Description: Padjadjaran Acute Care Nursing Journal (PACNJ) is a scientific nursing journal that publishes by the Faculty of Nursing, Universitas Padjadjaran. The PACNJ is published as a means for sharing research findings and development in nursing knowledge. The scope of the PACNJ is nursing and health areas, which focuses on nursing care or health care services in the context of an emergency, acute care conditions, and critical care in all fields and disaster nursing. The PACNJ is published every four months using a peer review system for selected articles. This journal provides an open access for readers to read abstracts and full papers. The PACNJ covers original research papers and reviews literature articles that have not been published previously. Articles submitted to the PACNJ will be filtered through an editorial board meeting to further make a decision. Articles that are decided to be accepted for review will be sent to the reviewer. The peer review process will take around 1-2 months. The accepted article will be returned to the author for revision, and resubmission would be required for the revised article. The publication process of articles in this journal would take about two to three months. The PACNJ reviewer’s team are people who have expertise in nursing and health in the relevant areas. Focus and scope: Launched in November 2019, Padjadjaran Acute Care Nursing Journal (PACNJ) is a newcomer publication place in the field of nursing carried out by the Faculty of Nursing at Universitas Padjadjaran. PACNJ aims to become an international journal to protect researchers around the world. PACNJ continues to improve quality by involving many experts in the field of nursing. PACNJ is an open-access journal. PACNJ focuses on receiving articles in various fields of nursing including; emergency nursing, critical nursing, emergency nursing in children, emergency nursing in maternity, emergency nursing in mental health, nursing emergency in medical surgery, emergency nursing in the community, and miscellaneous. Articles published in PACNJ have been selected by a team of editors and a peer review process from reviewers who are experts in the field of nursing. Articles published on PACNJ have also been completed by Digital Object Identifier (DOI) and indexed by Google Scholar, ISJD, and Kandaga.

Abstract

The COVID-19 outbreak is a worrying problem worldwide and health care workers have a big responsibility in responding to this condition. The increase in the number of COVID-19 patients which causes an increase in the workload of health care workers, scarcity of personal protective equipment, and stresses during work, can lead to psychological instability in health care workers.The study aims to identify the psychological conditions experienced by health care workers during the COVID-19 pandemic.The search for articles was carried out on the online digital database, Science Direct, and PubMed with the keywords 'COVID-19', 'psychological or psychology or mental health', 'nurse'. Then the articles are selected according to the inclusion criteria, namely the results of research in 2019-2020, full text, published in English, and articles discussing the psychological and mental conditions of health care workers who are assigned to the COVID-19 pandemic. In the end, 7 articles were analyzed in this literature review.In carrying out their duties during the COVID-19 pandemic, health care workers are prone to experiencing psychological disorders such as anxiety and insomnia. High workload, lack of personal protective equipment, lack of knowledge about the prevention and transmission of COVID-19, and pressures during work, are the main triggers for psychological problems experienced by health care workers. Therefore, coping strategies that come from internal and external need to be obtained by health care workers to reduce the psychological disturbance they experience.

Abstract

Acute pain is a significant manifestation in postoperative Coronary Artery Bypass Graft (CABG) patients. A sternotomy performed in open-heart surgery is a major cause of postoperative pain. Prolonged postoperative pain can lead to increased the use of analgesics, reduce infection resistance, delayed wound healing after surgery, and extended hospitalization. This study aims to evaluate pain management interventions in the form of deep breathing techniques and collaboration in administering morphine to acute pain in postoperative CABG patients. The method of this study is a case study. The sample of this study is a 69-year-old postoperative CABG patient who complained of chest pain in the chest and incision wound on a scale of 7 (0-10) in General Intensive Care Unit (GICU). Patients describe the pain as stabbing in the chest and getting worse when coughing, eating, and talking. After three days of intervention, the patient experienced a decrease in pain levels. This condition is seen from the decrease in pain levels to a scale of 2 (0-10), the patient's face appears relaxed, the patient does not appear restless, vital signs are within the normal range, and the patient can perform deep breathing techniques by himself when the pain is felt. Pain management through deep breathing techniques and collaboration on morphine administration can reduce acute pain in postoperative CABG patients. Researchers recommend pain management through deep breathing techniques and collaboration on morphine administration to treat acute pain in postoperative CABG patients.

Abstract

Nursing profession students need readiness to attend profession learning. Optimal readiness is directly proportional to optimal capabilities. The characteristics of emergency department tend to make nursing profession students the first and most frequent resource to deal with patients entering the emergency department. This study aims to determine the readiness of nursing profession students on handling emergency patients in emergency department.This study uses quantitative descriptive methods through approaches cross sectional. The population in this      study was a nursing profession students of the Faculty of Nurisng, Universitas Padjadjaran as many as 66 respondents. The total sampling technique was used in this study. This research measuring instrument uses the READI-JV The results of the study were calculated using quantitative descriptive analysis using a cross-sectional approach.The results showed that as many as 51 (77.3%) nursing profession students are not ready to handle emergency patients. The percentage of readiness obtained in the leadership and administrative support dimensions was 85.2%, the readiness obtained in the operational competency dimension was 63.9%, the readiness obtained in the establishment dimension was 63.1%, the readiness obtained in the stress handling dimension was 57.6%, the readiness obtained in the dimensions of group integration and identification of 57.5%, readiness obtained in the survival skills dimension was 56.6%, and readiness obtained in the clinical competency dimension was 47.2%.The unpreparedness of nursing profession students is because most of the respondents have not attended emergency department training. So that it is necessary to optimize the learning methods during pre- profession by applying in accordance with emergencies and based on case studies as well as providing stress management to students to reduce stress levels in nursing profession students.

Abstract

Stroke adalah suatu kondisi di mana defisit neurologis terjadi akibat penurunan aliran darah ke area otak yang terlokalisasi secara tiba-tiba yang dapat disebabkan oleh ttrombus, embolus, stenosis atau hemoragik. Tujuan studi kasus ini adalah untuk mendeskripsikan asuhan keperawatan pada pasien dengan stroke infark. Penelitian dilakukan menggunakan metode studi kasus dengan pendekatan Asuhan Keperawatan pada seorang wanita berusia 56 tahun di Ruang Azalea Rumah Sakit Umum Pusat Hasan Sadikin. Hasil pengkajian didapatkan dua masalah keperawatan prioritas yaitu risiko perfusi serebral tidak efektif dan gangguan mobilitas fisik. Intervensi keperawatan yang diberikan pada pasien adalah pemantauan TTIK dan dukungan mobilisasi. Setelah dilakukan intervensi keperawatan selama dua hari, keluhan nyeri kepala pasien berkurang dan kekuatan otot pasien meningkat. Intervensi pemantauan TTIK dan dukungan mobilisasi yang diberikan pada pasien terbukti efektif dalam mencegah terjadinya peningkatan TTIK dan mengatasi defist neurologis akibat stroke infark.

Abstract

The provision of phase I cardiac rehabilitation intervention in patients with cardiovascular disease needs to be given because it can accelerate recovery and determine the needs of the next phase of cardiac rehabilitation, namely phases II, III and IV.  The aim of this study was to find out the impact of phase 1 cardiac rehabilitation on acute coronary syndrome patients. This literature study examined scientific articles published from 2011 to 2021 and in Indonesian and English. Data was obtained from the database includes PubMed, Cinahl Ebscohost and Google Scholar search engine using keywords developed based on population, concept and context namely Cardiac Patients, Phase 1 of Cardiac Rehabilitation, Outcomes, Benefits and Effects. A total of 275,000 articles were obtained and after being selected based on duplication, title, abstract and inclusion criteria were obtained 9 articles that are eligible for analysis. The results of the study found that phase 1 cardiac rehabilitation in SKA patients was shown to have good impact and outcomes, such as improving quality of life and self-efficacy, reducing the need for the second phase of rehabilitation, and reducing the level of anxiety and depression experienced by SKA patients. Therefore, the results of this literature review can be the basis of the next research in the implementation of phase 1 cardiac rehabilitation in Indonesia.

Abstract

The mortality rate of Covid-19 in Indonesia is 8.9%, which is the highest rate in Southeast Asia. Indonesia announces that there are positive patients with Covid-19 in children under five and school age. Children have the potential to transmit the virus to other people. One of the efforts to prevent the spread of Covid-19 is to carry out hand hygiene according to the Covid-19 protocol. The research objective was to determine the knowledge of school age children about hand hygiene as an effort to prevent the spread of Covid-19. The research design is descriptive analytic. The population in this study were children of elementary school age, namely as many as 403 people. By using Simple Random sampling, the sample were found 100 respondents. The data were collected using a questionnaire that contains knowledge of school age children about hand hygiene. The results showed that 45% of respondents have low knowledge of hand hygiene and 55% of respondents have high knowledge of hand hygiene as an effort to prevent the spread of Covid-19. The lowest respondent's knowledge (46.8%) is about washing hands with the right steps which take a long time. It can be concluded that respondents still have low knowledge about hand hygiene as an effort to prevent the spread of Covid-19. It is hoped that parents and schools will disseminate how to properly practice hand hygiene, and facilitate the availability of a place to wash hands that is equipped with hand washing soap and water flow.

Abstract

Introduction: Rheumatic Heart Disease (RHD) is a disease that rarely affects individuals. RHD occurs due to inflammation caused by group A streptococcal bacteria and can cause heart failure. RHD in Indonesia is still rare, and it is being discussed about how to handle it. The purpose of this case report is to describe nursing issues in people with RHD.Method: The case is about 43-year-old woman who has been diagnosed with RHD since January 2022. She was taken to the hospital with complaints of heavy breathing, and her main complaint when she was treated was a feeling of palpitations all the time, and she was easily tired. The main nursing problem was found, namely decreased cardiac output. The interventions provided were non-pharmacological therapy in the form of providing a semi-Fowler's position, a cardiac diet, education, and collaboration in providing pharmacological therapy.Results: After intervention for three meetings, the result was no longer feeling short of breath and increased patient and family knowledge about self-management of heart disease.Conclusion: This case report describes PJR patients by applying evidence-based practice, so it is hoped that it will be useful for health professionals to understand the assessment process for evaluation and especially how to treat patients with non-pharmacological therapy for nurses.

Abstract

Patients who are receiving treatment at the ICU have at least one symptom of gastrointestinal dysfunction including decreased bowel noise, abdominal distension, constipation, decreased frequency of defecation, and increased GRV (gastric residual volume). Non-pharmacological interventions that can be done to improve gastrointestinal function include abdominal massage. This paper aimed to conduct a literature review on the effects of abdominal massage to overcome gastrointestinal dysfunction in patients in the ICU. Articles taken from the Google Scholar and Science Direct electronic databases with the keywords abdominal massage, gastrointestinal function, bowel function, and ICU were published from 2015 to 2019. There were four research articles discussing abdominal massage to improve gastrointestinal function. Three articles used the Randomized Controlled Design method and one article used the Quasi Experimental method. Abdominal massage has a positive effect to improve gastrointestinal function in patients receiving treatment in the ICU. Improved gastrointestinal function that can be seen from the existence of a faster time in the reduction in gastric residual volume (GRV), decreased abdominal circumference, decreased gastric distension, and decreased the incidence of constipation. It is expected that abdominal massage can be applied by ICU nurses considering this intervention has the effectiveness in terms of time and funding.Keywords: Abdominal Massage, Gastrointestinal Function, Intensive Care Unit

Abstract

Valvular heart disease (VHD) adalah keadaan di mana salah satu katup di jantung mengalami kerusakan atau mati. Jika katup mengalami kerusakan, jantung tidak dapat memompa darah secara efektif ke seluruh tubuh dan harus bekerja lebih keras untuk memompa. Hal ini dapat menyebabkan gagal jantung, henti jantung, dan kematian. Penelitian ini menggunakan metode desain case report atau laporan kasus. Perawat melakukan manajemen syok kardiogenik yang meliputi pemberian oksigen, pemasangan IV line, pemasangan folley catether, kolaborasi pemasangan ventilator dan tindakan kardioversi dengan Dokter. Manajemen syok kardiogenik merupakan intervensi keperawatan yang tepat dilakukan pada pasien VHD dengan masalah keperawatan penurunan curah jantung.

Abstract

In general, Acute Respiratory Distress Syndrome or ARDS is acknowledged to be the cause of 10% opatients requiring ICU care, and about 3 million patients worldwide experience it. Despite the data, up to these days there haven’t been many treatments that are considered to be effective in preventing or treating ARDS.This research is a literature review with a narrative approach in which the researchers analyze several research articles.  The present study aims to analyze the management of ARDS in reducing mortality, improving lung function and airway effectiveness. The articles were searched based on the publication year 2016-2020, with the keywords “Acute Respiratory Distress Syndrome” AND “Oxygen Saturation”, “Acute Respiratory Distress Syndrome” AND “Treatment”, “Acute Respiratory Distress Syndrome” AND “Management”; as well as the databases of PubMed, EBSCO and American Journal of Clinical Pathology.Based on the 14 articles selected using inclusion criteria, it was found that there were 8 treatments given to the ARDS patients to reduce mortality, namely Non-Invasive Ventilation (NIV), the use of corticosteroids, UC-MSC (umbilical cord mesenchymal stem cells), oxygen therapy, mechanical ventilation, HFNC, prone position, open lung approach (OLA), and simvastatin.Of the 8 analyzed treatments, 3 interventions were found to have the highest significance of improvement in lung function and quality improvement in several indicators such as decreased mortality rate, increased saturation, decreased hospitalization time, etc. These interventions were: (1) Non-Invasive Ventilation (p value <0.001); (2) simvastatin (p value <0.001); (3) Corticosteroid (methylprednisolone) (p value <0.001).
