Abstract

Background: HIV/AIDS is a global health problem, including in Indonesia. PLWHA's access to health services has not been evenly distributed. Only about 63% of PLWHA received initial antiretroviral therapy, 18% died, and 25.4% experienced drug withdrawal. Therefore, research is needed to determine the factors influencing the access of PLWHA to health services.Objective: This study aims to determine the factors related to access to health facilities in Indonesia for people living with HIV/AIDS (PLWHA) in Indonesia.Methods: This study uses secondary data from the 2017 IDHS. The case group was PLWHA, who had not visited a healthcare provider in the last six months. The control group was PLWHA patients who visited a healthcare provider during the last six months. The variables were the patient's and their partner's education level, types of residence, occupation, health insurance, type of health facility, and access to health facilities in the last six months. Multivariate analysis was performed by logistic regression.Results:The PLWHA access to health services was not significantly related to education level, spouse's education level, type of residence, occupation, and type of health insurance (p-value >0.05). The access is significantly related to the health facility types (OR= 4.04; 95% CI 1.173-13.955).Conclusion: The factor determining access to health services for PLWHA in Indonesia is the type of health facility. The government must make various efforts to improve the quality of facilities and services in all health facilities types to increase the number of PLWHA who undergo tests and therapy.

Abstract

Background: Coronavirus disease 2019 (COVID-19) pandemic causes individuals at risk of anxiety and sleep disorders. Anxiety can cause a person’s sleep quality to be poor.Objective: To investigate the association between anxiety and sleep quality among students in the Faculty of Medicine and Health Sciences UNIKA Atma Jaya during the COVID-19 pandemic.Methods: This cross-sectional study involved 635 preclinical and clinical students at FKIK UNIKA Atma Jaya. Measuring instruments used were the Generalised Anxiety Disorder 7 (GAD-7) and the Pittsburgh Sleep Quality Index (PSQI). Both instruments were distributed using Google Forms. Data analysis was performed descriptively and bivariate (Mann Whitney U Test).Results: There were 62% of respondents with anxiety; among them, 37% were mild anxiety; 16.9% were moderate anxious; 8.2% were severe anxiety. This study finds that 66.28% of women with anxiety and 62.8% with poor sleep quality. In 2020, 73.81% of preclinical respondents experienced anxiety and 69.84% experienced poor sleep quality. The bivariate analysis demonstrated a significant association between anxiety and sleep quality among preclinical and clinical students at FKIK UNIKA Atma Jaya during the COVID-19 pandemic (p<0.05).Conclusion: There is an association between anxiety and sleep quality in preclinical and clinical students in FKIK UNIKA Atma Jaya During the COVID-19 Pandemic.

Abstract

Background: Stunting is one of the global problems that can increase morbidity, mortality, poor development, and infections in children. Some risk factors that escalate stunting incidence are pregnant women, insufficient breastmilk, infection, and the environment.Objective: This study aims to determine the association between pregnancy history, exclusive breastfeeding, and immunisation status with stunting incidence.Methods: This observational study employed a case-control design. The independent variables were pregnancy history, exclusive breastfeeding, and immunisation status. Meanwhile, the dependent variable was stunting incidence. The study subjects were 40 respondents, consisting of 20 respondents diagnosed with stunting and 20 respondents of control.Results: This study revealed an association between a history of low birth weight (p=0.028), too early provision of complementary food in addition to breast milk (p=0.011), no antenatal care (ANC) examination (p=0.000), and exclusive breastfeeding (p=0.028). However, these factors are not significantly different from the immunisation status (p=0.548).Conclusion: This study concluded an association between pregnancy history and exclusive breastfeeding with stunting.

Abstract

Background: Age-related Macular Degeneration (AMD) is a degenerative disease caused by multiple factors, including inflammation. This condition may lead to irreversible macular damage. Vitamin D has anti-inflammatory and immune-modulating qualities, therefore, is expected to protect against the development of AMD.Objective: This study aimed to evaluate the correlation between plasma 25-hydroxyvitamin D (plasma 25-OHD) levels and neovascular Age-Related Macular Degeneration (nAMD).Methods: This was a case-control study involving AMD and control subjects. All subjects underwent a standardized eye examination to check for eligibility by a retinal specialist. Blood samples were drawn for analysis using a DRG Elisa Kit to determine the plasma 25-OHD (Total). Plasma 25-OHD levels were categorized into deficiency (<12 ng/mL), insufficiency (12 - <20 ng/mL), adequate (20 – 50 ng/mL), and high (>50 ng/mL).Results: Total subjects were 39, which consisted of 20 AMD subjects and 19 controls. There were 17 male subjects (43.6%) and 22 female subjects (56.4%). The mean age for the case and control group was 69.35±7.04 and 68.26±6.83, respectively (p=0.612). Those in their 70s dominated the age distribution with a percentage of 56.41% of all participants. The mean plasma 25-OHD in the case group was 32.30±17.10 ng/mL, while the control group was 50.63±11.95 ng/mL (p=0.066). There was no significant association between AMD and plasma 25-OHD levels in all groups (p>0.05).Conclusion: Plasma 25-OHD levels did not correlate positively with nAMD in our study population.

Abstract

Congenital heart disease (CHD) and invasive cardiac procedures are risk factors for infective endocarditis (IE) in children. IE with Achromobacter xylosoxidans is a rare case and has a high mortality rate. This case report and literature review aim to understand the risk factors, diagnosis, therapy, and outcome of IE wirh Achromobacter xylosoxidans. We observed a 14-months-old boy with ventricular septal defect (VSD), a history of cardiac catheterization, 9 days fever and febrile convulsion; and tachycardia and pansystolic murmur on the tricuspid valve were found. Transthoracic echocardiography revealed vegetation on the tricuspid valve. Monomicrobial clinical isolates of Achromobacter xylosoxidans were consistently found in three samples of aerobic blood culture bottles. The patient was treated with definitive antibiotic therapy with 200 mg/8 hours of ceftazidime for 21 days. Bacterial growth was not found in the blood culture evaluation which was performed after 48 hours of the therapy. Also, it was found that in the follow up period the patient had no fever on the sixth day, and the vegetation size was reduced. A patient with CHD, especially with VSD as an underlying condition who had a history of cardiac catheterization and accompanied by fever and heart murmur, should be suspected of having IE. Then the definitive antibiotic therapy with ceftazidime contributed to a better clinical outcome.

Abstract

Background: One indication of microvascular dysfunction in diabetes mellitus (DM) is decreased nitric oxide (NO) levels. Tithonia diversifolia leaves (TDL) extract has been scientifically demonstrated to decrease glucose levels in diabetic rats.Objective: This study aims to examine the effect of TDL extract on NO levels in diabetic rats.Methods: True experimental with pre-post test control group design used 25 Sprague-Dawley rats. Its were divided into 5 groups: healthy control (K.1); diabetic control (K.2); diabetes rats with ethanol extract of TDL at a dose of 25 (K.3); 50 (K.4); and 100 mg/kg BW (K.5) for 28 days. We use a combination of nicotinamide (230 mg/kg BW) and streptozotocin (65 mg/kg BW) to induce diabetes mellitus. The blood serum was taken before and after extract administration. NO level was assessed using spectrophotometry. Paired T-test, Wilcoxon, one-way ANOVA, and post hoc LSD were performed for statistical analysis.Results: There were significant differences in NO levels before and after treatment in all groups unless K.5 (100 mg/kg BW). Furthermore, there was no significant difference in NO levels in the healthy control and the K.5 group but significant differences in other groups.Conclusion: TDL extract can prevent the decrease in NO level in diabetic rats model with the effective dose of 100 mg/kg BW.

Abstract

Cutaneous larva migrans (CLM) is a skin disorder in the form of serpiginous spreading with erythematous and papule as a skin reaction to the invasion of hookworm. Apart from the skin infection symptoms, helminthic infections tend to most frequently involve the respiratory, gastrointestinal, neurological, and eye. We report the case of a 21-year-old male who came with erythematous papules and linear tracks in the entire body that felt itchy, especially on the abdomen and at the back. On the physical examination, there were erythematous papules, erythematous tracks in serpiginous patterns, excoriations, and erosions. The patient was diagnosed with extensive CLM. The patient was treated with oral Albendazole 400 mg once daily for 3 days, oral Cetirizine 10 mg once daily to reduce the itch, and oral Cefixime 200 mg twice daily for 3 days. At the end of the follow-up, all the lesions had resolved, and no other symptoms appeared. Extensive cases of CLM are noteworthy for the presence of Loeffler syndrome and superinfection. In remote areas, sometimes, the availability of drugs is limited. Therapy using oral Mebendazole only gives 70% resolution of the skin lesion compared to Albendazole. Combination therapy of Albendazole, antihistamines, and antibiotics gave satisfactory results.

Abstract

Puberty is a series of physical development and sexual maturation that aims to achieved reproductive capacity. The hypothalamic-pituitary-gonadal (HPG) axis has an important role in the normal process of puberty. Disruption in the HPG axis causes hormonal disturbances, one of which is hypogonadotropic hypogonadism, which is associated with delayed puberty. Hormonal disorders can also cause various problems besides the reproductive system, such as muscle weakness, lack of energy, and an increased risk of bone fractures. We report the case of a male patient who complained of predominant muscle weakness, did not show signs and symptoms of puberty, and had an idiopathic hypogonadotropic hypogonadism who unresponsive to human chorionic gonadotropin (hCG) therapy. We decided to administered Testosterone Undecanoate (TU) therapy to overcome the problem of delayed puberty and complaints of muscle weakness experienced by the patient. TU therapy has shown beneficial effects in these patients by increasing testosterone levels, increasing penis length, and eliminating complaints of muscle weakness.

Abstract

Background: Measuring service quality is an effort to assess and evaluate service performance. Currently, the measurement of clinical forensic service quality has never been done. There is no instrument for measuring service quality for clinical forensic patients. Objectives: This study aims to collect valid evidence of a modified questionnaire to measure clinical forensic service quality. Methods: This study protocol followed three phases from the World Health Organization guidelines on translating and adapting instruments for substance abuse. The study was conducted at Bhayangkara Hospital Pekanbaru, with ten respondents in phase pretesting and thirty in phase evaluation of the internal structure. The questionnaire developed was from the original version of SERVQUAL, consisting of 5 dimensions (reliability, responsiveness, assurance, empathy, and tangible) and 22 items with a five Likert scale. Content validity, response process, and internal structure are applied to collect valid evidence.Results: A modified SERVQUAL questionnaire was obtained for clinical forensic patients with 22 items through an expert panel and respondent interview. The majority had high validity (item to total correlation coefficient r > 0.6). This final version showed excellent internal consistency (Cronbach alpha 0.956). Conclusion: The final version of the modified SERVQUAL for clinical forensic service quality questionnaire has sufficient validity evidence for its content, response process, and some part of its internal structure, namely item to total correlation and reliability.
