Abstract

Dengue Haemoragic Fever (DHF) is an infectious disease caused by the dengue virus transmitted through the bite of the Aedes aegepty mosquito. Dengue Haemoragic Fever can cause complications namely dengue shock syndrome and cause death especially in children. Preschoolers with DHF often experience anxiety due to hospitalization, one of the treatments is play therapy, then an intervention is prescribed plasticine play therapy. Plasticine play therapy is playing night candles that are soft and easy to shape as desired with a variety of colors of choice and can play on the bed and can reduce anxiety. The purpose of this case study was to identify the level of anxiety before and after plasticine play therapy in preschool children (3-6 years) with dengue haemoragic fever due to hospitalization of moderate anxiety. Plasticine focuses on reducing anxiety in children and helps increase children's creativity. The method used is an analytical descriptive method in the form of a case study with a nursing process approach. The results of this study showed that children experienced anxiety, tension and fear, nursing actions were carried out so that the nursing problem raised was anxiety. Plasticine play therapy was carried out on one child using the Facial Anxiety Scale (FAS) instrument with 3 days of plasticine play therapy from April 30-May 2, 2024 in a day 1 meeting for 10-15 minutes with the results of anxiety resolved on a scale of 2 (no anxiety). This study obtained data that there was a decrease in anxiety levels in An. R age 6 years from a score of 4 (moderate anxiety) to a score of 2 (no anxiety). Abstract

Low levels of knowledge and awareness of the importance of monitoring fetal well-being can have a negative impact, even triggering increased hemorrhage and intrauterine fetal death. (IUFD). The purpose of this case study is to raise the knowledge and awareness of pregnant mothers about the importance of monitoring the well-being of the fetus independently. This case study was shown to pregnant primegravid mothers at the age of 14-28 weeks of pregnancy to perform fetal movement calculations. The intervention used in this case study was an education and demonstration counting fetal movement for 12 hours. The results of the case study showed an increase in the mother's knowledge of fetal motion from 62.5 to 83.3 on the last day of the intervention. It could indicate that there was an increase in knowledge. Application of the fetal Movement count education to pregnant mothers gives awareness of the importance of fetal monitoring.

Abstract

Based on Riskesdas data in 2018, the prevalence of heart disease in Indonesia is around 1,017,290 population. Congestive heart failure is a condition in which the heart is unable to pump blood throughout the body properly. Signs of symptoms that usually appear are dyspnea and edema, where edema is a buildup of fluid or hypervolemia in several organs of the body. If not treated immediately can cause complications in the form of cardiogenic shock, thrombolytic episodes and pericardial effusion or cardiac tamponade. One of the non-pharmacological management that can be done to overcome hypervolemia is a contrast bath. Contrast bath is a treatment by soaking the feet in warm water, followed by cold water alternately. The temperature of the warm water used is 36,6 – 43,3 °C and the temperature of cold water is 10 - 20 °C. This case study aims to illustrate nursing care with the application of contrast bath in congestive heart failure patients who have hypervolemia. This type of case study method used a descriptive method with 1 patient with inclusion criteria for congestive heart failure patients who have lower extremity edema. The assessment showed that the patient felt tight and there was edema of the lower extremities, so that the nursing diagnosis that was established was hypervolemia with the implementation of the application of contrast bath carried out 3 times a day for 2 consecutive days with a time of 15 minutes once an action. The results of the case study of the application of contrast bath showed that there was a decrease in the degree of edema in both lower extremities which was originally at a degree of +3, to a degree of +2 after the intervention. It can be concluded that this contrast bath can overcome hypervolemia in congestive heart failure patients, the author also recommended to patients and families to perform this intervention independently.

Abstract

Exclusive breastfeeding globally has only reached 48%, shy of the 2025 target of 50%. In Indonesia, 1 in 5 newborns is given food other than breast milk within first 3 days of life. One contributing factor to the failure of exclusive breastfeeding is poor latch-on. Inadequate breastfeeding can lead to risks such as malnutrition, childhood diabetes/obesity, lower cognitive development, and a higher risk of infection due to insufficient immunoprotection. Improper latch can also worsen mother condition like nipple trauma and psychological distress due to pain. This case study aims to implement nursing care for a post-SC mother, with a focused intervention on improving the baby's latch using the cradle hold position. The intervention was carried out over 3 days, during a single breastfeeding session each day. The case study revealed an improvement in the LATCH score from 7 (adequate) on the first day, to 8 (good) on the second day, and 10 (excellent) on the third day. Additionally, the mother reported increased confidence in breastfeeding, reduced pain and more optimal post-surgery recovery. Cradle hold position proved effective in enhancing infant latch and supporting successful breastfeeding of post-SC mother. The author recommends integration of breastfeeding education into health services, particularly at Dustira Hospital to prevent lactation issues and help ensure more successful exclusive breastfeeding programs.

Abstract

Stroke is the second leading cause of death after heart disease and the third leading cause of disability worldwide (American Heart Association, 2020). Non-hemorrhagic stroke refers to a decline in brain function due to disrupted blood supply to parts of the brain, resulting in vessel blockage or narrowing. This study aimed to evaluate the effectiveness of A.I.U.E.O speech therapy in enhancing verbal communication in non-hemorrhagic stroke patients in the medical rehabilitation unit at Cibabat Regional General Hospital. A.I.U.E.O speech therapy is an intervention that supports improved verbal communication in patients with speech disorders. This case study used a nursing process approach involving two patients with impaired verbal communication. The intervention was administered for 3 consecutive days, with each session lasting 15 minutes. Results showed improved verbal communication abilities by the third day following at least three therapy sessions. Patient 1 could articulate words clearly, construct longer sentences, showed more responsive facial expressions, improved conversation comprehension, and experienced reduced slurred speech. Patient 2 could articulate several words, showed responsive facial expressions, moderately improved conversation comprehension, and had moderate slurring. Contributing factors included diet in patient 1 and excessive stress in patient 2, which slowed recovery. In conclusion, A.I.U.E.O speech therapy is effective in enhancing verbal communication abilities in non-hemorrhagic stroke patients and is recommended as part of nursing care to support stroke rehabilitation.

Abstract

Bronchial asthma is a chronic respiratory disease caused by narrowing of the airway due to a hypersensitivity reaction in the bronchi, causing symptoms in the form of wheezing, coughing, and shortness of breath. Buteyko's breathing technique is a non-pharmacological method that focuses on conscious breath control, with the principle of reducing the frequency and volume of breathing through the practice of holding the breath and breathing slowly controlled through the nose.  The method of this case study is a descriptive method, aiming to describe nursing care in bronchial asthma patients. The subject of this case study used 2 patients as the subject material of the case study, with a nursing diagnosis of ineffective breathing patterns due to bronchial asthma. The focus of the intervention was to provide the application of buteyko breathing techniques for 10 minutes within 3 days with a frequency of 2 times daily. The results showed that both patients experienced improvement in respiratory condition after the 3rd day of application. In patient 1, the respiratory frequency decreased from 28x/min to 20x/min and oxygen saturation increased from 95% to 98%, the use of the respiratory and respiratory muscles of the nasal lobes also decreased significantly. Meanwhile, patient 2  showed a decrease in breathing frequency from 30x/min to 20x/minute, an increase in oxygen saturation from 94% to 97%, but the diffusion occurred more slowly than patient 1. Limitations in the application of buteyko breathing techniques include differences in response speed between patients and the need for repeated education so that patients understand the application correctly. Buteyko breathing techniques are recommended as an effective nonpharmacological intervention in the management of ineffective breathing patterns in bronchial asthma patients, both in hospital and independently at home with consistent training for optimal outcomes.

Abstract

Edema in the lower extremities is one of the most common physiological discomforts experienced by pregnant women in the third trimester. The World Health Organization reported in 2020 that there was an approximately 75% increase in the incidence of foot swelling during the second and third trimesters of pregnancy. This case study aims to implement nursing care, which includes assessment, data analysis, diagnosis determination, intervention planning, implementation, evaluation, and documentation. The focus of the intervention is a warm kencur (aromatic ginger) foot soak therapy as a natural herbal remedy to reduce the degree of edema and relieve acute pain in third-trimester pregnant women. The intervention was carried out for five consecutive days, with a duration of 10 minutes per day. Evaluation results showed a reduction in the degree of edema and pain scale. On the first day, the client experienced grade 3 edema, which decreased to grade 2 by the third day and reached grade 1 by the fifth day. This indicates that warm kencur foot soak therapy is effective in reducing the degree of edema in third-trimester pregnant women. The author recommends that third- trimester pregnant women experiencing foot edema regularly perform warm kencur foot soak therapy to help reduce swelling.
