Abstract

Tujuan: untuk mengetahui ekspresi caspase 3 dan p53 mutan dalam memprediksi respons operabilitas (pra-operatif) pada penderita kanker serviks IIB yang mendapatkan kemoterapi neoajuvan paclitaxel-platinum.Metode: ekspresi caspase 3 dan p53 mutan diperiksa dengan pengecatan immunohistokimia pada 40 potongan biopsi jaringan pasien kanker serviks IIB yang memenuhi kriteria inklusi, yang melakukan pengobatan di RSUD dr. Soetomo, Departemen Obstetri dan Ginekologi, Fakultas Kedokteran Universitas Airlangga, Surabaya mulai bulan Desember 2006 hingga Juli 2011. Kriteria inklusi meliputi penderita kanker serviks IIB yang mendapatkan rejimen kemoterapi neoajuvan paclitaxel-platinum (cisplatin 75 mg/m2 atau carboplatin AUC 5 – paclitaxel 175 mg/m2) sebanyak tiga seri berturut-turut 3 mingguan dan telah dilakukan evaluasi operabilitasnya pasca-kemoterapi neoajuvan yang dilakukan sebelum operasi, 3 minggu sampai 4 minggu setelah kemoterapi yang terakhir. Penelitian ini merupakan studi analitik dengan studi kasus kontrol.Hasil: hubungan antara ekspresi p53 mutan dan caspase 3 dengan operabilitas post kemoterapi neoajuvanpaclitaxel-platinum dianalisis dengan uji regresi logistik sederhana. Dari penelitian ini didapatkan sejumlah 22 (55%) pasien kanker serviks IIB yang mendapat kemoterapi neoajuvan memberikan respons inoperable dan 18 (45%) pasien memberikan respons operabel. Ekspresi p53 mutan terbanyak menunjukkan hasil ekspresi negatif, dengan rentang skor 0-1, yaitu sebanyak 33 (82,5%) penderita. Ekspresi positif didapatkan pada 7 (17,5 %) penderita, dan tidak didapatkan perbedaan yang signifikan (p 0,48; OR: 0,55 (CI 95%:0,11-2,87)). Jumlah ekspresi caspase 3 yang positif dan negatif menunjukkan jumlah yang sama besarnya baik pada luaran inoperabel maupun operabel, yaitu masing-masing 13 (72,2%) dan 11 (50,0%); dengan p=0,158, OR:2,60(CI 95%:0,69-9,81).Kesimpulan: ekspresi caspase 3 dan p53 mutan tidak dapat memprediksi respons operabilitas pada pasien kanker serviks IIB yang mendapatkan kemoterapi neoajuvan paclitaxel-platinum.Kata kunci: kanker serviks stadium IIB, kemoterapi neoajuvan paclitaxel-platinum, operabilitas pascakemoterapi neoajuvan

Abstract

Background: Worldwide, cervical cancer is the second highest cause of death after breast cancer. In Indonesia, cervical cancer is ranked second among all types of cancer. Psychological distress negatively affects the mortality rate of cancer patients, and there is no stress management treatment of standard integrative psychotherapy to be carried out in the process of individual therapy in cervical cancer.Objective: To treat psychological distress in cases of cervical cancer using an integrative psychotherapy approach, create a model of factors that influence psychopathology related to psychological distress, and an integrative psychotherapy model as the basis for making modules and standard operation procedures of stress management in cervical cancer patients.Methods: This is an integrated case study with descriptive, exploratory, and exploratory qualitative approaches in two women with cervical cancer who were given integrative psychotherapy (integrative psychoeducation combined with integrative guided imagery and music), data analysis using the constant comparative method. Presentation of data using tables and graphs.Results: Two models were illustrated, a model of factors that influence psychopathology related to psychological distress and an integrative psychotherapy model in stress management of cervical cancer patients. Evaluation of decreased distress and improved related physical symptoms (pain and sleep quality).Conclusion: A model of factors that influence psychopathology related to psychological distress and an integrative psychotherapy model that can be implemented in stress management of cervical cancer patients were obtained, with sessions and types tailored to the patient’s needs.

Abstract

Background: The development of screening methods for infertility patients during the Coronavirus-2019 (COVID-19) pandemic provides new challenges in the adaptation of children's want programs in the era of the COVID-19pandemic. Several screening methods have been developed but various factors affect the awareness of couples to want to do COVID-19 screening. This study aimed to analyze biopsychosocial factors associated with awareness of COVID-19 screening in infertile couples. Subjects and Method: A cross-sectional study conducted at The Sekar Polyclinic of Dr. Moewardi Regional General Hospital during the COVID-19 pandemic. A number of 60 infertile couple of reproductive age who underwent the ART program was selected by random sampling. The dependent variable was awareness for COVID-19 screening. The independent variables were anxiety, knowledge, age, and length of infertility. The data were obtained from medical record and questionnaire and analyzed by a multiple logistic regression. Results: Anxiety (OR= 26; 95% CI; p<0.001) and above-average knowledge levels (OR=16; 95% CI; p<0.001) affected awareness for COVID-19 screening. While the age and length of infertility do not affect awareness for COVID-19 screening (p >0.05). Conclusion: Anxiety and above-average knowledge levels affect awareness for COVID-19 screening.

Abstract

HIGHLIGHTS There were no significant differences in TMSC, sperm motility, sperm morphology, and DFI in sperm preparation using the swim-up method at 27°C and 37°C. However, this study provided an overview of the average improvement of DFI at 27°C compared to 37°C. There was no significant difference in the pregnancy rate of IUI in sperm preparation using the swim-up method at 27°C and 37°C.   ABSTRACT Objective: This study aimed to determine the effect of temperature during sperm preparation on total sperm motile count (TMSC), sperm motility, sperm morphology, DNA fragmentation index (DFI), and pregnancy rate. Materials and Methods: A quasi-experimental laboratory study with pre- and post-test control group was conducted at Sekar Fertility Clinic, Dr. Moewardi General Hospital, Surakarta, Indonesia. A total of 20 sperm samples from infertile patients were prepared using the swim-up method at 27°C (group 1) and 37°C (group 2). TMSC, motility, morphology, and DFI examinations were performed. In addition, IUI was performed to confirm pregnancy rate. Sperm DNA fragmentation was determined using Sperm Chromatin Dispersion/SpermFunc DNAf test. Sperm DNA fragmentation was characterized by a halo <30% of the volume of the sperm head. Results: Group 1 had mean TMSC of 13.77 ± 9.30, while group 2 had 14.82 ± 8.82; p=0.218. Group 1 had a motility value 82.25+12.77 and group 2 had 82.55 ± 11.69; p=0.968. The morphological value for group 1 was 11.25 ± 5.15 and group 2 was 11.6 ± 5.34; p=0.626. The mean DFI for group 1 was 17.79 ± 10.88 and group 2 was 18.18 ± 12.95; p=0.765. Pregnancy rate in group 1 was 10% and group 2 was 20%; p=1.000. Conclusion: There were no significant differences in TMSC, sperm motility, sperm morphology, DFI, and pregnancy rate in sperm preparation using the swim-up method at 27°C and 37°C.

Abstract

Kolaborasi antara tenaga medis dan masyarakat memiliki peran penting dalam mendukung penanganan infertilitas, sebuah kondisi yang sering menyebabkan kecemasan, stres, dan dampak negatif lainnya pada kesehatan mental. Tenaga medis berperan dalam diagnosis, perencanaan pengobatan, dan mendukung pasien mencapai tujuan mereka untuk memiliki anak, sementara masyarakat dapat memberikan dukungan emosional yang membantu mengurangi isolasi dan depresi pasien. Kegiatan pengabdian ini berfokus pada edukasi dan pemberdayaan tenaga medis serta masyarakat untuk meningkatkan pemahaman dan kapasitas mereka dalam mendukung individu dengan infertilitas. Evaluasi menggunakan pre-test dan post-test menunjukkan bahwa 25% peserta mengalami peningkatan pengetahuan setelah kegiatan, sementara 72% telah memiliki pemahaman yang memadai sebelumnya. Hasil ini menggambarkan efektivitas program dalam meningkatkan pemahaman terkait infertilitas. Selain itu, kegiatan ini berhasil menyebarkan informasi akurat dan memerangi mitos yang beredar di komunitas, sehingga meningkatkan kesadaran dan mengurangi stigma terhadap individu dengan infertilitas. Kesimpulannya, program ini efektif dalam meningkatkan dukungan sosial dan kapasitas tenaga medis untuk memberikan perawatan holistik yang lebih baik bagi pasien infertilitas, serta berkontribusi pada pengurangan dampak psikologis dan sosial yang dialami pasien.

Abstract

Objective : This study aims to explore the relationship between micturition disorders and adenomyosis and dysmenorrhea and provide insight into the factors associated with the severity of LUTS symptoms in these conditions. Methods : This study was a descriptive qualitative study. We retrospectively collected medical record data from the Obstetrics Gynecology Outpatient Ward of Dr. Moewardi Surakarta General Hospital in the period January 2021 to May 2023. The results of this study analyzed using Pearson correlation analysis if normally distributed and using Mann-Whitney if not normally distributed, thus, statistically analyzed. Results: There is a significant relationship between adenomyosis and dysmenorrhea on urinary disorders (p value of 0.016) where urinary disorders are found to be more severe in adenomyosis patients with dysmenorrhea. Conclusion : Lower urinary tract symptoms are common in symptomatic adenomyosis patients and greatly affect the patient's quality of life. Dysmenorrhea can be a potential risk factor that increases the occurrence of moderate to severe LUTS. Keywords: adenomyosis; dysmenorrhea; urinary disturbance.

Abstract

Background: Assisted reproductive technology (ART) programs have proven to be a solution to help infertility couples to conceive. However, most couples face a financial crisis related to the relatively expensive cost of ART treatment. This service aims to educate about ART's meaning, direction, and procedures. Thus, couples want children to be able to understand the ART program at an affordable cost (low-cost) and undergo the ART program without any concerns about costs.Method: Seminar participants be given socialization in person and online through zoom software by the service provider. It is in two stages, in-person and online, so participants who cannot attend in person can follow this service program. In addition, participants are given a pre-test and post-test questions to assess their understanding of the ART program at an affordable cost.Result: The pre-test and post-test results were analyzed using Wilcoxon, which showed that after providing education about the ART program, the post-test results were significantly better than the pre-test results. Down value: 5.5%, fixed value: 15.2%, value rise: 79.3% with significance p<0.001.Conclusion: There are significant differences in the pre-test and post-test scores, which means that the participants understood the explanation the servicer gave.

Abstract

Background: The assisted reproductive technology (ART) program for examining infertile couples requires face-to-face with a doctor. This face-to-face examination for the ART program has a risk for the transmission of COVID-19 to all medical personnel and infertile couples who want to undergo the ART program. This study aims to understand pre- and post-test education about COVID-19 screening on ART services.Methods: A total of 50 infertile couples in the ART program are given education online. The use of online methods using the Zoom application due to the COVID-19 pandemic. Evaluation of the level of understanding using pre and post-test providing education. Data analysis was carried out using the SPSS 23 application.Results: Analysis of the level of understanding of pre-test and post-test education using the Wilcoxon test. Pre-test results were 69.90±12.05, post-test 73.00±10.83 with significance values p<0.001. Chi-Square test analysis results obtained p<0.001 and OR = 15.68.Conclusion: There was a significant increase in understanding of COVID-19 screening in the post-test education ART service via the Zoom application compared to pre-test education.
