Abstract

Male infertility represents a major global health concern, with oxidative stress playing a central role in the deterioration of sperm quality. In recent years, growing attention has been directed toward herbal medicine as a potential alternative or complementary therapy due to its antioxidant and multi-target properties. However, a comprehensive long-term overview of the global research landscape in this field remains limited. A bibliometric and visualized analysis was conducted using publications indexed in the Scopus database from 2005 to 2025. Eligible peer-reviewed articles were retrieved following PRISMA guidelines. Bibliometric indicators were analyzed using Biblioshiny (R-Bibliometrix) to assess publication trends, leading contributors, and thematic evolution, while VOSviewer was employed to visualize international collaboration networks, co-citation patterns, and keyword co-occurrence clusters. A total of 562 articles were included, revealing a strong upward trend in scientific output, particularly after 2016, with an annual growth rate of 15.78% and a peak in 2025. Iran, China, and Nigeria emerged as the most productive countries, while the Faculty of Veterinary Medicine and several universities in Africa, the Middle East, and Asia were identified as leading institutions. Keyword analysis highlighted three dominant research fronts: semen quality assessment, testicular histopathology, and oxidative stress–related molecular mechanisms. The collaboration network indicated increasing global and South–South research synergy. The field of herbal medicine for male infertility has evolved into a mature and multidisciplinary research domain, increasingly grounded in mechanistic and experimental evidence. These findings provide a strategic overview of research hotspots and emerging trends, offering valuable guidance for future experimental and clinical investigations.

Abstract

Bullying is a form of aggressive behavior commonly experienced by adolescents and has significant impacts on both physical and mental health. Schools, as primary social environments for teenagers, often become settings where bullying occurs. Several factors contribute to this behavior, particularly peer influence, family roles, and social media use. This study aimed to analyze factors related to bullying behavior among adolescents in a junior high school, including respondent characteristics, peer influence, family roles, and social media usage, as well as their relationships with bullying behavior. This research used a quantitative approach with a cross-sectional design involving 71 eighth-grade students as respondents. Data were collected using structured questionnaires and analyzed with the Chi-Square test. The results showed significant relationships between peer factors (p=0.003), family roles (p<0.001), and social media use (p=0.001) with bullying behavior. A large proportion of respondents reported high social media usage (45.1%) and low family involvement (45.1%), while more than half (53.5%) were categorized as being at risk of engaging in bullying behavior. These findings indicate that adolescents are strongly influenced by their social environment, both offline and online. Negative peer interactions and limited family supervision can increase the likelihood of bullying behavior. In conclusion, peer factors, family roles, and social media use are significantly associated with bullying among adolescents. Therefore, collaboration between families, schools, and communities is essential through psychosocial assessments, increased family involvement, and character education programs to prevent and reduce bullying behavior.

Abstract

The PESIAR Program (Petakan, Sisir, Advokasi, dan Registrasi Map, Screen, Advocate, and Register) is an operational instrument of BPJS Kesehatan designed to accelerate the achievement of Universal Health Coverage (UHC) by increasing National Health Insurance (JKN) membership at the local level. However, the effectiveness of this program’s implementation is strongly determined by the institutional capacity and configuration that support it. This article aims to reposition institutional determination not only as a factor influencing program effectiveness, but as a model of administrative governance in achieving UHC. The study uses a mixed methods approach with a sequential explanatory design. Quantitative data were obtained through a survey of PESIAR Agents in Semarang City, while qualitative data were collected through open-ended questionnaires and interviews. The results show a strong, positive relationship between institutional determination and the effectiveness of the PESIAR Program. Qualitative findings further clarify that issues of program effectiveness largely originate from institutional capacity, the quality of cross-sector coordination, and the role of PESIAR Agents as field implementers. This study concludes that institutional determination deserves to be positioned as an administrative governance model for the PESIAR Program in promoting the achievement of UHC at the local level.

Abstract

The implementation of the tiered referral system and zoning policy by BPJS Kesehatan aims to ensure equitable access to health services. However, in practice, the utilization of services at Tere Margareth General Hospital (RSU Tere Margareth) in Medan remains suboptimal. This condition is reflected in the continuous decline of the Bed Occupancy Rate (BOR) from 2022 to 2024, as well as the low percentage of referrals from six primary health centers within its designated zoning area compared to other Class C hospitals. This study aims to explore the utilization of RSU Tere Margareth and to identify factors influencing its utilization following the implementation of the BPJS Kesehatan zoning and tiered referral policies. The study employed a qualitative approach using a phenomenological method. A total of 19 informants participated in this research, consisting of referred patients, heads of primary health centers, referral officers, and hospital management staff. Data were collected through in-depth interviews, observation, and documentation. Data analysis was conducted using the interactive model of Miles, Huberman, and Saldaña, which includes data reduction, data display, and conclusion drawing, supported by source triangulation. The findings indicate that the utilization of RSU Tere Margareth remains low. Contributing factors include the BPJS Kesehatan referral policy system, patients’ level of knowledge, accessibility, service quality, hospital image and reputation, clinical considerations by referral officers, as well as internal and external hospital conditions. It is recommended that the Medan City Health Office and BPJS Kesehatan Medan conduct periodic evaluations and monitoring to ensure equitable referral distribution among Class C hospitals, thereby promoting more balanced hospital utilization in Medan.

Abstract

Overweight is a growing global health concern, especially among female healthcare workers who are expected to serve as community role models. In Indonesia, adult obesity prevalence reached 23.4% in the 2023 SKI Survey, while Aceh recorded an increase from 24.4% in 2018 to 34.3% in 2023. This study aimed to analyze determinants of overweight among female healthcare workers at community health centers in Southeast Aceh Regency. The study was conducted from July to December 2025 using a cross-sectional design involving 191 respondents selected through cluster random sampling. Overweight status was measured anthropometrically, nutrient intake using the SQ-FFQ, physical activity through a 24-hour recall questionnaire, and sleep disturbances using the SSDA. Data were analyzed using chi-square and multiple logistic regression at α=0.05. Bivariate analysis showed significant associations between overweight and energy intake, protein intake, fat intake, physical activity, and sleep disturbances. Multivariate analysis identified physical activity as the dominant factor. Female healthcare workers with higher energy intake and low physical activity had a 98% probability of being overweight. Therefore, regular exercise and balanced nutritional intake are recommended to reduce overweight risk.

Abstract

Indonesia contributes nearly one-third of malaria cases in Southeast Asia (World Malaria Report 2023, 2023). In North Sumatra Province, Langkat Regency continues to report relatively consistent malaria incidence, particularly in forest fringe and plantation areas, although the absolute number of cases remains relatively low. A preliminary study conducted by (Nainggolan et al., 2022) identified several Plasmodium species in the region, including P. knowlesi, which is recognized as a cause of zoonotic malaria with long-tailed macaques serving as the natural host. The proximity of plantation habitats to forested areas increases the potential interaction among humans, vectors, and natural reservoirs of the disease, thereby facilitating cross-species transmission. This study aimed to analyze the factors influencing malaria incidence among plantation communities in Langkat Regency. This study employed an unmatched case-control design conducted from June to December 2025 in three selected healthcare facilities. Samples were collected using a consecutive sampling technique, consisting of 47 case groups and 94 control groups with a 1:2 ratio. Data were analyzed using univariate analysis, bivariate analysis with the chi-square test, and multivariate analysis using multiple logistic regression. The results showed that living in the same household with malaria patients (household clustering) (OR=22.89; p=0.004), activity time (OR=3.584; p=0.004), and residential distance from mosquito breeding sites (OR=7.838; p=0.018) were significantly associated with malaria incidence. One of the mosquito breeding sites identified in this study was latex collection cups in rubber plantations. Plantation environments adjacent to forest areas may strengthen the potential for residual transmission and reflect a focal transmission pattern at the village or sub-district level. This study concludes that malaria incidence among plantation communities is influenced by a combination of behavioral and environmental factors. Therefore, integrated malaria control measures are required through environmental health improvement, vector control, enhancement of community preventive behaviors, and strengthening surveillance in plantation and forest fringe areas.

Abstract

Ankle injuries are a common occurrence among students, particularly due to high levels of physical activity. The recommended initial treatment is the R.I.C.E. method, which stands for rest, ice, compression, and elevation. This study aims to analyze the impact of educational interventions using digital booklets on enhancing PMR students' ability to administer first aid for ankle injuries according to the R.I.C.E. method. The study employed a pre-experimental design featuring a one-group pre-test and post-test format. The population consisted of all PMR members at SMK Ahmad Yani Gurah Kediri, totaling 28 respondents selected through total sampling. The educational intervention took place over three sessions, each lasting 120 minutes, during which digital booklets were utilized. These booklets contained educational content, visual aids, and step-by-step instructions for applying the R.I.C.E. procedure. Data were gathered using psychomotor observation sheets and analyzed with the Wilcoxon Signed Rank Test. Findings revealed a significant improvement in the students' abilities; the average pre-test score rose from 44% to 82%, with a p-value of 0.000 (p ≤ 0.05). Prior to the digital booklet intervention, the PMR students' abilities were categorized as poor, reflecting limited knowledge and practical skills in first aid. Following the intervention, their abilities improved to a satisfactory level, demonstrated by their correct and sequential execution of the R.I.C.E. method. In conclusion, the use of digital booklets effectively enhanced PMR students' skills in administering first aid for ankle injuries using the R.I.C.E. method, thereby supporting the development of practical skills within the school environment.

Abstract

Job satisfaction is a key consideration for employees to remain with a company. Employees consider both internal and external satisfaction. Factors that can increase job satisfaction are individual characters, organizational support and work environment. The purpose of this study is to analyze the influence of individual characteristics, organizational support, and the work environment on job satisfaction. The researcher used a quantitative approach through the distribution of questionnaires and hospital data to 50 medical workers. The study findings show that individual characteristics do not significantly influence medical job satisfaction. Age, gender, education, employment status, length of service, and marital status did not significantly influence the results, with p-values > 0.05. Meanwhile, organizational support and work environment significantly influence the job satisfaction of medical workers at Pangeran Jaya Sumitra Hospital, Kotabaru. Superior support, reward support, and fairness support all showed significant effects, with p-values < 0.05. Similar results were found in the physical work environment and non-physical work environment, with a p-value <0.05. Therefore, based on this view, it can be concluded that hospitals need to pay attention to organizational support and the work environment in order to create a sense of satisfaction while working.

Abstract

The Free Nutritious Meal Program (MBG) is a government initiative aimed at improving population nutrition by providing safe, nutritious, and high-quality food. Food safety in this program is influenced by sanitation and hygiene conditions as well as the implementation of the Hazard Analysis Critical Control Point (HACCP) system. This study analyzes sanitation and hygiene conditions of facilities and buildings, food sanitation practices, personal hygiene of food handlers, and HACCP implementation at SPPG X and SPPG Y in Karo Regency in 2026. A descriptive observational design was used, with data collected through field observations, interviews, documentation, and checklists based on Permenkes No. 1096 of 2011. HACCP analysis was conducted from raw material receipt to food serving. Results show that SPPG X scored 58 (68.2%), below the minimum standard of 75%, while SPPG Y scored 65 (76%), meeting the standard. Several risks were identified in food processing, storage, and serving stages. Personal hygiene of food handlers was categorized as fairly good (>60%), although discrepancies between observations and questionnaires indicated possible response bias. SPPG X had more critical control points than SPPG Y. Improvements are needed in food processing, temperature control, PPE use, and monthly equipment sanitation. Consistent implementation of hygiene and HACCP principles is essential for ensuring food safety in the MBG program.

Abstract

Despite national efforts to promote exclusive breastfeeding, its coverage in Indonesia, including in Padang City, has not yet reached the national target of 80%. One of the factors contributing to this gap is the insufficient knowledge of pregnant women regarding exclusive breastfeeding. Therefore, this study sought to assess the level of knowledge among pregnant women concerning exclusive breastfeeding at the Dadok Tunggul Hitam Community Health Center in Padang City. A descriptive quantitative approach was employed involving 47 first-trimester pregnant women selected through simple random sampling from a population of 88 individuals. Data were gathered using a structured closed-ended questionnaire that assessed respondents’ knowledge of the definition, benefits, classification, and proper breastfeeding techniques, and were analyzed using frequency distributions. The findings indicated that more than half of the respondents (51.1%) demonstrated a high overall level of knowledge, while 42.6% had a moderate level and 6.4% had a low level. In terms of specific domains, knowledge of the definition of exclusive breastfeeding was predominantly moderate (44.7%), knowledge of its benefits was evenly distributed between the high and moderate categories (42.6% each), whereas knowledge of breastfeeding classification and proper breastfeeding techniques was mainly at the moderate level (46.8% each). These findings suggest that although the overall knowledge of exclusive breastfeeding among pregnant women was generally satisfactory, important knowledge gaps remain, particularly concerning breastfeeding classification and appropriate breastfeeding techniques. Strengthening health education programs through regular counseling sessions and the use of engaging visual educational media may help improve both knowledge and breastfeeding practices among pregnant women
