Abstract

Background: Stroke remains one of the leading causes of long-term disability worldwide, with hemiparesis being the most common motor deficit. Hemiparesis on the dominant side, such as sinistra hemiparesis, greatly impairs daily function. Motor Imagery Training (MIT) is a cognitive-based therapeutic approach that stimulates motor-related cortical areas through mental rehearsal of movement, offering a non-invasive, low-cost intervention to promote neuroplasticity during stroke rehabilitation. Objectives: This case study aims to evaluate the short-term effects of a 4-day Motor Imagery Training intervention on a post-stroke patient with hemiparesis sinistra, focusing on motor function improvement and cognitive engagement. Methods: Mrs SA, a 66-year-old female with a history of uncontrolled hypertension and prior stroke, presented with left-side weakness, facial asymmetry. The patient underwent MIT sessions for four consecutive days (30 minutes/day), guided by auditory scripts and visual imagery techniques targeting upper limb function. Motor performance was evaluated using the Fugl-Meyer Assessment for Upper Extremity (FMA-UE) and the Medical Research Council (MRC) scale. Results: After four MIT sessions, the patient demonstrated improved motor strength (MRC 4-/5), increased FMA-UE score from 29 to 36, and enhanced focus and engagement. Subjectively, the patient reported increased motivation and perceived movement initiation. These findings suggest early cortical activation and functional gains, even within a limited intervention period. Conclusion: This case supports existing evidence on the effectiveness of MIT in promoting neurofunctional recovery in stroke rehabilitation. Despite its brief duration, MIT contributed meaningfully to motor recovery and psychological readiness. Motor imagery is feasible for early rehabilitation and can be tailored to settings with limited resources.

Abstract

Background: Premature birth remains a leading cause of neonatal morbidity and mortality globally. Parents of premature infants often face challenges in providing proper care after hospital discharge due to limited knowledge, inadequate caregiving skills, and a lack of readiness. Health education plays a vital role in improving parental competency and ensuring safe and effective home care. Objectives: This study aimed to evaluate the effect of health education on improving the knowledge, caregiving skills, and readiness of mothers in caring for premature infants. Methods: A quasi-experimental pre-posttest design was conducted involving 25 mothers whose premature infants were admitted to the neonatal unit of RSI Siti Hajar Sidoarjo. The intervention included structured health education using printed leaflets and direct nursing demonstrations. Data were collected through validated questionnaires and observation checklists before and after the intervention. Statistical analysis was conducted using the Chi-square test with a significance level of p < 0.05. Results: Before the intervention, 76% of mothers demonstrated poor knowledge, 100% lacked caregiving skills, and 100% were unprepared for home care. Post-intervention, 76% exhibited good knowledge, 80% showed adequate caregiving skills, and 84% were ready to care for their infants. All improvements were statistically significant (p = 0.000). Conclusion: Health education significantly improved maternal knowledge, caregiving skills, and readiness in caring for premature infants. It is recommended that structured educational programs be routinely integrated into neonatal discharge planning to support safe and effective home care.

Abstract

Background: Stroke is a serious global health issue, ranking as the leading cause of disability and the second leading cause of death worldwide. One of the most common impacts of stroke is impaired physical mobility, particularly when damage occurs to the brain's motor centers or neural pathways that control body movement. In stroke patients, this impairment can manifest immediately after the acute phase and persist into the rehabilitation phase if not properly addressed. One effective intervention to overcome physical mobility problems in stroke patients is the application of a combination of Range of Motion (ROM) exercises and rubber ball gripping therapy. This approach not only accelerates motor function recovery but also prevents complications due to immobility and enhances patient independence. Objectives: This study aimed to analyze the application of combined ROM and rubber ball gripping therapy in managing physical mobility impairment in stroke patients, particularly in improving movement function and supporting a comprehensive rehabilitation process. Methods: This research used a descriptive case study design with a nursing care process approach to provide an in-depth overview of the intervention implementation in the patient. Results: The implementation of nursing interventions in the form of combined ROM exercises and rubber ball gripping therapy on Mr. E, who experienced physical mobility impairment due to a non-hemorrhagic stroke over six days, showed positive outcomes. Conclusion: The combined implementation of Range of Motion (ROM) exercises and rubber ball gripping therapy proved effective in improving muscle strength and motor function in ischemic stroke patients with impaired physical mobility. This intervention not only facilitated the recovery of movement but also supported patient independence and provided a feasible approach for continued rehabilitation at home.

Abstract

Background: Rapid and accurate initial management of emergency patients is crucial to prevent mortality and disability, especially in critical care units. However, variations in nurses’ knowledge and preparedness—often influenced by education, experience, and training—highlight the need to assess factors affecting their competence in emergency management. Objectives: This study aimed to assess nurses’ knowledge of initial emergency management and identify factors associated with their knowledge levels in the critical care units of Sumbawa Regional General Hospital. Methods: Analytical descriptive (cross-sectional) quantitative study conducted in June 2025. The target population comprised nurses working in the Emergency Room, ICU, NICU, and Surgical unit (N = 60). A total of 52 nurses participated (response rate 86.7%) and were included using convenience (accidental) sampling. Data were collected using a structured questionnaire assessing knowledge of initial emergency management. Knowledge was categorized as Good (≥75%), Sufficient (50–74%), and Lacking (<50%) based on the total score. Analysis included descriptive statistics and bivariate analysis using the Chi-square test (α = 0.05). Results: Of 52 respondents, 44.2% (n = 23) demonstrated Good knowledge, 25.0% (n = 13) Sufficient knowledge, and 30.8% (n = 16) lacked knowledge. Bivariate analysis showed significant associations between knowledge level and educational level (p = 0.041), length of work (p = 0.035), and previous emergency training (p < 0.001). Conclusion: Knowledge of initial emergency management among nurses at Sumbawa General Hospital is heterogeneous, with gaps associated with lower formal education, shorter work experience, and lack of training. We recommend regular, simulation-based, and blended BLS/PPGD training, periodic competence assessments, and hospital-level policies to ensure mandatory life-support certification for critical care nurses.

Abstract

Background: Pulmonary tuberculosis (TB) remains a major public health challenge in Indonesia, often leading to ineffective airway clearance due to excessive secretions. Non-pharmacological interventions, such as chest physiotherapy and deep breathing, are widely used; however, their effectiveness can be further enhanced through the Active Cycle of Breathing Technique (ACBT). Objectives: This study aimed to describe the nursing care process using ACBT and evaluate its effectiveness in improving airway clearance and respiratory function in a patient with pulmonary TB. Methods: A single-case descriptive study using an evidence-based nursing approach was conducted with an 82-year-old female patient diagnosed with pulmonary TB and ineffective airway clearance. ACBT was administered once daily for four consecutive days, each session lasting 15 minutes. Data were collected through observation, clinical documentation, and semi-structured interviews, and analyzed descriptively by comparing pre- and post-intervention respiratory parameters (respiratory rate, oxygen saturation, sputum characteristics, and lung sounds). Results: Clinical indicators showed progressive improvement. Respiratory rate decreased from 24 to 20 breaths/min, oxygen saturation increased to 97% on room air, and auscultation revealed reduced wheezing and rhonchi. The patient reported easier sputum expectoration and reduced dyspnea after four days of intervention. Conclusion: ACBT effectively enhanced airway clearance and respiratory function in a pulmonary TB patient. It is recommended as an evidence-based, non-pharmacological intervention for respiratory nursing practice.

Abstract

Background: Tuberculosis (TB) remains a major public health problem in Indonesia, where household environmental conditions, sanitation quality, and limited community awareness strongly influence transmission. Strengthening TB prevention requires integrated approaches that combine biomedical, behavioral, and environmental health strategies. The Community-Based Total Sanitation (STBM) program, alongside Contact Investigation (CI) and Tuberculosis Preventive Therapy (TPT), presents an opportunity for comprehensive community-level intervention. Objectives: This study aimed to evaluate the effectiveness of an integrated nursing education model combining clinical TB education, CI, TPT counselling, and STBM-based sanitation triggering in improving knowledge, hygiene practices, and environmental sanitation among TB-affected households in Jombang Regency. Methods: A quasi-experimental one-group pre–post design was conducted with 48 participants recruited purposively from households of active TB patients and at-risk community members. The intervention included structured TB education, symptom-based CI screening, TPT counselling, and participatory STBM triggering. Knowledge was assessed using a validated questionnaire, while hygiene behaviors and household sanitation conditions were evaluated through structured observations. Data were analysed using paired t-tests, McNemar tests, and effect size calculations. Results: Knowledge scores increased significantly from a mean of 54.76 to 74.67 (p < 0.001) with a very large effect size (Cohen’s d = 2.07). Hygiene practices, particularly handwashing with soap at critical times, showed significant improvement (p < 0.001). Environmental sanitation indicators, including improved latrine ownership and enhanced household ventilation, also demonstrated positive changes after the intervention. Conclusion: The integrated nursing education model effectively improved knowledge, hygiene behavior, and environmental sanitation among participants, demonstrating strong potential as a community-based TB prevention strategy. Although limited by the absence of a control group and a short follow-up period, the model is suitable for adoption in primary healthcare programs to strengthen holistic TB control efforts.
