Abstract

Introduction: Quality of life is an important parameter to be assessed, especially in progressive chronic diseases such as patients with Parkinson's Disease (PD). Parkinson's Disease Questionnaire-39 (PDQ-39) is the most widely used measurement tool in assessing the quality of life of PD patients. In PD, a series of features can affect the quality of life including the degree of disease severity and the number of medications. The aim of this study is to determine the relationship between the severity of the disease and the number of medications with the quality of life of people with PD so that it is hoped that it can help improve the quality of life of PD patients. Method: This study used a cross-sectional design. The research subjects were PD patients at the Neurology Polyclinic at Haji Adam Malik Hospital Medan from April - November 2020 who met the inclusion criteria. Assessing the severity of the disease using the Hoehn and Yahr and measuring the quality of life using the PDQ 39. Data analysis used the Spearman correlation test for the degree of severity and the Mann- Whitney test for the number of medications. Results: Of the 47 subjects who met the inclusion criteria, most were males (68.1%), with a mean age of 64.82 ± 9.52 years old, with University graduates as the majority of education level (46.8%) and the civil servant as the majority of a profession (34.0%). Stage 2 on Hoehn and Yahr Scales is the most common severity level of disease (55.3%), and polytherapy is the majority of the number of medications (74.5%). The median value of quality of life is 35, with the lowest score being 10 and the highest score being 98. From the results of the bivariate analysis, it was found that there was a very strong, significant association between the degree of disease severity and the quality of life (p <0.001 r=0.872), and there was a significant difference between the number of medications with the quality of life of Parkinson's disease patients (p=0.041). Conclusion: There is a very strong association between the degree of severity and the quality of life of people with Parkinson's disease and there is a difference in the quality of life of PD patients between those who receive monotherapy and polytherapy so there is an effect of the number of medication given on the quality of life of Parkinson's disease patients.

Abstract

Introduction: Heart failure (HF) is a complex clinal manifestation that cause abnormality of structural and/or functional of the heart. HF is due to diastolic dysfunction and systolic dysfunction of the left ventricle (LV). The aim of this study is to assess the relationship between grading of pre-discharge LV diastolic dysfunction and readmission at 30 days post discharge and 6 months post discharge. Method: This is a cohort retrospective study in HFrEF patients which are treated at Pusat Jantung Terpadu RSUP H. Adam Malik Medan on January 2021 until December 2021. Pre discharge echocardiography has been done to assess LV diastolic dysfunction and the we do the follow up of readmission at 30 days and 6 months by phone. Results: From 93 patients, there are 40 patients (43%) with grade I LV diastolic dysfunction, 22 patients (23,7%) with grade II LV diastolic dysfunction and 31 patients (33,3%) with grade III LV diastolic dysfunction. There are 34 patients (26,6%) with readmission at 30 days and 21 patients (26,6%) readmission at 6 months. There is a significant relationship between grading of pre-discharge LV diastolic dysfunction and readmission at 30 days post discharge (p value 0,000), but not at 6 months post discharge (p value 1,000). Conclusion: Grading of pre-discharge LV diastolic dysfunction on HFrEF patient is correlated with readmission at 30 days post discharge.

Abstract

Introduction: Chronic Obstructive Pulmonary Disease (COPD) is a heterogeneous lung condition characterized by chronic respiratory  symptoms due to abnormalities of the airways  and/or  alveoli (that cause persistent, often progressive, airflow obstruction. The main environmental exposures leading to COPD are tobacco smoking and the inhalation of toxic particles and gases from household and outdoor air pollution. COPD could lead to worse outcomes from COVID-19. Oxygen therapy is the main treatment method for patients with COPD and hypoxemia. Compared with NIV, HFNC can significantly reduce the incidence of pressure injuries such as facial and nasal bridge rupture. And also HFNC could be a valuable and feasible treatment option for patients with COVID-19 pneumonia, with remarkable clinical advantages. Method: This was a retrospective case report, after analysis of patient clinical data. The patient provided written informed consent to publish their case details and any accompanying images. Results: A 63-year-old man was presented with a complaint of shortness of breath that had been felt for two days before entering the hospital. The first day of treatment, the patient complained of increasing severe shortness of breath accompanied by fever. Later on, the patient was diagnosed with an acute exacerbation of COPD accompanied by COVID-19. During 4 days of hospitalization, the patient was admitted to the ICU because of worsening breathing and was given oxygen therapy with HFNC and other drugs for 8 days. Patient showed improvement and was then discharged after seven days of hospitalization. Conclusion: HFNC can be an alternative therapy in COPD therapy with COVID 19, because it has good tolerance and physiological effects

Abstract

Introduction: Chronic Kidney Disease are related to inflammation and stress levels therefore depression is a predictor that will increase the morbidity and mortality of patients with chronic kidney disease undergoing hemodialysis. Method: This research is an analytical study with a cross-sectional design which carried out in Januari until February 2023, at Hemodialysis Installation of RSUP. H. Adam Malik. To analyze the correlation between the Depression Level and Red Blood Cell Distribution Width (RDW) and Erythrocyte Sedimentation Rate (ESR), the Pearson correlation test was used if the data was normally distributed. If the data is not normally distributed, Spearman's correlation test is performed. Statistical analysis using 95% confidence level (p<0.05). Results: In this research, demographic characteristics of 85 patients with mean age of the patients was 49.56 years old with a standard deviation of 13.46. Based on Spearman rho test it was found that there was a significant correlation between RDW and the BDI II score with p value 0.003. The degree of correlation or r value was 0.318. This indicates that there is a weak correlation between the RDW score and the BD II score. Based on Pearson test it was found that there was a significant correlation between ESR and the BDI II score with p value 0.009. The degree of correlation or r value was 0.280. This indicates that there is a weak correlation between the ESR value and the BD II score. Conclusion: Level of depression were found to be correlated with Red Blood Cell Distribution Width (RDW) and Erythrocyte Sedimentation Rate (ESR) in CKD patients whose undergoing regular hemodialysis.

Abstract

Introduction: Patients with tumor usually show abnormal laboratory coagulation tests, indicating a subclinical hypercoagulable state that contributes to morbidity and mortality. Hematologic markers such as the platelet-lymphocyte ratio (PLR) can be used as an index of tumor progression, high PLR is associated with morbidity and mortality in patients with primary or metastatic tumors. KPS is a method that is widely used to assess the functional status of a patient. The aim of this study is to determine the relationship between coagulation profile and platelet-lymphocyte with Karnofsky Performance Status Scale in Brain Tumor Patients. Method: This study was an observational analytic study with a cross-sectional design using primary data sources taken consecutively from all brain tumor patients who were hospitalized in Inpatient Room at Haji Adam Malik General Hospital Medan who met the inclusion criteria. The parameters analyzed were Prothrombin Time (PT), Activated Partial Thromboplastin Time (aPTT), D dimer (DD), and PLR. Clinical outcome of brain tumor patients was assessed using KPS. To assess the relationship between coagulation marker factors and the ratio of platelets to lymphocytes to KPS, the Spearman and Gamma tests were used. Results: Of the 30 research subjects analyzed, the number of female subjects was comparable to that of males (50%). The highest age is in the age range of 61-70 years by 30%. The highest level of education was high school at 40%, the most research subjects were housewives at 33.3% and the most types of brain tumors were primary brain tumors at 56.7%. The highest KPS score during treatment was found in the <70 group of 76.7%. From the correlation test, it was found that there was a strong, significant relationship between the values of PT (p<0.02 r = -0.731), APTT (p<0.013 r = -0.761), D-dimer (p<0.001 r = -0.737) and PLR (p<0.001 r = 0.78) on the clinical outcome of brain tumor patients assessed by KPS. Conclusion: There is a strong relationship between PT, APTT, D-dimer and PLR values on KPS in patients with brain tumor.

Abstract

Background: Fatty liver is a broad term for a condition in which there is an increase triglyceride level in hepatocyte. In Indonesia there is not much research about this condition. Fatty liver is a major health problem in West countries, and is slowly increasing in Asian countries. For many individual with fatty liver, have no specific complaint or can have no symptoms at all. This aim of this study to analyzed the relationship between fatty liver images on abdominal CT scans with dyslipidemia. Methods: This study is an observational analytic study with a case – control type of research with the aim of knowing the relationship between CT scan images of fatty liver and dsylipidemia. The data in this study were all medical records who went to the Radiology Department for abdominal CT scan at the Haji Adam Malik General Hospital for a period of one year from January 1st, 2020 to December 31st 2020. Results: The subjects on the abdominal CT scan results with fatty liver and high triglyceride is 5 peoples out of 12 (41.7%). The test results obtained p value = 0.,688 (p>0,05), in other words there is no significant relationship between fatty liver and high triglyceride level. Conclusion: No significant relationship was found between the incidence of fatty liver and the incidence of dyslipidemia.

Abstract

Introduction: Myasthenia gravis (MG) is one of the most common neuromuscular junction disorders with various clinical presentations. Several studies showed cognitive function decline in MG patients which affects cognitive domains in memory, attention, executive function, and verbal. The involvement of the central cholinergic system, known as central cholinergic deficits are thought to manifests as impaired cognitive function in patients with MG. The purpose of this study was to evaluate cognitive function characteristic in patients with myasthenia gravis. Method: This study used a cross-sectional design, involved 33 myasthenia gravis patients in Neurology Outpatient Clinic of Haji Adam Malik Central General Hospital and 33 subjects in healthy control group. Cognitive function tests were performed using the Indonesia version of Montreal Cognitive Assessment (MoCA-Ina). Mann-whitney test was performed to evaluate cognitive performance difference in both groups. Results: : The results were compared between MG and healthy control group. The mean of MoCA-Ina score was significantly lower in MG group compared to healthy control group. The result of this study showed difference in cognitive performance between MG patients and healthy control group (p<0.001). This study showed delayed memory, attention, verbal, abstraction, visuospatial and executive function performance was decline in MG patients. Conclusion: This study concludes significant difference of cognitive performance based on MoCA-Ina scores between myasthenia gravis patients and healthy control group.

Abstract

Introduction: Tension-type headache (TTH) is one of the major public health concerns globally with an incidence of 1.89 billion people. Fatigue is potentially debilitating and affects an individual's quality of life and is known to be associated with the case of TTH. The purpose of this study was To find out the relationship between fatigue and the incidence of tension-type headache in the first semester resident doctors of the Faculty of Medicine Universitas Sumatera Utara. Method: The research design was an observational study with a cross-sectional approach which was undertaken at Haji Adam Malik Hospital and Network Hospital in January to February 2022 with all first semester resident doctors as research subject at Faculty of Medicine, Universitas Sumatera Utara in the period of August 2022. Fatigue was measured through the Fatigue Severity Scale (FSS), TTH frequency was measured using the third of the International Classification of Headache Disorders (ICHD-3) criteria, and TTH pain intensity was measured using numerical rating scale (NRS). Meanwhile, to analyze the relationship between fatigue and the incidence of tension-type headache, the Spearman correlation test was used. Results: : Total subjects who met the inclusion and exclusion criteria was 98.  Regarding to the results of statistical tests, a significant relationship was obtained between fatigue and TTH frequency (r= 0,294, p= 0,003), but there was no relationship between fatigue and the pain intensity (r= 0,143, p= 0,160). Conclusion: There was a significant relationship between fatigue and TTH frequency. However, the relationship was not significant between Fatigue and the pain intensity

Abstract

Introduction: Various factors including surgical, psychological, amount of acute pain relief have been proposed as risk factors for developing CPSP. Different treatment modalities like nerve and plane blocks, psychiatric counselling have been described to counter this pain. Case: The patient was a 30-year-old female with complaints of severe pain in the right lower quadrant of the abdomen and found a surgical scar with hyperalgesia and a history of open appendicectomy surgery, two times section caesarean section surgery. The patient experienced pain after section caesarean section surgery 6 months ago. Because the pain was also felt in the right hypochondrium, the patient was unable to move his body, unable to sit and stand. The patient was diagnosed as a case of chronic post surgical pain syndrome (CPSP). We performed transversus abdominis plane block (TAP) with ultrasound guidance and lidocaine 15 ml combined with 40mg methylprednisolone. VAS scores showed significant improvement from 8/10 and 2/10 during the 10-day follow-up. We conclude that ultrasound-guided TAP block can be a treatment option for CPSP cases. Conclusion: Chronic post surgical pain (CPSP) is a definite clinical entity after laparatomy. We report a successful management of such a case with ultrasound guided transversus abdominis plane block. The patient had very significant pain relief after procedure follow up continue until today without any side effects.

Abstract

Introduction: Coccydynia, or coccygodynia, is pain in the tailbone or coccyx area. Despite identifying chronic coccyx pain happened hundreds of years ago, its treatment can be difficult and sometimes controversial due to the multifactorial nature of coccyx pain. Many physiological and psychological factors contribute to its etiology. Most cases of coccydynia resolve within weeks to months with or without conservative treatment, but for some patients, the pain could be chronic and bothersome. Cases: This report describes a patient who experienced a traumatic fall from a height, causing chronic coccyx pain. Coccyx pain is local and does not radiate, usually gets worse when changing from sitting to standing or vice versa. This makes it difficult for patients to carry out their usual activities. After taking pain medications regularly for years, the pain does not improve. Conclusion: Prolotherapy is an injection therapy for chronic musculoskeletal injuries, including coccydynia. Injection prolotherapy aims to tighten and strengthen weak or lax tendons, ligaments, or joint capsules through the multiplication and activation of fibroblasts. The main principle is the injection of small amounts of irritant solution into the painful ligament and tendon insertions as well as in the adjacent joint space over several treatment sessions. This technique has been shown to reduce patient pain by approximately 50% of initial pain after the first session of prolotherapy. It is hoped that the repetition of this session will further reduce and even eliminate the chronic pain that the patient has been suffering.
