Abstract

Implementasi Rekam Medis Elektronik (RME) merupakan salah satu upaya transformasi digital pelayanan kesehatan yang bertujuan meningkatkan kualitas pelayanan, efisiensi kerja, dan pengelolaan informasi kesehatan. Keberhasilan implementasi RME dipengaruhi oleh tingkat penerimaan pengguna terhadap sistem yang digunakan. Technology Acceptance Model (TAM) merupakan salah satu metode yang banyak digunakan untuk mengukur penerimaan teknologi. Penelitian ini bertujuan untuk mengevaluasi implementasi Sistem Informasi Rekam Medis Elektronik di fasilitas pelayanan kesehatan berdasarkan pendekatan TAM. Metode penelitian yang digunakan adalah literature review sistematis dengan pendekatan PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses). Pencarian artikel dilakukan melalui database Google Scholar dan PubMed menggunakan kata kunci yang berkaitan dengan Rekam Medis Elektronik, Technology Acceptance Model, dan fasilitas pelayanan kesehatan. Hasil pencarian awal memperoleh 1.675 artikel, kemudian dilakukan proses identifikasi, skrining, dan seleksi berdasarkan kriteria inklusi dan eksklusi sehingga diperoleh 19 artikel untuk dianalisis. Hasil kajian menunjukkan bahwa Perceived Ease of Use dan Perceived Usefulness berpengaruh positif terhadap Attitude Toward Using dan Behavioral Intention to Use sistem RME. Mayoritas pengguna menilai RME mampu meningkatkan efisiensi pelayanan, mempercepat akses informasi pasien, serta mendukung ketepatan dokumentasi dan pengambilan keputusan klinis. Selain itu, faktor pelatihan, kualitas sistem, infrastruktur, dan dukungan organisasi turut memengaruhi keberhasilan implementasi RME. Berdasarkan hasil penelitian, dapat disimpulkan bahwa implementasi RME secara umum telah diterima dengan baik oleh pengguna. Namun, peningkatan kualitas sistem, pelatihan berkelanjutan, dan dukungan manajemen masih diperlukan untuk mengoptimalkan pemanfaatan sistem di fasilitas pelayanan kesehatan.

Abstract

Pneumonia is one of the leading causes of death in the world, including Indonesia. This case is also included in the top 10 inpatient diseases at Kaliwates General Hospital, Jember Regency, and the number continues to increase from 2018 to 2022. The purpose of this study is to predict the need for hospital beds in pneumonia patients and reduce the number of hospitalized patients. This study is a quantitative descriptive study that studies the daily population survey of inpatients at Kaliwates General Hospital. The data collection technique used is documentation. The results of the study show that the days of pneumonia case care every year from 2018-2022 tend to increase and decrease repeatedly. The results of the calculation of the estimated value from actual or predicted data show that the fitted value follows the actual data pattern. In addition, the addition of beds is quite a lot needed for pneumonia cases. The suggestion that can be proposed by the researcher is that Kaliwates General Hospital is expected to be able to analyze patient care days by predicting care days in order to find out the need for beds as a reference in making decisions regarding determining bed capacity. The hospital should also plan bed needs by relocating beds from several inpatient rooms with low disease accuracy.

Abstract

The process of borrowing and returning medical record documents at the Dental and Oral Hospital of the University of Jember is currently carried out manually using an expedition book. The absence of a tracer as a tool to track medical records makes it difficult for staff to locate borrowed documents, leading to misplaced documents in the filing racks. There are issues with late returns from clinics, which do not comply with the standard operating procedures for medical record document storage at the Dental and Oral Hospital of the University of Jember, and there is no record of these delays, resulting in a lack of problem documentation. The researcher aims to design and develop a web-based medical record borrowing and returning information system at the Dental and Oral Hospital of the University of Jember. The researcher used the waterfall development method, with data collection techniques including interviews, observations, documentation, and brainstorming. The results of the study show that the developed information system has several advantages, such as medical record barcode printing features, medical record tracers, and WhatsApp notifications as reminders for overdue medical record returns.

Abstract

Citra Husada Jember Hospital has been running a hospital management information system since 2019 with the help of a third-party vendor. However, during the preliminary study, several problems were still found in the use of SIMRS, including information content that was less concise, a display that was too complicated, and errors during data processing. The purpose of this study was to evaluate hospital management information systems. Researchers used quantitative research with a sample of 109 respondents. The data analysis used is relationship analysis with the spearman rank test using the help of the SPSS program. The results showed the significance value of all variables < (α = 0.05) and was positive. The correlation coefficient value of the content variable, format variable, accuracy variable is in the medium category, ease of use variable, timeliness variable is in the strong category. All variables in the EUCS method have a significant relationship to user satisfaction. The results of this study are expected that the hospital can improve the content features of SIMRS in the future to make it easier for users.

Abstract

Hospitals need to provide services that prioritize safety, health, comfort and convenience. Hospital staff reported that many elderly patients do not bring complete personal data and are often confused by the information in the application, especially regarding SPM, BPJS, and payment rates, even though the information is available. A usability test was conducted on the PUSPA application at Moh. Anwar Hospital involving 7 respondents using the System Usability Scale (SUS) method. The test results showed a usability score of 48.21, which is in the poor category. This study aims to redesign the user interface of the PUSPA application with a user-centered design approach. The method used is a combination of qualitative and quantitative approaches through interviews and questionnaires to collect data. The redesigned interface was evaluated using the SUS and UEQ tests. The SUS score increased to 70.71, indicating a good category. The UEQ test results for the new interface showed a "good" rating for the clarity and efficiency variables, and "above average" for attractiveness, reliability, stimulation, and novelty, so that application design updates need to be implemented immediately.

Abstract

Angina pectoris is left chest pain that occurs due to impaired blood flow to the heart, preventing the heart muscle from receiving adequate oxygen. This case ranked third in the top 10 morbidity report at Jember Clinic Plantation Hospital in 2022. The number of morbidity and mortality events of angina pectoris continued to increase from 2020 to 2022. This study aimed to analyze the relationship between diabetes mellitus with angina pectoris based on medical records of inpatients at the Jember Clinic Plantation Hospital. This type of research is quantitative research with a case control study design. The study sample consisted of 164 case and 164 control samples selected using simple random sampling. The results showed that there was a significant association of diabetes mellitus (p value = 0,001) with angina pectoris. The suggestion of this study is that people with diabetes mellitus check their blood sugar regularly to prevent angina pectoris.

Abstract

Claims are requests for payment of healthcare service costs by healthcare facilities to BPJS Kesehatan. Hospitals must complete the required documents according to BPJS verification procedures to avoid pending claims. The issue of pending claims still occurs at RS Tk.III Baladhika Husada Jember, with a pending rate of 11.4% from July to December 2023. This study aims to analyze the factors causing pending BPJS Kesehatan inpatient claims at RS Tk.III Baladhika Husada Jember using Robbins’ Performance Theory. This qualitative study collected data through interviews, observations, documentation, and brainstorming. The study found that pending BPJS Kesehatan inpatient claims are caused by motivation factors (lack of a reward system), opportunity factors (absence of job descriptions, insufficient staff for casemix and TPPRI), and ability factors (staff with less than 5 years of experience, lack of knowledge about BPJS inpatient claims). The study concludes that the causes of pending BPJS Kesehatan inpatient claims are motivation, ability, and opportunity factors. Proposed improvements include creating job descriptions, regularly maintaining internet networks, recruiting new staff with medical record qualifications, and providing training to enhance staff knowledge.

Abstract

Penerapan Rekam Medis Elektronik (RME) untuk meningkatkan efisiensi, mutu, dan keselamatan pelayanan pasien. Penelitian ini bertujuan menganalisis peran RME sebagai pendukung manajemen pelayanan pasien di fasilitas pelayanan kesehatan serta mengidentifikasi faktor pendukung dan penghambat implementasinya. Metode penelitian ini merupakan literature review dengan pendekatan sistematis berdasarkan pedoman Preferred Reporting Items for Systematic Reviews and Meta-Analyses. Sebanyak 16 artikel nasional dan internasional yang memenuhi kriteria inklusi dianalisis. Hasil penelitian ini RME terbukti meningkatkan efisiensi alur pelayanan, mempercepat akses dan keakuratan data pasien, mendukung pengambilan keputusan klinis dan manajerial, serta meningkatkan keselamatan pasien melalui dokumentasi yang terstruktur dan terintegrasi. Faktor pendukung meliputi komitmen manajemen, ketersediaan infrastruktur teknologi, integrasi sistem antar unit pelayanan, pelatihan sumber daya manusia, serta adanya standar operasional prosedur. Faktor penghambat meliputi keterbatasan kompetensi dan jumlah SDM, gangguan jaringan dan sistem, resistensi terhadap perubahan, peningkatan beban kerja pada tahap awal implementasi, serta belum optimalnya interoperabilitas sistem. Implementasi RME berhubungan erat dengan peningkatan efektivitas manajemen pelayanan pasien, namun optimalisasinya memerlukan penguatan kapasitas SDM, stabilitas infrastruktur, dan tata kelola sistem yang berkelanjutan.

Abstract

The number of inpatients at Citra Husada Hospital with type 2 DM continues to increase every year until 2023, there were 734 patients. The purpose of this study was to examine how gender, a history of obesity, and a history of hypertension relate to risk factors for type 2 diabetes. Based on medical record data from inpatients at Citra Husada Hospital in 2023, this study sought to examine the association between the occurrence of type 2 DM and risk variables for the disease, including gender, history of obesity, and history of hypertension. This kind of study used secondary data in analytic observational case control methodology. Using basic random sampling, 133 case samples and 133 control samples made up the study sample. descriptive analysis for univariate data analysis and the chi-square test for bivariate data analysis. The results showed variables associated with Type 2 DM, namely a history of obesity (p = 0.000) with a risk of 3.297 times developing Type 2 DM and a history of hypertension (p = 0.020) with a risk of 1.836 times developing Type 2 DM and unrelated variables, namely Gender (p = 0.376).

Abstract

The standard time for provided outpatient medical records was ≤ 10 minutes. Based on the results of preliminary study, based on 99 files, 90 medical records (90.9%) experienced delays in provision. The purpose of this study was to analyze the factors caused the delay in provided outpatient medical records at Doctor Mohamad Saleh Hospital, Probolinggo City used the performance theory from Robbins. Type of research used qualitative, data collection techniques were interviews, observations, documentation and brainstorming. The research subjects consisted of 1 head of medical records, 7 outpatient medical record storage officers, 1 medical record distribution officer, 6 outpatient registration officers and 3 nurses. The results of the study were based on ability factors were the lack of knowledge of officers about the procedure for providing outpatient medical records, officers didn’t have medical record education qualifications, officers never participated in medical record management training, and work experience less than 3 years. Based on the opportunity factor were errors in SIMRS, insufficient shelves and storage space for medical records and there was no SOP for provision of outpatient medical records. Meanwhile, the opportunity factor is in the sub-variables of SIMRS, shelves and storage rooms for medical records and SOP. The improvement plan are apply for training, replace officers according qualifications, switch to using RME, improve facilities and infrastructure, and make SOP.
