Abstract

Chronic Kidney Disease (CKD), or kidney failure, is a progressive and irreversible renal dysfunction in which kidney function in maintaining metabolism and fluid and electrolyte balance declines, resulting in uremia. In 2025, there were 368 patients with chronic kidney disease (CKD) in West Sumatra, and 52% of them underwent hemodialysis. The purpose of this study was to determine the relationship between physical activity and quality of life in Chronic Kidney Disease (CKD) patients in the Hemodialysis Unit of Dr. Reksodiwiryo Hospital, Padang, 2025. This study was a quantitative study using a cross-sectional approach. Data collection was conducted from August 4-22, 2025, at the Hemodialysis Unit of Dr. Reksodiwiryo Hospital, Padang. The population of this study was 106 Chronic Kidney Disease (CKD) patients admitted to the Hemodialysis Unit of Dr. Reksodiwiryo Hospital, Padang, from January to March 2025. The sample size was 51 respondents using purposive sampling. Data were analyzed using univariate and bivariate methods using Chi-Square statistical tests. The results showed that more than half (66.7%) had a poor quality of life, and more than half (54.9%) reported light physical activity. There was a relationship between physical activity and quality of life in Chronic Kidney Disease (CKD) patients at the Hemodialysis Unit of Dr. Reksodiwiryo Hospital, Padang (p=0.028). The results of this study concluded that there is a relationship between physical activity and quality of life in Chronic Kidney Disease (CKD) patients. Hospital heads and ward heads are expected to provide information through health education on the relationship between physical activity and quality of life in patients with Chronic Kidney Disease (CKD), so that patients can adapt to their current situation.

Abstract

Butterfly pea (Clitoria ternatea L.) is a plant that contains several bioactive compounds, including flavonoids and anthocyanins, which can act as natural antioxidants. The level of antioxidant activity depends on the type of solvent used in the extraction process. This review article talks about evaluating the antioxidant activity of butterfly pea flower extracts based on the use of different solvents using the DPPH technique. The approach taken is a literature review from various publications discussing butterfly pea flower extraction using ethanol and water solvents through maceration, remaceration, infusion, soaking, and decoction methods. The results show that the IC50 values of butterfly pea extracts vary from very strong to weak depending on the solvent type and extraction method used. Ethanol usually works better as an antioxidant than water because it’s more effective at extracting bioactive molecules. Based on many studies that were reviewed, it can be concluded that the choice of solvent affects the antioxidant activity of butterfly pea flower extract.

Abstract

Medication reconciliation is an essential process in pharmaceutical care aimed at preventing medication discrepancies that may lead to medication errors and compromise patient safety. Clinical pharmacy plays a strategic role in this process through verifying, identifying, and clarifying patients’ medication histories. This review aims to analyze the role of clinical pharmacists in medication reconciliation to reduce medication discrepancies based on various research findings. This study used a literature review method by examining 10 journals related to medication reconciliation, medication discrepancies, and the role of clinical pharmacists in hospital settings. The results show that medication discrepancies are still frequently found at different stages of care, particularly during patient transitions. The most common discrepancies include omitted medications, dose differences, therapeutic duplication, and undocumented medication changes. The involvement of clinical pharmacists has been shown to reduce discrepancies through medication verification, interprofessional communication, and support from information systems and complete documentation. In conclusion, clinical pharmacists play an important role in improving the effectiveness of medication reconciliation and reducing medication discrepancies. Strengthening the role of clinical pharmacists, improving documentation systems, and enhancing interprofessional collaboration are essential to improve patient safety.

Abstract

According to the 2023 patient safety incident report from the KKP-RS, adverse events (KTD) in Indonesia increased from 46.2% to 63%. Patient safety is a significant issue in West Sumatra, with incident reporting occurring in only 3.8% of hospitals. The purpose of this study was to examine the relationship between ward head supervision and patient safety implementation in the Inpatient Ward of Reksodiwiryo Level III Hospital, Padang, in 2025. This was an analytical study with a cross-sectional design. The study was conducted from March to August 2025. Data collection took place from June 15 to 30, 2025. The population in this study was all nurses in the inpatient ward at Reksodiwiryo Hospital Level III, Padang, in 2025. A total of 104 nurses were selected, with a sample size of 51. The sampling technique used was simple random sampling. The study used a questionnaire and interviews. Data were analyzed univariately and bivariately using the Chi-Square statistical test with a 95% confidence level and a α value of 0.05. The results showed that more than half (60.8%) of respondents performed poorly on patient safety measures. Less than half (49%) of ward heads performed poorly on patient safety measures. There was a correlation between ward head supervision and patient safety implementation in the Inpatient Ward of Dr. Reksodiwiryo Hospital Class III, Padang, in 2025 (p-value = 0.000). The conclusion of the supervision of the head of the room is related to the Implementation of Patient Safety in the Inpatient Room of RS. Level III Reksodiwiryo Padang in 2025. It is hoped that the hospital will provide training on supervision to the head of the room. To improve the culture of patient safety in the inpatient room, the head of the room should be someone who has experience with a work period of > 5 years.

Abstract

The elderly are an age group vulnerable to degenerative diseases, one of which is hypertension, often influenced by psychological factors such as anxiety. In Indonesia, in 2021, the prevalence of hypertension increased to 34.1%. Based on data from the Padang City Health Office in 2023, as many as 51.67% of the elderly suffered from hypertension, along with indications of anxiety among elderly patients with hypertension. Anxiety can trigger an increase in blood pressure through physiological mechanisms, potentially worsening hypertension in the elderly. The purpose of this study was to determine whether there is a significant relationship between anxiety levels and the incidence of hypertension among the elderly at Andalas Public Health Center, Padang. This study employed a quantitative method with a descriptive correlational design using a cross-sectional approach. The research was conducted at Andalas Public Health Center, Padang, from May 6 to May 19, 2025. The population consisted of 285 elderly patients with hypertension aged 60–74 years who visited the health center. A total of 74 respondents were selected using accidental sampling. Data were collected using the Hamilton Anxiety Rating Scale (HARS) questionnaire and blood pressure measurements. Data analysis was carried out using univariate and bivariate methods. The results of the study showed that out of 74 respondents, 11 (14.9%) elderly people experienced severe anxiety. Of the 74 respondents, 33 (44.6%) experienced stage II hypertension. There was a significant relationship between anxiety levels and hypertension incidence in the elderly at the Andalas Padang Community Health Center in 2025. The conclusion of this study is that there is a meaningful relationship between anxiety levels and the incidence of hypertension among the elderly at Andalas Public Health Center, Padang. It is recommended that healthcare providers pay greater attention to psychological aspects, particularly anxiety, in delivering nursing care to elderly patients with hypertension, and intensify education and preventive efforts to reduce the risk of complications.

Abstract

The development of tablet dosage forms in Teknologi Farmasi requires optimal formulation to achieve products that meet pharmacopeial quality standards. Wet granulation is a widely used method due to its ability to improve flowability, compressibility, and content uniformity of tablets. Studies show that the combination of fillers such as Avicel PH 101 and lactose produces granules and tablets with desirable physical characteristics, including flow rate, angle of repose, hardness, friability, and disintegration time that comply with requirements. In addition, natural binders such as gelatin, starch, and cellulose demonstrate comparable performance to synthetic binders in producing high-quality tablets. These materials  offer  advantages  in  terms  of  biocompatibility and  environmental sustainability. However, variations in composition and hygroscopic properties remain challenges in standardization and long-term stability, which are closely related to Stabilitas Obat. Overall, the  proper selection and combination of  fillers and  binders in  wet  granulation significantly influence the  quality of  granules and  tablets. Therefore, further optimization is  necessary to  ensure consistencyandstabilityduringstorage.

Abstract

Sustained-release tablets are pharmaceutical preparations designed to release active ingredients gradually and in a controlled manner over a longer period of time. The type and concentration of the matrix used are the main determining factors in controlling the drug release profile. This study aims to examine the effect of various types and concentrations of matrices on the drug release profile in sustained-release tablets based on scientific literature published between 2011 and 2025. This study used a literature review method (review article) by collecting and analyzing ten scientific articles from Google Scholar, PubMed, and Scopus databases. The results of the study showed that hydrophilic matrices such as Hydroxypropyl Methylcellulose (HPMC), xanthan gum, and pectin proved to be the most effective in controlling drug release through a gel layer formation mechanism. Increasing the concentration of hydrophilic polymers generally slows the rate of drug release by forming a thicker and more stable gel. Hydrophobic matrices such as ethyl cellulose provide release control through a pore diffusion mechanism that produces a profile approaching zero-order kinetics. In contrast, modified starch-based matrices such as phosphate-cross-linked pregelatinized sago starch and glucomannan have not demonstrated adequate release control. Combinations of two polymers, such as HPMC-xanthan gum and HPMCAS-HPC, have been shown to produce more stable and controlled release profiles than either polymer alone. These findings suggest that the selection of matrix type and concentration should consider the physicochemical properties of the drug, the desired release control mechanism, and dissolution requirements based on the drug's pharmacokinetic profile.

Abstract

Pill dosage forms remain one of the most widely used traditional solid pharmaceutical preparations due to their good stability, ease of manufacturing, and suitability for herbal active ingredients. Advances in pharmaceutical technology have encouraged the optimization of pill formulations through the selection of excipients, manufacturing techniques, and quality evaluation to improve product performance and therapeutic effectiveness. This review aimed to summarize recent developments in herbal pill formulation based on published scientific literature. A literature review approach was conducted using articles retrieved from PubMed, Google Scholar, and other scientific databases. The review focused on turmeric (Curcuma longa) pills, Stenochlaena palustris herbal pills, and studies investigating the influence of binders, disintegrants, powder fineness, and drying conditions on pill quality. The reviewed studies demonstrated that formulation variables significantly affect weight uniformity, hardness, disintegration time, stability, and overall product quality. Continuous innovation in formulation strategies contributes to the development of herbal pills with improved pharmaceutical characteristics, enhanced stability, and better patient acceptance, supporting the modernization of traditional herbal dosage forms.

Abstract

Acne is a common skin disorder caused by the activity of Propionibacterium acnes. The development of topical preparations based on natural ingredients in gel form has become an alternative treatment due to their perceived safety and minimal side effects. This study aimed to review the effectiveness of various anti-acne gel formulations in inhibiting the growth of P. acnes. The method used was a literature review of 12 scientific articles obtained from academic databases and selected based on inclusion criteria. The data analyzed included research methods, gel formulations, and antibacterial activity results expressed as inhibition zone diameters.The results showed that all gel formulations exhibited antibacterial activity against P. acnes with varying levels of effectiveness. Ingredients such as Allium sativum, Piper betle, and Zingiber cassumunar demonstrated the strongest antibacterial activity, while nanoemulgel formulations showed the highest overall effectiveness compared to conventional gels. In addition, peel-off gel masks also showed good results by increasing the contact time of active compounds with the skin.In conclusion, natural-based anti-acne gel formulations are effective in inhibiting the growth of Propionibacterium acnes, especially when supported by appropriate selection of active ingredients and formulation innovations.

Abstract

Chronic kidney disease (CKD) is a disease that affects many people around the world. One of the consequences of CKD is a disruption in the patient's quality of life. The quality of life of CKD patients can be influenced by spiritual well-being. High spiritual well-being in CKD patients can help them feel closer to God and find meaning and purpose in their lives. This study aims to determine the relationship between Spiritual Well-Being and quality of life in CKD patients undergoing hemodialysis. This study uses a quantitative approach with a cross-sectional study design. This study was conducted at Dr. Reksodiwiryo Hospital from May to June 2025. The study sample consisted of 75 CKD patients undergoing hemodialysis, selected using purposive sampling. Data collection was conducted using the SWBS questionnaire and the KDQOL 36 questionnaire through interviews. Data analysis was performed using univariate and bivariate analysis with a chi-square test p-value ≤0.05. The univariate results showed that 31 (41.3%) patients had moderate spiritual well-being. More than half (44 or 58.7%) had a good quality of life. The chi-square test yielded a p-value of 0.001. The results of this study indicate that there is a relationship between spiritual well-being and quality of life in CKD patients undergoing hemodialysis at Dr. Reksodiwiryo Hospital in Padang. It is hoped that hospital medical staff can provide support to CKD patients undergoing hemodialysis in the form of audiovisual therapy and spiritual education such as recitation of the Qur'an and dzikr.
