Abstract

Rheumatoid arthritis (RA) is a chronic autoimmune disease that frequently causes progressive joint damage, particularly in the hands, resulting in impaired functional ability in daily activities. This study aims to describe the hand function of patients with rheumatoid arthritis based on age, gender, and family history at Royal Prima Hospital Medan. This research employed a descriptive quantitative design with a cross-sectional approach involving 55 respondents who met the inclusion criteria. Data were collected using a structured hand function questionnaire assessing right hand, left-hand, and bilateral hand function across various activities. Data analysis was conducted using univariate and bivariate descriptive methods through cross tabulation. The results showed that hand function impairment was most prominent in patients aged >40 years, who predominantly fell into the “poor” function category. Female respondents demonstrated a higher level of functional limitation compared to males. Meanwhile, a family history of RAcontributed as a predisposing factor but did not directly correlate with the severity of functional impairment. Patients with a family history tended to fall within the “moderate” category, while more severe impairment was observed among those without a family history. Older age and female gender are major factors associated with reduced hand function in RA patients, whereas family history serves as a predisposition rather than a determinant of severity.

Abstract

This research examined the potential of essential oil–based bio-additives derived from citronella, clove, and patchouli to enhance the physicochemical properties, engine performance, and emission characteristics of Pertalite fuel. The study utilized Response Surface Methodology (RSM) with a Central Composite Design (CCD) to assess additive concentrations between 0.1 and 0.3 mL per liter of Pertalite. Findings indicated that these low-level additions did not significantly affect density, which remained similar to pure Pertalite. Likewise, power output and exhaust emissions (CO2 and O2) showed no notable improvements. Statistical evaluation using Design Expert® revealed all regression models were non-significant (p > 0.05), and optimization attempts failed to identify a valid optimum, with desirability values below 0.5. The minimal impact is attributed to the very low additive dosage, insufficient to influence atomization, combustion, or emission reduction. Additionally, gasoline’s non-polar nature limits sensitivity to parameters such as pH, while operational variations during engine testing (temperature, RPM, and load) may have masked minor effects. In summary, essential oil bio-additives at concentrations below 0.03% by volume are ineffective for measurable improvements. Future studies should investigate higher concentrations, purification of active compounds, improved blending techniques, and stricter engine testing protocols. The contribution of oxygenated compounds to combustion enhancement also merits further exploration.

Abstract

The goal of this study was to find out how to make smart choices that would lead to more sales and more loyal customers at AHASS Bersama Jaya Prima, the official Honda workshop in Bekasi. Even though the economy is getting better again after the pandemic, their business is still stuck. This is mostly because service visits have dropped by about 40% from what they were before the pandemic. For this study, we ran a qualitative case study. We basically talked to key people, like the customers, the staff, and the suppliers, to understand the whole picture. The main way we analyzed the data was using Root Cause Analysis (5 Whys), and then we used the Theory of Planned Behavior (TPB) idea to explain what was going on. The results showed that the biggest problem is that things aren't run efficiently. Especially, they keep messing up the Standard Operating Procedures (SOPs) because everyone is so focused on serving many people fast instead of giving good quality service. This situation just gives customers a bad time, makes them wait longer, and makes them unhappy with the service. We then used Analytic Hierarchy Process (AHP) to pick the best move, and the clear winner was setting up a digital queue and booking system. This new system should help customers feel more in control (perceived behavioral control), fundamentally smooth out service bottlenecks, and definitely help bring customer loyalty back up so the business can recover properly.

Abstract

In business-to-business firms that are in the commodity markets with limited product differentiation, customer retention is very crucial. This study studies customer purchasing behavior and retention variables in the industry North Sumatra livestock industry by the case study of PT Eldira Fauna Asahan which is cattle fattening company that has gone through a five-year period of fifty five percent sales decline even though product and price remained the same. Using qualitative case study as the research method, semi-structured interviews were carried out on five business customers and three employees of the company to learn about the process of organizational buying decisions in the eight-stage business buyer decision process. The Service Quality Gap Model analysis indicates that the main cause of customer attrition is the inconsistency of service delivery at the peak periods and not the product or price quality. Customers stated high satisfaction with the quality of cattle and price competitiveness although the level of service declined with high levels due to delays in communication, facing uncertainty to coordinate delivery and to be responsive to customers when staff capacity became limiting. Internal analysis reveals that there is a lack of customer service expectations, lack of standard service protocols and reactive operation methods in the management that create gaps in the services that undermine customer trust. The study suggests a holistic approach, which has four priorities: organized communication channels with four-hour turnaround guarantees, relationship reinforcement through periodic business reviews and selective payment options, scheduling reliability through structured schedules, and customer recovery through individual customer outreach. The implementation is aimed at achieving ninety-five percent retention rate, 90 percent on time delivery, and 20 percent growth in revenues in eighteen months. The study adds empirical support showing the impact of dimensions of service quality on buyer decision making in an organization and retention of customers in agricultural supply chains where there is trust in relationships and product differentiation is low.
