Abstract

In recent years, rapid advancements in digital technology have significantly transformed global healthcare delivery. The COVID-19 pandemic acted as a catalyst, accelerating the integration of information technology into healthcare systems, particularly through the increased adoption of telemedicine. In the field of internal medicine, which often involves long-term care and management of chronic illnesses, telemedicine presents a promising solution to enhance service accessibility, improve care efficiency, and sustain favorable clinical outcomes. This study aims to conduct a narrative review to explore the impact of telemedicine on patient clinical outcomes, healthcare worker workload, and healthcare system efficiency within internal medicine practice. The review is based on an analysis of peer-reviewed literature published between 2020 and 2025, sourced from major databases including PubMed, Scopus, and Web of Science. Article selection was based on relevance to the topics of telemedicine and internal medicine, contextual applicability, and full-text availability. The findings of the review reveal that telemedicine contributes positively by expanding access to care, reducing travel time and logistical barriers for patients, accelerating clinical response times, and decreasing patient congestion in healthcare facilities. These outcomes are particularly beneficial for patients with chronic conditions who require regular monitoring and consistent communication with healthcare providers. However, the implementation of telemedicine also introduces new challenges, including increased administrative responsibilities and digital workload for healthcare professionals. The study underscores the importance of equipping healthcare systems with adequate digital infrastructure, implementing supportive and flexible policies, and providing comprehensive training for healthcare workers. These measures are crucial to ensure the successful and sustainable adoption of telemedicine in internal medicine. Overall, telemedicine holds strong potential to reshape the future of internal medicine, provided that its integration is strategically managed to balance technological advancement with human-centered care.

Abstract

Stunting is a chronic nutritional problem in toddlers that remains a global concern, including in Indonesia. This condition is characterized by height that is not appropriate for age due to long-term malnutrition, repeated infections, and environmental and social factors. The impact of stunting not only affects physical growth but also cognitive development, future productivity, and increases the risk of degenerative diseases in adulthood. Various factors contribute to stunting, including biological factors such as the child's gender and behavioral factors such as maternal knowledge about nutrition and health. This study aims to analyze the relationship between child gender and maternal knowledge and the incidence of stunting in toddlers. The research design used a quantitative cross-sectional approach. A sample of 73 respondents was selected using a stratified proportional sampling technique. Data collection was conducted through structured interviews using questionnaires and anthropometric measurements of toddlers. Data analysis was performed using univariate analysis to describe frequency distributions, and bivariate analysis using the chi-square test with a 95% confidence level. The results showed no significant association between child gender and stunting (p > 0.05). This indicates that biological factors such as gender are not the primary determinant of stunting in the study area. Conversely, there was a significant relationship between maternal knowledge and the incidence of stunting in toddlers (p < 0.05), indicating that mothers with low levels of knowledge are at greater risk of having stunted children. These findings emphasize the importance of improving maternal knowledge through ongoing nutrition education programs, health counseling, and promotive-preventive activities at community health centers. These interventions are expected to prevent stunting and other health problems in toddlers and support the achievement of the national stunting reduction target.

Abstract

This study investigates the adoption of Electronic Medical Records (EMR) by registration staff at Tangkiling Health Center, Palangka Raya, using the Technology Acceptance Model (TAM) framework. Out of 61 total healthcare workers, five medical record staff with educational backgrounds in midwifery, nursing, dental nursing, and non-medical high school were purposively selected as research participants. The study employed a qualitative descriptive method, combining in-depth interviews and direct field observations. Results indicate that while the staff acknowledged the usefulness of EMR in improving service efficiency and data accessibility, several barriers hindered its effective use—primarily a lack of training, limited digital literacy, unstable internet connectivity, and inadequate hardware such as computers and printers. Observations showed that only the registration unit had a single shared computer, while other units like outpatient clinics, pharmacy, and cashier still relied on manual documentation. This fragmentation has led to duplicate workloads and disrupted patient data continuity. Despite these challenges, the staff demonstrated a positive attitude and strong behavioral intention to adopt EMR if provided with sufficient infrastructure and regular training. The study concludes that successful EMR implementation in primary healthcare requires not only user acceptance but also institutional readiness, adequate resources, and integrated systems across all service units.

Abstract

Background: Primary dysmenorrhea is abdominal pain, cramps and back pain experienced by women at the beginning of menstruation for 48-72 hours. This condition is most often experienced by women in their late teens to young adults (15-25 years) due to optimization of uterine function so that prostaglandin secretion increases. In Indonesia, the prevalence of primary dysmenorrhea reaches 54.89%, with a significant impact on adolescent activities. One of the effective non-pharmacological methods to overcome primary dysmenorrhea is abdominal effleurage massage, which is performed directly on the site of pain so that endogenous analgesics (endorphins) are released and pain transmission to the cerebral cortex is inhibited. Purposes: To analyze the effect of abdominal effleurage massage on reducing primary dysmenorrhea in adolescents. Research method: This study used a quantitative method with a pre-experimental one group pretest posttest design method. The population of this study was 45 adolescents. The sample was taken using a simple random sampling technique of 21 female students who experienced primary dysmenorrhea. Data collection was carried out using the NRS (Numeric Rating Scale) pain scale observation sheet. Data were analyzed using the Wilcoxon Signed Rank Test. Results: Based on the analysis test using the Wilcoxon Signed Rank Test, a significance value of 0.00 (p <0.05) was obtained, which means Ha was accepted. In other words, abdominal effleurage massage has an effect on reducing primary dysmenorrhea in adolescents. Conclusion: There is an effect of abdominal effleurage massage on reducing primary dysmenorrhea in adolescents.

Abstract

Modern healthcare services are undergoing transformation toward a more humanistic and patient-centered approach, emphasizing hospitality based services in hospitals. However, many healthcare institutions, particularly in outpatient settings, have not fully adopted this concept. Patients often experience psychological distress, and when coupled with an unsupportive physical environment, their comfort and overall experience may be diminished. These conditions can negatively affect perceptions of service quality, patient satisfaction, and loyalty toward hospital services. This study aimed to analyze the influence of patients’ psychological conditions and the physical environment on hospitality-based services, with patient experience as an intervening variable, at the internal medicine outpatient clinic of BM Hospital. A quantitative research design using a cross-sectional approach was employed. The study involved 200 respondents selected through purposive sampling. Data were collected using structured questionnaires with Likert-scale items and analyzed with Structural Equation Modeling Partial Least Squares (SEM-PLS). The results showed that patients’ psychological conditions, the physical environment, and patient experience simultaneously and significantly influenced hospitality-based services. Hypothesis testing confirmed that both psychological conditions and the physical environment had a direct and significant impact on hospitality-based services. Furthermore, patient experience played a mediating role, strengthening the relationship between these factors and perceived service quality. The findings highlight the need for hospitals to integrate psychological support and a conducive physical environment into outpatient care delivery. Enhancing these dimensions can improve patient experience, foster greater satisfaction and comfort, and build stronger loyalty toward hospital services

Abstract

The advancement of digital technology has transformed how people search for information and access health services. RSIA Clas C Cipondoh faces challenges related to its brand image, as indicated by low patient engagement with the hospital’s website and digital platforms, based on a preliminary survey. This study aims to analyze the simultaneous and partial effects of digital literacy, website quality, and ease of access on the brand image with digital marketing strategy as a moderating variable. A quantitative research method with a cross-sectional approach was employed. The study involved 152 outpatient respondents who had accessed the hospital website. Data were analyzed using index analysis with the Three Box Method and Moderated Regression Analysis (MRA) to test the moderating effect. Findings reveal that digital literacy, website quality, and ease of access simultaneously and significantly influence the hospital’s brand image. Partially, each of these variables also has a positive and significant effect on brand image. Moreover, the digital marketing strategy effectively moderates the relationships between digital literacy, website quality, and ease of access with brand image. The Three Box Method analysis indicates that although website quality falls into the high category, weaknesses remain in the interaction quality dimension. The study concludes that strengthening digital literacy, improving website quality, and ensuring ease of access supported by an effective digital marketing strategy can enhance the hospital’s brand image. Hospitals are advised to add interactive features such as chatboxes or live chat to improve patient engagement and online service experience.

Abstract

Student mental health is an important issue in higher education, yet many students remain reluctant to utilize counseling services. Several factors are suspected to influence students’ willingness to participate in counseling, including stress levels, social support, public stigma, and self-stigma. This study aimed to determine the relationship between stress level, social support, public stigma, and self-stigma with students’ intention to engage in mental health counseling. The research used a quantitative design with a cross-sectional approach. A total of 202 first-year nursing students from Campus I of Poltekkes Kemenkes Semarang were selected using a total sampling technique. The research instrument was a questionnaire that had been tested for validity and reliability, and data were analyzed using Spearman’s rho correlation test. The results showed that most students had a moderate level of stress (52.8%), adequate social support (58.1%), moderate public stigma (53.7%), and moderate self-stigma (55.3%). Students’ willingness to seek counseling was also in the moderate category (63.4%). Statistical analysis indicated a significant relationship between stress level (p=0.000), social support (p=0.002), public stigma (p=0.000), and self-stigma (p=0.000) with the intention to engage in mental health counseling. These findings highlight the importance of promotive and preventive approaches from educational institutions to enhance students’ awareness and access to mental health counseling services.

Abstract

Background: Patient safety culture forms the cornerstone of safe and high-quality healthcare delivery. However, its implementation often encounters barriers, particularly the persistence of a blaming culture that discourages staff from reporting patient safety incidents. Clinical leadership and the intensity of incident reporting are believed to play a pivotal role in shaping and sustaining a positive patient safety culture. Objective: This study aims to examine the influence of clinical leadership and patient safety incident reporting intensity on patient safety culture, with blaming culture serving as an intervening variable at Sentra Medika Cikarang Hospital. Methods: A quantitative research approach with an explanatory design was applied. The study involved 147 nurses selected through stratified random sampling. Data were collected using structured questionnaires and analyzed using Structural Equation Modeling (SEM) with the Partial Least Squares (PLS) method to test the direct and indirect relationships among variables. Results: The findings revealed that both clinical leadership and incident reporting intensity significantly influence patient safety culture, both directly and indirectly, through the mediation of blaming culture. Strong clinical leadership and a high level of incident reporting were associated with a more positive patient safety culture, while a high blaming culture weakened this relationship. Conclusion: The study underscores the importance of fostering supportive clinical leadership and cultivating a non-punitive reporting environment to strengthen patient safety culture. Hospital management should focus on leadership development and the creation of open, blame-free communication systems to enhance safety outcomes.

Abstract

This study aims to evaluate the Resource Development and Installation (RDI) technique to reduce symptoms of Posttraumatic Stress Disorder (PTSD) in female victims of domestic violence. Domestic violence (which falls under the category of intimate partner violence/IPV) is a global public health problem that contributes significantly to women's mental health disorders, including post-traumatic stress disorder (PTSD). Recent systematic evidence shows a strong association between various forms of IPV and adverse mental health outcomes in women, with large variations in prevalence across contexts. Resource Development and Installation (RDI) is a psychological stabilization procedure within EMDR aimed at generating positive resources in individuals exposed to traumatic experiences. The research method used in this study was multiple baseline with a single case (single case research), namely research conducted on subjects with the aim of determining the magnitude of the effect of treatment given repeatedly over a certain period. This study involved three adult female participants who experienced trauma symptoms due to violence perpetrated by an intimate partner in a domestic context. The data analysis technique used in this study was visual inspection by comparing changes in trauma scores in respondents between the baseline and intervention phases. Trauma symptoms were measured using the Harvard Trauma Questionnaire (HTQ). There was a decrease in HTQ scores from an average of 2.9 (presence of PTSD symptoms) before the intervention to 2.1 (minimal). Resources Development and Installation were effective in reducing trauma symptoms in female victims of violence. This was evident in the decrease in trauma symptom scores between before and after the provision of Resources Development and Installation to female victims of violence.
