Abstract

Mental health is a state of physical, mental, and social well-being that enables individuals to function optimally. Mental disorders such as schizophrenia can lead to distortions of reality, one of which is delusion, defined as a belief that is not consistent with reality but is strongly maintained. Delusions can disrupt thought processes, emotions, and behavior, thus requiring appropriate management. One of the nursing interventions that can be applied is reality orientation therapy. This study aims to describe the implementation of nursing care in a patient with a thought process disorder: grandiose delusion. This study used a case study design involving one patient with schizophrenia who had a nursing diagnosis of grandiose delusion, conducted over three days. The results showed that the implementation of the Implementation Strategy (SP) through reality orientation therapy improved the patient’s ability to orient to person, place, and time, and gradually reduced the intensity of delusional symptoms. The conclusion indicates that reality orientation therapy is effective as a nursing intervention in managing patients with grandiose delusions

Abstract

Children with intellectual disabilities have limitations in self-care, attention, memory, communication, and fine motor coordination. These conditions may affect their ability to maintain dental hygiene, particularly toothbrushing, which is an essential daily self-care activity. This case study aimed to describe the application of the backward chaining technique to improve dental hygiene independence among children with intellectual disabilities. The study involved two children at SDLB Bhakti Luhur Maumere who had difficulty brushing their teeth independently. Data were collected through direct observation, nursing assessment forms, toothbrushing skill checklists, and daily SOAP progress notes. The intervention was implemented for six consecutive days by training the children from the final step of toothbrushing to the initial steps, supported by simple instructions, demonstration, gradual assistance, and positive reinforcement. The results showed improved toothbrushing skills in both children, although the level of progress differed according to their motor ability, concentration, and understanding of instructions. The children became more able to hold the toothbrush, follow simple instructions, perform several toothbrushing steps, and reduce dependence on assistance. The findings indicate that backward chaining can be used as a structured nursing intervention to improve dental hygiene independence in children with intellectual disabilities.

Abstract

The three critical risk factors for adolescent reproductive health (TRIAD KRR) is an important issue that requires more attention. PIK-R staff are agents of change and act as partners who can influence young people themselves. Therefore, there is a need to improve staff knowledge about the TRIAD KRR by providing reproductive health education as a key strategy in efforts to improve PIK-R staff knowledge about the TRIAD KRR. This study aims to analyze the impact of health education on the knowledge level of PIK-R program staff about this triad. The study used a quasi-experimental design with a single group and pre- and post-tests. The sample consisted of 15 PIK-R program staff selected using a large-scale sampling method. The statistical analysis in this study was based on the Wilcoxon Signed-Rank test. The results showed a statistically significant difference in the knowledge level of the participants before receiving the health education program, with 10 participants (66.7%) having a good level of knowledge and 5 participants (33.3%) having a sufficient level of knowledge. After the program, 12 participants (80%) demonstrated a good level of knowledge, while 3 participants (20%) demonstrated a sufficient level of knowledge. Statistical analyses showed a statistically significant difference (p = 0.001, p < 0.05) between the level of knowledge before and after the health education program on the KRR TRIAD model. Therefore, it can be assumed that the health education had a significant impact on the knowledge of the participants in the PIK-R program on the TRIAD KRR model

Abstract

Insomnia in the elderly is a health problem that impacts quality of life and cognitive function. Non-pharmacological interventions such as physical activity are effective alternatives in improving sleep quality. This study aims to analyze the effectiveness of elderly exercise and ergonomic exercise on changes in the insomnia scale in the elderly and to compare the effectiveness of the two interventions. This study used a quasi-experimental design with a two-group pretest-posttest. The sample consisted of 40 elderly people divided into two groups, each with 20 respondents. The intervention was carried out for four weeks with a frequency of three times per week. Measurements used the Insomnia Severity Index (ISI). Data analysis was performed using paired sample t-tests and independent t-tests. The results showed a significant decrease in insomnia scores in the elderly exercise group (p=0.001) and ergonomic exercise group (p=0.000). In addition, the results of the independent t-test showed a significant difference in effectiveness between the two groups (p=0.023), where ergonomic exercise was more effective than elderly exercise in reducing insomnia scores. The conclusion of this study indicates that both interventions are effective in reducing insomnia in older adults, but ergonomic exercise provides more optimal results. These findings can inform the development of community-based nursing interventions to improve sleep quality in older adults

Abstract

Maintaining preventive health behaviors among adolescents remains a significant challenge in building a resilient society. Compliance driven solely by external regulations is often temporary and fragile. To achieve sustainable behavioral transformation, a deep understanding of how policy frameworks interact with adolescent internal motivation is required. This study aims to analyze the relationship between policy support and adolescent motivation in internalizing long-term preventive behaviors. A quantitative study with a cross-sectional design was conducted on 427 adolescents selected through purposive sampling. Data were collected using structured questionnaires and analyzed using Pearson correlation and linear regression tests to measure the strength of the relationship between variables. The analysis revealed a mean policy score of 17.90 and an adolescent motivation score of 39.50. A significant positive correlation was found between policy and adolescent motivation (p=0.001; r=0.431). These results indicate that inclusive and effective policies act as catalysts, enhancing adolescents' internal drive to independently comply with health standards. Public policy should no longer be viewed as an instrument of coercion, but rather as a supportive ecosystem that fosters internal motivation. Sustainable transformation of preventive behavior requires policy approaches that empower adolescents, rather than merely restricting them. Future health strategies must focus on integrating participatory policies with the strengthening of adolescent health autonomy, ensuring that preventive behavior becomes an ingrained aspect of their daily lives.

Abstract

The kidneys are a pair of vital organs roughly the size of a fist and account for 1% of the total human body weight. Chronic kidney disease (CKD) is a medical condition in which kidney function progressively declines, resulting in the body’s inability to perform metabolism efficiently. One of the treatments for chronic kidney disease is hemodialysis, which aims to address the temporary or permanent loss of kidney function. The process requires vascular access to circulate blood, typically through the insertion of a needle into an arteriovenous fistula. This procedure causes a pain response that can lead to anxiety and fear during hemodialysis. This study discusses a case analysis regarding the management of CKD patients using acupressure at the LI-4 point to reduce pain intensity during the AV fistula cannulation process in the Hemodialysis Unit at Parikesit Tenggarong Regional General Hospital. The innovative intervention involved applying acupressure at the LI-4 point to alleviate pain for 2–3 minutes prior to and immediately before the AV fistula puncture. The study results showed a significant difference in patients’ pain scores after the acupressure intervention was applied. On the first day, the pain score decreased from 5 to 2; on the second day, from 4 to 2; and on the third day, from 4 to 1. The use of acupressure at the LI-4 point effectively reduced the intensity of pain experienced by patients during the AV fistula needle insertion process

Abstract

Dementia is a cognitive disorder that commonly occurs in older adults and may affect memory, concentration, thinking ability, communication, and daily activities. One non-pharmacological intervention that can be implemented to help maintain cognitive function in older adults is Group Activity Therapy (GAT). This study aims to describe the implementation of a picture-guessing therapy in improving the cognitive function of older adults. The method used was Group Activity Therapy with a picture-guessing game approach, conducted among older adults at PSTW Budi Mulia 2. The results showed that the participants were able to follow the activity well, appeared more active and focused, and demonstrated improvements in memory and concentration. In addition, this therapy had a positive impact on the participants’ mood and social interaction. Therefore, picture-guessing Group Activity Therapy can be considered a simple, safe, and applicable non-pharmacological nursing intervention to help maintain cognitive function and improve the psychosocial well-being of older adults.

Abstract

Pulmonary tuberculosis is a chronic, recurrent infectious disease that usually affects the lungs, and can even affect all organs. Pulmonary TB is spread through the air from person to person. When a person with pulmonary TB coughs, sneezes, or spits, TB germs are released into the air, which can be inhaled by others, potentially infecting them. Psychological distress in families with members with pulmonary TB can also occur due to fear of contracting TB, worry about the lengthy treatment process, and even concern about the social environment where negative stigma toward pulmonary TB persists. This literature review was conducted by searching the Google Scholar database and selecting articles based on inclusion criteria, resulting in the selection of 10 primary articles for analysis. Family anxiety in caring for a family member with pulmonary TB is a manifestation of multiple psychological burdens. The majority of families indicated that despite having the resilience to care, they remain emotionally distressed. Family anxiety in caring for a patient with pulmonary TB is a complex phenomenon triggered by a combination of a lack of accurate medical information, economic pressures, gender, age, and minimal social support in the surrounding environment. Education should not only focus on patient medication schedules but also address family anxiety management. Furthermore, it's crucial for healthcare facilities to initiate support groups for patient families to share emotional burdens and prevent caregivers from feeling socially isolated. Keywords: abstract keywords
