Abstract

The management of expired medications in community health centers (Puskesmas) is crucial because it directly relates to the quality of healthcare services, patient safety, and environmental impact. Medications that have passed their expiration date no longer guarantee effectiveness and safety, and therefore require proper handling in accordance with applicable regulations. Furthermore, improper management can lead to waste and the risk of misuse. This study was conducted to describe the expired medication management system in community health centers (Puskesmas) in Pemalang Regency, specifically regarding separation, recording, reporting, and destruction. This study used a descriptive method with a cross-sectional approach to describe the conditions in the field. The data used were primary data obtained through direct observation and interviews with pharmacists at the community health centers. Sampling was conducted using purposive sampling at five community health centers: Losari, Purwoharjo, Sarwodadi, Rowosari, and Kebandaran. The instrument used was an observation sheet as an interview guide, while the collected data was analyzed descriptively to provide an overview of the implementation of expired medication management. The study results indicate that all community health centers (Puskesmas) have properly separated, recorded, and reported expired medications in accordance with regulations. Overall, the management of expired drugs in health centers in the Pemalang Regency area has been running well and in accordance with the guidelines of the Indonesian Food and Drug Supervisory Agency (BPOM) Regulation Number 20 of 2025 concerning Good Distribution Practices for Medicines (CDOB), reaching 100%.

Abstract

The performance of health workers is an important element in ensuring the quality of primary health services. In addition to technical and structural factors, psychological aspects such as job meaningfulness and work engagement are increasingly recognized as playing a role in improving performance. Health workers who interpret their work as something of value tend to show higher work attachment, which ultimately has an impact on performance. This study aims to analyze the effect of job meaningfulness on the performance of health workers through work engagement at the Mokoau Health Center, Kendari City. This study used a quantitative approach with a cross-sectional design. All health workers at the Mokoau Health Center as many as 70 people were involved as respondents through the total sampling technique. Data were collected using a structured questionnaire and analyzed using the Partial Least Squares–Structural Equation Modeling (PLS-SEM) method using SmartPLS version 4.0. The results of the analysis showed that job meaningfulness had a positive and significant effect on work engagement (β = 0.563; p < 0.05). Furthermore, work engagement had a positive and significant effect on the performance of health workers (β = 0.638; p < 0.05). The indirect influence test proved that work engagement significantly mediated the influence between job meaningfulness and performance (β = 0.360; p < 0.05). Job meaningfulness improves the performance of health workers through strengthening work engagement. Therefore, the HR management strategy at the Health Center needs to be directed at strengthening the meaning of work and work attachment to improve performance in a sustainable manner.

Abstract

Data on malnutrition in toddlers at the Amondo Health Center in 2023 were 47 toddlers, then in 2024 from early January to November, malnutrition data increased by 64 toddlers. This study aims to determine the Risk Factors for Malnutrition in Toddlers in the Working Area of ​​the Amondo Health Center, South Palangga District, South Konawe Regency in 2025. The design of this study is quantitative with a cross-sectional approach. The population of this study was all toddlers registered in the working area of ​​the Amondo Health Center, South Palangga District, South Konawe Regency in 2024 with a total of 966 people. The sampling used was Accidental Sampling. To obtain a representative sample, the selection of subjects from each stratum or certain region was determined in a balanced or comparable manner in each matched region. The sample size used was 91 respondents. The data were analyzed using the SPSS application with the Odds Ratio test. The OR test results obtained knowledge values ​​(OR = 1.08), eating habits (OR = 0.70), family income (OR = 1.10), carbohydrate intake (OR = 2.47), protein intake (OR = 0.91) and fat intake (OR = 1.30). The conclusion that maternal knowledge, children's eating habits, family income, and nutrient intake (carbohydrates, proteins and fats) in the past are risk factors for malnutrition. It is recommended that health workers always provide information about malnutrition to the community, carry out vegetable planting movements in the community, do not carelessly give food to toddlers, always consume and provide nutritious food for the family

Abstract

Background: Nurses’ workload is one of the factors that influence the quality of nursing care and patient safety. The level of patient dependency is a key factor determining the magnitude of nurses’ workload because it relates to the time, energy, and attention required to provide nursing care. Objective: To analyze nurses’ workload based on the level of patient dependency in Ward Lily 1 at Soerojo Hospital in Magelang. Methods: A descriptive quantitative design with a cross-sectional approach. The study sample consisted of 21 nurses selected using total sampling. Data were collected using a nurse workload questionnaire and the Douglas method’s patient dependency level classification form, then analyzed using the chi-square test. The results showed that the majority of nurses experienced heavy workloads (14 nurses, 67%), while 5 nurses (24%) experienced moderate workloads. Result: Patient dependency levels were dominated by the partial care category (12 patients, 57%), followed by minimal care (4 patients, 19%) and total care (5 patients, 24%). The chi-square analysis yielded a significant p-value of 0.001 < α = 0.005. Implication: It was concluded that there is a relationship between patient care levels and workload.

Abstract

Background: Acute pain is a common problem experienced by patients after cholelithiasis surgery due to tissue injury and the inflammatory process resulting from surgical procedures. Uncontrolled pain can interfere with mobilization, rest, and the recovery process. Slow deep breathing is a non-pharmacological intervention that can help reduce pain by promoting relaxation and decreasing sympathetic nervous system activity. Objective: To determine the application of Evidence-Based Nursing through slow deep breathing techniques for acute pain management in post-cholelithiasis surgery patients. Methods: A case study was conducted on post-cholelithiasis surgery patients experiencing acute pain. Pain intensity was assessed using the Visual Analog Scale (VAS) before and after the implementation of slow deep breathing therapy for approximately 5–10 minutes. Data were collected through interviews, observations, physical examinations, and nursing documentation. Results: The implementation of slow deep breathing resulted in a reduction in pain intensity. Patients reported decreased pain levels, appeared more relaxed, showed reduced facial grimacing, and experienced improved comfort after the intervention. Implications: Slow deep breathing is effective in reducing acute pain in post-cholelithiasis surgery patients and can be applied as a complementary non-pharmacological nursing intervention in pain management.

Abstract

Pre-conference and post-conference are essential components of nursing management functions that play a significant role in enhancing communication, team coordination, continuity of nursing care, and patient safety. However, their implementation in inpatient wards continues to face several challenges, including high nursing workloads, limited time availability, and suboptimal adherence to Standard Operating Procedures (SOPs). This study aimed to analyze the effectiveness of a refresher program on the implementation of pre-conference and post-conference activities in an inpatient ward. A descriptive case study design was employed involving 21 nurses as research participants. Data were collected through direct observation, in-depth interviews, and document review of pre-conference and post-conference practices before and after the intervention. The refresher intervention consisted of SOP education, interactive discussions, and role-play sessions focusing on the proper implementation of pre-conference and post-conference activities. The findings demonstrated a notable improvement in the implementation of both activities following the intervention, as reflected by increased compliance with SOPs, enhanced nurses' understanding of conference procedures, and improved communication and coordination among nursing staff. The Wilcoxon Signed Ranks Test revealed a statistically significant difference between pre- and post-intervention conditions (p < 0.001), indicating that the refresher program was effective in improving the implementation of pre-conference and post-conference activities. These findings suggest that regular refresher programs may serve as a sustainable strategy for improving the quality of nursing services and promoting patient safety in inpatient settings.

Abstract

Chronic Kidney Disease (CKD) is a chronic kidney condition characterized by progressive and irreversible decline in kidney function. CKD patients undergoing hemodialysis often experience fatigue due to anemia, accumulation of uremic toxins, and the prolonged duration of the hemodialysis process. Unresolved fatigue can reduce quality of life and the ability to perform daily activities. One nonpharmacological intervention that can be used to address fatigue is slow deep breathing therapy. The purpose of this study was to determine the application of slow deep breathing in CKD patients undergoing hemodialysis. This study used a qualitative approach, specifically a case study with a nursing care approach. The study subjects were two CKD patients. The therapy was administered using slow deep breathing techniques for 5 minutes, with 1–3 cycles, over three sessions. Data collection methods included interviews and observations. Data collection instruments comprised nursing assessment forms, head-to-toe physical examinations, and supporting data.  After providing nursing care interventions over three sessions to the two patients, the results showed a reduction in complaints of weakness and fatigue; the patients appeared more relaxed and comfortable, and were better able to perform light activities.Slow deep breathing therapy as a nonpharmacological intervention to help reduce fatigue in CKD patients undergoing hemodialysis

Abstract

Delayed onset of breast milk production is a common problem experienced by postpartum mothers and can lead to failure to achieve exclusive breastfeeding. This condition may be influenced by insufficient stimulation of the hormone oxytocin, maternal anxiety, and a lack of knowledge regarding proper breastfeeding techniques. The purpose of this study was to analyze the application of oxytocin massage on breast milk let-down in postpartum mothers with ineffective breastfeeding nursing problems. This study used a case study design with a nursing care approach on five postpartum mothers experiencing delayed breast milk let-down in the Mawar Ward of Kardinah Regional General Hospital, Tegal City. Data collection was conducted through interviews, observations, physical examinations, and document reviews. The nursing interventions provided included breastfeeding education, family support, and oxytocin massage for 10–15 minutes over three consecutive days. The study results showed that all patients experienced improved milk let-down, characterized by smoother milk flow, increased breastfeeding frequency, babies appearing calmer after feeding, and improved maternal proficiency in performing proper breastfeeding techniques. Additionally, mothers’ knowledge regarding exclusive breastfeeding and oxytocin massage also increased. The application of oxytocin massage has been proven to help increase milk production in postpartum mothers and can therefore be used as an evidence-based nursing intervention to support the success of exclusive breastfeeding.

Abstract

Chronic Kidney Disease (CKD) is a progressive and irreversible disorder requiring hemodialysis as renal replacement therapy. Fatigue is a frequent symptom among hemodialysis patients and negatively affects quality of life and daily activities. This case study aimed to determine the effect of the Benson relaxation technique on fatigue in a CKD patient undergoing hemodialysis at RSUD Gunung Jati, Cirebon. The method used was a single-subject descriptive case study with nursing intervention applied in three sessions during pre, intra, and post hemodialysis periods. Data were obtained through assessment, observation, and fatigue evaluation. The intervention consisted of deep breathing combined with repetition of calming spiritual or meaningful words according to patient belief. Results showed a decrease in fatigue, indicated by reduced weakness, increased relaxation, improved comfort, and better emotional stability. The patient also reported increased energy after the intervention period. This study suggests that the Benson relaxation technique can be an effective non-pharmacological nursing intervention to reduce fatigue in hemodialysis patients and may be applied as a complementary approach in nursing care.

Abstract

Diabetes Mellitus (DM) is a chronic metabolic disease characterized by elevated blood glucose levels (hyperglycemia) resulting from impaired insulin secretion, insulin action, or both. Non-adherence to dietary recommendations is one of the factors that may trigger hyperglycemia in patients with DM, as poor dietary adherence can increase blood glucose levels (random blood glucose [RBG] > 200 mg/dL). According to the World Health Organization (WHO), approximately 422 million people worldwide have diabetes mellitus, particularly in low- and middle-income countries, including Indonesia, with approximately 1.6 million deaths occurring each year (World Health Organization, 2018). This study aimed to determine the relationship between dietary adherence and the incidence of hyperglycemia. The study was conducted in July 2026 using a quasi-experimental design with a one-group pretest-posttest design. The results of the study conducted in one of the working areas of Biru Public Health Center showed that some community members adhered to the recommended diet, while others did not, which may affect blood glucose levels. The level of dietary adherence among patients with DM was reported to be 65%; however, only 19% of patients actually followed the recommended diet. The findings are consistent with previous research indicating that maintaining normal blood glucose levels is important to prevent the effects of elevated blood glucose, including glucose loss through urine.
