Abstract

Background Adherence to fluid intake restrictions is a necessary part of therapy for hemodialysis patients in order to maintain the balance between fluid intake and output. However, many hemodialysis patients fail to comply with these restrictions. This non-adherence poses a significant problem, as it can negatively affect the patient’s condition. Family support is one of the key factors that plays an important role in encouraging patient adherence to therapy. With optimal support from family members, patients are more likely to comply with the recommended treatment or therapeutic process. Objective This study aims to examine the relationship between family support and adherence to fluid restriction among CKD patients undergoing hemodialysis at Sentra Medika Hospital, North Minahasa. Methods A quantitative study was conducted using a cross-sectional design. A total of 143 respondents were selected through purposive sampling. Data were collected using a family support questionnaire and a fluid restriction adherence questionnaire. Statistical analysis was performed using the Gamma correlation test. Results The results indicated that the majority of respondents reported a moderate level of family support (62.94%) and low adherence to fluid restriction (84.6%). The Gamma correlation test revealed a significant relationship between family support and adherence to fluid restriction (p = 0.000). Conclusion There is a statistically significant relationship between family support and adherence to fluid restriction in CKD patients undergoing hemodialysis. Abstract

Background: Hemodialysis is the primary therapy used to remove metabolic waste from the blood in patients with chronic kidney disease (CKD). Although hemodialysis is crucial for the survival of CKD patients, the procedure has several physical impacts, one of which is fatigue. This fatigue can contribute to low adherence to the prescribed hemodialysis regimen. Objective: This study aimed to determine the relationship between fatigue and adherence in chronic kidney disease patients undergoing hemodialysis. Methods: This research used a quantitative approach with a cross- sectional design. The sample consisted of 137 hemodialysis patients selected using purposive sampling. Results: Based on the statistical analysis using Spearman’s rho correlation test, the result showed a p-value of 0.019 (p < 0.05), indicating a significant relationship between fatigue and adherence. The correlation coefficient was r = -0.200, which indicates a weak negative correlation. This means that the higher the level of fatigue experienced by patients, the lower their adherence tends to be. Conclusion: There is a significant relationship between fatigue and adherence in chronic kidney disease patients undergoing hemodialysis at Sentra Medika Hospital, North Minahasa. Keywords: Fatigue, Adherence, Chronic Kidney Disease, Hemodialysis.

Abstract

Abstract Background. Acute Decompensated Heart Failure (ADHF) causes decreased cardiac output to respiratory failure requiring mechanical ventilation. However, this condition causes increased secretions due to the inability to cough spontaneously, so suction is needed to maintain a patent airway. Pre- and post-suction hyperoxygenation is important to avoid complications of hypoxemia and hemodynamic disorders. Objective. This scientific paper aims to analyze nursing care providing pre- and post-suction hyperoxygenation interventions in ADHF patients with nursing problems of Ineffective Airway Clearance. Method. This scientific paper is made in the form of a case study with a sample of Mr. FM. Implementation is carried out by administering hyperoxygenation for 1 minute pre-post suction. Evaluation using a bedside monitor by monitoring hemodynamic parameters such as Mean Arterial Pressure (MAP), heart rate (HR) and oxygen saturation (SpO2). Results. The results of the intervention for 2 days of administering 1 minute pre- and post-suction hyperoxygenation showed changes in oxygenation status, hemodynamic parameters, and the amount of airway secretions. Conclusion. Based on the evaluation results, hyperoxygenation for 1 minute pre and post suction was proven effective in maintaining oxygenation and hemodynamic stability within the normal range in ADHF patients with ventilators as seen from the MAP, HR and SpO2 parameters. Keywords: Acute decompensated heart failure, case study, hyperoxygenation, suction.
