Abstract

This study aims to analyse the defect rate of threaded reinforcing bars, identify the dominant defects, determine Six Sigma capability, and design conceptual improvements based on Poka Yoke. The method used is Six Sigma with the DMAIC approach, combined with the Poka Yoke concept, supported by observational data, interviews, and historical data collected over a 15-day period. The results of the study indicate that the production process is not yet stable, with an average value of 3.96 Sigma and a Defects Per Million Opportunities (DPMO) of 6,831.37. A Pareto analysis identified dents (74.25 per cent) and tears (15.60 per cent) as the dominant defects. The FMEA evaluation found that the root cause with the highest priority was the absence of machine initial setup records (RPN 392) and parameter sheets (RPN 343). As a solution, it is proposed to implement Visual and Administrative Poka-Yoke measures via a One-Point Lesson (OPL) board to prevent ‘blind setup’, as well as to make the completion of check sheets mandatory. This proposal is projected to reduce the RPN for tear defects to 128 and for dent defects to 112, which will subsequently be formalised in Standard Operating Procedures (SOPs).

Abstract

The design and implementation of a Direct Current (DC)-based Solar-Powered Street Lighting System (PJUTS) represents an alternative solution to improve energy efficiency while promoting the utilization of renewable energy on university campuses. This study aims to design and implement a DC-based solar-powered street lighting system for the campus environment of Soerjo University, Ngawi. The research methodology included field observation, system design, component selection, installation, and performance testing of the proposed system. The implemented system consists of a 20 Wp solar panel, a 12 V 7.5 Ah VRLA battery, a 10 A solar charge controller, and a 20 W DC LED lamp. The solar panel converts solar energy into electrical energy during the daytime, which is stored in the battery and automatically utilized to power the lighting system at night through the controller. The total cost of designing and installing the system was IDR 1,460,000. The results indicate that the system operates effectively and provides adequate illumination for the campus environment according to the planned specifications. System performance is influenced by several technical factors, including installation location, pole height, solar panel tilt angle, cable installation quality, and protection system suitability. Potential problems include short circuits, cable damage, and component performance degradation due to environmental conditions. Therefore, the use of standardized components and regular maintenance is essential to ensure system reliability, efficiency, and service life. This study is expected to contribute to the development of efficient, economical, and environmentally friendly solar-powered street lighting systems, particularly in educational institutions.

Abstract

Product quality is a key factor in improving the competitiveness of manufacturing companies. PT. X, a spunbond fabric manufacturer, continues to face product defects that affect production quality. This study aims to identify Critical to Quality (CTQ) characteristics, determine the dominant defect types, measure process capability using the Six Sigma method, and propose quality improvement recommendations. The study used all production and defect data as the population, with six months of production data (May–October 2025) selected through purposive sampling based on data availability and completeness. Data were analyzed using the Six Sigma approach, including CTQ analysis, Pareto diagrams, P-Charts, Defects Per Million Opportunities (DPMO), sigma level calculations, and fishbone diagrams. The results indicate that Black Cont (D1), Fil Drop (D7), and Cont Net (D3) are the dominant defects, contributing approximately 92% of total defects. The DPMO values ranged from 12,755,102 to 17,699,115, with sigma levels between 3.87 and 3.99, indicating that the production process performs reasonably well but still requires quality improvement. The novelty of this study lies in the integrated application of Six Sigma to prioritize quality improvement in spunbond fabric production. The findings provide practical recommendations, including improving operator competence, machine maintenance, SOP implementation, raw material quality control, inspection systems, and production environment management to reduce product defects.

Abstract

Holland Bakery is a company engaged in the bakery industry that sells bread directly to end consumers. The concept of the bullwhip effect is inherent in the supply chain, there is an effect that spreads in the supply chain called the bullwhip effect, Problems that occur in the supply chain of Holland Bakery products that are Perishable quickly damaged so that it causes demand in product inventory causing the bullwhip effect. The purpose of this study is to Identify indicators of the occurrence of the bullwhip effect in the Holland Bakery supply chain using a fishbone diagram and Determine alternative solutions to reduce the bullwhip effect based on the assessment weight of the Fuzzy AHP (Analytical Hierarchy Process) method criteria. The results of this study The results of the identification of the causes of the bullwhip effect in the Holland Bakery supply chain using the fishbone diagram approach there are 6 causes of the bullwhip effect based on the analysis of Demand forecasting, Order batching, Price fluctuations seen from 4 aspects: Humans (man), Methods (method), Materials (material), Machines (machine). The alternative solution selected based on the Fuzzy AHP weighting assessment yielded a weighting score of 0.7696. Alternative B was the first priority to be selected as an alternative solution to reduce the bullwhip effect in the Holland Bakery supply chain.

Abstract

This study aims to identify the potential risk of work accidents, determine the main causative factors, and develop risk mitigation strategies using the House of Risk (HOR) method in the gasket production area at PT XY. This research is motivated by the high potential for work accidents caused by the use of production machines, especially press machines that are not equipped with safety systems, as well as the implementation of risk management that still needs to be improved. The research uses a quantitative descriptive method by collecting data through direct observation and interviews with parties involved in the production process. The analysis was carried out using HOR Phase I to identify and determine the priority of risk agents based on the Aggregate Risk Potential (ARP) value, then continued with HOR Phase II to determine the most effective mitigation actions. The results of the study showed that there were 10 risk events and 14 risk agents that had the potential to cause work accidents. The main causative factors include high production targets, reduced operator concentration, machines without safety guards, non-compliance with SOPs, less than optimal preventive maintenance, damaged electrical cables, and inconsistent use of personal protective equipment (PPE). Prioritized mitigation strategies include increasing compliance with the use of PPE, implementing regular preventive maintenance, strengthening the implementation of SOPs through routine inspections, Occupational Safety and Health (K3) training, implementing the 5R/5S program, and installing safety sensors on press machines. The application of the HOR method provides a systematic approach in determining risk control priorities so that it can improve work safety in the production area.
