancer patients were obtained, with sessions and types tailored to the patient’s needs.

Abstract

Background: Colorectal cancer (CRC) is a malignant epithelial tumour in the colon and rectum, and its main cause is distant metastasis. The main process of distant metastasis is the epithelial-mesenchymal transition (EMT) that may cause mesenchymal phenotype and malignant capabilities of tumour cells. Then it is regulated by one of transcription factors, the Snail.Objective: This study aims to determine differences between Snail gene expression in CRC that has not metastasized and Snail gene expression in CRC that has metastasized. Methods: The subjects of this study consisted of two groups of patients. The first group consisted of 20 patients with CRC (tumour), and the second group consisted of 12 patients with CRC with metastasis (metastasis tumour). The Snail gene expression was measured in both groups by using the RT-qPCR method and supported by laboratory examination to measure ALT and AST. Then all obtained data were analysed by using a statistical software analysis.Results: This study indicated that there were differences of the Snail gene expression between both the groups, and the highest expression was in the metastatic tumour group with a significance level of 0.002 (p<0.05). This was also supported by AST and ALT which were significantly correlated between both the groups. The correlation value on the AST was 0.506 with a significance level of 0.003, and the ALT correlation value was 0.532 with a significance level of 0.002. Conclusion: This study suggested that the Snail was a potential marker of metastasis in the CRC. Therefore, further research is needed to determine the role of Snail regulation in the CRC metastasis.
