Abstract

Chronic kidney disease (CKD) is a global public health issue, with its prevalence increasing annually. CKD involves a pathophysiological process with diverse etiologies, such as uncontrolled diabetes, uncontrolled hypertension, or chronic glomerulonephritis, which lead to progressive kidney function decline, requiring patients to undergo routine hemodialysis. This study aims to determine the relationship between the Albumin- Creatinine Ratio (ACR) and the frequency of hemodialysis in patients with chronic kidney disease. This research employs a systematic review method, collecting data from various research journals available on Google Scholar and PubMed. The results indicate that CKD patients were predominantly male and were most vulnerable between the ages of 40-59 years. Hemodialysis was typically performed 2-3 times per week. The longer the duration of hemodialysis treatment, the more significant its impact on the albumin and creatinine ratio. While the albumin-creatinine ratio can be used to identify early kidney disease, its relationship with albumin and creatinine examination parameters was weak The albumin-creatinine ratio (ACR) test can serve as an early diagnostic support for chronic kidney disease. Further research is needed on the quality of life of CKD patients based on the duration of hemodialysis. Additionally, there should be increased awareness among CKD patients about their dietary patterns, particularly protein intake.

Abstract

Adolescence is a time of transition from childhood to maturity, and as such, it requires self-control and psychological care because it is undoubtedly going through a process of self-discovery. Routine, education, leisure, and worry are all impacted by the COVID-19 pandemic, which impacts children's and adolescents' mental and psychological health. We used secondary data to investigate how the COVID-19 pandemic has affected children's and teenagers' mental health. 1182 people from different educational institutions in Delhi-NCR, ranging in age. Additionally, a survey of 56 US states was conducted. The paper used associative research techniques to determine how the COVID-19 pandemic affected various age groups' performance on online tests, health problems during social separation, and stress management. The survey's findings also revealed a yearly percentage increase in the difficulty of focusing, remembering, and making judgments in 56 US states using correlation and basic linear regression analysis. Better educational advice is needed for adolescents, as this is thought to have an impact on their mental health.

Abstract

Chronic migraine significantly impacts quality of life and remains a therapeutic challenge, with many patients experiencing limited relief from traditional treatments. Greater occipital nerve (GON) blockade has emerged as a potential intervention, utilizing bupivacaine to alleviate migraine frequency and severity. This meta-analysis assesses the efficacy of GON blockade in reducing headache days per month in patients with chronic migraine. A meta-analysis was conducted, encompassing four randomized, placebo-controlled studies published between 2014 and 2024. The studies included a total of 202 participants (105 in the intervention group and 97 in the placebo group) who received GON blockade with bupivacaine or a placebo. The primary outcome measured was the mean difference in monthly headache days between the intervention and placebo groups. Statistical heterogeneity across studies was also assessed. The pooled analysis revealed a statistically significant reduction in monthly headache days in the GON blockade group compared to placebo, with an average difference of -5.7 days (95% CI: -8.5 to -2.9; p < 0.0001). Individual studies demonstrated consistent reductions in headache days in the intervention group, with mean differences ranging from -1.1 to -12.8 days. Heterogeneity was moderate (I² = 30.3%), indicating some variability across studies but an overall trend favoring the intervention. This meta-analysis supports the efficacy of GON blockade with bupivacaine as a promising treatment option for chronic migraine, showing a meaningful reduction in headache frequency. While GON blockade demonstrates short-term benefits, further research with extended follow-up periods and larger, more diverse samples is recommended to evaluate its long-term effectiveness and generalizability. These findings suggest that GON blockade may offer substantial relief for patients suffering from chronic migraines, providing a potential alternative for those who have not responded to conventional therapies.

Abstract

Mild Cognitive Impairment (MCI) is a transitional phase between normal cognitive aging and dementia, characterized by noticeable cognitive decline that is not severe enough to interfere with daily life. Recent studies suggest that probiotics, particularly Bifidobacterium breve, may play a role in enhancing cognitive function in older adults with MCI.This literature review synthesizes findings from multiple studies published between 2018 and 2024, focusing on the effects of Bifidobacterium breve supplementation on cognitive improvement.Seven articles were identified that addressed the impact of Bifidobacterium breve on cognitive functions in older adults with MCI. These studies utilized various methodologies, including randomized controlled trials and open-label studies, and consistently reported positive outcomes in cognitive performance. The results indicate that Bifidobacterium breve supplementation can enhance cognitive functions by modulating the gut microbiota, reducing inflammation, and promoting neuroprotective effects. These findings suggest a potential therapeutic avenue for managing cognitive decline in older adults. The evidence supports the hypothesis that Bifidobacterium breve may improve cognition in older patients with MCI, highlighting the need for further research to confirm these findings and explore the underlying mechanisms.

Abstract

Glaucoma is one of the leading causes of irreversible blindness worldwide, characterized by progressive damage to the optic nerve and visual field loss. Automated perimetry is an important method for detecting and monitoring the progression of glaucoma. However, conventional static perimetry has limitations, especially in advanced glaucoma cases where visual sensitivity is often below the threshold of detection. Moving stimuli have been proposed as an alternative or complementary method to overcome these limitations, by exploiting different visual mechanisms. This review aims to analyze the efficacy and variability of moving stimuli compared with static stimuli in glaucoma perimetry. A systematic literature review was conducted using four online databases: PubMed, Scopus, Google Scholar, and Cochrane. The PICO framework was used, with glaucoma patient population, intervention as moving stimuli, comparison with static stimuli, and outcomes evaluated including sensitivity, test-retest variability, and patient preference. The articles analyzed were published between 2011-2023, in English, and used study designs such as RCT, cohort, case-control study, or cross-sectional. Three relevant studies were found. Moving stimuli showed significant advantages in increasing sensitivity, especially in advanced visual field loss, extending the dynamic range to below 19 dB compared to static stimuli. Test-retest variability was also 44% lower with moving stimuli, indicating higher reliability. Most patients (62%) preferred moving stimuli due to greater comfort. However, there are limitations such as generalizability of results to a broader clinical population and the need for an updated normative database. Moving stimuli have significant advantages in glaucoma perimetry, including increased sensitivity, reduced variability, and better patient preference. These results demonstrate the potential of moving stimuli as an alternative or complementary method to static perimetry in the diagnosis and monitoring of glaucoma. Further studies are needed for validation in a broader clinical population as well as integration of moving stimuli into automated perimetry systems.

Abstract

To evaluate the effectiveness of spectacle lenses with aspherical lenslets in myopia control. Literature retrieval on PubMed, Cochrane Library, Embase, and Web of Science databases was conducted until December 29, 2023. Studies were selected based on predefined inclusion and exclusion criteria, and RevMan 5.3 software was used for meta-analysis. A total of four randomized controlled trials (RCTs) were included in this meta-analysis, involving 770 myopic children. The results demonstrated that spectacle lenses with aspherical lenslets significantly reduced the progression of myopia in children when compared with single vision spectacle lenses (SVLs) (WMD = -0.21 D, 95% CI: -0.41, -0.01, p < 0.05). However, no significant difference was found in the control of axial length (AL) growth in myopic children (WMD = -0.10 mm, 95% CI: -0.21, 0.01, p = 0.07). The results indicated that spectacle lenses with aspherical lenslets were more effective in controlling the progression of myopia (OR = 0.73, 95% CI: 0.52, 1.20, p < 0.001) and delaying the growth of AL (OR = 44.25, 95% CI: 8.84, 221.58, p < 0.001) compared to SVLs. Spectacle lenses with aspherical lenslets can effectively control the progression of myopia compared with SVLs, but they do not significantly delay the growth of AL.

Abstract

Toric multifocal intraocular lenses (TMIOLs) are an important innovation in cataract treatment, especially in patients with astigmatism. TMIOLs are designed to improve multifocal vision while correcting astigmatism, providing significant benefits to patients after cataract surgery. A literature search was conducted in four major databases, namely PubMed, Scopus, Google Scholar, and Cochrane. Included studies were observational studies published between 2010 and 2023. Search keywords included “multifocal toric intraocular lens” OR “toric IOL” AND “visual outcomes” OR “patient satisfaction.” Inclusion criteria included adult patients after cataract surgery with or without astigmatism who underwent TMIOLs implantation. A total of five relevant studies were analyzed. The results showed that TMIOLs significantly improved distance and near visual acuity without additional aids, with a patient satisfaction rate of 94%. Side effects such as glare and halos were reported in a small number of patients, but newer lens technologies are able to minimize these effects. The use of TMIOLs has been shown to be effective in improving visual efficacy and patient satisfaction levels after cataract surgery. Despite the potential for side effects, the overall benefits of TMIOLs outweigh the risks. Further research is needed to evaluate long-term efficacy and develop better lens technologies

Abstract

Human Herpesvirus 4 (HHV-4), also known as Epstein-Barr virus (EBV), is a member of the herpes virus family that has significant genetic variation. Molecular studies show that this variation can occur in genes that code for capsid proteins, DNA-binding proteins, and transcription-regulating proteins. In silico methods are used to determine mutations and genetic variations. The purpose of this study was to explore genetic variability in the LMP-1 HHV 4 gene sequence from NCBI PopSet 2581709082 using RFLP in silico. Restriction enzyme screening using restriction enzyme digest of DNA sites. RFLP in silico using the Benchling site. Genetic variability using the MEGA 11 application. The results obtained in silico screening of restriction enzymes and RFLP showed arelationship between PopSet 2581709082 genetic variation and HHV 4 LMP-1 genes, marked by the presence of five alleles, namely A1, A2, A3, A4, and A5.

Abstract

This study reveals symbolic violence perpetrated by the state in the name of food estates. Several theories used to examine the forms of symbolic violence that occurred include Pierre Bourdieu's theory of symbolic violence and Jurgen Habermas's theory of deliberative democracy. The study employed qualitative methods. The results revealed two forms of violence perpetrated by the state: psychological violence, and symbolic violence. In food estate programs and policies, symbolic violence operates through inequality in the relationship between the community as the controlled party, the state as the party that controls the community but is also controlled by companies, and companies as the party that controls the government, resulting in food estate programs and policies that have proven to fail repeatedly. In fact, legal regulations were created to accommodate the food estate program. In the context of democratizing livelihoods, the food estate program, from the era of President Soeharto to President Jokowi, represents a dismal reflection of democracy in Indonesia. The lack of public space is characterized by the absence of dialogue between citizens and state administrators in choosing methods for protecting and managing food resources. Deliberative democracy must be the government's path to addressing Indonesia's food crisis and ending the symbolic violence perpetrated in the name of food estates. At the same time, actions must be taken to overthrow the dominant discourse (Doxa) regarding food estates by adopting a new discourse (Heterodoxa) based on local knowledge and traditional practices. This new discourse must be able to fill the public space, which must be guaranteed by the government.
