Abstract

Introduction: Subarachnoid hemorrhage is a dangerous neurological condition with sudden onset, often without warning. About half of the patients die before reaching the hospital. The purpose of this case report is to evaluate the management and clinical outcomes of a 59-year-old female patient with SAH. Case Presentation: A 59-year-old woman presented with sudden, severe headache, nausea, vomiting, and neurological deficits. CT scan revealed subarachnoid hemorrhage. She received treatment to manage intracranial pressure, blood pressure, and prevent vasospasm, with monitoring for complications like hydrocephalus. Conclusion: Sudden, severe headache that has never been experienced before is the most common symptom of subarachnoid hemorrhage. Management primarily aims to prevent recurrent bleeding and other complications.

Abstract

Background: This study aims to serve a comprehensive systematic review on the oncological outcomes following surgical management of localised renal cancer in literatures of the last 5 years. Methods: The systematic review followed PRISMA 2020 standards and examined full-text English literature published between 2019 and 2024. This review excluded editorials, review papers from the same journal, and submissions without a DOI. Literature was sourced from online platforms such as PubMed, SagePub, SpringerLink, and Google Scholar. Result: A total of  8,824 articles were retrieved from online databases (PubMed, SagePub, SpringerLink and Google Scholar). After three rounds of screening, five articles directly relevant to the systematic review were selected for full-text reading and analysis. Conclusion: The evolving management of renal cancer emphasizes balancing oncologic effectiveness with preserving renal function. With PN's acceptance and minimally invasive options, tailored treatments improve outcomes. However, personalized decisions based on tumor and patient factors remain crucial for optimal results.

Abstract

Background: Hyperpigmentation, which happens when a skin region is darker than its surroundings, is typical in people with dark skin tones. In these subjects more than in lighter-toned ones, hyperpigmentation usually lasts longer and is harder to treat. The aim: The aim of this study to show about efficacy and effectiveness of laser and light therapy for the treatment of cutaneous disorders of hyperpigmentation. Methods: By the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) 2020, this study was able to show that it met all of the requirements. This search approach, publications that came out between 2014 and 2024 were taken into account. Several different online reference sources, like Pubmed, SagePub, and Sciencedirect were used to do this. Result: Eight publications were found to be directly related to our ongoing systematic examination after a rigorous three-level screening approach. Subsequently, a comprehensive analysis of the complete text was conducted, and additional scrutiny was given to these articles. Conclusion: Highlighting Q-Switched laser treatments as the gold standard for hyperpigmentations. In our cohort of patients, lighter phototypes, lesions located on the trunk or limbs, and superficial melanosis better responded to the treatment.

Abstract

Introduction: Diabetic ketoacidosis (DKA) is a critical condition often encountered in pediatric patients with type 1 diabetes. Objective: This review aims to summarize current strategies and guidelines for managing pediatric diabetic ketoacidosis (DKA), focusing on fluid resuscitation, insulin therapy, and long-term outcomes. Methods: A comprehensive review of recent literature from the past decade was conducted. Key studies were selected based on their relevance to pediatric DKA management, particularly regarding fluid resuscitation methods and insulin regimens. Results: The review highlights that effective management of pediatric DKA requires a balance of fluid resuscitation and insulin therapy. Recent studies emphasize the importance of using a structured clinical pathway to improve outcomes and reduce complications. Conclusion: Proper management of pediatric DKA involves timely recognition, appropriate fluid resuscitation, and insulin therapy, with a focus on monitoring for complications such as cerebral edema. The use of structured clinical pathways and liberal fluid administration in the absence of cerebral edema are supported by recent evidence to improve patient outcomes.

Abstract

Background: Knee osteoarthritis (KOA) is an active, heterogeneous, and low-grade inflammatory condition leading to functional disability and pain. The aim: The aim of this study to show about the effect of intraosseus infiltration of platelet rich plasma for knee osteorthritis. Methods: By the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) 2020, this study was able to show that it met all of the requirements. Result: Eight publications were found to be directly related to our ongoing systematic examination after a rigorous three-level screening approach. Subsequently, a comprehensive analysis of the complete text was conducted, and additional scrutiny was given to these articles. Conclusion: Intraosseous (IO) PRP injections to target subchondral bone can provide a more comprehensive treatment by stimulating biological processes that lead to an environment primed for regeneration. Restoring joint homeostasis also influences the behaviour of MSCs that coordinate bone remodelling of the subchondral bone.

Abstract

Introduction : Dextrocardia is a rare congenital anomaly affecting the heart. The range of cardiac problems linked to dextrocardia is extensive. This case report seeks to enhance the current knowledge by providing a clinical description of dextrocardia accompanied by hypertensive emergency and congestive heart failure. Case Presentation : A 39-year-old woman came to the Emergency Department of RSI Sultan Hadirin Jepara, having vomitted more than ten times since last night. Vomiting was preceded by nausea. The patient has been experiencing shortness of breath for three years, particularly when sleeping with only one pillow, necessitating the use of multiple pillows. Through additional physical and supporting examinations, the patient was diagnosed with dextrocardia with hypertensive emergency and congestive heart failure. Conclusion : Dextrocardia is a rare birth defect characterized by a variety of complicated structural abnormalities in the heart. There is no difference in symptoms and signs in patients with congestive heart failure in dextrocardia or levocardia. So that the approach to managing congestive heart failure in our patient is identical. Gaining a comprehensive understanding of this uncommon ailment is essential for delivering suitable medical treatment and effectively managing any problems in affected persons.

Abstract

Introduction : Cervical disc herniation most likely happens in people over 30-years-old-age with a job that need holding a posture that can make body alignment easily diverge. Case presentation : A 48-year-old male presented with radiate pain from the wrist going upward to the neck. He also felt numbness or tingling sensation on the right side of the arm. MRI showed protrusion of intervertebral disc C5-6 and C6-7 with narrowing of right and left neural foramen.  The patient was diagnosed with C5-C6 cervical disc herniation, Radiofrequency Ablation (RFA) was performed twice. An Anterior Cervical Discectomy and Fusion (ACDF) procedure was suggested but the patient declined it and then he received 3 months of physical therapy, and he performed physical activity regularly. Eventually the patient reported resolution of all the symptoms after. Conclusions : This case report emphasizes the effectiveness of combining physical activity with medical rehabilitation in a patient with cervical herniated nucleus pulposus after RFA procedure. The combination of these approaches yielded positive results measured by NPRS and ROM examination, with the patient experiencing reduced pain, improved daily function, and return to their daily activities.

Abstract

Background: Congenital hypothyroidism is defined as a deficiency of thyroid hormone at birth and is one of the most common endocrine disorders in newborns. Congenital hypothyroidism, with a prevalence of 1 in 3000-4000 live births, is the most common preventable cause of mental retardation in children. The aim: The aim of this study to show about gender and risk of congenital hypothyroidism. Methods: By the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) 2020, this study was able to show that it met all of the requirements. Result: Eight publications were found to be directly related to our ongoing systematic examination after a rigorous three-level screening approach. Subsequently, a comprehensive analysis of the complete text was conducted, and additional scrutiny was given to these articles. Conclusion: many risk factors are involved in the development of congenital hypothyroidism in children. It should be noted that the irreversible consequences of this disease for children including mental retardation is one of the cases which to control and remove it. Accordingly, awareness of the birth season could help policymakers who plan preventive programs to reduce the false negative results among neonates, especially girls.

Abstract

Background: Overweight and obesity under 18 years of age has been linked to an increased risk of premature mortality from middle adulthood onward. The aim: The aim of this study to show about risk factor, management and outcome of obesity in children. Methods: By the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) 2020, this study was able to show that it met all of the requirements. Result: Eight publications were found to be directly related to our ongoing systematic examination after a rigorous three-level screening approach. Subsequently, a comprehensive analysis of the complete text was conducted, and additional scrutiny was given to these articles. Conclusion: the broad principle of treatment is to adjust the chronically positive energy balance. While lifestyle and behavior modifications with dietary changes and increased physical activity are the integral part of obesity management, the efficacy and sustainability are limited, especially for those with severe obesity. With emerging evidence on new pharmacological agents, it is hoped that obesity management in children could be improved in the future.

Abstract

Background : Diabetes Mellitus (DM) is a metabolic disorder characterized by pancreatic cell dysfunction and insulin resistance. Fibroblast Growth Factor 21 (FGF21) is thought to have a cardioprotective role. Through FGF21 signaling, AMP-activated protein kinase (AMPK) activity reduces apoptosis, inflammation, hypertrophy, and fibrosis in the heart. Hibiscus sabdariffa Linn. (HSL) contains flavonoids that have hypoglycemic effects and are anti-inflammatory and antioxidant-active. This study aims to assess the effect of HSL on FGF21 and AMPK levels in the heart of diabetic mice models. Methods : An experimental study using Deutschland Denken Yoken (DDY) mice aged eight weeks were divided into four groups: Control, DM Control, HSL 200, and HSL 400. In the DM control, HSL 200, and HSL 400 groups, diabetes was induced by giving a High Fat Diet(HFD) and STZ 40mg/kgbb. The HSL 200 group was given HSL 200mg/kgBB supplementation for three weeks, and the HSL 400 group was given HSL 400 mg/kgBB supplementation for three weeks. FGF21 and AMPK levels were measured using the enzyme-linked immunosorbent assay (ELISA) method. Results : There was no significant difference between FGF21 and AMPK levels between the control and DM control groups. FGF21 and AMPK levels in the HSL 400 group were higher than in the control, DM control, and HSL 200 groups. Conclusions : These findings suggest that HSL supplementation at specific doses can potentially increase the activation of FGF 21 and AMPK signaling in the heart of diabetic mice models.
