Abstract

Hypertension remains a major non-communicable disease and a public health concern in Indonesia, with psychological stress recognised as an important modifiable risk factor. This study aimed to analyse the association between stress level, promotive efforts, and preventive efforts with the incidence of hypertension at Dandy Dinda Clinic, Tembung. A quantitative observational study with a cross-sectional design was conducted from May to June 2026. The population consisted of 107 hypertensive patients recorded between January and May 2026, and 84 respondents were selected using the Slovin formula with a 5% margin of error. Primary data were collected through structured questionnaires, including the Perceived Stress Scale (PSS-10), while secondary data were obtained from clinic records. Data were analysed using univariate distribution and the Chi-Square test at a 95% confidence level. The results showed that most respondents experienced moderate stress (59.5%) and stage 1 hypertension (61.9%). The Chi-Square test indicated a significant association between stress level and hypertension (p=0.006), promotive efforts and hypertension (p=0.024), and preventive efforts and hypertension (p=0.009). These findings suggest that stress management, health education, and preventive behaviours should be strengthened at the primary care level to support hypertension control.

Abstract

Type 2 diabetes mellitus (t2dm) is a chronic disease caused by insulin resistance and insufficient insulin production, resulting in elevated blood glucose levels. Proper blood glucose control can prevent both microvascular and macrovascular complications of diabetes. Effective management is therefore needed to monitor blood glucose levels through healthy lifestyle modifications, including a healthy diet and physical exercise. This study aimed to determine the difference between healthy diet education and physical exercise in monitoring blood glucose levels among patients with t2dm at darussalam public health center, medan. This quantitative study employed a quasi-experimental two-group pretest-posttest design. The population consisted of 294 patients with t2dm, from which 44 participants were selected using purposive sampling. Data were collected using an observation sheet to record blood glucose levels among patients with t2dm. The data were analyzed using paired t-test and independent t-test. The results showed a significant difference in blood glucose levels between the healthy diet education and physical exercise groups, with a p-value of 0.000 (p < 0.05). The healthy diet education intervention resulted in a decrease in blood glucose levels of 23.86 mg/dl, while the physical exercise intervention resulted in a mean decrease in blood glucose levels of 52.72 mg/dl. There was a difference of 28.86 mg/dl in the change in blood glucose levels between the healthy diet education and physical exercise groups, with a p-value of 0.000. It can be concluded that there was a significant difference between healthy diet education and physical exercise interventions in monitoring blood glucose levels. It is recommended that the public health center establish regular physical exercise programs and strengthen support for dietary management by developing a non-communicable disease prevention community through healthy lifestyle programs. Posbindu health cadres are expected to serve as facilitators in implementing these programs.

Abstract

Background: Acute myocardial infarction (AMI) is a severe manifestation of coronary artery disease, commonly caused by atherosclerotic plaque rupture and thrombus formation. Dual antiplatelet therapy with aspirin and P2Y12 inhibitors is a cornerstone of AMI management but remains limited by bleeding risk and nonspecific drug distribution. Platelet membrane-coated nanoparticles (PM-NPs) are biomimetic drug delivery systems with potential for targeted therapy. Objective: To review the potential of PM-NPs for delivering aspirin and P2Y12 inhibitors in AMI management. Methods: A literature review was conducted using PubMed, ScienceDirect, and Google Scholar with the keywords “platelet membrane-coated nanoparticles,” “aspirin,” “P2Y12,” and “acute myocardial infarction.” Articles published within the last 10 years, available in full text, and written in English or Indonesian were selected according to predefined criteria. Twelve relevant articles were identified after screening. Results: PM-NPs demonstrated potential for targeting thrombi, atherosclerotic plaques, and injured myocardial tissue. They may improve drug stability, circulation time, and targeted delivery while reducing oxidative stress, inflammation, and ischemia-reperfusion injury. However, evidence regarding combined aspirin and P2Y12 inhibitor delivery remains limited and predominantly preclinical. Conclusion: PM-NPs represent a promising targeted antithrombotic platform for AMI. Further studies are required to establish the safety and efficacy of combined aspirin and P2Y12 inhibitor delivery.
