Abstract

Vascular complications, particularly impaired peripheral perfusion as reflected by reduced Ankle Brachial Index (ABI) values and the occurrence of peripheral neuropathy, are frequently observed in individuals with type 2 diabetes mellitus (T2DM). These conditions significantly contribute to an increased risk of diabetic foot ulceration and lower limb amputation. Therapeutic exercise targeting the lower extremities represents a non-pharmacological preventive strategy, among which Heel Raise Exercise (HRE) is considered practical and feasible for routine implementation. This study aimed to evaluate the effect of HRE on ABI values and peripheral neuropathy scores in patients with T2DM. This study employed a quasi-experimental design with a nonequivalent control group approach. A total of 62 participants were recruited using purposive sampling from the service area of Buleleng I Primary Health Care Center. Group allocation into intervention and control groups was conducted through non-random assignment based on participant enrollment order and operational considerations at the study site. ABI measurements were performed using an 8 MHz Doppler ultrasound device in conjunction with a sphygmomanometer, while peripheral neuropathy was assessed using the Michigan Neuropathy Screening Instrument (MNSI). Statistical analysis involved paired t-tests to examine within-group differences before and after the intervention and independent t-tests to compare mean differences between groups. The results demonstrated that the intervention group exhibited an increase in mean post-intervention ABI values to 0.93, accompanied by a reduction in peripheral neuropathy scores to 2.89. In contrast, no statistically significant changes were identified in the control group. The mean differences in ABI (0.07 ± 0.04) and peripheral neuropathy scores (0.40 ± 0.61) in the intervention group were statistically significant. In conclusion, Heel Raise Exercise was shown to be effective in improving peripheral vascular status and reducing peripheral neuropathy severity in patients with type 2 diabetes mellitus.

Abstract

Trauma is a global health problem, which is a serious issue, claiming around 8% of all lives lost. Proper treatment is crucial, and accurate assessment is key. Coming hotfooting out of nowhere or being involved in a sudden accident are two of the most common causes.  Many methods have been established for the determination of trauma and for the guidance of treatment. This study attempted to compare the performance of NEWS, MREMS, and RTS in predicting death and ICU admission among trauma patients in Indonesia. This cross-sectional design analyzed 90 trauma patients admitted to the hospital in 2024. Patients aged ≥ 16 years with complete medical records were included. The instruments used to predict mortality and ICU admission were NEWS (National Early Warning Score), RTS (Revised Trauma Score), and MREMS. The analysis was conducted by comparing the predictive performance using AUCROC with the optimal threshold value by the Youden index. The Odds Ratio (OR) value was also determined. The analysis of this study was conducted using SPSS version 29. Interpretation results with p < 0.05 were considered significant. The results of this study analysis showed that the three scoring systems (NEWS, RTS, and MREMS) had excellent discriminatory ability to predict mortality (AUCROC ≥ 0.95, p < 0.001), with NEWS achieving the highest performance (AUCROC 0.979, sensitivity 0.966, specificity 0.883) compared to other instruments. In the ICU admission variable, the predictive performance results were lower, but NEWS still had a better value (AUCROC 0.816) compared to the others. This analysis concludes that the three instruments have acceptable discriminative power. NEWS is a superior instrument, as evidenced by better descriptive results for sensitivity and specificity in predicting trauma mortality and ICU admission. This finding further validates the usability of NEWS as an effective triage tool for predicting early risk of mortality and ICU admission in trauma patients.

Abstract

Metabolic syndrome is a cluster of risk factors for cardiovascular disease and type 2 diabetes mellitus in adults. The development of a health application-based instrument is important. This study aims to evaluate the Content Validity Index (CVI) and System Usability Scale (SUS). This study used a cross-sectional study design. Content validity was analyzed using the Content Validity Index (CVI) method involving five experts. Researchers evaluated the usability of the application using the System Usability Scale (SUS) and tested it on nine users. Researchers analyzed the data descriptively to obtain CVI and SUS scores. The test results showed that the I-CVI value was 1.00 and the S-CVI/Average was 1.00, indicating that all items in the application were highly relevant. The SUS score was 76.11, and the application was categorized as acceptable (good), indicating that the application was easy to use and well received by users. This mobile application has excellent content validity and good usability, making it suitable for use as an early detection instrument for metabolic syndrome. This application has the potential to support health promotion and prevention efforts to reduce non-communicable diseases in the community

Abstract

Gross motoric skill development in children is a basic component of children’s growth and their physical functional movement. This study was conducted using pilot quantitative study that evaluated the effectiveness of traditional Riau games in improving gross motoric skills, specifically balance, speed, and agility, among pre-school children. This study used a non-randomized, single-group pretest–posttest design without a control group, conducted on 50 participants. Gross motoric performance was assessed pre and post intervention by using an instrument of TGMD (Test of Gross Motoric Development). Data were analyzed using paired sample t-tests, with effect sizes calculated to estimate strong effects taken from this study. The results indicated statistically significant improvements in balance, speed, and agility following the intervention (p < 0.001), with a very large effect size (Cohen’s d = 2.39 ≥ 0.80). These findings suggest that traditional Riau games may represent a culturally appropriate and effective strategy for enhancing children’s gross motoric skills. Nevertheless, the absence of a comparator group and the pilot nature of the study warrant cautious interpretation, and further controlled trials are recommended to confirm these results.

Abstract

Diabetes Mellitus (DM) is the second most common disease in Indonesia after hypertension. Diabetes Mellitus can cause complications, one of which is diabetic ulcer. Diabetic ulcer can lead to amputation of the extremities, especially the lower extremities. Prevention measures that can be taken to prevent diabetic ulcer include foot care therapy. Foot care consists of three preventive measures, starting with education on foot care, diabetic foot exercises, and management of foot injuries/wounds. Foot care training was conducted for five weeks. The study aimed to determine the effect of Diabetes Foot Care Training on the foot care behavior of D patients. Foot care behavior was assessed using the Nottingham Assessment of Functional Footcare (NAFF) tool. The research was conducted over 5 weeks. The study involved 60 respondents divided into an intervention group and control group. The results of the control group showed a p-value > 0,005 (0,647), indicating no difference in foot care behavior in the control group. The Wilcoxon test for the intervention group showed a p-value < 0,005 (0,002), indicating a difference in foot care behavior before and after the intervention. The Mann Whitney test showed a p-value < 0,005 (0,027). The conclusion of the study states that Difoca intervention has an effect on improving foot care behavior in DM patients.

Abstract

Understanding the predictors of self-care behavior in heart failure (HF) patients is crucial for designing effective interventions tailored to patient needs. The situation-specific theory of HF self-care emphasizes the interaction between the person, problem, and environmental as factors influencing self-care. This study aimed to identify predictors of self-care behavior in HF patients by examining individual characteristics, disease-related variables, and environment influences. This cross-sectional study was conducted on 110 HF patients at the outpatient cardiac clinic of Kendari City Hospital, selected using consecutive sampling method. Self-care behavior was measured using the European Heart Failure Self-Care Behavior Scale-9. The predictor variables included person-related factors (age, gender, occupation, self-efficacy as measured by the CSES, and spiritual well-being as measured with the FACIT-Sp12), problem-related factors (HF duration and symptom severity), and environmental factors (social support as measured by the 12-item MSPSS). A Binary logistic regression was used to identify the significant predictors. Variables with p-value < 0.05 in multivariate analysis were considered significant predictors of self-care. Among the participants (65.5% male; mean age 62.27 ± 9.10 years), 66.4% reported inadequate self-care. Higher Self-efficacy (AOR = 1.35; 95% CI: 1.15–1.58) and greater social support (AOR = 1.08; 95% CI: 1.01–1.16) were significantly associated with better self-care. In contrast, HF duration ≥ 2 years (AOR = 0.66; 95% CI: 0.46–0.96) was associated with poorer self-care. These findings recommend that nursing interventions should integrate person, problem, and environmental components, with an emphasis on strengthening self-efficacy and enhancing social support to optimize self-care among HF patients.

Abstract

Depression in the elderly is a public health issue that is sometimes influenced by low social interaction, but community-based studies are still limited. This study aims to analyze how social engagement relates to depression severity among elderly. This study used cross sectional approach and recruited 56 elderly participants aged 60–72 years. Depression level measured by the 30 item Geriatric Depression Scale (GDS). Social engagement was assessed with the Lubben Social Network Scale (LSNS). Data were analyzed using descriptive statistics, Spearman correlation, and linear regression analysis. Participants averaged score are 13.70 on depression (SD=5.77) and 17.57 on social engagement (SD=2.79). We found a negative correlation: less social contact linked to higher depression level (r = –0.288, p = 0.031). Regression confirmed this pattern which social engagement predicted depression levels (B = –0.398, p = 0.002). Social engagement is important to reduce depressive sign and symptoms among the elderly. Community-based programs is needed to strengthen social support to improve mental health in elderly.

Abstract

Hypertension remains a common health problem in the elderly and often requires additional non-pharmacological interventions in addition to pharmacological treatment. This study aims to be able to evaluate the effect of progressive muscle relaxation combined with lavender aromatherapy on systolic and diastolic blood pressure in the elderly with hypertension. This quantitative research uses a pre-experimental design of one pretest-posttest group. The target population consists of elderly people with hypertension who live in Balapulang Wetan Village, Indonesia. A total of 23 participants were selected as research samples through proportional random sampling in all environmental units or RWs. All selected participants underwent progressive muscle relaxation exercises with a combination of lavender aromatherapy, performed once a day for three consecutive days. Lavender essential oil with 15 drops dissolved in 30 mL of water is given using a diffuser at each session. Blood pressure was measured before and after the intervention using a digital sphygmomanometer. Data was analyzed with a pair of sample t-tests. The results of the study indicate a statistically significant decrease in systolic and diastolic blood pressure after the intervention or p < 0.05. The combination of progressive muscle relaxation and lavender aromatherapy has been proven to reduce blood pressure in the elderly with hypertension and can be used as an effective non-pharmacological nursing intervention.
