Abstract

Fraud in the national health insurance is a form of deliberate effort to create a benefit that should not be enjoyed by individuals or institutions and could harm other parties. This study aims to further analyze the potential for fraud in the implementation of national health insurance at health facilities. The method used a systematic review based on PRISMA with a qualitative approach through descriptive analysis. The articles selection based on the specified inclusion and exclusion criteria. The study found that the potential for fraud can occur in the primary care health facilities (FKTP) and secondary care health facilities (FKRTL) in the form of potency such as inappropriate capitation fund management, maximizing the number of claims, upcoding, dissatisfaction with the salaries received, and inadequate internal controlling/supervision. As the conclusion of this study, the potential fraud in the implementation of national health insurance in Indonesia occurs in the primary care health facilities (FKTP) and secondary care health facilities (FKRTL). The prevention of potential fraud can be done by implementing the principles of Corporate Governance and the implementation of fraud prevention based on the Regulation of the Minister of Health Number 16 of 2019.

Abstract

Several previous studies show that adherence to hormonal therapy increases the survival rate in Breast Cancer (BC) with ER+ or PR+. Tamoxifen is one of oral chemotherapy which is the most used hormonal therapy in breast cancer treatment regimens. The focus of the study was to determine the level of adherence and factors influencing the adherence of the treatment of BC patients undergoing hormonal therapy with Tamoxifen in patients at Dharmais Cancer Hospital in the year 2018. The study is an observational study with a cross-sectional design. The study was followed by 109 respondents. Medication adherence levels assessed via questionnaire modified MARS-5. Sociodemographic and clinical characteristics obtained from interviews using structured questionnaires. Medication adherence’s level of Tamoxifen was 90.9% with the proportion of adhering patients was 75.2% (82 of 109 patients). Multivariate analysis showed that the age of respondents, level of education, level of knowledge is the most influential factor and determine the level of treatment adherence. Education level is the variable that has the greatest effect on medication adherence. Through this study, it is recommended to pay more attention to patients with specific characteristics such as patients with low education and or low income.

Abstract

Nurse anesthetist are health workers who are competent to carry out anesthetic administration care in hospitals. The quality of the anesthetic administration care provided reflects the quality of the hospital. Anesthesiologists in carrying out their competence are in a work setting with high stressors. Therefore, it is necessary to pay attention to the Quality of Work Life in the anesthesiologist. The purpose of this study was to photograph the Quality of Work Life of anesthesiologists in a hospital. This study uses a quantitative descriptive method. The study population was all anesthesiologists working in the hospital. The research sample used simple random sampling method with a total of 108 respondents. The instrument used is a questionnaire that is given online using four dimensions, namely the dimensions of life at work/home, the dimensions of work design, the dimensions of the work context and the dimensions of the world of work. The analysis used is univariate using a mean value of 105. Data collection using gform, The results obtained are the mean value of 124.28. For each dimension, the dimensions of life at work/home are 20.03, the work design dimension is 29.08, the context dimension is 61.70, and the world of work is 13.46. The conclusion is that the Quality of Work Life of the anesthesiologist in the hospital is in the good category. Suggested in this study is the development of human resources that lead to a high workload and a great risk of anesthesia

Abstract

Patient safety is important because people have easy information and hospital reports related to medical errors. Patient identification is the most important cause of patient safety incidents. In 2019, 12 near misses due to misidentification of patients in the inpatient, laboratory, and pharmacy at the private class D hospital in Blitar. This prompted the authors to conduct a case study and analyze the causal factors of the problems. The method used in this research is descriptive and case study approach. Identification of the root problems was done by observation and interviews with implementing inpatients rooms nurses using observation sheets. Determination of the root problems is determined through a two-stage focus group discussion (FGD) using a fishbone diagram. Six root problems were found that affect the low compliance in the implementation of patient identification in inpatient installations, they are low supervision and supervision from the leadership, no resocialization regarding patient identification, a small number of patients, lack of safety culture, lack of supporting tools, and patients’ willingness to be served quickly. Knowing the root factors that affect the officers’ compliance is essential so supervision methods that are appropriate to the conditions of the hospital can be conducted.

Abstract

COVID-19 is a global health problem. The number of positive cases keep increasing globally towards the end of 2020, including in Indonesia. This is a challenging situation for the healthcare workers who provide services to the patient without knowing whether the patients infected by COVID-19 or not, especially for those who work in emergency unit of the hospital. This study aims to explore the stressors and coping strategies among emergency unit healthcare workers in Tanjungpinang, Indonesia. The sample was taken from the emergency unit of Raja Ahmad Tabib Kepulauan Riau Regional Public Hospital as the only referral hospital in Tanjungpinang. Data was collected through interview and then analyze by using thematic analysis approach. The result shows that the source of stress that the emergency unit healthcare workers feel during Covid-19 pandemic is the fear of contacting the disease, fear of infecting family members, as well as negative perceptions from the public. As for the coping methods, it was found out that the emergency unit healthcare workers adhere to health protocol, adding insight on Covid-19 related issues, performing religious activities, adopting healthier lifestyle, seeking social support, and doing relaxing activities in the spare time.

Abstract

Distance to a health care facility has long been discussed as a key barrier to maternal health care utilization in rural areas. Maternity Waiting Homes (MWHs) has been proved to the reductions of maternal mortality. The objective of this study was to investigate the pregnant women’s Willingness to Pay (WTP) toward the maternity waiting homes (MWHs). The cross-sectional study was conducted in two rural areas in South Sulawesi, Indonesia. The data were collected through survey which was distributed to pregnant women and their families. A total of fifty-one pregnant women and forty-nine families participated in the study as two groups of respondents. For the results of this study, there were 66.7% of pregnant women agreed if they should stay at MWHs. For a group of pregnant women, the amount of WTP which was capable to be paid was 100.000 IDR in a month. Moreover, for a group of family, the amount of WTP which they could pay for staying at MWH was 200.000 IDR in a month. The pregnant women’s acceptance to stay in MWH is good, but their willingness to pay is still very low.

Abstract

Nursing turnover is problem for hospitals around the world, including at XYZ Hospital in Tangerang, Indonesia. Studies on the causes of turnover are required to reduce turnover effectively and efficiently. We aimed to study the effect of interprofessional collaboration and transformational leadership on turnover intention, with job satisfaction as intervening factor. This associative quantitative study gathered primary data obtained through survey on nurses at XYZ Hospital, Tangerang in April 2021, using Turnover Intention Scale-15, Interprofessional Collaboration Scale, Transformational Leadership Behavior Inventory, and McCloskey/Mueller Satisfaction Scale. Analysis was performed with Structural Equation Modeling (SEM) method. From a total of 95 subjects, we found that there was significant negative effect of Interprofessional Collaboration (β = -0.560, p = 0.004) and Transformational Leadership (β = ‑0.271, p = 0.031) on Turnover Intention through Job Satisfaction as intervening factor (β = -0.560, p = 0.004) and significant negative effect of Job Satisfaction on Turnover Intention (β = 0.808, p = 0.001). Also found was positive effect of Interprofessional Collaboration (β = 0.692, p <0.001) and Transformational Leadership (β = 0.335, p = 0.032) on Job Satisfaction. Therefore, promoting job satisfaction, interprofessional collaboration, and transformational leadership may be beneficial in reducing nursing turnover intention.

Abstract

Factors that influence the work satisfaction of medical record officer is the work environment and work stress. Uncomfortable work environment and work stressing with a lot of work demands make the staff will experience severe pressure. Based on the preliminary study on April 1st, 2018, it's found that seven officers who often absent (above 10%), small workspace, awkward movement, inadequate facilities, double jobs, and double shifts that made 10 out of 13 officers experience unenjoyable in their work. The purpose is to analyze the effect in the work environment and work stressing on the work satisfaction of medical record officers in the X Hospital. It is analytical with a cross-sectional approach using ordinal logistic regression and questionnaire as an instrument. The sample used is 13 medical record officers—the work environment and work stressing as independent variables and work satisfaction as dependent variables. The significance value of the work environment and work stress variable is 0.002. It has a significance value <α (P <0.05), proving that the work environment and work stressing have a significant effect on work satisfaction. This study's suggestions need to be the expansion of workspace, additional facilities, and officers to avoid double jobs and double shifts.

Abstract

Indonesia is a disaster-prone country, so research on hospital preparedness for disasters is necessary. One of the tools to measure the readiness of hospitals in facing disasters using the Hospital Safety Index (HSI) from WHO. This study aims to determine the readiness of hospitals to deal with disasters using HSI. Research location in PKU Yogyakarta, PKU Bantul, PKU Kotagede, PKU Nanggulan, and PKU Wonosari. These PKU that have been accredited by the Hospital Accreditation Committee in Yogyakarta Province. The approach used is descriptive qualitative. The independent variable is the HSI from WHO, and the dependent variable is the Hospital Accreditation Committee (Komite Akreditasi Rumah Sakit/KARS) accredited hospital. The respondents are the person in charge of the structural part of the disaster management system. The checklist refers to the HSI form from WHO. Each statement provides three types of answers, namely low, medium, and high. All statements are a total number of low scores, medium scores, and high scores using the HSI calculator. The results of the study are that hospitals with plenary accreditation have higher HSI scores so that they prepared to face disasters than hospitals that have not been plenarily accredited.

Abstract

A frequent problem in hospitals is nurse turnover, which reaches 27.3% annually in Indonesia. The solution to this problem is increasing spirituality intelligence through the formation of spirituality in the workplace. Good spirituality in the workplace can increase productivity and organizational commitment, and reduce turnover. The study was conducted to determine the effect of spirituality in the workplace on nurse turnover intention. The study used a quantitative design with a cross-sectional approach. The sample used was 92 nurses who were determined by Proportionate Random Sampling. Data were processed using Partial Least Square (PLS). The PLS results showed T statistics of 8.927 with direct coefficients of -0.423. The application of spirituality in the workplace shapes nurses’ perceptions more positively towards hospitals, so that, nurse turnover intention is low.
