Abstract

Timely reporting of critical laboratory values is essential for patient safety, service quality, and clinical decision-making. However, Amanda Hospital in North Cikarang recorded 684 critical laboratory results in 2024, of which 79 were reported beyond the established 30-minute standard. This study aimed to evaluate the reporting of critical laboratory values, examine its implications for service quality, identify process waste, and develop an improved reporting model. A qualitative phenomenological design was employed. Data were collected through interviews with five informants, direct observation, and reviews of standard operating procedures, medical records, patient progress notes, and quality reports. Three experts participated in validating the proposed model. Data were analyzed thematically using NVivo 12 Plus, while lean management principles and the DMAIC framework were applied to evaluate process efficiency. The findings revealed inconsistent compliance resulting from reporting delays, incomplete documentation, difficulties in contacting attending physicians, high service workloads, and nonstandardized communication practices. Lean analysis streamlined the laboratory examination process from 12 to 9 stages, reduced the critical-value reporting process from 5 to 4 stages, and shortened the mean examination time from 21.2 to 17.35 minutes. The proposed model incorporated automated notifications, structured documentation, clearly defined responsibilities, and systematic evaluation. In conclusion, integrated communication systems and lean process redesign can improve reporting timeliness, service quality, operational efficiency, and patient safety.

Abstract

Reliable utility maintenance supports hospital safety and continuity of services; however, damage reporting systems may remain ineffective when reported problems are not translated into timely corrective actions across hospital units. This study examined the effects of the damage reporting system and IPSRS technician competence on reporting system effectiveness, with technician response as a mediating variable, at Pasar Minggu Regional General Hospital. A mixed-methods design combined a survey of 210 employees selected through purposive sampling with interviews involving 10 informants. Quantitative data were analyzed using covariance-based structural equation modeling (CB-SEM) with IBM SPSS Amos, while qualitative findings were used to strengthen the interpretation of the quantitative results. The damage reporting system significantly improved reporting system effectiveness (? = 0.436; p < 0.001) and technician response (? = 0.157; p = 0.002). Technician competence did not have a significant direct effect on reporting system effectiveness (? = 0.144; p = 0.058), but it had a strong positive effect on technician response (? = 0.709; p < 0.001). Technician response significantly improved reporting system effectiveness (? = 0.380; p < 0.001) and mediated the effects of both the damage reporting system and technician competence on reporting system effectiveness. Interview findings confirmed that complete reporting information, diagnostic competence, and prompt corrective action were essential, while limited competence in managing condensation problems in central air-conditioning systems hindered repair effectiveness. Therefore, effective damage reporting requires integrated improvements in reporting quality, technician competence, and the speed and accuracy of technician responses.

Abstract

Nurses’ performance is essential for service quality and patient safety; however, inconsistent rewards, delayed corrective actions, limited professional development, and varying levels of engagement may constrain performance in private hospitals. This study examined the effects of transactional leadership and continuance organizational commitment on nurses’ performance, with work engagement as a mediating variable, at a Class B private hospital in Jakarta. A quantitative cross-sectional study was conducted from March to July 2026 among 119 nurses selected through purposive sampling from a population of 170 nurses. Data were collected using a four-point Likert-scale questionnaire and analyzed using descriptive statistics, confirmatory factor analysis, structural equation modeling (SEM) with AMOS, and Sobel tests. Transactional leadership, continuance organizational commitment, and work engagement collectively explained 67.7% of the variance in nurses’ performance. Transactional leadership positively affected work engagement (? = 0.434; p < 0.001) and nurses’ performance (? = 0.219; p = 0.004). Continuance organizational commitment demonstrated the strongest effects on work engagement (? = 0.642; p < 0.001) and nurses’ performance (? = 0.472; p < 0.001), while work engagement positively affected nurses’ performance (? = 0.353; p < 0.001). Work engagement partially mediated the relationships between transactional leadership and performance, as well as between continuance organizational commitment and performance. The findings demonstrate that integrated leadership practices, organizational commitment, and employee engagement strategies are essential for improving nurses’ performance.
