Abstract

Vulnus Laceratum is a disruption of the continuity of tissue so that there is a separation of tissue that was initially normal; torn wounds occur due to severe violence that severs the tissue. Vulnus Laceratum is the third most common type of injury in Indonesia, with a percentage of 23.2%. Nursing problems that often arise in patients with vulnus laceratum are acute pain, impaired skin and tissue integrity, and risk of infection. The knowledge and role of nurses are essential in the rapid treatment of vulnus laceratum patients at risk of bleeding, infection and even disability. This case study aims to analyze the application of nursing care to patients with vulnus laceratum. Nursing care methods using the SDKI, SLKI, and SIKI approaches. The results of the assessment obtained four male patients, the cause of vulnus laceratum 2 cases due to vehicle accidents, 1 case due to blunt objects, and 1 case due to sharp objects. The nursing diagnoses that appear are impaired skin and tissue integrity, acute pain, risk of ineffective cerebral perfusion, and risk of infection. Nursing interventions include wound care, wound suturing, pain management, vital sign monitoring, and infection prevention. During the implementation process, all interventions were carried out. Evaluation of nursing care with impaired skin and tissue integrity disorders resolved on the 10th and 30th days, acute pain resolved on the third day, risk of infection on the seventh day, and risk of ineffective cerebral perfusion 1 hour after implementation. The results of this case study are expected to increase knowledge and complement existing sources so that knowledge and management of vulnus laceratum are appropriate and fast.

Abstract

Stunting is one of the nutritional problems that the government focuses on because it risks disrupting child development. This study aims to the development of stunted children aged 2-5 years at the Sukorejo Community Health Centre in Blitar City. The research method used is descriptive quantitative with a population of 91 stunted children aged 2-5 years taken by purposive sampling technique conducted on 25 March to 20 April 2024 with the DDST (Denver Development Screening Test) sheet instrument with normal interpretation if there is only 1 caution on the age line, abnormal if there are 2 or more cautions on the age line, Suspect if there are more than 2 cautions or more than 1 delay. Based on the results of the analysis of the overall development of stunted children aged 2-5 years, 75 children (82%) were normal, 7 children (8%) were abnormal, and 9 children (10%) were suspect. The conclusion in this study is that most children show normal development because parents provide regular stimulus, although there are a number of children who have delays due to lack of stimulus from parents. This study provides insight for researchers, educational institutions, health centres, and parents to increase attention to the development of stunted children.

Abstract

Cerebrovascular Accident is a neurological deficit disease caused by bleeding or blockage with symptoms and signs according to the affected part of the brain, which can cause disability or death. The assessment found several complaints in the form of weakness of the extremities, not taking medication regularly, and the family did not know the care for family members with CVA. The nursing diagnosis in CVA patients is impaired physical mobility, ineffective family health management, and readiness to increase knowledge. The interventions determined for CVA patients are based on the previous diagnosis and theory. The implementation carried out on CVA patients is based on the previously determined interventions and is adjusted to the conditions and situations of the patient. Evaluation of CVA patients during two meetings found that the problem of ineffective family health management nursing was resolved, the problem of readiness to increase knowledge was resolved, and the problem of impaired physical mobility nursing was partially resolved due to time constraints in implementing it.

Abstract

Mental health has serious consequences for the future of adolescents. Poor mental health affects routine activities, damages relationships, and hinders development and productivity. Adolescents with mental health problems have difficulty completing schoolwork. Various sources state that self-esteem is a predictor of mental health. This study aims to determine the relationship between self-esteem and mental health in adolescents. The research design was correlational with a cross-sectional approach. The population was 279 adolescents of Srengat junior high school in grade 8. A sample of 164 people was determined using a purposive sampling technique. The independent variable is self-esteem, and the dependent variable is mental health. Data collection used a questionnaire, which involved filling out a Google form—analysis using Spearman-Rho with a confidence value (α) of 0.05. The results showed that most respondents had high self-esteem, namely 59.1%, and almost all had good mental health, namely 87.2%. The Spearman Rho test results showed a p-value = 0.000 (> α = 0.05), r = 0.424, meaning that there is a relationship between self-esteem and mental health of adolescents at Srengat junior high school with moderate strength. This study concludes a significant relationship between self-esteem and adolescents’ mental health; the better the self-esteem, the better the mental health of adolescents. Schools and parents should pay more attention to adolescents by taking a personal approach in an effort to improve mental health by increasing adolescent self-esteem.

Abstract

Controlling blood sugar levels is the key to the treatment program by reducing weight, diet, and exercise. Physical activity is one factor affecting glycemic management for Diabetes Mellitus (DM) patients. This case study aims to maintain the stability of blood sugar levels in DM patients by applying walking exercises. The design of this research is a case report with a nursing process approach carried out on Diabetes Mellitus patients with nursing problems of unstable blood sugar levels. The Bendo Community Health Center carried out nursing care for seven days. The intervention implemented is walking exercises. The case study results are presented in the form of nursing documentation. The case study results showed that the patient's blood sugar levels tended to be stable starting on the second day of the intervention. An increase in whole-body insulin sensitivity mostly explains exercise-induced glycemic control. The long-term advantages of frequent exercise on glycemic control are related to the cumulative effect of transitory gains in insulin sensitivity and glycemic control after each bout of exercise rather than structural changes in insulin sensitivity.

Abstract

Stunting cases continue to increase along with the high risk of malnutrition, increasing prevalence of malnutrition, and decreasing productivity. Several factors related to the incidence of stunting in toddlers include birth weight, history of infectious diseases and birth spacing. This research identifies the relationship between maternal employment and the incidence of stunting in children. The research design was cross-sectional, carried out on a population of mothers with toddlers, all of whom were taken as a sample of 20 people. Instruments used in collecting questionnaire data. The results of this study show that there is a significant relationship between maternal employment and the incidence of stunting, with a p value of 0.000 (p ≤ 0.05). These data imply that maternal employment plays an important role in children's nutritional attainment. Employment status is directly related to the household's ability to provide quality and sufficient food.

Abstract

Early identification of pregnancy risk factors using the Maternal and Child Health (MCH) handbook and the Poedji Rochyati Score Card can be carried out by pregnant women or community health volunteers (cadres). Supporting cadres in identifying high-risk pregnancies facilitates timely detection and appropriate intervention. This study aimed to develop a pregnancy risk detection booklet based on the MCH handbook. A cluster randomized controlled trial with a pretest-posttest control group design was conducted in eight villages, involving 30 selected cadres recommended by local midwives based on their active participation. The intervention group received training and assistance using the developed booklet. The results demonstrated that cadres in the intervention group had significantly greater improvement in pregnancy risk detection skills compared to the control group, with a mean difference of 18.7 (95% CI: 15.2–22.2, p=0.001). In addition, cadre performance in antenatal care using the 10T approach improved by 15.7% (95% CI: 11.3–20.1, p=0.001) in the intervention group relative to the control. These findings suggest that the booklet is an effective tool for enhancing cadres’ competencies in early pregnancy risk screening and service delivery. Further development of the booklet with more engaging and culturally relevant content may enhance its utility and adoption in community health settings.
