Abstract

Fatigue is a common symptom experienced by many patients undergoing hemodialysis, whether due to situational, physiological, or long-term factors. Foot massage is the manipulation of connective tissue by rubbing, squeezing, or patting, which improves circulation, improves muscle tone, and provides a relaxing effect. The purpose of this study was to determine the application of foot massage therapy with olive oil to reduce fatigue levels in patients with chronic kidney disease (CKD) undergoing hemodialysis. This study used a pre-test-post-test case study design. The sampling consisted of two respondents who met the inclusion criteria. The instrument used was the 13-question Functional Assessment of Chronic Illness Therapy-Fatigue Scale (FACIT-Fatigue Scale) questionnaire, administered before and after the intervention.The study found that patients experienced changes in fatigue levels after receiving foot massage therapy with olive oil for two days. The fatigue level changed from an initial score of 32-23 (severe-moderate) to 18-13 (moderate-mild) on the first day. On the second day, the score decreased from 19-18 (moderate) to 16-14 (mild).

Abstract

Thought content distortion in schizophrenic patients with grandiose delusions is a major challenge in clinical psychiatric nursing care. This study aims to identify the effectiveness of applying reality orientation interventions (including Cognitive Behavioral Therapy for Psychosis and Metacognitive Training) in improving the thought content of patients with grandiose delusions. Through a systematic literature review method based on the PRISMA guidelines, 10 selected international articles published between 2021 and 2026 were analyzed narratively. The review results indicate that reality orientation interventions are proven effective in reducing delusional beliefs and improving cognitive insight, not by directly confronting delusional conclusions, but by modifying how patients process sensory data and test reality within their environment. These findings are relevant for the development of psychiatric nursing care standards focused on thought content improvement, aiming to accelerate patient recovery in integrated mental health care facilities. The implementation of structured and sustained interventions is expected to improve thought content, enhance adaptive functioning, and support the patient recovery process in integrated mental health care facilities.

Abstract

Patients who have undergone cholecystectomy generally experience acute pain due to the surgical incision, which, if not properly managed, can reduce comfort, hinder mobility, slow healing, and lower quality of life. One nonpharmacological therapy that can be administered is Benson relaxation therapy. This study investigates medical-surgical nursing care for patients undergoing post-operative laparoscopic cholecystectomy with the nursing issue of discomfort and pain. It employs a case study research design in the form of family-centered nursing care, involving 1 research subject with a medical diagnosis of cholelithiasis. Data collection instruments included an observation sheet and the Numerical Rating Scale (NRS). Results: Administration of Benson relaxation therapy once daily for 3 days, each session lasting 10 minutes, led to a reduction in pain scores when evaluated on the third day. Specifically, the first patient’s pain score was 6 before the intervention and decreased to 2 after the intervention. Conclusion: Benson relaxation therapy can reduce acute pain in patients following cholecystectomy. The results of this study can serve as a routine therapy for patients to perform when experiencing pain.

Abstract

 The implementation of clean and healthy living behavior, especially handwashing using handwash, is one of the important efforts in preventing diarrheal disease among preschool children. Diarrhea remains a common health problem in children due to low awareness of maintaining hand hygiene before eating and after activities. This study aimed to determine the implementation of evidence-based nursing on handwashing using handwash as an effort to prevent diarrhea at TK Asuhan Ibu. The research method used was a descriptive case study with a community nursing care approach. The subjects of this study were preschool children at TK Asuhan Ibu who had improper handwashing habits. Data were collected through interviews, observations, and documentation. The intervention was carried out through health education, demonstrations, and re-demonstrations of the six-step handwashing technique using handwash. The assessment results showed that most children did not wash their hands before eating and were unable to perform the six steps of handwashing correctly. After the intervention, there was an improvement in the children’s knowledge and ability to perform proper handwashing using handwash. The children appeared to better understand the importance of handwashing as an effort to prevent diarrhea and began to develop the habit of washing their hands before eating and after playing. Support from teachers and families also increased in encouraging clean and healthy living behaviors among children. The implementation of handwashing education using handwash was effective in improving clean and healthy living behavior and served as a preventive effort to reduce the risk of diarrhea among preschool children at TK Asuhan Ibu.

Abstract

Skin irritation is one of the most common health problems experienced by farmers due to pesticide exposure during spraying activities. This study aimed to analyze the relationship between the duration and frequency of pesticide spraying and skin irritation complaints among farmers in Raanan Baru Village, West Motoling District. This quantitative analytical study employed a cross-sectional design and was conducted from March to April 2026. A total of 55 respondents were selected from 286 active pesticide-using farmers using a purposive sampling technique. Data were collected through structured questionnaire-based interviews and analyzed using the Spearman's rank correlation test with a significance level of p < 0.05. The results showed that the average spraying duration was 2.35 hours per day, while the average spraying frequency was 2.38 times per week. There was a significant positive correlation between spraying duration and skin irritation complaints (r = 0.315; p = 0.019) and between spraying frequency and skin irritation complaints (r = 0.346; p = 0.010), with both correlations classified as weak. The findings indicate that longer spraying duration and higher spraying frequency are associated with increased skin irritation complaints among farmers.

Abstract

Background: Human Immunodeficiency Virus (HIV) remains a major public health problem affecting both mothers and children due to the risk of mother-to-child transmission during pregnancy, childbirth, and breastfeeding. HIV screening among pregnant women is an essential strategy to prevent vertical transmission. Objective: This study aimed to analyze the association between maternal knowledge, family support, and HIV screening among pregnant women at Kolongan Community Health Center, North Minahasa Regency. Methods: A quantitative study with a cross-sectional design was conducted from February to May 2026. Eighty pregnant women were selected through purposive sampling from a population of 473 pregnant women. Data were collected questionnaires and analyzed using univariate analysis and Spearman rho correlation test. Results: Most respondents had good knowledge (mean=3.69) and received good family support (mean=39.71). Bivariate analysis showed a significant association between maternal knowledge and HIV screening (p=0.030; r=0.242), as well as between family support and HIV screening (p<0.001; r=0.385). Conclusion: Maternal knowledge and family support were significantly associated with HIV screening among pregnant women. Suggestion: Strengthening health education and involving family members, particularly husbands, are recommended to improve HIV screening coverage among pregnant women.

Abstract

Background: Carpal Tunnel Syndrome (CTS) is a compression neuropathy of the median nerve at the wrist, commonly associated with repetitive occupational activities. Rubber farmers represent a high-risk group due to the continuous, repetitive hand motions involved in rubber tapping. Objective: To analyze factors associated with CTS among rubber farmers in Tuindrao Satu Village, South Nias Regency. Methods: A cross-sectional study was conducted in March 2026. The total population of 53 rubber farmers was recruited, yielding 38 respondents via total sampling. Data were collected through structured interviews, questionnaires, and anthropometric measurements. Bivariate analysis was performed using the Chi-Square test at a 95% confidence level (α = 0.05). Results: The prevalence of CTS history was 65.8% (n = 25). Repetitive motion was the sole factor significantly associated with CTS incidence (p = 0.044). Age (p = 0.222), body mass index (p = 0.362), awkward hand posture (p = 0.122), and ergonomic working position (p = 0.576) showed no significant associations. Conclusions: Repetitive motion during rubber tapping is the primary occupational risk factor for CTS in this population. Targeted ergonomic interventions, including scheduled rest breaks and stretching exercises, are recommended to mitigate CTS risk among informal agricultural workers.

Abstract

Background: Gestational Diabetes Mellitus (GDM) is a pregnancy-related condition that can increase the risk of maternal and neonatal complications. The management of GDM requires adequate self-care practices, including dietary regulation, physical activity, glucose monitoring, adherence to antenatal care, and stress management. Family-based health promotion is needed to strengthen mothers’ ability to perform self-care continuously. Objective: To analyze the relationship between family-based health promotion and self-care among pregnant women with GDM at Mitra Sejati Hospital. Methods: This study employed a quantitative analytical approach with a cross-sectional design. A total of 75 pregnant women with GDM were selected using consecutive sampling. Data were analyzed using univariate and bivariate analyses with the Chi-Square test at a significance level of α = 0.05. Results: Most respondents received family-based health promotion in the moderate category (42.7%) and demonstrated good self-care (57.3%). Among respondents who received good family-based health promotion, 80.0% demonstrated good self-care, whereas only 27.8% of those who received inadequate health promotion demonstrated good self-care. The Chi-Square test showed a statistically significant relationship between family-based health promotion and self-care (p = 0.003). Conclusion: Family-based health promotion is significantly associated with self-care among pregnant women with GDM.

Abstract

Non-communicable diseases (NCDs) are the leading cause of global mortality, and behavioural risk factors including physical inactivity, unhealthy diet, tobacco use, and harmful alcohol consumption are highly prevalent among working-age adults. Digital health promotion has emerged as a scalable strategy to address these risks, but the evidence base is heterogeneous. This literature review aimed to synthesize evidence on digital health promotion strategies for the prevention of NCDs among working-age adults, including intervention approaches, behavioural and health outcomes, implementation characteristics, and research gaps. A structured search was conducted in PubMed, Scopus, Web of Science, and ScienceDirect, complemented by Google Scholar, focusing on peer-reviewed articles published between 2021 and 2026. Findings indicate that mobile health applications, wearable activity trackers, text-messaging platforms, social-media campaigns, and telehealth-supported programmes are the most frequently deployed modalities. Digital interventions produced small-to-moderate improvements in physical activity, dietary quality, weight, glycaemic control, and short-term smoking abstinence, particularly when they incorporated behaviour-change techniques such as goal setting, self-monitoring, feedback, and social support. Effectiveness was moderated by engagement, adherence, and digital health literacy, while sustainability and equity remained key concerns. Digital health promotion offers a promising complement to conventional strategies for NCD prevention among working-age adults, and future research should prioritize long-term outcomes, equity, and integration into workplace and primary-care settings.

Abstract

Background: Shoulder sprain is a ligament injury of the shoulder joint resulting from repetitive work activities, non-ergonomic postures, and heavy lifting. Oil palm harvesters represent a high-risk group for this injury because harvesting is performed manually using long-handled tools with the hands raised above shoulder level repeatedly. Musculoskeletal disorder prevalence in the plantation sector continues to rise, and shoulder-specific injuries remain understudied. Objective: This study aimed to analyze the relationship between ergonomic factors (work posture, workload, length of employment, and work duration) and the incidence of shoulder sprain among oil palm harvesters in Namu Ukur Village, Langkat Regency, 2026. Methods: A quantitative cross-sectional study was conducted. The population comprised all 30 oil palm harvesters in the study village, selected via total sampling. Data were collected using REBA for posture assessment, observation sheets for workload, questionnaires for work duration and employment length, and the Nordic Body Map for shoulder sprain detection. Data were analyzed using Chi-Square test (α = 0.05). Results: Twenty respondents (66.7%) experienced shoulder sprain. Significant associations were found between work posture (p=0.017), workload (p=0.007), length of employment (p=0.045), and work duration (p=0.000) with shoulder sprain incidence. Conclusion: All examined ergonomic factors were significantly associated with shoulder sprain. Comprehensive ergonomic interventions including posture training, workload reduction, job rotation, and provision of more ergonomic harvesting tools are recommended to reduce shoulder injury prevalence among oil palm harvesters.
