Abstract

Non-alcoholic fatty liver disease (NAFLD) is characterized by hepatic steatosis in the absence of other causes of liver fat accumulation. The prevalence of NAFLD is increasing and now among top causes of hepatocellular carcinoma. The diagnosis of NAFLD requires evidence from liver biopsy. There are currently no specific drugs for therapy of NAFLD. Current mainstay for management of NAFLD is lifestyle interventions focused around weight loss. Lifestyle interventions can be done by dietary modification or increasing physical activity level. Pioglitazone and vitamin E are only recommended for patient whom was confirmed NAFLD by liver biopsy. New agents such as obeticholic acid, elafibranor, selonsertib, and cenicriviroc are still waiting for phase III result to be recommended as therapy for NAFLD. Bariatric surgery can be advised for NAFLD patient with obesity who is not adequately controlled by medical therapy.

Abstract

ABSTRACTBackground: Increased eradication of Helicobacter pylori decreases the incidence of gastric cancer. Vonoprazan is a potassium-competitive acid blocker (PCAB) with a stronger acid production suppression effect than proton pump inhibitors (PPI). Vonoprazan has been recognized as a therapy for gastric acid reflux disease since 2015 in Japan, but the comparison of the effectiveness of PCAB first-line therapy with PPI for adult patients with H. pylori infection is still controversial.Method: A literature search was carried out on three databases: Pubmed, Cochrane, and Scopus on March 30th 2023, using keywords vonoprazan, potassium competitive acid blocker (PCAB), proton pump inhibitor, and H. pylori. There were 7 studies on Pubmed, 26 studies on Cochrane, and 144 studies on Scopus. Critical screening was conducted using the Oxford Centre for Evidence-Based Medicine for systematic reviews and randomized controlled trials. Results: Based on screening of inclusion and exclusion criteria, one study met the criteria. The intention to treat analysis (ITT) showed eradication of H. pylori in the combination of three therapies with PCAB vs PPI of 91.81% vs 75.5% (1,18 [1.08−1.28], p0,0001), no heterogeneity found (I2 = 43%). Analysis per protocol showed PCAB versus PPI eradications of 92.99% vs 78.57% (1,13 [1,02−1,26], p = 0.02), heterogeneity obtained (I 2 = 75%). Conclusion: Generally, PCAB has better eradication than PPI as first-line therapy for adult patients with H. pylori infection Keywords: potassium competitive acid blocker, proton pump inhibitors, helicobacter pylori

Abstract

Aim: This study aims to evaluate the efficacy and safety of endoscopic band ligation (EBL) for bleeding grade III internal hemorrhoids in an evidence-based case report.Method: A search was conducted on PubMed and Cochrane databases based on clinical query, with keywords "endoscopic band ligation AND internal hemorrhoids". The inclusion criteria were meta-analysis, systematic review and randomized clinical trial, and written in English. The critical appraisal was performed for validity, importance, and applicability.Results: Of 18 studies, only 2 studies that suited the criteria. Compared to bipolar electrocoagulation (BPEC), EBL had significantly higher success rate in controlling bleeding and reducing the grade of hemorrhoids. In comparison with band ligation with rigid proctoscope (BL-RP), EBL was associated with higher success rate, lesser treatment sessions, and earlier symptom control. Mild complications and symptomatic recurrence were comparable among EBL, BPEC, and BL-RP.Conclusion: Endoscopic band ligation is effective and safe for bleeding grade III internal hemorrhoids. It is feasible procedure in primary referral hospital.

Abstract

Introduction: Infantile colic is a functional gastrointestinal disorder (GSCF) found in 20-30% of infants aged 5 months which is characterized by recurrent prolonged periods of infant irritability, fussing, or crying without obvious cause and cannot be prevented or resolved by parents. Infantile colic is often associated with higher mothers depression and lower quality of life. Mothers' knowledge and behavior of infantile colic varies greatly. This research to investigate the knowledge and behavior of Indonesian mothers regarding infantile colic and related factorMethods: A cross-sectional analytic observational study with the target of 100 mothers with babies aged 0-12 months was performed using an electronic questionnaire in Google forms. The questionnaire consists of multiple-choice questions regarding the mothers' knowledge and behavioral to infantile colic.Results: As many as 82% of mothers understand that infantile colic as recurrent prolonged periods of crying that occur without obvious cause and cannot be prevented or resolved by parents, however only 12% of mothers know that most infantile colic will disappear by the age of 5 months. Forty percent mothers had babies with infantile colic; 97.5% of them were worried about their baby's condition and 7.5% of the mothers gave them medication. There was no significant difference between mothers' age and number of children with inadequate mother-infant interaction (P = 0.770 and P = 0.647)Conclusion: The knowledge and behavior of mothers in Indonesia regarding infantile colic is quite good, however parental reassurance still needs to be improved, especially to reduce maternal anxiety.

Abstract

ABSTRACTIntroduction: Treatment of hepatitis C in end stage renal disease patients  still a clinical challenge. Elbasvir/grazoprevir is a type of direct acting antiviral (DAA) drug used in treatment of Hepatitis C virus patients with renal disease. However, research on effectiveness of elbasvir/grazoprevir in hepatitis c virus patients with stage 5 chronic kidney disease (CKD) was limited. Therefore, this study aims to describe the effectiveness of elbasvir/grazoprevir on hepatitis C patients with stage 5 chronic kidney disease (CKD).Method: This research using analytic observational design with cross-sectional study approach. The effectiveness of elbasvir/grazoprevir in hepatitis C patients with stage 5 chronic kidney disease (CKD) were evaluated the viral load, SVR12, and kidney function. Data were obtained from medical records of hepatitis C patients at Hepatology Outpatient Clinic dr. Saiful Anwar hospital from January 1st 2021 until December 31st 2022. Data analysis used the chi-square test, paired t test, and the Wilcoxon test with a significance level of p0.05 and 95% confidence level.Results: This study obtained 52 respondents. Most of the patients after treatment elbasvir/grazoprevir had an SVR12 value of 94.23%. Creatinine levels significant increase from 122±40 mg/dl to 133.05±36.12 mg/dl (p=0,032). Urea levels significantly different from 12±3 mg/dl to 12.18±3.56 mg/dl (p=0,446).Conclusion: Elbasvir/grazoprevir was effective to treat hepatitis C patients with stage 5 CKD. Keywords: Hepatitis C, stage 5 CKD, effectiveness, elbasvir/grazoprevir

Abstract

Inflammatory Bowel Disease (IBD) is a chronic condition characterized by persistent inflammation of the gastrointestinal tract, comprising ulcerative colitis (UC) and Crohn’s disease (CD). While the current global incidence of IBD exceeds 0.3% in Western nations, Indonesia reports a comparatively lower prevalence however with projections indicate a potential rise in incidence from 2020 to 2050. Hepatobiliary manifestation is one of the most common extraintestinal manifestations of IBD. Some of the liver disease associated with IBD had similar background immune response, inflammatory reaction, similar risk factor, or due to the treatment of IBD. Primary sclerosing cholangitis (PSC) has close association with IBD and both affect the prognostic and disease progression of each other. Non-alcoholic fatty liver disease (NAFLD) shares common metabolic risk factors. Several drugs used as the treatment of IBD might cause hepatic injury especially due to the long treatment duration of IBD. Hepatitis B reactivation is another concerning event found after prolonged used of immunosuppressive drugs of IBD. Therefore, close monitoring of liver function test periodically is a mandatory test to screen liver disease in IBD patients.  Keyword: Inflammatory bowel disease, liver disease, autoimmune hepatitis, drug-induced liver injury

Abstract

Background: In the era of laparoscopic procedure, innovative non-surgical approaches have emerged for managing biliary tract disorders. Techniques such as therapeutic endoscopic retrograde cholangiopancreatography (ERCP) and interventional endoscopic ultrasound (EUS) are increasingly being utilized. However, a clear consensus on a stepwise approach to managing difficult biliary disorders remains elusive. Therefore, this study was performed to evaluate the impact of endoscopic approach management on the outcomes of complex biliary cases.Methods: A retrospective study was conducted using an endoscopy database over two-year period. The characteristics of the study subjects, including demography and clinical data, were presented descriptively. The technical success rate was defined as the completion of the procedure performed on the patients. A bivariate analysis was conducted to evaluate the outcomes of therapy.Results: Sixty-one subjects with similar proportions in malignant (44.3%) and non-malignant (55.7%) etiologies in this retrospective database study were considered as complex biliary cases. Among them, 16.4% underwent a combination of therapeutic ERCP and EUS in one session based on the complexity of the case. Additionally, 8.2% of the subjects underwent ERCP with additional single operator cholangioscopy procedure. One subject (1.6%) underwent a rendezvous ERCP procedure through a percutaneous approach, while 4.9% underwent EUS-guided biliary drainage procedure. This study revealed no significant association between all mortality outcomes and baseline characteristics of the patients. Furthermore, there were no significant associations between mortality and the need for re-intervention, post-procedural pain, or the occurrence of acute pancreatitis.Conclusion: Complex biliary cases require a good clinical approach algorithm to decide which procedure comes first based on a comprehensive evaluation consisting of the patient’s factor, expertise, cost, and the risk of complications.

Abstract

Colorectal cancer is the third most common malignancy and the fourth most common malignancy causing death in the world. The development of colorectal cancer is influenced by several factors, including external, internal environmental, and genetic factors. One of the environmental factors recently known to be associated with colorectal cancer is gut microbiota. Generally, gut microbiota has various functions and is influenced by several factors, such as food, drugs, activities, habits, and diseases. Microbiota may become imbalanced, otherwise known as dysbiosis, which may lead to several changes and disturbances that can initiate the development of malignancy. There were differences in the composition and diversity of the types of microbiota in colorectal cancer patients. This difference has the potential to be used as a diagnostic marker and also the development of treatment in patients with colorectal cancer.Keywords: gut microbiota, colorectal cancer, diversity, abundance
