Abstract

Banten Province which stood by the passing of Law No. 32 on October 4, 2000. Ahead of the 13th anniversary of politics in Banten province was shaken by the arrest of Tubagus Chaeri Wardana (TCW) who is also the younger brother of Ratu Atut Chosiyah by the Corruption Eradication Commission (KPK) in a hand-catching operation which then detained Ratu Atut Chosiyah in the bribery case Pemilukada Lebak Regency 2013. The implementation of legislative elections will be the first test of the shock experienced by the Family Politics Clan Ratu Atut Chosiyah. This research uses quantitative approach method. Hypothesis in this research is with the shocks on Family Clan Ratu Atut Chosiyah then the level of election of family member Atut Chosiyah in legislative election year 2014 become low. Allegations of political observers about the fall of the RAC family's political clan after the arrest of RAC and TCW by the KPK proved to have no realities. Legislative Election 2014 is true to prove to the audience that the Atut family political clan is not exhausted and still stands firmly. The people still trust their political rights to be channeled by members of the RAC Family Clans. Thus members of RAC family already have the stability and political existence in the eyes of the people of Banten. The RAC Political Clan termed by many observers as the Political Dynasty has turned out to be a self-repairing system even though some elements have problems. Have a good regeneration system where other family members can continue their political clan in Banten. Conclusions regarding the election level of members of the RAC core family clan in 2014 Legislative Election in their respective districts (1) H. Andika Hazrumy has a very high degree of election, (2) Hj. Ade Rossi Khoerunnisa has a high degree of election, (3) Andiara Aprilia Hikmat has a very high degree of election, (4) Tanto Warsono Arban has a very high degree of election, and (5) H. Aden Abdul Kholiq has a very low level of election. Therefore, all stakeholders of democracy in Banten Province should be more participative and more concerned about politics, and mobilize elements of democracy such as political parties, interest groups, academics, press, NGOs to become watchdogs of democracy in Banten, so that Atut Chosiyah political clan re-entrenched strongly in Banten does not violate the norms and rules of law applicable in Indonesia.

Abstract

Introduction: Dengue Hemorrhagic Fever (DHF) causes 40% of world population at risk for infection, and 50 million people get infection with as many as 24,000 die every year. Incidence rate in Indonesia, particularly in Maritengngae Subdistrict, Sidenreng Rappang District, is increasing and needed to be controlled. Disease distribution has not been documented regionally. The dynamics of the incidence,that comprises the characteristics of the patient, environmental conditions, susceptibility of mosquitoes resulted from insecticide use, transovarial infectivity, and the incidence pattern were unknown.Objectives: To identify the dynamics of DHF incidence with spatio-temporal approach, includes gender, age, the habit of cleaning the water containers, house density, the density of water containers within the radius of 100 meters from the house of the patient, resistance of the mosquito, and transovarial transmission of Dengue virus.Methods: Subjects were DHF patients in Maritengngae Subdistrict. It was an epidemiological observational study with cross-sectional design and spatio-temporal approach to variables of the study, which included the patient characteristics (age, gender, habit of cleaning the water containers), environmental conditions (house density, water container density within 100 meter radius around the patient’s house), and vector conditions (resistance status to organophosphate insecticide, and transovarial transmission of Dengue virus).Results: Cases of DHF in Maritenggae Sub district, Sidenreng Rappang District, South Sulawesi decreased from 95 cases in 2008 to 38 cases in 2009, except in Kelurahan Majeling. Incidence distribution of DHF in 2008-2009 based on gender were not different (p=1.000), patients with age <15 years old were different from age ≥15 years (P=0.016 and p = 0.013), house density and density of water containers around the patient’s house were not different (p=0.829 and p = 0.538). The habit of cleaning water containers at the house was 43.84%. There were 1.94% of Aedes aegypti samples showed medium resistance (tolerance) status against organophosphat insecticide and 33.33% of samples showed transovarial transmission of Dengue virus. The highest of transovarial transmission occured in Kelurahan Majeling with transovarial transmission index of 100%.Conclusion: The transmission dynamic of DHF in Maritengngae sub district is not influenced by gender, house densities, water container densities, and the frequency of source reduction, but influenced by ages. Most of Ae. aegypti mosquitoes are still susceptible against organophosphate insecticides. The highest of transovarial transmission of dengue virus in Ae. aegypti in Kelurahan Majeling with Transovarial transmission index of 100% may contribute in the increasing of DHF cases in the Kelurahan Majeling in 2009.Keywords: dynamics, dengue, spatio-temporal, Ae. aegypti, Maritengngae.

Abstract

Introduction : Malaria is a public health problem that still causes mortality, particularly in high risk population. Kabupaten Nias is one of the malaria endemic areas. Malaria diagnosis is mainly determined according to physical examination, despite the fact that laboratory examination is the gold standard of malaria diagnosis. To help health workers in diagnosing malaria accurately, it is necessary to develop a decision support system for malaria diagnosis.Objectives: To develop a prototype of malaria diagnostic decision support system.Methods: It was a descriptive study with action research design to explore each phase in the development of a prototype of malaria diagnostic decision support system. Participants of the study consisted of 5 general practitioners in RSU Gunungsitoli and 2 nurses in Puskesmas Gunungsitoli.Results: The study created an application model of computer-based malaria diagnostic decision support system designed using PHP programming language and MySQL database. This system worked by entering malaria clinical symptoms into the expert system, and data of symptoms were processed by the expert system to determine diagnosis and medical advice that was useful to assist health staff in making decision.Conclusion: Malaria diagnostic decision support system that had been developed attracted the interest of health workers and help them in diagnosing malaria clinically.Keywords: diagnostic decision support system, prototype, malaria, computer application, Gunungsitoli

Abstract

Health care-associated infections (HAI) are the most frequent adverse event in health-care delivery worldwide, including in Asian countries. Asian countries produced 4,819 HAI publications. Majority of the publications (68%) appeared in the last decade. The average annual growth rate was 14.8%. Japan was the most productive country that contributes about 13.5% of total publications, followed by China (11.7%), and Taiwan (11.6%). Asian HAI research were cited in 76,737 articles with an average of 15.9 citations per publication. The top two most highly cited articles were published in the New England Journal of Medicine followed by the Journal of Clinical Microbiology. The top three productive journals were Journal of Hospital Infection (309 papers; 6.4%), American Journal of Infection Control (198 papers; 4.1%) and Infection Control and Hospital Epidemiology (170 papers; 3.5%). The ten top journals contained 25% of the HAI literatures. Emerging research topics in the last years include ventilator-associated pneumonia, MRSA (methicillin-resistant staphylococcus aureus), acinetobacter baumannii, carbapenems, Republic Korea and tertiary care center.

Abstract

Introduction: The ‘Cempaka' Tropical Cyclone hit south Java in November 2017, causing heavy rainfall and severe flooding. Changes in climate variability and extreme weather events may shift the geographic and seasonal patterns of neglected tropical diseases such as leptospirosis. This study analyses the Spatiotemporal pattern between flooding, weather, and human leptospirosis cases after a tropical cyclone. Methods: This was an ecological study that collected monthly flooding data, weather data, and human leptospirosis data cases per village from November 2017 to October 2018, a year after the ‘Cempaka' Tropical Cyclone in Bantul. Spatio-temporal analyses were calculated to 0-3 months lag by Pearson's correlation, spatial mapping, and time-series graphs. Results and Discussion: As many as 99 people infected with leptospirosis were found in all 75 villages in Bantul. The villages affected by flooding were 44% and leptospirosis cases spread in 68% of villages. A 1-month lag (r = 0.6849; p < 0.05) and 3-months lag (r = 0.6666; p < 0.05) of relative humidity along with a 1-month lag (r = 0.7451; p < 0.05) and 3-months lag (r = 0.8561; p < 0.05) of rainfall were found to be correlated to human leptospirosis cases. Heavy rain due to the Cempaka Tropical Cyclone caused flooding and transmission of Leptospira bacteria into the water and heightened contact between humans, animals, and the environment. Conclusion: It is found that flooding, relative humidity, and rainfall after the Cempaka Tropical Cyclone would be followed by human leptospirosis outbreaks 1 and 3 months later. Cross-sectoral cooperation of public health authorities should integrate climatic information as an early warning for disaster-prone areas and community groups at risk.

Abstract

Malaria is an infectious disease in tropical countries including Indonesia which causes serious clinical manifestations and even death. Indonesia Ministry of Health targets that malaria is eliminated from this country by 2030. In Java, malaria is targeted to be eliminated by 2015. However, elimination has not been achieved because some areas, including Kulon Progo, Daerah Istimewa Yogyakarta often experiences outbreaks (Kejadian Luar Biasa) in recent years. As microscopic diagnosis is a gold standard diagnosis for malaria, thus accurate microscopic examination is important. Disappearance of malaria cases in malaria pre-elimination areas causes laboratory workers/microscopists are less exposed to experience in identifying malaria. As part of quality assurance, continuous quality control is expected to maintain the accuracy of malaria diagnosis in endemic areas. However, the cross-check sistem as a part of this quality control strategy still has constraints, such as: transportation costs, limited human resources and irregularity. Ibm team of Faculty of Medicine UGM has successfully developed a digital microscopic cross-check sistem. This method is able to bridge the distance and time constraint also transportation costs of slide submissions to be cross-checked to the referral laboratory. Our study partners were microscopists of Kulon Progo Primary Health Centers (Puskesmas) facilitated with digital devices installed in a microscope. Partners were trained to: 1. take microscopic images of suspected malaria parasites and 2. familiarize with digital cross-check sistem. The successful implementation of digital malaria cross-check is expected to improve the accuracy of malaria diagnosis and assist the Government in malaria elimination program especially in Kulon Progo. This system has been registered for Intellectual Property Right no. 091306 obtained from Department of Justice and Human Right of Republic of Indonesia.

Abstract

Blockchain first introduced and implemented in digital currency management and transactions. Its application to medical records data management is a novelty. This paper described the implementation of blockchain technology in the healthcare industry, especially in medical records data management A literature review was conducted on three popular databases, ScienceDirect, SpringerLink, and IEEE Xplore with the keywords "health", "medical record" and "blockchain" with "research article" and "conference proceeding" filters. There are a few articles that meet the criteria to review indicated that the implementation of blockchain technology in medical records data management is a novelty and still in the early phase. Blockchain is a potential technology in supporting the implementation of electronic medical records, especially related to data integration and privacy. Several scientific publications related to the implementation of blockchain for medical records data management shown that the implementation of this technology will make the patient have full control over their health data. Yet there are still many challenges in the implementation both from the user side and the technology infrastructure.

Abstract

ABSTRACT Background: Electrocardiogram (ECG) has become a crucial examination in the management of cardiac emergencies. Accordingly, improvement of ECG interpretation skills is mandatory for general practitioners as the front-liners in emergency cases. The Mobile ECG application was developed as mobile learning media to facilitate continuing improvement of ECG interpretation skills.Aims: This study aimed to investigate the impact of the Mobile ECG application toward ECG interpretation skills of general practitioners and medical students and evaluate its usability.Methods: A pilot quasi-experimental study was conducted in a 1-week timeframe using webinar and the Mobile ECG application. Subjects were recruited through consecutive sampling. They met the following criteria: 1) registered as general practitioners or medical students, 2) completed the basic ECG pre and post-tests, and 3) agreed to participate in the study. The Mobile ECG is a web-based application which consists of modules, quizzes, and gallery of ECG interpretations. Pre and post-test analysis and system usability scale (SUS) questionnaire were used to evaluate the impact and usability of the application.Results: A total of 252 subjects were recruited and 80.2% were general practitioners. There was a significant increase in post-test scores compared to pre-test (p=0.000) for all subjects. General practitioners significantly gained more score increment than medical students (1.08 vs 0.16, p=0.001). Based on the SUS score of 67.5, the application was marginally accepted by the users.Conclusion: To conclude, the implementation of the Mobile ECG application did improve basic ECG interpretation skills. According to the SUS score, this application still needs improvement.

Abstract

ABSTRACTBackground: The rapid development of science and technology in the globalization era, particularly information technology enables data or information to be processed quickly and accurately. Nutrition service in the form of food provision in hospitals requires precision and accuracy  as well as speed in its process. Therefore the utilization of science and technology especially computer-based information system will be of much help. Nutrition installation of Malang Military Hospital still uses manual method in calculating the need for food stock so that it needs more time and nutrition staff to do the work.    Method: The study was a quasi experimental study (before and after) without control group to fi nd out the difference in duration of food stock need calculation before and after the implementation of computer-based information system in the food provision at Nutrition Installation of Malang Military Hospital.Result: The implementation of computer-based information system at nutrition installation improved nutrition staff and time effi ciency in the calculation of food stock needs. Only one nutrition staff was needed to operate computer, whereas in the manual calculation method two nutrition staff were needed to do the work. The time needed to calculate food stock before the implementation of computer-based information system in average was 3.5 hours and after the implementation was 1.6 hours, based on statistical test at signifi cance level 95% p<α(0.05) whereby 0.003<0.05. This meant there was difference in duration of time needed before and after the implementation of computer-based information system in food provision at Nutrition Installation of Malang Military Hospital.Conclusion: The development of computer-based information system improved nutrition staff and time effi ciency in the calculation of food stock need in food provision at Nutrition Installation of Malang Military Hospital.KEYWORDS: information system, effi ciency, food stock calculation, nutrition installation, food provisionABSTRAKLatar Belakang: Perkembangan iptek pada era globalisasi yang begitu pesat, terutama teknologi informasi akan memungkinkan proses data atau informasi dengan cepat dan akurat. Pelayanan gizi, dalam hal penyelenggaraan makanan rumah sakit dalam prosesnya memerlukan ketelitian dan ketepatan serta kecepatan waktu yang digunakan. Untuk itu, pemanfaatan ilmu pengetahuan dan teknolgi dalam hal ini sistem informasi berbasis komputer akan sangat membantu. Instalasi Gizi Rumah Sakit Militer Malang (RSMM), dalam kegiatan perhitungan kebutuhan bahan makanan masih menggunakan penghitungan secara manual sehingga memerlukan waktu dan tenaga kerja gizi yang lebih untuk menyelesaikannya.Tujuan: Mengembangkan sistem informasi berbasis komputer untuk meningkatkan efi siensi waktu dan tenaga kerja gizi dalam perhitungan kebutuhan bahan makanan pada penyelenggaraan makanan di Instalasi Gizi RSMM.Metode: Penelitian ini merupakan penelitian quasi experimental (before-after) tanpa kelompok kontrol untuk mengetahui perbedaan durasi waktu yang digunakan dalam perhitungan kebutuhan bahan makanan sebelum dan sesudah menerapkan sistem informasi berbasis komputer pada penyelenggaraan makanan di Instalasi Gizi RSMM.Hasil: Penelitian ini menunjukkan bahwa dengan penerapan sistem informasi berbasis komputer pada instalasi gizi akan meningkatkan efi siensi tenaga kerja gizi dan waktu dalam perhitungan kebutuhan bahan makanan. Satu tenaga kerja gizi cukup digunakan untuk operasional computer yangsebelumnya 2 tenaga kerja gizi untuk perhitungan secara manual. Waktu yang diperlukan untuk perhitungan bahan makanan sebelum dan sesudah menerapkan sistem informasi berbasis komputer berbeda nyata dengan rata-rata adalah 3,5 jam dan 1,6 jam(p<0,05).Kesimpulan: Pengembangan sistem informasi berbasis komputer akan meningkatkan efi siensi tenaga kerja gizi dan waktu dalam perhitungan kebutuhan bahan makanan pada penyelenggaraan makanan di Instalasi Gizi RSMM.KATA KUNCI: sistem Informasi, komputer, efi siensi tenaga kerja gizi dan waktu, perhitungan kebutuhan bahan makanan

Abstract

Background: Low compliance as one cause of high prevalence of anemia in Indonesia is mainly due to forgetfulness. Nutrition education and short message service (SMS) are expected to become media to send nutrition message and remind pregnant mothers of taking iron tablet.Objective: To identify impact of nutrition education and nutrition message through SMS to knowledge, behavior and compliance of pregnant mothers with taking iron tablet.Method: The study was a quasi experiment that used pre and post test control/non equivalent control group design involving 3 groups of experiment, i.e. nutrition education only, nutrition education and SMS, and control as comparator. Subject of the study consisted of 121 pregnant mothers. The study was carried out from November 2009 to April 2010 at Palangka Raya Municipality in Pahandut, Kayon and Tangkiling Health Centers. Statistical analysis was performed by using paired t-test, ANOVA, Tukey test, and multivariate test.Results: Pregnant mothers that got nutrition education and SMS intervention significantly improved their knowledge and behavior (p<0.05). Pregnant mothers that got nutrition education and SMS had knowledge 18 point higher (coef=18.26), behavior 4 point better (coef=4.07) and probability 4 times more compliant than the control group (OR=3.79; p=0.03). Nutrition education and SMS intervention could increase compliance and significantly affect increase of hemoglobin (Hb) level up to 1.3g/dl (p<0.05). Increase of Hb level went in line with increase of compliance (dose-response) in pregnant mothers with preliminary Hb level <11g/dl. Meanwhile in pregnant mothers with preliminary Hb level ≥11g/dl, increase of Hb level was statistically significant when compliance reached ≥90%. Conclusion: Pregnant mothers that got nutrition education and SMS had better knowledge, behavior and compliance with taking iron tablet than those that only got nutrition education and the control group. High compliance could significantly increase Hb level.
