Abstract

The implementation of a digital health information system in improving the quality of hospital services, especially in the management of electronic medical records (ERM). Although it has been implemented, various obstacles are still found such as system disruptions, delays in data input, and limited user capabilities. Therefore, this study is aimed evaluate the digital health information system in medical record management using the HOT-Fit method at Dharma Yadnya General Hospital. The study method used is a qualitative descriptive study with a case study approach. Data collection was carried out through in-depth interviews. The number of Participants is 25 informants the EMR system. Out of those participants, 7 key informants were selected using a purposive sampling technique based on certain criteria, namely officers who were directly involved and had experience in using the system. The selection of informants was carried out until data saturation was achieved (data saturation), observation, and documentation of several informants involved in using the system. Data analysis was carried out by referring to the HOT-Fit model components, namely human, organizational, technological, and net benefit aspects. The results of the study showed that from the human aspect, users felt the system helped their work. From an organizational perspective, management and policy support are in place, but monitoring and evaluation are not yet optimal. From a technological perspective, the system is considered quite good, but technical challenges such as errors and downtime persist. The conclusion of this study is that the implementation of the digital health information system at Dharma Yadnya General Hospital has been quite successful, but not optimal in several HOT-Fit aspects. Improved human resource training, strengthened organizational support, and improved technological infrastructure are needed to optimize the system's benefits.

Abstract

Nutritional concerns for children aged 1–9 years remain a priority, particularly regarding the fulfillment of energy, protein, and calcium requirements. The development of nastar as a snack made from snakehead fish flour is an effort to address these issues. The objective of this study was to determine the macronutrient and calcium content of the product and its contribution to children’s energy and nutritional adequacy. The study design employed a Completely Randomized Design (CRD) with six formulations of catfish flour supplementation: F0 (0%), F1 (10%), F2 (20%), F3 (30%), F4 (40%), and F5 (50%), each with three replicates. Tests for macronutrients (carbohydrates, protein, fat), calcium, and acceptability were conducted by 25 panelists based on the parameters of aroma, taste, texture, and color. Data analysis used a One-Way ANOVA test and Duncan’s multiple range test at a 5% significance level. The results showed that the substitution of snakehead fish meal had a significant effect (p<0.05) on the levels of carbohydrates, fat, and calcium. Preference tests on F1 (10%) and F2 (20%) were preferred by the panelists compared to the other formulations. F1 contained carbohydrates (72.27 g), protein (0.11 g), fat (15.65 g), and calcium (17.15 mg). F2 contains carbohydrates (66.7 g), protein (0.11 g), fat (20.05 g), and calcium (29.87 mg). F1 provides 10–15% (172.16 kcal) of the RDA for children aged 10–3 years, and F2 provides 10–15% (179.12 kcal) of the RDA for children aged 4–9 years, with a serving size of 40 g.

Abstract

Lip balm is a topical preparation used to maintain moisture and protect the lips from various environmental factors. The use of natural ingredients in lip balm has increased and is considered safer. Cherry leaves (Muntingia calabura) and moringa leaves (Moringa oleifera) are known to contain various secondary metabolites with potential applications in cosmetic formulations. This study aimed to formulate a lip balm based on a combination of ethanolic extracts of cherry and moringa leaves and to evaluate its physical stability. The formulation was evaluated through physical stability tests, including organoleptic properties, homogeneity, pH, and spread ability. Stability testing was conducted at room temperature for 14 days, with observations performed on days 0, 7, and 14. The results showed that all formulations exhibited good organoleptic characteristics, were homogeneous, and did not undergo significant changes during storage. It can be concluded that the lip balm formulation containing a combination of ethanolic extracts of cherry (Muntingia calabura) and moringa (Moringa oleifera) leaves demonstrated good physical stability and has potential for further development as a natural-based cosmetic product.

Abstract

Abstract Biomedical science serves as the fundamental foundation of medical education, bridging the understanding of basic sciences with clinical reasoning. In competency-based curricula, formative assessment is designed to support the learning process and prepare students for summative evaluation. However, during the early phase of medical education, discrepancies between formative and summative assessment outcomes are still frequently observed and may reflect problems related to constructive alignment and the implementation of block-based learning. This study aimed to analyze the discrepancy between formative and summative assessments in the Biomedical Science 1 Block and to examine contributing factors based on Monitoring and Evaluation (Monev) findings. This study employed a mixed-methods design with an explanatory sequential approach. Quantitative analysis was conducted on the formative and summative assessment scores of 36 first-year medical students in the Medical Education Program at Universitas X during the Biomedical Science 1 Block (Anatomy, Histology, and Physiology) in the odd semester of the 2025/2026 academic year. Descriptive and comparative statistics were used to describe score distribution, pass rates, and discrepancies in learning achievement. Qualitative data were obtained from structured monitoring and evaluation reports involving block coordinators, lecturers, tutors, and the Quality Assurance Unit, and were subsequently analyzed. Data integration was performed during the interpretation phase. The results demonstrated a significant discrepancy between formative and summative assessments. Although most students achieved high formative assessment scores, only 12 out of 36 students (33.3%) passed the summative examination. The disparity in score distribution indicated the low predictive value of formative assessments. Monitoring and evaluation findings identified several contributing factors, including weak constructive alignment, less challenging formative assessment design, and high cognitive load. This study suggests that discrepancies between formative and summative assessments may serve as one of the indicators in learning monitoring and evaluation to identify potential problems in the implementation of block-based learning during the early phase of medical education.

Abstract

The development of information technology in educational environments has increased internet accessibility to support learning activities, but it has also contributed to cyberslacking behavior, namely the use of the internet for non-academic activities during study hours. This study aims to analyze the effectiveness of MikroTik-based DNS Filtering in controlling access to non-productive websites within educational networks. The research employed a quantitative approach with a quasi-experimental design by comparing network conditions before and after the implementation of DNS Filtering. Additional testing was conducted under Virtual Private Network (VPN) and DNS over HTTPS (DoH) scenarios to identify system limitations. Data analysis was performed using the Confusion Matrix method with evaluation parameters including accuracy, precision, recall, F1-score, and specificity. The results indicate that DNS Filtering effectively blocks access to non-productive websites under normal network conditions with a high level of accuracy without disrupting access to educational resources. The system was also able to classify productive and non-productive websites appropriately based on the implemented filtering rules. However, the effectiveness of the system decreased when users utilized VPN and DoH as bypass methods, indicating the limitations of conventional DNS-based filtering approaches. This study concludes that MikroTik-based DNS Filtering is a lightweight, easy-to-implement, and sufficiently effective solution for controlling cyberslacking in educational environments, although it still requires integration with additional network security methods to improve resistance against bypass techniques.

Abstract

Long-term use of synthetic drugs can induce adverse side effects, prompting society to return to traditional medicine, such as kirinyuh leaves (Chromolaena odorata L.). Kirinyuh leaves contain flavonoid compounds that possess antipyretic potential by inhibiting the cyclooxygenase (COX) enzyme in prostaglandin synthesis. A transdermal patch formulation was selected to avoid absorption difficulties in the gastrointestinal tract and to improve patient compliance. This study aimed to formulate and evaluate the physical stability of transdermal patches containing kirinyuh leaf extract (at doses of 250 mg, 300 mg, and 350 mg) and to determine the most effective antipyretic dose in male Wistar rats (Rattus norvegicus) induced by 10% peptone. The extract was obtained through maceration using 96% ethanol, and the patches were prepared using the solvent casting method. Physical evaluation encompassed organoleptic testing, weight uniformity, pH, moisture content, thickness, and folding endurance. The antipyretic test was conducted on 25 male Wistar rats that had been acclimatized for 7 days. Baseline body temperatures were recorded prior to induction. The rats were then induced with 2 mL of 10% peptone via intraperitoneal injection, after which their temperatures were remeasured. The subjects were divided into 5 groups of 5 rats each: positive control (Bye-Bye Fever), negative control (blank patch), and three treatment groups (doses of 250 mg, 300 mg, and 350 mg). Rectal temperatures were monitored every 15 minutes for the first 2 hours and every 30 minutes during the final hour. The physical evaluation indicated that all patch formulas were stable and conformed to the physical quality standards for transdermal preparations. All three kirinyuh leaf extract patch formulas demonstrated antipyretic activity in reducing the rectal temperature of the rats. The 350 mg dose formulation provided the most effective reduction in body temperature 3,040C at the 180th minute, which was comparable to the positive control. The transdermal patch of kirinyuh leaf extract meets the physical stability requirements for transdermal preparations, and the 350 mg dose is the most effective antipyretic formulation.

Abstract

The common cold is one of the most common illnesses experienced by boarding school students, caused by food or beverages consumed or the entry of microorganisms (bacteria, viruses, fungi) into the respiratory tract, leading to common cold symptoms. Many students engage in self-medication or self-treatment, particularly for the common cold, as it is considered mild and easily manageable without needing to visit a doctor. The purpose of this study is to determine the level of knowledge regarding self-medication for the common cold among students at the Roudlatul Quran Metro Islamic boarding school and to identify the relationship between the level of knowledge and self-medication behavior for the common cold. The research method used is a quantitative approach with a cross-sectional study design, involving a sample size of 222 respondents. The instrument used in this study is a questionnaire. Based on the research results, the level of knowledge among students at the Roudlatul Quran Metro Islamic boarding school was categorized as good (36.94%), adequate (61.71%), and poor (1.35%). Self-medication behavior was categorized as good (53.60%), adequate (42.79%), and poor (3.60%). This study concludes that there is a correlation between the level of knowledge and self-medication behavior for the common cold among students at the Roudlatul Quran Metro Islamic boarding school, with a P-value of 0.00.

Abstract

Schizophrenia is a chronic psychiatric disorder characterized by disturbances in thought processes, perception, emotion, and behavior and is frequently accompanied by medical comorbidities. This case report aimed to describe the clinical manifestations and management of paranoid schizophrenia accompanied by type 2 diabetes mellitus and chronic suppurative otitis media. A descriptive case-report method was applied to a 37-year-old woman. Data were collected through history taking, mental status examination, physical examination, supporting investigations, and review of medical records. Clinical assessment revealed findings consistent with paranoid schizophrenia, accompanied by type 2 diabetes mellitus and chronic suppurative otitis media. These comorbidities increased the complexity of the patient’s condition, requiring comprehensive evaluation of psychiatric symptoms, metabolic control, chronic ear infection, treatment adherence, and potential adverse effects of antipsychotic medication. The patient was managed using an integrated approach involving psychiatric treatment and appropriate management of the accompanying medical conditions. This case highlights the importance of multidisciplinary care, regular clinical monitoring, and continuity of treatment to achieve optimal outcomes in patients with schizophrenia and multiple medical comorbidities.

Abstract

Hemophilia A with Factor VIII Inhibitor in a Geriatric Patient: A Case Report. Hemophilia is an inherited bleeding disorder caused by a deficiency or abnormality of coagulation proteins. Its clinical manifestations are diverse, ranging from hemarthrosis, muscle hematomas, and mucosal bleeding to life‑threatening events such as intracranial hemorrhage. This article presents a case of a 60‑year‑old woman who developed a massive hematoma in the posterior femoral region, associated with factor VIII deficiency and the presence of factor VIII inhibitors. The discussion highlights the diagnostic approach, supportive laboratory investigations, pathophysiological aspects, and management strategies for hemophilia A in elderly patients complicated by hematoma.

Abstract

Meningioma is the most common primary intracranial tumor, originating from the meninges surrounding the brain and spinal cord. Although its exact etiology remains unclear, exposure to ionizing radiation, hormonal factors, and genetic predisposition are considered important risk factors. This systematic review aimed to identify the most common anatomical location and clinical manifestations of brain meningioma worldwide. A literature search was conducted in PubMed, MedLine, and ProQuest for studies published between 2015 and May 2025. Eligible studies included original research focusing on intracranial meningioma, while studies involving other central nervous system tumors or non-journal publications were excluded. Of 95 identified articles, 10 studies involving more than 500 patients met the inclusion criteria. The findings consistently demonstrated that supratentorial meningiomas were the most frequent, particularly those located at the convexity (30%) and parasagittal region (24%). The most common clinical presentation was nonspecific headache, followed by vomiting, decreased level of consciousness, and other neurological deficits depending on tumor location. These findings highlight that supratentorial meningiomas represent the predominant anatomical site and that nonspecific headache remains the most frequent presenting symptom, emphasizing the importance of early clinical recognition and neuroimaging evaluation.
