Abstract

Chronic Obstructive Pulmonary Disease (COPD) is a progressive lung disease characterized by persistent airflow limitation and chronic respiratory symptoms such as dyspnea and excessive sputum production. Ineffective airway clearance is one of the most common nursing problems in COPD patients due to mucus retention. Postural drainage and effective coughing techniques are non- pharmacological interventions aimed at facilitating sputum removal and improving ventilation. This descriptive case study aimed to describe nursing care through the implementation of postural drainage and effective coughing techniques in a COPD patient with ineffective airway clearance at Kenanga Ward, TK II Dustira Hospital Cimahi. The intervention was carried out twice daily for three consecutive days, with a duration of 20 minutes per session. The evaluation results showed improvement in airway clearance, characterized by decreased adventitious breath sounds (rhonchi and wheezing), reduced respiratory rate, and easier sputum expectoration. It can be concluded that postural drainage and effective coughing techniques can improve airway clearance in COPD patients. These interventions are expected to be applied independently by nurses and families to prevent respiratory complications.

Abstract

Burns are traumatic injuries that frequently occur in children with significant impact on hand function. Grade 2-3 burns on bilateral hands can cause severe pain, limitation of range of motion (ROM), decreased muscle strength, edema, and impairment of daily functional activities. To determine the effectiveness of physiotherapy management with exercise modalities on pain reduction, ROM improvement, edema reduction, muscle strength enhancement, and functional ability in a case of bilateral grade 2-3 hand burns in a 12-year-old child. This study employed a case study method with a pre-post intervention design on one 12-year-old male patient diagnosed with grade 2-3 burns on both hands (4% total body surface area). Physiotherapy intervention was conducted for 6 treatment sessions following the wound healing phases (acute inflammation, transition, and early proliferation) with a program comprising: continuous elevation, positioning, passive range of motion (PROM), active-assisted range of motion (AAROM), massage, circulation exercises, isometric exercises, and functional training. Measured parameters included: pain (VAS), wrist and finger ROM (goniometer), muscle strength (MMT), edema (circumferential measurement), and functional ability (Barthel Index). After 6 treatment sessions, significant improvements were observed in all parameters. Resting pain decreased from VAS 8/10 to 4/10 (50% reduction), pain with movement from 9/10 to 6/10 (33.3% reduction). Wrist ROM showed improvement: right dorsiflexion- palmarflexion from 15°-0-20° to 35°-0-40° (133% and 100% increase), left from 20°-0-25° to 40°-0-45° (100% and 80% increase). MCP ROM increased 10°, PIP 10°, and DIP 5° bilaterally. Muscle strength for grip improved from unmeasurable to 2+/5, pinch strength to 2+/5 (right) and 3-/5 (left), wrist muscles from 2/5 to 3/5. Edema reduced by 0.9 cm (right) and 1 cm (left). Barthel Index increased from 70 to 85 (21.4% improvement). Physiotherapy management with exercise modalities adapted to wound healing phases is effective in reducing pain, improving ROM, decreasing edema, enhancing muscle strength, and improving functional ability in a case of bilateral grade 2-3 hand burns in a child. A comprehensive and progressive therapy program is crucial to prevent long-term complications such as contractures and hand function impairment.

Abstract

Hypertension is one of the chronic diseases that causes cognitive impairment and a decline in the quality of life for the elderly. Hypertension is a condition where blood pressure rises above normal values, with systolic blood pressure ≥140 mmHg and diastolic blood pressure ≥90 mmHg. The prevalence of hypertension increases every year, reaching 34.11% in Indonesia, with higher rates among women and in urban areas. The application of finger hold therapy is one of the complementary therapies that can be useful in lowering blood pressure in hypertensive clients, especially the elderly. Finger hold therapy is an alternative therapy that can stimulate the parasympathetic nervous system, thereby reducing blood pressure, heart rate, and respiratory rate. The method used in this case study is descriptive. This case study aims to describe nursing care for elderly families (≥60 years) with hypertension through the complementary therapy of finger hold therapy. The instruments used were a sphygmomanometer, standard operating procedure for finger hold therapy technique, and an observation sheet to determine the results before and after the intervention. The finger hold therapy technique was performed for 3 days for 30-60 minutes once in the morning. The case study results show that there was a decrease in blood pressure after the finger hold therapy technique was applied, in accordance with the target to be achieved. The evaluation obtained in this case study is that the nursing diagnosis of ineffective peripheral perfusion was resolved. It can be concluded that the finger hold therapy technique can be administered to elderly clients with hypertension because the finger hold therapy performed over 3 days was able to reduce the initial blood pressure from 160/100 mmHg to 140/90 mmHg. The author's suggestion to the client is to perform finger hold therapy as a complementary therapy to the core therapy with medication.

Abstract

Surgical procedures can lead to various postoperative clinical problems such as pain, anxiety, nausea and vomiting, sleep disturbances, and fatigue, all of which require comprehensive management. Reflexology, as a non-pharmacological complementary therapy, has the potential to support postoperative management through stimulation of reflex points on the soles of the feet, hands, and ears. This study aims to synthesize scientific evidence regarding the effectiveness of reflexology as a complementary intervention in postoperative patient management. A systematic review was conducted following the PRISMA 2020 guidelines. Literature searches were performed in PubMed, Scopus, Web of Science, CINAHL, EBSCOhost, and the Cochrane Library for publications from January 2010 to December 2024. The inclusion criteria were determined using the PICO framework, and methodological quality was assessed using the JBI Critical Appraisal Tools. Out of 1,032 identified articles, 15 studies met the inclusion criteria (13 randomized controlled trials, 1 clinical trial, and 1 randomized study). The synthesis results indicate that reflexology significantly: (1) reduces postoperative pain intensity, (2) decreases anxiety levels, (3) stabilizes physiological/hemodynamic parameters, (4) improves sleep quality, and (5) reduces fatigue. These effects were consistently observed across various surgical procedures, including coronary artery bypass graft (CABG), abdominal surgery, hysterectomy, and cesarean section. Reflexology has the potential to serve as a safe, effective, and practical complementary intervention to support postoperative patient management. Further studies with multicenter designs, larger sample sizes, and more standardized protocols are recommended.
