Abstract

Background: Improper management of all types of hospital waste has become a significant problem relevant to public health issues. However, even though there are regulations regarding waste management, the implementation still varies between hospitals. The most neglected factor is the waste management supervision frameworks.  Purpose: To identify the emerging concern of supportive supervision in hospital waste management. Method: This formative delphi study employed qualitative methods and focus group discussions (FGDs). Each FGD lasted 60 minutes. The study incorporated the concept of stakeholder engagement in hospital waste governance, focusing on factors driving stakeholder participation, knowledge-sharing structures, information exchange, decision-making processes, and the institutionalization of various centers of power and decision-making to diversify waste management oversight. Results: Participants were most likely to decide to improve waste management. Of the 18 questions, three received a higher number of "yes" responses (53.8%). The first question concerned managers in the environmental, occupational safety, and health department providing recognition/appreciation for staff performance or achievements in waste management at the hospital. The second question concerned information provided by managers to staff regarding open and periodic performance appraisals. The third question concerned the use of audit results to improve the existing waste management system. Conclusion: Supervision is an activity aimed at improving service quality. It requires a conditional and open environment to accept problem-solving. To enhance the quality of service, supervision must be systematic and require continuous follow-up.

Abstract

Background: Depression is prevalent in hemodialysis patients and may contribute to anemia, yet its impact on hemoglobin levels remains underexplored. Purpose: To examined the association between depression severity and hemoglobin levels in Indonesian hemodialysis patients. Method: A cross-sectional study was conducted at a single-site hemodialysis unit from October to December 2024. A total of 101 patients were selected using purposive sampling. Depression severity was assessed using the Beck Depression Inventory-II (BDI-II), and hemoglobin levels were measured via venous blood samples. Spearman’s correlation and linear regression analysis were used to analyze the association. Results: Depression severity was negatively correlated with hemoglobin levels (r = -0.62, p < 0.01). Patients with severe depression had hemoglobin levels nearly 3 g/dL lower than those with minimal depression. Regression analysis showed depression severity significantly predicted hemoglobin levels (Adjusted R² = 0.38, p < 0.01). Conclusion: Higher depression severity is associated with lower hemoglobin levels in hemodialysis patients. Routine mental health screening should be integrated into nephrology care, with nurses playing a key role in early detection and intervention. Future multi-center studies are needed to enhance generalizability.

Abstract

Background: Patients with chronic diseases generally experience repeated hospitalizations. Cultural diversity can affect communication and engagement from health care providers and recipients. An interactive culturally sensitive telenursing application is needed as a form of continuous nursing intervention when patients are at home. Purpose: To build and test the culturally sensitive telenursing application of Suster Kita. Method: Using the ADDIE (Analysis, Design, Development, Implementation, Evaluation) process design. The research was conducted from August 2022 to February 2024 and tested on 20 respondents in the DKI Jakarta area. The evaluation of the effectiveness of the Suster Kita culturally sensitive telenursing application at the implementation stage was carried out using a questionnaire on the use of the application developed by the research team. The final results of the application are evaluated 2 weeks after the respondent uses the application. Results: the use of the application showed that the average knowledge of chronic diseases and their treatment consisting of 10 knowledge components increased before and after the trial of the use of the culturally sensitive application of Suster Kita. Conclusion: the intervention is acceptable to users and is worth using. Application suggestions are integrated with laboratory results.

Abstract

Background: Mental health is an important aspect of human life. Traumatic experiences in childhood are associated with increased mental health morbidity. Factors such as lack of parental support, low self-esteem, and social difficulties play a role in increasing the risk of risky behavior in adolescents. Purpose: To determine the determinant factors that influence the incidence of depression in adolescents living in orphanages in Central Aceh Regency. Method: Used a Cross Sectional design in four orphanages in August 2023. The sample consisted of 302 adolescents aged 8-18 years, selected by purposive sampling. The analysis used logistic regression with 95% confidence. Results: A total of 121 respondents (40.07%) experienced depression. Related factors were education, region of origin, family visits, social support, and social discrimination. Social support was the most dominant factor (AOR=5.64; p=0.000). Conclusion: Social support plays a major role in preventing depression in adolescents in orphanages, so interventions that increase social support need to be prioritized. Suggestion: Explore other psychosocial factors such as quality of relationship with caregivers, trauma experiences, and adolescent coping strategies.

Abstract

Background: Bullying in schools is a growing global problem that affects victims, perpetrators, and the surrounding environment. Environmental and individual factors, such as family, mass media, and social pressure, play a significant role in the emergence of bullying behavior. In Indonesia, this phenomenon is still often overlooked, even though it has long-term consequences for students' psychological and academic well-being. Purpose: To analyze the factors that contribute to bullying behavior in schools. Method: A descriptive qualitative design with an exploratory focus was conducted in April 2025 at State Vocational High School 1 Banda Aceh. A total sampling technique was utilized, resulting in the selection of four participants, consisting of key informants including teachers and the headmaster. Data collection was carried out through triangulation, integrating multiple data sources, methods, and theoretical perspectives to enhance the credibility and validity of the findings. In-depth interviews served as the primary data collection method. The collected data were analyzed using thematic analysis, allowing for the identification of recurring patterns and the extraction of core meanings from the participants' narratives. Results: Bullying behavior among students is influenced by family, media, and peer environments. A lack of support or harsh treatment at home can lead children to imitate negative behavior at school, while exposure to violent media and poor parental supervision further contribute to aggression. Peer pressure also plays a significant role, as teenagers often follow group behavior to feel accepted. Conclusion: Preventing bullying requires a comprehensive approach involving families, schools and communities.

Abstract

Background: Endometriosis is a condition characterized by the growth of endometrial tissue outside the uterus, which can cause pelvic pain and problems in the reproductive system. Endometriosis usually occurs during reproductive age. The symptoms of endometriosis depend on the location of the endometrial cells. The main complaint that often appears is pelvic pain, so about 71-87% of cases are diagnosed due to severe chronic pain during menstruation, while only 38% are detected due to infertility problems. Proper treatment of endometriosis is necessary, as many women are often diagnosed too late, leading to suffering and decreased quality of life. One of the medical approaches used is hormone therapy. Purpose: To aprove Leuprorelin acetate hormone therapy in helping the treatment of Ovarian Endometriosis. Method: This descriptive analytic research used a case study approach involving a 37-year-old female patient with ovarian endometriosis at the Obstetrics and Gynecology Polyclinic of Dompu General Hospital. The study followed ethical principles and obtained informed consent. Data were collected through interviews, observation, physical examinations, medical records, and diagnostic documentation, using a head-to-toe assessment format. Data analysis was conducted concurrently with data collection, comparing findings with existing theories to support discussion and intervention recommendations. The analysis was narrative and observational, aiming to interpret the patient’s condition in relation to theoretical knowledge. Results: Hormone therapy with Leuprorelin acetate was administered in this case at a dose of 3.75 mg per injection for three months. The results of the ultrasound examination showed a reduction in the size of the ovarian mass, and the patient's pain significantly decreased. Conclusion: Leuprorelin acetate hormone therapy can serve as an effective alternative to surgery in managing symptoms of ovarian endometriosis.

Abstract

Background: In the context of increasing competition within the healthcare sector, patient loyalty has emerged as a vital determinant of the long-term viability of primary healthcare clinics. Purpose: To explore and assess evidence-based management strategies that effectively foster patient loyalty. Method: A systematic literature review was conducted following the PRISMA framework, utilizing five prominent electronic databases and focusing on publications from 2020 to 2025. Results: Patient loyalty is significantly influenced by factors such as healthcare provider communication, service quality, patient satisfaction, clinic reputation, and marketing efforts. The findings suggest that the integration of these five elements forms the most effective strategic framework for enhancing patient loyalty. Conclusion: It is recommended that primary healthcare clinics implement targeted interventions, including structured communication training for staff, regular service quality assessments, systematic patient feedback collection, strategic reputation management, and comprehensive marketing initiatives to strengthen patient loyalty.

Abstract

Background: Lung cancer remains a leading cause of cancer-related mortality worldwide, with surgical resection being the primary curative approach for early-stage non-small cell lung cancer. Postoperative wound infections following thoracic surgery significantly impact patient outcomes, healthcare resources, and quality of life. Purpose: To evaluate the incidence, risk factors, management, and outcomes of postoperative wound infections comparing Video-Assisted Thoracic Surgery (VATS) versus open thoracotomy in lung cancer patients. Method: A systematic review with article selection guided by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The systematic review process begins with the formulation of clinical questions relevant to the topic. Before this, the authors establish PICOS criteria, which stand for: P (problem, patient, or population), I (intervention, prognostic factor, or exposure), C (comparison or control), O (outcome), and S (study design). For this article, P: Adult patients with diagnosed lung cancer who underwent surgical resection, I: Video-Assisted Thoracic Surgery (VATS), C: Open thoracotomy, O: Incidence of postoperative wound infections, infection severity, causative microorganisms, time to infection development, treatment approaches, and impact on hospital stay, S: Randomized controlled trials. The article search was conducted systematically using the keywords: "Video-Assisted Thoracic Surgery,” AND "VATS," AND "Open Thoracotomy," AND "Lung Cancer," AND "Pulmonary Carcinoma," AND "Surgical Site Infection," AND "Wound Infection," AND "Postoperative Complications," AND "Surgical Wound Dehiscence," AND "Surgical Outcomes," AND "Minimally Invasive Surgery". Results: The analysis showed that VATS had lower overall complication rates, reduced mortality, and shorter hospital stays compared to open thoracotomy, especially in high-risk patients like those with obesity. VATS conversion to thoracotomy significantly increased complications and mortality. Robot-assisted thoracic surgery offered additional benefits, including less blood loss and lower conversion rates than conventional VATS. Conclusion: VATS is preferred over open thoracotomy for lung cancer due to lower complications, mortality, and shorter hospital stays. Conversion to thoracotomy increases risks. Robotic surgery offers additional benefits but at higher costs. Overall, minimally invasive approaches reduce wound-related complications.

Abstract

Background: Burnout is characterized by emotional exhaustion, depersonalization, and decreased personal accomplishment, often occurring in workers with intense social interactions, such as healthcare workers. Nursing is a profession vulnerable to burnout, given the high job demands, long working hours, and significant responsibility in patient care. Burnout can cause physical, emotional, and mental exhaustion, impacting the quality of healthcare services and nurses' well-being. Purpose: To determine the relationship between mindfulness, job satisfaction, and personality factors with burnout among nurses in hospitals. Method: A descriptive-analytical method with a cross-sectional design was applied. The target population included all 1,989 nurses employed at Central Hospitals in Indonesia, from which a sample of 328 respondents was drawn. Data collection was carried out using a questionnaire. For data analysis, the chi-square test was employed in the bivariate analysis, while multiple logistic regression was used for the multivariate analysis. The variable most strongly associated with the outcome was job satisfaction (OR = 17.874), whereas mindfulness showed the weakest association (OR = 4.932). Results: The logistic regression analysis revealed a significant relationship between mindfulness, job satisfaction, and personality factors with work-related fatigue (burnout) among hospital nurses, indicated by a p-value of 0.001. Conclusion: There is a relationship between mindfulness, job satisfaction, and personality factors with work fatigue (burnout) among hospital nurses.

Abstract

Background:  Gynura procumbens (Lour.) is a medicinal plant known for its potential in managing various metabolic disorders, including hypercholesterolemia. However, scientific data supporting its cholesterol-lowering effects remains limited. Purpose: To evaluate the effect of ethanol extract of Gynura procumbens leaves on total cholesterol levels in male Wistar rats induced with a high-fat diet. Method: A quasy experiment approach, specially focusing on post-test only group. The study took place from March-August 2023, in Research Laboratory of the Faculty of Medicine, Universitas Muslim Indonesia. The independent variable was gynura procumbens extract dependent variables is total cholesterol levels. Using purposive sampling and federer formula, 21 participants were selected which were divided into 3 groups. Group I (negative control) received a placebo, group II (treatment group) received gynura procumbens extract, and group III (positive control) received simvastatin, who met specific inclusion and exclusion criteria. Analysis proceeded with a univariate test to determine the frequency distribution of respondent’s characteristics, followed by bivariate analysis using man whitney and anova test. Results:  After induction, all groups showed a marked increase in cholesterol levels. Following the treatment period, the group receiving Gynura procumbens extract demonstrated a significant reduction in cholesterol levels (123.43 ± 24.80 mg/dl, p = 0.018), which was statistically comparable to the simvastatin group (112.83 ± 8.52 mg/dl, p = 0.033). In contrast, the negative control group showed no significant change (231.33 ± 90.56 mg/dl, p = 0.075). Conclusion: These findings indicate that Gynura procumbens leaf ethanol extract has a significant cholesterol-lowering effect in hypercholesterolemic rats, with efficacy comparable to simvastatin as standard pharmacological therapy.
