Abstract

Background: Early Postoperative Small Bowel Obstruction (EPSBO) is a primary factor contributing to death and morbidity after pelvic and abdominal surgery. Although it is frequently regarded as a surgical emergency, the most recent research indicates that non-operative management is always preferable unless there are complications.Case: A 53-year-old woman came with symptoms of small bowel obstruction and a history of hysterectomy three weeks earlier. The patient refused surgical intervention, so a conservative approach was taken. She was given methylprednisolone 500 mg/day in two doses daily for three consecutive days, in addition to nasogastric tube insertion and symptomatic therapy. After three days of follow-up, the patient showed symptoms improvement, and the abdominal X-Ray and CT Scan showed no abnormal findings.Discussion: The majority of Early Postoperative Small Bowel Obstruction (EPSBO) cases, according to the most recent research, may be managed without surgery. Additionally, due to their anti-inflammatory properties, some medications, like corticosteroids, have been found to be beneficial and effective for EPSBO. This is because they may help resolve the obstruction and lessen the edema and fibrin deposition associated with EPSBO following abdominal surgery.Conclusion: Conservative therapy with steroids appears promising due to its lower risk profile and greater patient comfort. However, the exact mechanism and its efficacy in treating EPSBO have yet to be fully concluded, so further research is still needed.

Abstract

Globally, the elderly population are more increasing each year. The enhancement of life expectancy is also followed by the enhancement of chronic illness, which one of them is liver disease. In elderly, there are also several physiological and biochemical changes in liver. Several studies show that the reduction of liver function will affect the severity of liver clinical manifestation in older people. This review article aims to discuss more about liver disease in older population. Hepatitis A in elderly has higher mortality and morbidity rates compared to young people. More over, the progressivity of acute hepatitis B to chronic hepatitis B is also greater in older people than young people. The treatments of hepatitis B and hepatitis C are safe and effective to be applied in elderly. Polypharmacy and fraility affects the elderly to be more susceptible to drug induced liver injury (DILI). This review aims to address knowledge gaps in understanding the morphological and functional changes in the aging hepatic system, their implications for disease progression, and the effectiveness of current therapeutic strategies. By critically analyzing recent evidence, we identify challenges in treating liver diseases in the elderly and highlight areas requiring further research.

Abstract

Treatments for Crohn's Disease (CD) and Ulcerative Colitis (UC), two conditions that fall under the umbrella of Inflammatory Bowel Disease (IBD), must be both safe and effective. This review compares the effectiveness and safety of anti-TNF agents and IL-12/23 inhibitors, specifically in the context of IBD management. Randomized controlled trials (RCTs), observational studies, and meta-analyses involving adult IBD patients were all included in the review. Anti-TNF agents (infliximab and adalimumab) were shown to significantly improve clinical remission rates and reduce complications, particularly with early intervention. However, these agents were associated with adverse effects such as infections and autoimmune reactions. IL-12/23 inhibitors, particularly risankizumab, showed superior efficacy in achieving and maintaining remission with a better safety profile. The findings prove that IL-12/23 inhibitors might be more effective for patients who are unresponsive to anti-TNF therapies. In conclusion, both anti-TNF agents and IL-12/23 inhibitors effectively manage IBD, with IL-12/23 inhibitors offering a favorable safety profile. Early intervention and personalized treatment strategies are significant for optimizing patient outcomes. It will take additional research to validate these results and improve treatment strategies.

Abstract

Ulcerative Colitis (UC) is a chronic idiopathic inflammation that appears from the rectum and can extend to colon causing diffuse friability and superficiality. In this case report, Woman, 44 years old, admitted to hospital because of bloody diarrhea, frequency of 4-6 times daily with mucous, malaise, and history of weight loss. Patient already had oral sulfasalazine and oral corticosteroid to maintain her condition, but she stopped the medication at will. On physical examination, showed anemic conjunctiva and increasing in bowel sound, fecal examination showed brown to red, porridge-like consistency, blood, mucous, and positive Benzidine test. Colonoscopy examination revealed, normal anus; in the sigmoid rectum, colon descendent found small swellings, an ulcer, and few bloody. On histopathology, epithelial surface with mucin depletiondistortion of crypt architecture, dilatation and elongation of crypt, as well as shortening of the crypt that does not reach muscular mucous and lamina propria contains solid aggregate inflammation cells, dominated by plasma cells. The patient was given  combination of 5-aminosalicylates and glucocorticoids. On re-evaluation in week 8 of therapy, frequency of defecation becomes 1-2 times daily with no blood occurring and improvement on colonoscopy, rectal mucous, colon sigmoid improved, no bleeding and clean mucous.  5-aminosalicylates is the first treatment in UC,  it also helps to achieve remission and prevent carcinoma development. UC is a disease that also causes extra-intestinal manifestation, so the treatment should be done holistically. Compliance and regular monitoring will help to improve the quality of life.

Abstract

Background: The American College of Gastroenterology (ACG) currently recommends the Truelove and Witts scoring system for evaluating Ulcerative Colitis (UC) disease activity. Inflammatory markers such as Matrix Metallopeptidase 9 (MMP-9), High-sensitivity C-Reactive Protein (HsCRP), and Erythrocyte Sedimentation Rate (ESR) have previously been shown to correlate with intestinal inflammation. To evaluate the association of MMP-9, HsCRP, and LED with disease activity and treatment duration in UC according to the Truelove and Witts criteria.Methods: An observational analytic study with a cross-sectional design was conducted involving subjects with UC diagnosed pathologically and receiving 5-ASA therapy at the Gastroenterohepatology Outpatient Department of Dr. Soetomo Hospital. MMP-9, HsCRP, and ESR serum levels were measured. The disease activity was determined based on the Truelove and Witts criteria. Analysis was done by spearman's correlation. P-values and correlation coefficients (r) were presented.Results: Among the 31 subjects, the majority were female (67.7%) with an average age of 46.84 years. Approximately 35.5% subjects had been treated for more than 6 months. The MMP-9 and HsCRP serum levels were above normal range (71% and 93.5% subjects, respectively). The majority (74.2%) of the subjects had normal ESR levels. Among the investigated inflammatory markers, only ESR levels significantly had a very weak positive correlation (p=0.010; r=0.004).Conclusion: ESR serves as an effective biomarker for UC disease activity compared to MMP-9 and HsCRP. Further study is warranted to explore the role of these biomarkers in UC management.

Abstract

Background: Peutz-Jeghers Syndrome (PJS) is a rare hereditary polyposis syndrome that is autosomal dominant and has the main characteristics of hamartoma polyps, mucocutaneous pigmentation, and increased susceptibility to malignancy. Case: This case report presented a 19-year-old man referred from a secondary hospital with complaints of diarrhea for one month before hospital administration. The symptoms happened five times a day with residue in the stool. Mucus or bleeding was not found. Around ten days prior, the polyp came out from the anus, but the patient could pull it back by himself. Diarrhea had been recurring since 2014. The patient then underwent polypectomy (while still children) and until now has undergone polypectomy 11 times. Abdominal CT scan showed multiple polyps with varying morphology in the luminal mucosa of the duodenum, jejunum, ileum, descending colon, sigmoid colon, and rectum, in accordance with the presentation of Peutz-Jeghers syndrome. The colonoscopy examination found a Peutz-Jeghers appearance after a polypectomy with a hemoclip installed. The patient was diagnosed with Peutz-Jeghers Syndrome post polypectomy. The patient received a plan for post-operative complete blood count examinations and periodic colonoscopies. Conclusion: PJS is an important diagnosis to consider in young patient with polyposis, even without mucocutaneous pigmentation.

Abstract

Epiploic appendages are fat-filled outpouchings arising from the serosal surface of the colon and have uncertain physiological functions. Epiploic appendagitis is characterized by inflammation of these appendages. Primary appendagitis often results from ischemic injury, while secondary appendagitis may arise from the contiguous spread of inflammation or infection from adjacent structures. Clinical features of epiploic appendagitis include acute or subacute abdominal pain, most commonly in the lower quadrants, without significant concurrent constitutional or other gastrointestinal symptoms. Diagnosis relies on imaging modalities, with Computed Tomography (CT) being the preferred method. Management of epiploic appendagitis is mainly conservative, with analgesic and anti-inflammatory agents being the mainstay of treatment. Surgical indications are poorly defined, but laparoscopic excision of the inflamed appendage may be considered in cases of recurrent or refractory symptoms. This review article explores the anatomy, pathophysiology, clinical features, diagnosis, and management of epiploic appendages and appendagitis.

Abstract

Background: Functional dyspepsia is characterized by upper gastrointestinal symptoms like epigastric bloating, pain, and early satiation without structural disease evidence, including negative findings on endoscopy. Psychological triggers, such as stress and anxiety, may influence its occurrence. Previous research in Indonesia, including studies at Tarumanegara University, Mulawarman University, and the Islamic University of North Sumatra, has found associations between psychological factors and functional dyspepsia symptoms. Most respondents experienced dyspepsia linked to stress and anxiety. A study on patients at Dr. M. Jamil Hospital also confirmed this relationship. Building on the data mentioned, this study aims to explore and confirm the relationship between stress, anxiety, and the occurrence of functional dyspepsia in medical students at Sumatera Utara University.Methods: This research utilized a cross-sectional analytical approach with a sample of 261 medical students from Sumatera Utara University, selected via quota sampling, and applied the Rome IV and DASS-21 criteria questionnaires. Data were collected from September 2022 until October 2022.  Results: This study showed a 47.9% prevalence of functional dyspepsia among respondents, with 44.1% experiencing moderate stress and 72.4% having very severe anxiety. The Chi-square test revealed significant relationships between stress (p=0.026) and anxiety (p=0.038) with functional dyspepsia incidence. Spearman correlation tests further confirmed significant relationships between stress levels (p=0.049) and anxiety levels (p=0.030) and functional dyspepsia. Conclusion: There is a relationship between stress and anxiety with the incidence of functional dyspepsia in medical students of Sumatera Utara University.

Abstract

Background: The EVendo score and Baveno VII criteria are non-invasive methods in predicting the presence of esophageal varices in patients with cirrhosis. The EVendo score is more practical to use in daily practice than the Baveno VII criteria because it does not require a liver stiffness measurement. This study aims to compare the performance of the EVendo score with the Baveno VII criteria for predicting the presence of esophageal varices in patients with liver cirrhosis.Methods: This study was diagnostic study with cross-sectional approach on cirrhosis patients who had previously undergone upper gastrointestinal endoscopy who had laboratory examination data and liver stiffness measurement values. Based on the existing data, calculation and comparison of the diagnostic value of the EVendo score and the Baveno VII criteria were carried out. The research was conducted at dr. Hasan Sadikin based on patient data from April 2017 to May 2023.    Results: The Evendo score had a sensitivity of 80.5%, specificity 50%, PPV 84.6%, NPV 42.9%, positive likelihood ratio 1.61, and negative likelihood ratio 0.39. The Baveno VII criteria had a sensitivity of 95.1%, specificity of 25%, PPV 81.2%, NPV 42.9%, positive likelihood ratio 1.27, and negative likelihood ratio 0.2.Conclusion: The results of this study show that the EVendo score and Baveno VII criteria have high sensitiviy but low specificity for predicting esophageal varices in patients with liver cirrhosis.

Abstract

Background: Hepatic Encephalopathy (HE) is a serious complication with a wide spectrum of clinical symptoms, from minimal changes to profound coma. HE is hard to diagnose without advanced laboratory parameters such as ammoni. This study aims to develop a scoring model to diagnose HE using clinical and laboratory parameters. Methods: An analytical cross-sectional study collected data from 96 hospitalized patients with liver cirrhosis from November 2021 to January 2022. Employing multivariate logistic regression analysis, the study aimed to identify autonomous factors associated with HE. Each significant variable was used to calculate patient probabilities. The score for each variable was computed utilizing the (B/SE)/lowest(B/SE) formula, demonstrating robust discriminatory capability. The scoring model was formulated and evaluated based on its sensitivity and specificity.Results: Nineteen point eight percent, equivalent to ninenteen patients, were admitted with HE. The scoring model was crafted based on nineteen variables. There were four significant variables in this model: Aspartate Aminotransferase (AST) (p=0.01), Total Bilirubin (p=0.007), Fibrosis-4-Index (FIB-4) (p=0.014), and Ascites (p=0.016). Each variable was scored as 1 for AST, -1 for total bilirubin, 1 for FIB-4-index, and 1 for Ascites. The probability was 2%, 14.2%, 57%, 91.4%, and 50%, following the total score of -1, 0, 1, 2, and 3, respectively. The sensitivity and specificity of the scoring model were 68.4% and 85.3%, respectively (AUC=84.7%).Conclusion: Daily laboratory and clinical manifestations related to hepatic cirrhosis could give a clue to diagnosing hepatic encephalopathy.
