Abstract

Background: The application of patient safety goals is an effort made by hospitals to improve the quality of health services. Handover activities that are not accompanied by effective communication can cause errors that can harm the patient. Recommendations from WHO require nurses to improve communication patterns, especially when carrying out handovers, namely by using SBAR communication which consists of Situation, Background, Assessment and Recommendation. Based on the data summary of reported incidents that occurred in the hospital as many as 20 incident cases in the past year, including cases of KTD, KNC, KTC and KPCS. From these incidents, there were 6 cases related to miscommunication during handovers. Some nurses still do not apply the SBAR communication method when carrying out handovers, resulting in cutting/lagging of the information provided and resulting in incidents. Objective:to determine the reltionshipbetween SBAR Communication during handover and the implementation of patient safety at Bakti Timah Karimun Hospital. Methods: This research is quantitative with a cross-sectional approach. The population of this study were all inpatient nurses at Bakti Timah Karimun Hospital with total sampling, which consisted of 63 respondents. Results: showed that 41 respondents (65%) used SBAR communication during handovers in the good category and implemented patient safety. Conclusion:This means that the implementation of SBAR communication during the handover is good but not optimal because there are still 35% who have not implemented it. Based on the chi square test, there is a relationship between SBAR communication during handover and the application of patient safety (p=0.000 <0.05). Suggestion:, it is expected that nurses can carry out SBAR communication during handovers effectively so that they can implement patient safety optimallyKeywords : Newbron, Skin wrap, hypothermia.

The purpose of this study was to analyze nursing care for Mr. B with Stage 1 HIV at the Seroja Polyclinic, Gunung Jati Hospital, Cirebon City. The research design used was a descriptive case study including assessment, data analysis, nursing diagnoses, interventions, implementation and evaluation. This research was conducted in the seroja room of Gunung Jati Hospital, Cirebon City. Based on the results obtained, based on the assessment data, two nursing problems emerged, namely hyperthermia, related to the disease process marked by fever and secondly the risk of opportunistic infections associated with decreased immune system which was marked with a CD4 value close to 200. Actions taken were monitoring body temperature and collaborating on drug administration with doctors, providing health education about HIV, adherence to taking medication and advising regular eating patterns and adequate rest. It is recommended that health workers provide education about HIV/AIDS care and also provide medication taking assistance to prevent non-adherence in taking medication, social and spiritual.

The type of research used in this study was analytic with a cross-sectional approach. Cross sectional is a research approach that emphasizes the time of measurement/observation of independent variable data and depends only once at a time. The chi-square test results obtained a p value of 0.0050.005. This means that there is a significant relationship between the quality of the registration section and the satisfaction of pregnant women. Reliability where the p value = 0.000. Responsiveness with p value = 0.000. Assurance with a p value = 0.000. Empaty with p value = 0.002.

Abstract

Background; Progressive Mobilization Level I can affect blood pressure and oxygen saturation in critically impaired patients with reduced consciousness. Progressive mobilization can affect oxygen saturation, this is because after being given level 1 progressive mobilization in the Head of Bed position, gravity will pull the diaphragm down resulting in better lung expansion (distributing oxygen in the lungs) so that the oxygen bound by hemoglobin increases. increase in oxygen saturation values. Objective; Knowing the results of applying level I progressive mobilization to blood pressure and oxygen saturation in patients with decreased consciousness. Method; The application was carried out using a case study descriptive method to 2 respondents in patients with decreased consciousness for 3 days with a frequency of 2 times a day for 15 minutes. Results; Based on the results of the application that has been carried out, there is a decrease in blood pressure and an increase in oxygen saturation before and after the application of level I progressive mobilization. Conclusion; Level I Progressive Mobilization can be used as a non-pharmacological technique or independent intervention in patients with decreased consciousness to reduce blood pressure and increase oxygen saturation.

Methods: This research uses descriptive case studies by observing nursing care for diabetic ulcer patients using aloe vera gel at the Rasika Gunungkidul Wound Clinic with an application 6x and application is carried out every 2 days using the Bates-Jensen Wound Assessment Tool. Results: The results of the assessment score before application in respondent 1 score 32 points with categories wound degeneration and respondent 2 score 21 points with categories wound degeneration. After application with aloe vera gel the score on respondent 1 was 21 points with categories wound degeneration while on respondent 2 was 10 points with categories wound regenration. The two respondents had a decrease in score which indicated that the wound had progressed significantly. Conclusion: The application of aloe vera gel can be used as an effective non-pharmacological method for diabetic ulcer wound.
