Abstract

Patent ductus arteriosus (PDA) is a ductus arteriosus that cannot be closed spontaneously after birth due to failure of physiological closure of the fetal ductus. The initial pharmacological treatment for PDA is ibuprofen. The other drugs that can be given, such as paracetamol which has the same role as ibuprofen by inhibiting the synthesis of prostaglandins. Several studies have compared the effectiveness and side effects of the two drugs and obtained different results have shown that paracetamol cannot be used in all premature infants. This literature review was conducted to determine the comparison of paracetamol and ibuprofen in premature infants diagnosed with patent ductus arteriosus. From the analysis of 15 journals, the comparison of the effectiveness of paracetamol and ibuprofen in premature infants has the same effectiveness and there is no statistically significant difference. But the effectiveness of ibuprofen is much better in premature infants with very low birth weight (VLBW) and extremely low birth weight (ELBW) infants than paracetamol. The adverse effects that are often caused by ibuprofen are gastrointestinal bleeding, thrombocytopenia, increased BUN levels, increased serum creatinine levels, increased incidence of AKI, hyperbilirubinemia, and oliguria. Meanwhile, other side effects such as BPD, IVH, NEC, ROP, sepsis, SGOT and SGPT levels and death were not found to be statistically significant differences in the two drugs.

Abstract

Psoriasis is a chronic recurrent inflammation of the skin caused by excessive activity of the immune system. Based on data from WHO psoriasis cases are 0.09% - 11.43%, while in Indonesia it reaches 2.5% of the total population. Of the various types of psoriasis, the most common is plaque psoriasis or psoriasis vulgaris, up to 90%. The severity of moderate to severe plaque psoriasis is a problem because it affects both physical and psychological life. This study review aims to determine the effectiveness and safety of treatment with secukinumab in moderate to severe plaque psoriasis. Secukinumab is the choice because the main target of this biological agent is to neutralize IL-17A. Secukinumab is available in Indonesia and was approved by the FDA in 2015. Parameters of treatment success were seen from the PASI, DLQI and BSA scores. Side effects that often occur are nasopharyngitis, skin infections, upper respiratory tract infections and gastrointestinal disorders.

Abstract

One of the physiological changes in pregnant women in the third trimester is the gradual musculoskeletal changes associated with the occurrence of low back pain due to increased body weight and changes in body posture. One of the new technologies in physical therapy is the use of elastic therapeutic bandages. This literature review aims to determine the effectiveness of elastic therapeutic taping (ett) against LBP in the third trimester of pregnancy. Of the 11 studies reviewed by the authors, it was shown that ETT support for low back pain relief was usually installed in the lumbar region 5 (L5) and sacrum (S1) with a band pattern I. ETT can be done 2 times a week and the ETT is replaced 3 days every 3 weeks for best results. optimally with a duration of 24-72 hours. ETT can reduce pain scores more significantly when combined with other therapies such as Paracetamol plus ETT, TENS plus ETT, ETT with oblique pelvic exercises, back exercises with ETT. The conclusion is that ETT can relieve low back pain and have a very real effect in reducing low back pain intensity in the last trimester of pregnancy.

Abstract

The habit of people who consume coffee will have an impact on increasing blood pressure, where if you drink 4 or more cups of coffee in one day, the systolic pressure will increase by approximately 10 mmHg and around 8 mmHg in diastolic blood pressure. The purpose of this study was to see the relationship between the habit of consuming milk coffee and black coffee with blood pressure in students of the Faculty of Medicine at Krida Wacana Christian University. This research used a cross-sectional approach with a population of 2020 Ukrida Faculty of Medicine students. The sample in this study was 102 people with a simple random sampling technique. It was found that the average 2020 Ukrida Faculty of Medicine student consumed coffee 1-2 times a day at a rate of 1-2 cups per day, but there were also some who consumed 3-4 cups of coffee a day. Data analysis used the Person Chi-Square and obtained the p-value for milk coffee: 0.126 and the p-value for black coffee: 0.656. So this shows that both milk coffee and black coffee have no significant relationship with an increase in blood pressure.

Abstract

Telemedicine as an online consultation tool is also developing in Indonesia. in addition, Indonesia currently does not have a clear legal umbrella for telemedicine issues. This literature review aims to provide an overview of telemedicine services in Indonesia from an ethical perspective. Of the 7 studies reviewed by the authors, there are various expert opinions that differ from an ethical perspective regarding telemedicine, so that it can be concluded that telemedicine can be used to provide health services, but priority must be given to the application of ethical and legal principles. Inappropriate use of telemedicine will cause problems for the patient and the doctor himself. Problems that can occur such as the problem of demands given by patients to doctors in this case malpractice. Respect for ethical and legal principles must always be obeyed by every doctor because it saves doctors from lawsuits and demands while at the same time reflecting the personality of the doctor as a noble and noble profession of all time. It is necessary to make regulations related to telemedicine services in the medical field and the need for further research related to the extent of the use of telemedicine in Indonesia.

Abstract

Understanding the immune response to COVID-19 is critical to help develop appropriate management. Understand the description and response of the immune system that is formed in humans through COVID-19 infection. Conduct a critical review of research journals on the immune response to SARS-CoV-19. Search for articles in research journal databases using the keywords COVID-19, SARS-CoV-2, and Immunity. COVID-19 patients have increased serum cytokine levels (TNFα, IL-2, IL-6, IL-10, IFNγ) compared with healthy patients. IL-6, IL-10, and CRP increased significantly with disease severity. The number of T cells decreases in COVID-19 patients, especially those who are more severe. There were no significant differences in antibody titers based on gender, age, or BMI in post-vaccine individuals. Smokers had significantly lower antibody titers after the second dose of vaccination. COVID-19 patients experience a decrease in the number of lymphocytes and an increase in CRP and cytokines such as TNFα, IL-2, IL-6, IL-10, and IFNγ, especially in severe cases. A positive correlation exists between CRP and IL-10. The number of T cells such as CD4+T and CD8+T also decreases in COVID-19 patients, especially those who are more severe. This could be a diagnostic and prognostic marker for COVID-19.

Abstract

Cockroach is one of the types of insects that exists everywhere. Cockroach can live in a lot of places, such as the gutter, the trash, and even in the bathroom. Cockroach is one of the agents that is responsible for the spreading of diseases such as diarrhea, dysentery and cholera in children. In Indonesia, there exists a lot of different types of cockroach, one of it is the German Cockroach or Blattella germanica L., and the cockroach problem in Indonesia is mainly handled by using chemicals insecticides, this happens because there’s a lot and its commonly found in a lot of places. Pyrethroid based chemicals insecticides are one of the most common chemical insecticides used by the public. These chemical insecticides have various kinds of adverse effects because they leave residues that can be inhaled by humans and also consumed by humans through food products. Some of the effects of chemical insecticides are diseases of the liver, kidneys, and also problems with the central nervous system. Therefore, an alternative solution is needed in dealing with the cockroach problems in Indonesia, namely Bioinsecticides. Bioinsecticides are insecticides made using natural ingredients without the addition of chemical substances. Ginger is one of the spices that can be found anywhere, in ginger there are compounds called flavonoids, and these compounds can be used as bioinsecticides. Bioinsecticides are made by taking the ginger extract then putting the extract into a spray bottle according to the dose using a pasteur pipette.

Abstract

Bacteria are one of the component parts in every ecosystem. Staphylococcus aureus (S. aureus) is a normal surface flora of the human body, as a temporary contaminant on inanimate objects such as clothing, cutlery and doorknobs, but also a oportunistic even pathogen on the host. The mutated strain of S. aureus has a cross-resistant strain against all antibiotics of the Beta Lactam, Methicillin, Oxacillin and Flucloxacillin groups, called Methicillin Resistant Staphylococcus aureus (MRSA). This research is needed because MRSA is a strain of bacteria that can be everywhere, especially in public facilities, such as doorknobs, which can be a place of transmission (transfer) of bacteria from one host to another. The samples in this study were taken by swabbing from the door handle in building X, Jakarta. A total of 62 samples were then cultured on Mannitol Salt Agar (MSA) media, the growing isolate was tested by Gram staining, catalase test, and mannitol. Isolate suspected of Staphylococcus aureus, ascertained whether MRSA strain using 30 μg antibiotic C efoxitin (fox) disc. The results of this study showed that 58 (87%) isolates were Staphylococcus aureus and sensitive to C efoxitin antibiotics, 8 (13%) isolates, mnitol test negative, sensitive to Ceroxitin antibiotics. The study concluded that the bacterial isolate found was Staphylococcus aureus sensitive methicillin (MSSA), no MRSA was found.

Abstract

Peripheral Arterial Disease (PAP) requires good evaluation to reduce amputation rate. The aim of this study is to assess non-invasive detection methods in evaluating microvascular conditions in PAP. The potential use of Near Infrared Spectroscopy (NIRS) in identifying disorders in patients with PAP, either with or without DM, was also assessed. A systematic literature search was conducted through PUBMED until October 2023. Based on the literature review, it can be concluded that the use of NIRS in evaluating microvascular status in PAP is a non-invasive testing method that has expanded its role. Evaluation with NIRS provides metrics such as recovery time, deoxygenation, oxygen consumption (VO2), tissue oxygen saturation (StO2), total haemoglobin (HbT), and oxyhemoglobin area under the curve (O2 HbAUC). NIRS also has the potential to assist multidisciplinary teams in treating high-risk patients, especially in diabetic patients whose symptoms are compensated by peripheral neuropathy. This study shows that NIRS can be used as a useful tool in evaluating microvascular status in PAP, by providing important information that can be used to optimise patient care and prevent more serious complications. It is concluded that NIRS can be used as a tool to evaluate the effectiveness of treatment and prevent worsening of the condition of patients with PAP in diabetic foot.
