Abstract

Introduction: Conjunctivitis is an eye infection accounted as the third leading ophthalmology cases after cataract and glaucoma globally, with Streptococcus pneumoniae as the main causes in children. Several studies had indicated that Piper betle L. extract inhibited Streptococcus pneumoniae growth. Methods: This study was an analytical study with true experimental design and pre-test-post-test control group. Streptococcus pneumoniae colony growth media was analyzed using Mueller Hinton agar.  Streptococcus pneumoniae colony was calculated using Total Plate Count methods. Results: Sample of this study involved 30 male wistar rat aged 4 – 9 minggu. Kruskal Wallis test reveald a significant differences of Streptococcus pneumoniae colony based on the treatment group (p=0,023). Subsequent test using Posthoc revealed that there was a significant differences in Streptococcus pneumoniae colony between placebo group and group using 25% (p = 0,039) and 30% (p=0,006) piper betle. Test results also revealed that no significant difference was found between placebo group and groups using 20% piper betle (p=0,159) and ofloxacin (p=1,000). Conclusion: There was a significant difference in Streptococcus pneumoniae colony among placebo group and piper betle eyedrops 25 % and 30 % group. However, significant result among placebo group and piper betle L. 20 % and ofloxacin group were absent. Keywords: Bacterial conjungtivitis, Streptococcus pneumoniae, Piper betle L.

Abstract

Introduction & Objective: Brolucizumab is the newest anti-vascular endothelial growth factor (anti-VEGF) drug. It showed superior anatomic outcomes compared to other anti-VEGF and noniferiority best corrected visual acuity (BCVA) results for Wet-Aged Related Macular Degeneration (Wet-AMD) patients. In Yogyakarta, its been only used in few patients and we want to report the Effectiveness and safety of brolucizumab in Wet-AMD patients in Yogyakarta. Method: In this retrospective study, all neovascular AMD (nAMD) Patients who underwent brolucizumab intravitreal injections between December 2021 and January 2023 at YAP Eye Hospital were studied. Patient’s demographic data, before and after intravitreal brolucizumab injections (central subfield thickness, intraocular pressure and BCVA) were assessed. Result :This study included 6 eyes of 6 patients ( 3 women) with a mean age of 69.5 years old. The mean follow- up period was 27 ± 10.1 weeks after the first injection of brolucizumab. Three patients were naïve nAMD patient, never got any intravitreal injections and three anti-VEGF experienced nAMD patients with ranibizumab or aflibercept injections prior to brolucizumab. Mean BCVA at baseline (before brolucizumab injection) was 0.12 decimal (6/48) and was 0.29 decimal (6/20) at the last follow-up. Five out of six patients visual acuity were improved,one patient had constant bcva. Four out of six patient central subfield thickness were reduced, none of the patient intraocular pressure increased over 21 mmhg. None of the patient reported any signs of inflammation, vasculitis, or any other ocular or systemic adverse effects. Conclusion :This limited data demonstrated that brolucizumab was safe and effective in stabilizing BCVA for both of nAMD patients who had undergone previous treatment with other anti-VEGFs agents and naive. Future larger multicenter collaborative studies are warranted.

Abstract

Introduction and Objective: Macular hole (MH) is vitreomacular interface disorder that could cause visual impairment. Clinical characteristics of MH are important for predicting MH treatment outcomes. This study purpose was to describe MH patients’ characteristics and postoperative outcomes in Dr. Kariadi Hospital. Method: A retrospective study held on January 2021–December 2022 utilizing patients’ medical records. Preoperative data: sex, age, symptoms, duration of symptoms, origin, preoperative visual acuity (VA), modified Gass classification stage, macular hole index (MHI), and hole forming factor (HFF) were obtained. Postoperative outcomes data: postoperative VA and anatomical closure were also collected. Result: 20(20 eyes) patients were obtained, with 14(70%) female, 12(60%) aged >60 years old, 13(65%) metamorphopsia, 12(60%) central visual loss, 10(50%) preoperative VA 6/24-6/60, 10(50%) duration of symptoms <3 months, and 16(80%) idiopathic origin. OCT (Optical Coherence Tomography) findings revealed base diameter mean was 780,7 um(+ SD 312). 13 patients (65%) stage 3 of MH, 12(60%) MHI <0,5, 12(60%) HFF > 0,9. All patients underwent MH surgery procedures. Postoperative examination showed 10 patients(50%) with VA 6/6-6/20. 11(100%;p<0,001) with preoperative VA>6/60, 10(100%;p<0,001) with duration of symptoms <3 months, 8(100%;p=0,012) with MHI > 0,5, and 11(100%;p<0,001) with HFF > 0,9 (p<0,001) had improved visual acuity and anatomical closure. It also revealed 17(85%) had anatomical closure. Conclusion: The most common characteristics found in MH patients were female sex, age >60 years old, and disorder of idiopathic origin. Preoperative VA, duration of symptoms <3 months before surgery, MHI > 0,5, and HFF > 0,9 were positive prognostic factors for better postoperative functional and anatomical outcomes.

Abstract

Introduction: Orbital tumor removal surgery represent a small but significant portion of ophthalmologic surgery, and few reports of the associated complications are available. Methods: Descriptive retrospective study was carried out 138 patients who undergone orbital tumor removal surgery during July 2021-March 2023. Surgery approach and postoperative complications were collected from medical record. Results: Of approximately 138 patients, 12 (6%) patients had postoperative complications. Five (5%) of 75 patients who undergone anterior orbitotomy had postoperative complications. One percent had lagophthalmos, 1% had ptosis, 2% had decreased vision, and 1% had ocular movement disorders. Lateral orbitotomy was done in 21 patients. Three (12%) patients had postoperative complications. Four percent had ptosis, 4% had decreased vision, and 4% had ocular movement disorders. Four (60%) of 6 patients who undergone medial orbitotomy had ptosis (16%) and decreased vision (50%). Posterior orbitotomy was done in 8 patients. One (12,5%) patient had complication of ptosis.   Conclusion: Anterior orbitotomy was the most common surgery approach for orbital tumor removal. The most common postoperative complications were ptosis and decreased vision. Informed consent is an important aspect to improve patient’s knowledge of postoperative complications.

Abstract

Introduction: Thyroid Eye Disease (TED) is an autoimmune inflammatory disease of the orbit that affects orbital fat, extraocular muscles, periocular tissue and lacrimal glands. TED activity can be assessed with a Clinical Activity Score (CAS) to identify active or inactive TED conditions. Dry Eye Diseases (DED) is one of the manifestations in patients with TED with a prevalence that varies from 27%-96%. The Asia Dry Eye Society (ADES) 2017 proposed a new concept for DED diagnosis and therapy, namely Tear Film Oriented Diagnosis (TFOD) and Tear Film Oriented Diagnosis Therapy (TFOT) with Tear Film Breakup Pattern (TBUP) examination. TBUP examination help in determine specific management of DED. The aim of this study is to determine the differences in TBUP in TED patients based on active and inactive. Methods: This is an analytical observational study with a cross-sectional design. The subjects were TED patients who came to the Cicendo Eye Hospital and met the inclusion criteria. The study was conducted at the National Eye Center of Cicendo Eye Hospital in April - June 2022. The data were analysed with the Chi-Square Test. Result: This study was conducted on 23 patients or 46 eyes. This study found a statistically significant differences in TBUP features in active and inactive TED patients (p=0.0001). The most common features of TBUP was found in active TEDs was dimples (55%). Conclusion: There are differences of TBUP features in active and inactive TED patient.

Abstract

Introduction: Studies showed 38% refractive errors after intraocular lens (IOL) implantation are caused by deviations in calculation of IOL power. The difference between the formula’s refractive prediction and patient’s final outcome highly increases in cases of high myopia, but needs to be more research on this topic. This study aims to show difference between predicted and final refractive outcomes using Barrett II Universal (BU-II) and SRK/T in patients with high myopia who underwent phacoemulsification and IOL implantation. Method: This analytical observational study with cross-sectional design was conducted on high myopia patients (axial length ? 26.0 mm) who underwent phacoemulsification and intraocular lens implantation between January 2021 and January 2022 at Dr. Kariadi General Hospital Semarang. Data used in this study consisted of biometric measurements and difference between predicted refractive outcomes using BU-II and SRK/T formulas and final refractive outcomes of patients one month after surgery. Data were analyzed using one-sample T-test, and significance level of p < 0.05 was considered statistically significant. Results: Thirty-five eyes participated in this study, the mean axial length was (29.33  2.01). BU-II formula has mean difference in refraction prediction with patient's final refraction result closer to zero (1.191.31) than SRK/T formula (1.25  1,12). The difference between two formulas was statistically significant (p < 0.05). Conclusion: The BU-II formula has lower difference between predicted and final refractive outcomes of high myopia patients after phacoemulsification and IOL implantation than SRK/T formula.

Abstract

Introduction and Objective: Unilateral cataract presents from 19.6% to 55.5% of pediatric cataract. This condition is treatable, but delayed treatment may cause visual impairment. This study aims to understand characteristic of unilateral cataract in children. Methods: This study was descriptive review of patients aged 0-17 years old with unilateral cataract at Dr. Kariadi Hospital, Semarang. Data were collected from medical record of patients from January 2019 to December 2022.   Results: Sixty patients were included in this study, 56.7% of whom were male and 43.3% were female. The mean age at presentation is 71.50 ± 48.34 months with whitish eye being the most common main complaint (65%), followed by blurred vision (35%) and squinting of eye (5%). Most of patients had congenital or infantile cataract (60%), followed by traumatic (36.6%), uveitis (1.7%) and iatrogenic (1.7%). Some of patients had microcornea (21.7%), persistent pupillary membrane (8.3%), retinopathy of prematurity (3.3%), persistent fetal vasculature (8.3%), nystagmus (8.3%), amblyopia (20%) and strabismus (40%), with exotropia being the most common type (33.3%). Most of patients did not have systemic disease (93.3%), but we found two patients had congenital rubella syndrome, one had congenital cytomegalovirus, one had down syndrome. Conclusion: Etiology of unilateral pediatric cataract mostly idiopathic followed by traumatic. Leukocoria was the most common complaint, followed by blurred vision and strabismus. In unilateral cataract, exotropia was more common type of strabismus.

Abstract

Introduction and Objective: Unresolved cataract primarily has caused patients to have disturbance in carrying out daily activities, thus lowering their quality of life (QoL). We aim to use the Visual Function Index-14 (VF-14) questionnaire, one of the most frequently used tools, to discover the extent of daily activities disturbance in patients with cataract. Methods: In this observational study, we conducted eye screening to 47 subjects in Jonggol subdistrict, Bogor, Indonesia. Patients were asked several questions based on the translated VF-14 questionnaire into Bahasa and also tested to find out their best-corrected visual acuities. Lens examination was carried on using handheld slit lamp. Result: Out of all 47 subjects, 34 (72,3%) patients had cataract that was marked by lens opacities in either one or both eyes. According to distant vision impairment categories by International Classification of Diseases 11, 8,8% of cataract patients had normal vision from the better eye, 20,6% had mild, 64,7% had moderate, 0% had severe impairment, and 5,9% had blindness. From the assessment using VF-14 translated questionnaire, 14,7% subjects had no disruption in doing daily activities, 82,3% had disruption, and 2,9% could not do the mentioned activities at all. From the Spearman correlation test, there was a significant association with moderate correlation between the degree of vision impairment and QoL (p = 0.006, rs = 0.459). Conclusion: The majority of cataract patients in Jonggol subdistrict had moderate vision impairment and had disruption in doing daily activities, therefore lowering their QoL.

Abstract

Introduction Myopia is a refractive error that causes images of distant objects to be focused in front of the retina by the non-accommodating eye. Myopia is one of the most common visual disturbances in the world. It is believed that the use of spectacle can reduce the progressivity rate of myopia. Spectacle can help light fall on the retina and prevent axial elongation of the eye. This theory needs to be investigated further so that it can be used as a reference. This study aimed to know the correlation between compliance of spectacle wear and myopia progressivity rate in medical students of Diponegoro University (UNDIP). Methods This research was an observational analytical study with cross-sectional design on 54 subjects ranging from 18 to 22 years old. Data are primary data obtained from questionnaires and objective eye examinations using an autorefractometer. The research was conducted from October to November 2021. Data were analyzed using Spearman Rank test which p<0,05 was considered significant. Results The mean of compliance of spectacle wear and myopia progressivity rate in medical students of UNDIP are (1,57±0,108) and (0,259±0,515) dioptres respectively. The correlation between compliance of spectacle wear and myopia progressivity rate are p=0,082 and r=0,239. Conclusion There are no correlation between compliance of spectacle wear and myopia progressivity rate in medical students of UNDIP. Abstract

Introduction and Objective: Eyelid laceration is one of the most common ocular injuries with complications. This study examines patient characteristics with eyelid lacerations at Kariadi General Hospital Semarang. Methods: An observational study used medical records data from April 2019 to January 2023. Age, gender, eyelids, grading, area of laceration, mechanism of trauma and injury, canthal and canalicular involvement, associated injury, and surgery were recorded. Results: From 75 patients, lacerations were more common in men (56, 75%) and ages 16-<50 years (43, 57%). Lacerations occurred in right lower (21, 28%), right upper (17, 23%), right upper and lower (16, 21%), left lower (9, 12%), left upper (8, 11%), and left upper and lower (4, 5%) eyelids. They were mainly medial (52, 69%), lateral (12, 16%), and central (7, 9%). Full-thickness lacerations (58, 77%) outnumbered lamellar thickness (17, 23%). Blunt trauma (63, 84%) was the main mechanism, with motor vehicle accidents leading 30 (40%). Canthal involvement was absent in 41 cases (55%), medial in 29 (39%), and lateral in 5 (7%). Canalicular involvement was absent in 39 cases (52%) and present in 36 (48%). The three most frequent of associated injury of the patients was 16 (21%) subconjunctival bleeding, 12 (16%) orbital hematom, 9 (12%) conjunctival laceration. All patients underwent surgery, with eyelid repair most common (32, 43%). Conclusion: Eyelid lacerations mainly involved right lower eyelid, medial region, and are full-thickness. Blunt trauma, particularly motor vehicle accidents, was the main cause. Most cases lack canthal or canalicular involvement. Eyelid repair was the most frequent choice of surgery.
