Abstract

Introduction and Objective: Surgery is one of the treatment options for strabismus. The success rates of surgical interventions, as reported in a number of different studies, vary widely. There is no database of surgical outcome in large angle exotropia patient undergoing surgery in Semarang, Central Java. This study aims to evaluate the surgical outcome of strabismus correction in large angle exotropia in Dr. Kariadi General Hospital.  Methods: A retrospective study held from the medical record of patients with diagnosis large angle (>50?) exotropia undergoing strabismus correction surgery from January 2020-December 2022 in Dr. Kariadi General Hospital, Semarang. The clinical characteristics: sex, age, angle of exodeviation, amblyopia condition, sensory deprivation, and postoperative alignment were obtained. Surgical outcome measured with follow up of angle deviation in one month dan three months follow up after surgery.  Result: Totally 59 large angle exotropia patients were obtained with 31 (53%) were woman and 28 (47%) were man, range aged 1 year to 53 years old had preoperative angle of exodeviation 51->85 ?, 6 (10%) had amblyopia, 11(18.6%) had sensory deprivation. By the 3 months post-operative visit, 100% of all patients reported orthophoria.  Conclusion: This study found the outcome of strabismus correction for large angle exotropia is satisfactory. Two- or three-muscle surgery provides an effective means for treatment of large angle exotropia.  Keywords: large angle exotropia, exodeviation, strabismus

Abstract

Introduction: Chemical ocular injury is an emergency case and commonly occured among children. Alkaline chemical ocular injury is more destructive especially due to saponification. One of the rare chemical ocular injury is alkaline trauma by Potassium Permanganate (KMNO4). Case Report: A case of a three-year-old boy patient brought by his mother in emergency department at Cicendo Eye Hospital with severe pain, redness, and swelling of the left eye. Ten hours earlier, the patient was playing and accidentally exposed to KMNO4. Visual acuity of the right eye was 6/6 and the left eye was difficult to examine. Ophthalmological examination revealed superior and inferior palpebra are swelling, KMNO4 adhered tightly on conjunctival tarsal superior, inferior, and conjunctival bulbi inferior until fornix and temporal area. Cornea was hazed and iris shadow test positive so he was diagnosed with Chemical Trauma Ropper Hall grade II OS. Treatment begins with irrigation 2 litres of normal saline until pH 7 along with debridement of KMNO4 from conjunctival tarsal superior inferior and bulbi. The patient underwent installation of transplanted amniotic membrane and debridement under anasthesia. Discussion: Management in the initial phase is irrigation. The definitive therapy requires surgery to speed up the healing process. Complications that often occur are corneal limbal cell deficiency, dry eye, symblepharon formation, secondary glaucoma, and complicated cataracts. . Conclusion: Alkali chemical injury is hazardous and can lead to severe vision threatening complications because of saponification. Prompt and aggressive management is required in such cases to give them the best chance of a more favourable outcome. Abstract

Introduction: Occipital stroke involves the occipital lobe of the brain, which can impair the visual acuity in patients. Several risk factors have been proven as predisposing conditions for occipital stroke. This study investigates the risk factors for occipital stroke in tertiary eye center in Yogyakarta. Methods: The method of this study was cross-sectional research by analyzing secondary data from medical records of patients with occipital stroke hospitalized in Dr. Yap Eye Hospital, a tertiary eye center in Yogyakarta. Patients diagnosed with occipital stroke from January 2021 to December 2022 were included in this study. Demography, clinical data, and risk factors associated with occipital stroke were recorded. The descriptive statistic was performed to evaluate the risk factors for occipital stroke.  Results: Sixty-one subjects were analyzed, comprised of 44 males (72%) and 17 females (28%). The majority of subjects within age ?45 years old (93%). The attack occurred during activity (n=54, 89%) and wake-up (n=7, 11%). The involvement right eye (n=8, 13%), left eye (n=9, 15%), and bilateral (n=44, 72%). Of 122 eyes, 74 had hemianopia (61%), 34 had quadranopia (28%), and 14 had bilateral hemianopia (11%). Twenty-nine subjects had right occipital stroke (48%), 25 subjects had left occipital stroke (41%), and seven subjects had bilateral occipital stroke (11%). Most subjects had one risk factor (n=29, 48%) and two risk factors (n=23, 38%). Eight subjects had three risk factors (13%), and only one subject had four risk factors (2%).  Conclusion: Most risk factors of occipital stroke involve only single and double risk factors. Only a minority of subjects had multiple risk factors.

Abstract

Introduction and Objective: To investigate the correlation between retinal nerve fibre layer (RNFL) thickness and the past RNFL thickness, related risk factor (gender, age, myopia, intraocular pressure (IOP), blood pressure). And RNFL parameter (symmetricity and four quadrants RNFL thickness) and optic nerve head parameter (rim area and vertical cup disc ratio) Methods: 39 right eyes of both sex and various age underwent examination including IOP measurement, subjective refraction for determining refractive error, and optical coherence tomography (OCT) for RNFL and ONH analysis. The follow up were done after one year. Result: The RNFL symmetricity, superior, nasal, inferior, temporal RNFL thickness showed significance correlation with RNFL thickness (R= 0.487, p= 0.002), (R= 0.915, R= 0.749, R= 0.897, R= 0.702 with p= <0.001), but not showed significance correlation with rim area and vertical cup disc ratio (R= 0.175, p= 0.286) and (R= -0.093, p= 0.574). A significance correlation also showed with increasing age, gender, and systolic blood pressure (R= -0.387, p= 0.015), (R= 0.441, p= 0.005) and (R= -0.418, p= 0.008). But not showed significance correlation with history of past RNFL thickness, refractive error, IOP, and diastolic blood pressure (R= 0.258, p= 0.113), (R= 0.005, p= 0.975), (R= -0.234, p=0.152), and (R= 0.060, p=0.717). Conclusion: RNFL thickness measurements showed significance correlation with RNFL parameter. It is not clear if past RNFL thickness, refractive error and IOP related with RNFL thickness even showed significance correlation with age, gender, and systolic blood pressure. Abstract

Health philanthropy plays a crucial role in supporting charitable activities in the field of ophthalmology. The Indonesian Ophthalmologists Association (PERDAMI) has been actively engaged in various social initiatives to improve eye care services. However, it is imperative to consider long-term sustainability in funding to ensure the continuity of philanthropic efforts. Additionally, the recent regulatory changes in Indonesia highlight the need for philanthropic practices to comply with legal frameworks. This study presents the key highlights of a social-entrepreneurship session presented at the 48th Annual Scientific Meeting of the PERDAMI in August 2023, with aim to emphasize the importance of collaboration among stakeholders involved in ophthalmic philanthropy, including professionals, institutions, and grantors. The future sustainability of vision care is vital, and it requires the exploration of innovative funding models, the promotion of public-private partnerships, and the cultivation of a culture of giving. By raising awareness of this issue and incorporating it into strategic planning, stakeholders can work towards ensuring the availability and accessibility of quality eye care services in Indonesia. In conclusion, this study highlights the importance of integrating collaboration, needs assessment, good governance, and sustainability considerations in health philanthropy practices within Indonesian ophthalmology. These efforts will help shape the future of vision care and empower the PERDAMI to deliver meaningful and lasting impact in eye health.

Abstract

Introduction & Objective: Sutureless Scleral fixation surgery is one of the secondary intraocular lens (IOL) implantation procedures for aphakic patients with absent or deficient capsular bag support that has gained more attention these days. This study aimed to describe the characteristics, indications for and visual outcomes of the patient who had undergone scleral fixation of intraocular lens at Sardjito Hospital. Methods: A retrospective cross-sectional study of 43 eyes that undergone sutureless scleral fixation intraocular lens at Sardjito Hospital, Yogyakarta. The data were collected from electronic medical records of the patient from October 2020 to December 2022. The data was analysed descriptively by presenting number and percentage for categorical variables and mean ± standard deviation (SD) for numerical variables. Outcome include pre- and postoperative uncorrected visual acuity (UCVA). Results: There were 43 eyes obtained from 40 patients who underwent scleral-fixation intraocular lens in this study, consisting of 35 males (79,55%) and 9 females (20,45%). The mean age of the subjects was 50.04 ± 19.2 years. Lens subluxation was the most of indication for surgery (32,56%), followed by intraocular lens luxation (20,93%), dropped nucleus (16,28%), lens luxation (16,28%), intraocular lens decentration (9,30%), and endophthalmitis (4,65%). Before surgery, most of the patients had visual acuity of less than 3/60 (81,4%) and after surgery, 32,56% had ?6/18, 34,88% had <6/18-6/60 and 18,60% had <6/60-3/60. Conclusion: The characteristics of patient who underwent scleral fixation intraocular lens in Sardjito Hospital was predominantly male and diagnosed with lens subluxation. There were enhancement in visual outcomes.

Abstract

Introduction : Endophthalmitis is a medical emergency caused by infection of tissue or fluids inside the eyeball. Vitrectomy for endophthalmitis is one of the treatment choice and leads to VA gains in some cases. Timing of surgery and visual outcome after surgery are vital to acknowledge. This study aims to analyze the difference of visual outcome between early and delayed vitrectomy in endophthalmitis patients operated in Dr. Kariadi General Hospital Methods : A Retrospective study held from medical record of patients with endophthalmitis underwent vitrectomy surgery from January 2021 – January 2023 in Dr. Kariadi General Hospital, Semarang. Endophthalmitis patients caused by trauma were excluded. All of the patients had immediate vitrectomy, and based on the onset, the patients were divided into two groups, early vitrectomy (onset  ? 7 days) and delayed vitrectomy (onset > 7 days). Independent t- test method was used to analyze the data (significant if p<0.05) Result : Totally 30 endophthalmitis patients (30 eyes) were obtained. There were 18 patients (60%) had early vitrectomy and 12 patients (40%) had delayed vitrectomy. Postoperative examination showed 13 (72%) patients with early vitrectomy had improved VA, and 5 (28%) had constant VA. On the other hand, patients with delayed vitrectomy showed 5 (42%) had improved VA, 6 (50%) had constant VA, and 1 (8%) had decreased VA. The difference between two groups was significant (p=0.014) Conclusion : This study found endophthalmitis patients with early vitrectomy had significant better visual outcome than delayed vitrectomy.

Abstract

Introduction: Vogt-Koyanagi-Harada (VKH), an auto-immune disorder driven by melanocyte antigen, is rare in children. The mainstay treatments are corticosteroid and immunomodulators. Methotrexate is one of immunomodulator that widely used due to its effectiveness with minimal side effects. Case Report: A 9-years-old boy get diagnosed Vogt-Koyanagi-Harada. His best corrected visual acuities (BCVA) were 20/400 in both eyes. Anterior segment examination showed granulomatous uveitis. Posterior segment examination revealed optic disk swelling, Dallen Fuch’s nodule and exudative retinal detachment. We also found poliosis and vitiligo in his lips. He had been previously treated with topical corticosteroids, oral corticosteroids and topical cycloplegics. The dose was tapered every month. After taking medication for 5 months with the last dose oral corticosteroid 16 mg/day and topical corticosteroid every 8 hours, his BCVA recovered to 20/25 RE and 20/40 LE. During the treatment time, the patient discontinued the medication due to parental disobedience and then came with decrease of vision. Based on the consideration of rebound phenomenon and the use of maximum dose of corticosteroids, we consulted the patient to the pediatrician, and decided to give methotrexate as an immunosuppressant. After receiving 15 mg/week of methotrexate for 3 months, inflammation calmed down but unfortunately the vision did not improve. Discussion: In children, the main treatment for VKH disease is high-dose corticosteroids. Considering the side effects of prolonged systemic corticosteroid therapy, methotrexate become the first line of corticosteroid sparing agent, especially in chronic stage. Abstract

Introduction and Objective: Laser peripheral iridotomy (LPI) is the standard first-line therapy in angle closure disease to increase anterior chamber angle width. Spectral Domain Anterior Segment Optical Coherence Tomography (SD AS-OCT) is an instrument that has a quantitative parameter to measure anterior chamber angle. Angle opening distance (AOD) and trabecular iris space area (TISA) are quantitative parameters in SD AS-OCT. The purpose of this study is to compare AOD and TISA before and one week after LPI in angle closure disease. Methods: This is an analytical prospective study with a cross-sectional study design. Twenty-two eyes with angle closure disease that met inclusion and exclusion criteria underwent LPI and SD AS-OCT before and one week after LPI. Analysis was performed using SD AS-OCT parameters, such as AOD (AOD500, AOD750) and TISA (TISA500, TISA750) in the temporal, nasal quadrants, and average. Results: Twenty-two eyes with angle closure are divided into nine eyes with primary angle closure suspect, seven eyes with primary angle closure, and six eyes with primary angle closure glaucoma. The mean age of participants was 57.86±7.63 years. There was a significant increase in AOD500, AOD750, TISA500, and TISA750 in the temporal and nasal quadrants, as well as in the average (P=0.0001) based on a paired t test or Wilcoxon as an alternative. Average AOD750 had the highest T-value (T= 6.329) with a mean 0.20±0.096 before LPI and 0.31±0.144 one week after LPI. Conclusion: There was a significant increase in AOD and TISA after one week LPI in angle closure disease.
