Abstract

Background: Oral health problems during childhood can have long-term impacts on oral health in adulthood, negatively impacting children's general health and academic achievement. Furthermore, children's oral health is influenced by various determinants and shows differences between urban and rural areas. Purpose: To analyze the determinants and disparities in oral health among children in urban and rural areas. Method: This systematic review used the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines to analyze and evaluate research findings identified through a literature search in PubMed and Google Scholar using the keywords "oral health" AND "children" AND "rural" AND "urban" AND "differences." Results: There were differences in the burden of untreated oral disease and lower access to health information among children in rural areas compared to urban areas. Conclusion: Disparities in oral health among children exist between rural and urban areas, influenced by various social, economic, behavioral, and access to health services factors. Children in rural areas tend to be at higher risk of oral health problems due to limited access to services, low health literacy, and uneven distribution of dental health professionals. Suggestion: Promotive and preventive interventions are needed to improve parental knowledge and behavior and ensure equitable access to dental health services, especially in rural areas. Furthermore, research should encompass a wider range of regions and variables, including research in Indonesia, to obtain a more comprehensive and contextualized picture of the determinants and disparities in children's oral health.   Keywords: Determinants; Health Disparities; Oral Health; Rural Areas; Urban Areas.

Abstract

Background: Adolescence is a developmental stage marked by the process of self-identity discovery. During this stage, adolescents are vulnerable to various problems, one of which is bullying. Lack of knowledge about bullying is one factor that can contribute to this behavior. Therefore, efforts are needed to improve adolescents' knowledge, one of which is a psychoeducational approach through digital media. Purpose: To analyze the effectiveness of psychoeducation through google sites in increasing knowledge about bullying among junior high school adolescents. Method: This quantitative study used a quasi-experimental one-group pre-test and post-test design. The population in this study were all 80 students at Cikalongkulon 4 State Secondary School. The sample size was 25 participants selected using a purposive sampling technique. Data were collected using a bullying knowledge questionnaire and statistically analyzed using the Wilcoxon Signed Rank Test. Results: Before psychoeducation, participants' knowledge was still suboptimal. After psychoeducation, knowledge improved in the majority of participants (92%) and was categorized as good. The statistical test results showed a p-value of 0.000 < 0.05, indicating that psychoeducation through Google Sites was effective in increasing knowledge about bullying among junior high school adolescents. Conclusion: Psychoeducation through Google Sites was effective in increasing knowledge about bullying among junior high school adolescents. This medium can be used as an alternative in implementing psychoeducation programs in schools, particularly in preventing bullying among adolescents.   Keywords: Adolescents; Bullying; Knowledge; Google Sites; Psychoeducation.

Abstract

Background: Musculoskeletal disorders in the elderly with degenerative diseases experience decreased muscle strength in the lower extremities, resulting in decreased mobility and quality of life. Non-pharmacological interventions such as heat-based reflexology and range of motion (ROM) exercises can improve circulation and muscle strength. Purpose: To compare the effectiveness of range of motion (ROM) exercises and heat-based reflexology in improving lower limb muscle mobility in the elderly. Method: A quasi-experimental study using a two-group pre-posttest design involving elderly participants from the Prolanis program was conducted. The first group performed range of motion (ROM) exercises, while the second group received heat-based reflexology applied at 39-40°C for 15-20 minutes. Data were analyzed using linear regression and independent t-tests. Results: Parameter estimation analysis showed that range of motion (ROM) exercises resulted in greater improvements in muscle mobility compared to heat-based reflexology. The regression coefficient for the ROM group was B = –0.153; 95% CI = –0.282–0.025; p = 0.020, indicating a significantly lower effect on improving muscle mobility compared to the ROM group. However, heat-based reflexology compression contributed to muscle relaxation, improved blood circulation, and physical comfort among elderly participants. Conclusion: Range of motion (ROM) exercises and heat-based reflexology compression both improved muscle mobility in elderly participants, but ROM exercises were more effective. Therefore, ROM is recommended as the primary intervention, while heat compression is used as a supportive therapy to enhance the effectiveness of the exercises.   Keywords: Elderly; Heat-Based Reflexology Massage Compress; Musculoskeletal Degenerative; Muscle Weakness; Range Of Motion (ROM).

Abstract

Background: Major Depressive Disorder (MDD) is a mental health condition with a high prevalence that significantly impacts patients’ quality of life. Although pharmacological therapy is the primary treatment option, its effectiveness is often limited by slow clinical response, medication side effects, and low patient adherence. Electroacupuncture at points GV 20 (Baihui) and EX-HN 3 (Yintang) offers a potential complementary intervention through neurobiological stimulation to regulate neurotransmitters and brain activity. Purpose: To analyze the effectiveness of electroacupuncture therapy at points GV 20 and EX-HN 3 in reducing symptom severity in patients with major depressive disorder. Method: This quantitative study employed a quasi-experimental design using a one-group pretest–posttest approach and was conducted from April to May 2026 at Rumah Tabinda in Malang. The sampling technique used was purposive sampling, with a sample size of 30 respondents. The independent variable in this study was electroacupuncture therapy at the GV20 and EX-HN3 acupoints, while the dependent variable was the reduction in the severity of depressive symptoms. Data analysis included univariate analysis in the form of frequency distributions and bivariate analysis using a paired-sample t-test. Results: Measurements of depression levels showed a decrease in the mean HAMD score from 22.37 ± 3.92 at the pretest to 15.85 ± 2.37 at the posttest. Statistical analysis revealed p=0.000 (p<0.05), demonstrating a significant effect of the intervention on reducing the respondents’ depression levels. Conclusion: Electroacupuncture therapy at the GV20 (Baihui) and EX-HN3 (Yintang) acupoints is effective in reducing the severity of depression in patients with Major Depressive Disorder.   Keywords: Electroacupuncture; EX-HN3; GV20; Hamilton Depression Rating Scale; Major Depressive Disorder.

Abstract

Background: Diarrhea is a health problem in toddlers that continues to cause high morbidity and mortality. Low parental knowledge about clean and healthy living behaviors can impact diarrhea prevention efforts in toddlers. Purpose: To determine the relationship between parental knowledge and diarrhea prevention in toddlers. Method: A descriptive correlation design was used with a cross-sectional approach. The study population was all parents of toddlers, with a sample size of 48 respondents drawn using a total sampling technique. Data were collected using a questionnaire regarding knowledge levels and diarrhea prevention in toddlers. Data were analyzed using univariate and bivariate methods using the chi-square test with a 95% confidence level (α=0.05). Results: The majority of respondents (27 respondents) were aged 31–40 years, 27 (56.3%) were housewives, and 22 (45.8%) had a high school education. Respondents with low knowledge mostly reported poor diarrhea prevention practices (22 respondents) (45.8%). The chi-square test yielded a p-value of 0.024 (<0.05), indicating a significant relationship between parental knowledge and diarrhea prevention in toddlers. Conclusion: There is a relationship between parental knowledge and diarrhea prevention in toddlers. The better the parental knowledge, the better the diarrhea prevention efforts.   Keywords: Diarrhea Prevention; Parental Knowledge; Toddlers.

Abstract

Background: Nutritional status plays an important role in maintaining reproductive health among adolescent girls. Both undernutrition and overnutrition may disrupt the balance of reproductive hormones, including estrogen, progesterone, luteinizing hormone (LH), and follicle-stimulating hormone (FSH), potentially leading to menstrual cycle irregularities. Purpose: To describe the nutritional status and menstrual characteristics of adolescent girls attending all junior high schools. Method: A descriptive study with a quantitative approach was conducted among adolescent girls enrolled in six junior high schools in Manganitu District. Total sampling was employed. A total of 161 participants were included in the nutritional status analysis, while menstrual characteristics were analyzed among 132 participants who had experienced menarche. Nutritional status was assessed through anthropometric measurements of weight and height and classified using the Body Mass Index-for-Age (BMI-for-Age) according to WHO standards. Menstrual characteristics were collected using a structured questionnaire through face-to-face interviews. Data were analyzed descriptively using frequencies, percentages, means, and standard deviations. Results: The mean age of the participants was 13.17 ± 0.81 years (range: 12–16 years). Most participants had normal nutritional status (78.8%), while 10.5% were obese, 6.2% were overweight, and 4.5% were undernourished. Among the 132 participants who had experienced menarche, 68.2% had a normal menstrual cycle (21–35 days), whereas 31.8% had an abnormal menstrual cycle. Furthermore, 72.7% of participants reported experiencing dysmenorrhea, and 81.8% reported mood changes during menstruation. Conclusion: Most adolescent girls had normal nutritional status and a normal menstrual cycle. However, undernutrition, overweight, obesity, and abnormal menstrual cycles were still observed in a considerable proportion of participants. These findings highlight the importance of continuous nutritional monitoring and reproductive health education to promote the health and well-being of adolescent girls.   Keywords: Adolescent Girls; Junior High School; Menstrual Cycle; Nutritional Status; Reproductive Health.

Abstract

Background:  Stunting remains a major public health problem, particularly in developing countries, including Indonesia. This condition affects not only physical growth but also cognitive development and future productivity. Maternal behavior plays a crucial role in stunting prevention efforts, particularly during the first 1,000 days of life. Psychoeducation is one approach that can be used to improve stunting prevention behaviors through an interactive and participatory learning process. Purpose: To analyze the effect of psychoeducational interventions on stunting prevention behaviors in mothers of young children. Method: This study used a quasi-experimental design with a pretest-posttest control group approach. The study was conducted in the PB Selayang II Community Health Center in Medan. A total of 76 respondents were divided into two groups: 38 participants in the intervention group and 38 participants in the control group. Behavioral data were collected using a structured questionnaire and analyzed as a composite score. Because the data were not normally distributed, non-parametric tests, namely the Wilcoxon and Mann-Whitney tests, were used. Results: The median score for stunting prevention behavior in the intervention group increased from 86.39 in the pretest to 98.33 in the posttest. In contrast, the control group showed no significant change, from 85.97 to 85.28. The Wilcoxon test showed a significant difference in the intervention group (p < 0.001), while there was no significant difference in the control group (p = 0.421). Furthermore, the Mann–Whitney test showed a significant difference between the two groups (p < 0.001). Conclusion: The psychoeducational intervention significantly improved stunting prevention behavior in mothers of young children. Abstract

Background: The smoke-free zone policy is one of the government's efforts to create a healthy environment and protect the public from exposure to cigarette smoke, particularly in educational settings. Schools play a strategic role in preventing smoking among adolescents, as the number of new smokers continues to rise. However, the implementation of the smoke-free zone policy in schools is often suboptimal due to limited resources, lack of supervision, and the absence of clear operational guidelines. Purpose: To analyze the implementation of the smoke-free zone policy in educational settings. Method: This study used a qualitative descriptive design. This study was conducted at State Senior High School 5 in Jambi City, located at Simpang IV, Sipin Village, Telanaipura District, Jambi City, from March to April 2026. Informants included the principal, teachers, guidance counselors, students, security officers, and canteen vendors. Data collection was conducted through in-depth interviews, observation, and documentation. Data analysis was carried out through data reduction, data presentation, and drawing conclusions. Results: Policy implementation has not been optimal. Communication regarding the smoking ban has been implemented through regulations and socialization, but coordination with external parties remains limited. The resources, facilities, and infrastructure supporting the policy remain inadequate. Although the entire school community generally supports the policy, violations are still found due to low awareness and weak oversight. Furthermore, there is no organizational structure or specific standard operating procedures (SOPs) for smoke-free areas. Conclusion: The implementation of the smoke-free area policy still requires improvement, particularly in terms of supervision, provision of supporting facilities, and enforcement to create a healthy and smoke-free school environment. Suggestion: Future research should involve a larger sample size and location, and use quantitative or mixed methods to evaluate the effectiveness of the smoke-free area policy and identify factors influencing its successful implementation.   Keywords: Education System; Policy Implementation; Smoke-Free Area.

Abstract

Background: Health policy and insurance system reforms are crucial instruments for achieving universal health coverage (UHC) and ensuring equitable access to medical services. With aging populations and rising healthcare costs across various nations, restructuring insurance frameworks has become a global priority. Purpose: To analyze the trajectory of development, policy implementation, financing, and insurance system reforms within healthcare services. Method: This study is a systematic literature review utilizing relevant and objective academic databases. Searches were conducted using keywords related to policy implementation and healthcare financing across Scopus, PubMed, and ScienceDirect. The identification phase yielded 52 articles, which were subsequently examined using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Data were synthesized and presented thematically, grouping relevant findings according to established key themes: enhancing administrative efficiency, policy reform, and the reform of primary healthcare systems within insurance models and financing structures. Following systematic identification and review, 13 articles were deemed suitable for inclusion in the final synthesis. Results: Three key pillars of national insurance system reform were identified as essential for achieving universal health coverage (UHC) and bolstering public health resilience: policy reform, support for fiscal sustainability, and administrative reform driven by digital integration. Conclusion: Health insurance reform is an ongoing process that balances social equity with fiscal realities. The most successful reforms are grounded in three main pillars: fiscal innovation, technology adoption, and legislative support. To achieve sustainable universal health coverage (UHC), policymakers must prioritize data interoperability and human resource training. Suggestion: Future research should examine the long-term impact of AI-based premium setting on patient trust and equity within the insurance system.   Keywords: Health Service Administration; Health Service Policy; Insurance Reform; Universal Health Coverage.

Abstract

Background: Limited access to and quality of social security administering agency services in underdeveloped, frontier, and outermost (3T) regions is a major challenge for low-income communities and has the potential to reduce patient satisfaction at community health centers. Geographical barriers, limited facilities, and a lack of health resources often impact the effectiveness of health services in remote areas. Purpose: To analyze the relationship between service accessibility and service quality, as well as patient satisfaction among participants of the social security administering agency in low-income communities in 3T regions. Method: This study used a quantitative observational design with a cross-sectional approach. A total of 106 respondents were selected using a multistage and purposive sampling technique. This study was conducted at the Muara Kibul Community Health Center and the Aur Berduri Community Health Center in Merangin Regency since February 2025. Data were collected using a structured questionnaire that had been tested for validity and reliability. Data were analyzed using univariate and bivariate analyses, using the chi-square test to determine the relationship between variables. Results: Most respondents rated service affordability as good, particularly in economic, language, and social aspects, although geographic barriers remain a challenge. Service quality was rated as quite good in terms of reliability, but responsiveness, assurance, empathy, and tangibles still need improvement. Statistical analysis showed a significant relationship between service affordability and patient satisfaction (p = 0.000), as well as between service quality and patient satisfaction (p = 0.000). Respondents with poor service affordability and quality were at higher risk of experiencing dissatisfaction with healthcare services. Conclusion: Service affordability and quality are important factors influencing patient satisfaction among participants of the social security administration for health in the 3T regions. Improvements in health infrastructure, transportation access, responsiveness of healthcare workers, and supporting facilities are crucial to increasing patient satisfaction and strengthening healthcare services in remote areas. Suggestion: Further research is recommended to use a larger sample size and cover more health centers in the 3T region, so that the research results are more representative.   Keywords: Low-Income Communities; Patient Satisfaction; Service Affordability; Service Quality; Social Security Administration for Health.
