Abstract

The risk factors of medically unassisted baby delivery in Banjarnegara DistrictPurpose: This study aims to identify risk factors of medically unassisted baby delivery in Banjarnegara Regency. Method: Mix method research design (case-control completed with in-depth interview). The sample size was 112 with a ratio of 1: 1 (56 cases and 56 controls), taken by simple random sampling based on secondary data PWS-KIA Banjarnegara District Health Office in 2018 including a qualitative sample of 5 people taken purposively. Data collection was carried out by means of interviews using questionnaires and interview guides. Analysis of bivariable data with McNemar test and multivariable with multiple logistic regression using the STATA 13.1 program. Results: Thirty-four (60.7%) respondents who gave birth without the help of health workers had a low educational level. The result of multivariable analysis showed a significant relationship between maternal education (aOR = 4.06; 95% CI = 1.32-12.45), history of ANC (aOR = 4.26; 95% CI = 1.24-14.64 ) and family support (aOR = 7.16; 95% CI = 1.73-29.51). Qualitative results show that prior delivery experience was the reason why women chose to give birth at home without the help of health workers. Conclusion: Childbirth without the help of health workers in Banjarnegara District was influenced by maternal education, ANC history, and family support. It is expected that families can be involved in providing Information and Education Communication (IEC) during ANC visits of pregnant women to increase awareness and knowledge related to safe pregnancy and childbirth.

Abstract

Scoping review hepatitis a outbreaks in Indonesia: 1998-2018Purpose: The purpose of this study was to determine the distribution of cases and risk factors of outbreaks of hepatitis A in Indonesia during 1998-2018.  Method: Scoping review conducted on electronic data from journal articles as well as published and unpublished reports (grey literature) from 1998-2018. Online databases such as Google Scholar and Pubmed. Grey literature is obtained from the library and from the unpublished report document results obtained from the University of Indonesia (UI) and University of Gadjah Mada (UGM). In the process of select and data extraction aided by 2 reviewers. Results: Outbreaks of hepatitis A were reported to occur in 47 incidents during 1998-2018 with the outbreak area mostly occurring in East Java, West Java and Central Java. Men are more at risk (63.4%) of hepatitis A than women and 15-34 years is the age most at risk of hepatitis A. The hepatitis A outbreak area is the most at school / campus with proportion (41.5%), cottage boarding schools (31.7%) and in residential or community environments (24.4%) The risk factors that most contribute to outbreaks of hepatitis A at school/campus and in the community are the habit of not washing hands with soap before eating and consuming foods that are not hygienic. While in boarding schools is the use of shared eating/drinking tools. Conclusion: School age and young adults are the age groups most at risk for hepatitis A. Unhygienic individual behavior (not washing hands before eating) is a major contributing factor to the transmission of hepatitis A.

Abstract

Breeding sites and larvae density with malaria cases in PT. Freeport Indonesia, mimika regency in 2013-2017: spatio-temporal approachPurpose: This study aims to determine the relationship between breeding sites and larval density in malaria cases in the Lowland region of PT. Freeport Indonesia Mimika Regency 2013-2017. Method: This study uses secondary data from PT. Freeport Indonesia in 2013 - 2017. The data used in this study included malaria case data, breeding sites and breeding sites and larval density in the lowland PTFI area from 2013-2017. Data analysis included descriptive analysis to see the distribution picture, bivariate analysis to examine the relationship between variables and spatial analysis to find out the distribution of cases and breeding sites.Results: The number of malaria cases from 2013-2017 in the Lowland PTFI region was 267 (37%) from a total population of 713 people. The highest case occurred in 2014 as many as 70 cases. Distribution of cases based on sex occurs mostly in men, in the age group of 46-50 years, based on the relatively equal employment status between employees and non-employees and based on the location, many occur in Kuala Kencana. Types of breeding sites with larvae density have a significant relationship (p <0.0001), larvae density with malaria cases also have a significant relationship (p 0.0013 and r 0.40) especially density in xpool which has a density of 2.01/dip.Conclusion: Distribution of malaria cases focused on locations where employees live and locations with high mobility. The types of breeding sites are related to larval density. The highest density of larvae is found in breeding sites of the xpool type. Larval density and malaria cases have a significant relationship with larvae density in the xpool breeding site.

Abstract

Maternal perceptions of MR vaccination in work area of Puskesmas Beji: explanations of reasons for and againts Purpose: Data from the Ministry of Health shows that as of October 2018, the coverage of MR vaccination nationally still has not reached the target. Depok is one of the cities that has not reached the target. This is due to the rejection of the child's parents. Therefore, the purpose of this study was to find out how the mother's perception and find out the reasons for acceptance and rejection of MR vaccination. Method: This research is a qualitative study with 23 informants consisting of mothers of children under five (mothers agree and disagree), cadres, midwives, and heads of Puskesmas uses Focus Group Discusion and In-depth Interview. Results: The results of this study found that for knowledge, mothers who agreed were mostly know about MR vaccination, while most mothers who did not agree admitted that they did not know. Whereas for perceptions, most of the mothers who disagree have a negative perception of MR vaccination, while most mothers who agree have positive perceptions of MR vaccination. There are also various reasons why mothers accept or refuse MR vaccination. Conclusion: Mother's perception of MR vaccination is one of the causes of rejection, resulting in MR vaccination coverage in Depok not yet reaching the national target.

Abstract

Latar Belakang : Gestasional Diabetes Melitus (GDM) adalah gangguan toleransi glukosa yang pertama kali ditemukan pada saat kehamilan. Sebuah tinjauan sistematis menyarankan bahwa wanita dengan GDM memiliki peningkatan 7,4 kali lipat dalam risiko terkena diabetes tipe 2 setelah melahirkan dibandingkan dengan mereka yang tidak memiliki GDM.Tujuan : Mengetahui pengalaman hidup ibu postpartum dengan riwayat Gestational Diabetes Mellitus (GDM) di kota Yogyakata 2019.Metode : Penelitian ini menggunakan metode kualitatif dengan pendekatan deskriptif fenomenologi. Teknik pengambilan sampel yang digunakan adalah purposive sampling dengan criterion sampling dengan alasan karena subjek penelitian dipilih dengan kriteria tertentu yang sesuai dengan tujuan penelitian. Dalam penelitian ini melibatkan 5 ibu dengan riwayat Gestational Diabetes Mellitus (GDM). Pengambilan data dilakukan pada bulan Mei-Juli 2019 di 3 Puskesmas kota Yogyakarta yaitu Puskesmas Umbulharjo 1, Puskesmas Danurejan 1 dan Puskesmas Kotagede 1. Data penelitian dianalisis menggunakan Colaizzi.Hasil : hasil analisis didapatkan 4 tema yaitu pengalaman ibu dengan Gestational Diabetes Mellitus, pengetahuan tentang Gestational Diabetes Mellitus, kehamilan dengan Gestational Diabetes Mellitus dan postpartum dengan riwayat Gestational Diabetes Mellitus.Simpulan : Dari hasil penelitian yang dilakukan ditemukan bahwa pengetahuan ibu mengenai GDM masih kurang, kurangnya kesadaran ibu untuk melakukan kontrol gula darah masa nifas, kurangnya informasi yang diterima dari bidan, aktifitas fisik yang dilakukan hanya berupa olahraga ringan, KIE gizi hanya diperoleh dari ahli gizi, peran dukungan keluarga sangat membantu ibu dalam proses mobilisasi.

Abstract

Experiences related to management competency of doctors in disadvantaged areas in Indonesia: a qualitative studyPurpose: Working in disadvantaged areas is challenging. Geographical barriers, lack of health facilities and accessibility has been impacted to health services delivery which is provided by doctors. This study explored the experiences related to management competency when doctors on duty in disadvantaged areas. Method: We conducted a qualitative study with phenomenological approach in Aceh Singkil Regency (an area in Province of Aceh which is still categorized as disadvantaged areas regarding to Presidential Regulation Number 131 year of 2015). Twelve doctors who work in three different characteristic areas (archipelago, watershed and land areas) underwent in-depth and structured interview. Doctors were recruited purposively. Data were analyzed using thematic analysis. Results: This study reveals that experiences related to management competency is induced by three situations, namely: a) Cultural diversity in community; b) Dealing with difficult situations; and c) Need to survive in difficult areas. Conclusion: Challenges and obstacles which is experienced by doctors when working in disadvantaged areas generate valuable experiences which related to management competencies, as communication skills, cultural competence, creativity, and leadership skills.
