Abstract

Background: The implementation of an early warning system (EWS) is a systematic strategy for early detection of acute deterioration in patients through structured monitoring of physiological parameters. However, various studies have shown that EWS implementation in inpatient settings remains suboptimal, primarily due to nurses' limited knowledge, confidence, and clinical skills in interpreting and responding to the resulting scores. Simulation-based education is considered an effective approach to improving clinical competence because it allows for active, reflective, and applied learning in a safe environment. Purpose: To analyze the effectiveness of the EWS simulation method in improving nurse competence through a scoping review approach. Method: This study adopted a scoping review approach based on the Joanna Briggs Institute (JBI) guidelines, with reporting structured according to the PRISMA framework. A systematic search of the literature was carried out across PubMed, ProQuest, ScienceDirect, Scopus, SAGE, and EBSCO to identify English-language publications from 2014 to 2024. The search strategy was developed using the PICOS framework, incorporating a range of keywords related to nursing, simulation, and early warning system (EWS). Results: The findings demonstrate that simulation-based EWS training, including high-fidelity and web-based approaches, consistently improves nurses’ knowledge, clinical skills, confidence, and attitudes in recognizing and responding to patient deterioration. This effectiveness is attributed to the ability of simulation to provide realistic, practice-based learning experiences. Most studies reported significant enhancement in early detection and clinical decision-making abilities following simulation interventions. Conclusion: Simulation-based EWS training is an effective strategy for strengthening nurses’ competence in detecting and managing acute patient deterioration. Integrating simulation into continuous nursing education is recommended to enhance clinical performance, support patient safety, and improve the overall quality of care. Suggestion: Future research can use a more robust experimental design and involve various clinical settings to evaluate the effectiveness of simulation more comprehensively, especially in the aspects of team communication and clinical response to patient deterioration.   Keywords: Acute Deterioration; Early Warning System; Nurses’ Competency; Simulation.

Abstract

Background: Labor pain is a physiological process experienced by almost all women, especially during the first stage of labor due to uterine contractions and cervical dilation. Pain intensity generally ranges from moderate to severe and can affect the mother's physical and psychological well-being. Inadequate pain management can increase anxiety, prolong labor, and reduce the quality of the birth experience. Therefore, effective and safe non-pharmacological interventions are needed. One method that can be used is the counterpressure technique, which applies pressure to the sacral area to reduce pain perception. Purpose: To analyze the effectiveness of the counterpressure technique in reducing pain intensity during the first stage of labor. Method: This study used a literature review design by examining scientific articles obtained from Google Scholar, PubMed, and ScienceDirect databases. Articles were searched using the keywords "counterpressure," "sacral pressure," and "labor pain." Article selection was based on the PRISMA guidelines, which include identification, screening, due diligence, and inclusion. Inclusion criteria included studies with experimental and quasi-experimental designs published between 2021 and 2026, available in full text, and relevant to the research topic. Results: The counterpressure technique significantly reduced the intensity of first-stage labor pain, with all studies reporting a p-value <0.05. This intervention was effective across a variety of study designs, sample sizes, and pain measurement instruments, such as the Numeric Rating Scale (NRS) and Visual Analogue Scale (VAS). This technique was generally administered during the active phase of labor for 10–30 minutes and showed a reduction in pain intensity from severe to moderate or mild. Conclusion: The counterpressure technique is an effective, safe, and easy-to-implement non-pharmacological intervention for reducing first-stage labor pain. The technique's mechanisms of action include inhibiting pain impulse transmission through gate control theory, increasing endorphin release, and reducing pressure on the sacral region, which also provides a relaxing and comforting effect on the mother. Suggestion: The counterpressure technique is recommended for integration into obstetric care practices, particularly in primary health care facilities. Health workers and midwives are encouraged to receive training in the application of this technique to improve maternal comfort and the quality of the birth experience. Further research is needed to examine the optimal duration and frequency of this intervention to achieve maximum results.   Keywords: Counterpressure; First Stage of Labor; Labor Pain.

Abstract

Background: Drug-resistant tuberculosis (Drug-resistant TB) is a serious public health challenge in Indonesia. Limited access to healthcare services, delayed diagnosis, and barriers to treatment contribute to the development of drug resistance. Purpose: This study aims to analyze the relationship between access to healthcare services and the incidence of DRT. Method: This quantitative observational study with a cross-sectional design involved 1,298 respondents selected through purposive sampling. Data were obtained from the 2023 Indonesian Health Survey. The incidence of DRT was designated as the dependent variable, while access to healthcare services was the main independent variable, with confounding factors including residence, gender, age, education, employment status, and economic status. Data analysis was performed using univariate tests, chi-square tests, and logistic regression. Results: A total of 39.06% of respondents were diagnosed with DRT, and 88.98% experienced difficulty accessing healthcare services. Multivariate analysis showed no significant association between access to healthcare services and DRT. Age ≥65 years was the only factor significantly associated with drug-resistant TB (DR-TB), and other variables did not show a significant effect. Conclusion: Access to health services as measured in this study did not significantly influence DR-TB due to its limited scope, which only captured the physical dimension of access. Suggestion: Future research should use multidimensional access indicators to understand the factors influencing DR-TB in Indonesia.   Keywords: Access; Drug Resistance; Health Services; Tuberculosis (TB).

Abstract

Background: Stunting remains a major public health problem due to its long-term consequences on human resource quality and continues to be one of the national health priorities. Prevention strategies require a comprehensive approach, including improving the maturity of marriage age among premarital adolescents. Social factors, particularly social support and parental democratization, are considered important determinants that influence adolescents’ readiness to enter marriage and contribute to stunting prevention efforts. Purpose: To analyze the determinants of marriage age maturity based on social support and parental democratization among premarital adolescents as an effort to prevent stunting. Method: A quantitative analytic design with a cross-sectional approach involving 260 premarital class participants in Kulon Progo Regency. Samples were selected using simple random sampling. Data were collected using structured questionnaires measuring parental democratization and social support, including family support, peer support, and health worker support. Data were analyzed using univariate analysis, Chi-square test for bivariate analysis, and multiple logistic regression for multivariate analysis. The strength of association was presented using odds ratio (OR) with a 95% confidence interval. Results: The multivariate analysis showed that parental democratization (p=0.000; OR=8.987) and health worker support (p=0.003; OR=5.508) were significant determinants of marriage age maturity. The logistic regression model showed a classification accuracy of 79.6% and explained 50.1% of the variation in marriage age maturity. Family support (p=0.105; OR=2.756) and peer support (p=0.498; OR=0.741) were not statistically significant determinants. Conclusion: Parental democratization and health worker support are dominant factors associated with marriage age maturity among premarital adolescents. Strengthening democratic parenting practices and optimizing the role of health workers may support efforts to improve marriage readiness and prevent stunting.   Keywords: Marriage Age Maturity; Parental Democratization; Premarital Adolescents; Stunting Prevention; Social Support.

Abstract

Background: Diabetes Mellitus (DM) is the fastest-growing non-communicable disease in the world and a major health problem in Indonesia. The increase in cases is influenced by risk factors such as obesity and population growth and aging. To date, projections have focused more on prevalence and therefore have not accurately reflected the number of cases as a basis for health service planning. Purpose: To project the number of DM cases in Indonesia from 2025 to 2030, taking into account trends in DM prevalence, obesity, and population growth. Method: This study used a quantitative approach with a descriptive-analytical design and trend projections based on secondary data from the 2007–2018 Basic Health Research, the 2023 Indonesian Health Survey, and the Central Statistics Agency. This study covered residents aged 25–64 years. Researchers conducted descriptive analysis to identify trends, then used Poisson and Negative Binomial regression to test the influence of variables. They also applied linear regression combined with population projections to estimate the number of future cases. Results: The study shows that the number of cases in Indonesia continues to increase over time. The Negative Binomial regression model was the best model because it was able to overcome overdispersion in the Poisson model. Age significantly influenced the number of cases (p=0.04) (p<0.05). Individuals aged 55–64 years have an incidence rate ratio (IRR) of 23.414, indicating that this group experiences an incidence rate of diabetes mellitus that is 23.4 times the incidence rate of the 25–34 years. In contrast, obesity did not show a statistically significant effect (p>0.05). Projections using linear regression also show an increasing trend in the number of cases from 2025 to 2030, reflecting an increase in the future disease burden. Conclusion: The number of cases in Indonesia will continue to increase until 2030 and is significantly influenced by age. Suggestion: Prevention and control efforts are needed, especially for at-risk age groups, and the projection results should be used as a basis for planning future health policies and services.   Keywords: Diabetes Mellitus; Obesity; Projection.

Abstract

Background: Dengue fever is a major public health problem in tropical countries like Indonesia. Previous research evaluating larval monitoring and dengue fever, particularly in rural areas, is limited. Purpose: To analyze the achievement of dengue control programs in rural areas. Method: This case study uses secondary data from the 2023-2024 surveillance report, focusing on Orok Solong Hamlet, Kabul Village, one of the working areas of the Batu Jangkih Community Health Center. Descriptive analysis was conducted on trends in dengue cases, house index, and larva-free rates compared to the national target (≥95%). Validation was conducted through a review of program documents and cross-sectoral reports. Results: The larva-free rate increased from 85% in 2023 to 97% in 2024 after strengthening periodic larva monitoring activities and routine mosquito nest eradication. The active involvement of two mosquito larvae monitoring cadres, along with support from the village government, village supervisory officer, and hamlet heads, contributed to increased community participation and the effectiveness of vector control. Dengue fever cases decreased to two cases by the end of 2024. Conclusion: The periodic mosquito larvae monitoring and 3M Plus mosquito nest eradication program effectively increased the mosquito larvae-free rate from 85% to 97% and reduced dengue fever cases in Orok Solong Hamlet. Success was influenced by consistent interventions, the role of mosquito larvae monitoring cadres, community surveillance, and cross-sector support. Abstract

Background: Cardiovascular nursing practice often prioritizes mechanical hemodynamic monitoring while neglecting interpersonal engagement. Qualitative findings support the value of therapeutic communication; however, there remains a lack of quantitative synthesis regarding its effect size on physiological stability. Purpose: To measure the impact of therapeutic communication on cardiac anxiety and heart rate variability (HRV) Method: This literature review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The article search was designed using the PICOS framework: P (problem, patient, or population), I (intervention, prognostic factor, or exposure), C (comparison or control), O (outcome), and S (study). The literature search was conducted in PubMed, Scopus, and CINAHL using the keywords “therapeutic communication” AND “nurse-patient communication” AND “cardiovascular patients” AND “cardiac anxiety” AND “heart rate variability” AND “psychological outcomes” AND “physiological outcomes.” The publication timeframe was from 2020 to 2026, and the search included articles in both English and Indonesian. The search yielded 15 relevant and usable articles. Results: The study demonstrated a significant reduction in cardiac anxiety (SMD = -0.89, 95% CI -1.15 to 0.63) alongside a significant improvement in physiological indicators. The observed heterogeneity (I²) of 83% was primarily attributed to cultural and environmental moderators. Conclusion: Therapeutic communication serves as a highly promising clinical intervention equivalent to adjunctive pharmacological stabilization, effectively reducing the cardiotoxic effects of increased sympathetic activity.   Keywords: Anxiety; Cardiovascular Nursing; Heart Diseases; Therapeutic Communication.

Abstract

Background: Cervical cancer remains a major public health concern in developing countries, where screening coverage is still low. Co-testing HPV DNA and IVA/VIA has the potential to improve early detection; however, its success is influenced by women's behavior in accessing and completing the screening pathway. Purpose: To identify the determinants of early cervical cancer detection behavior among women in developing countries in the context of co-testing HPV DNA and IVA implementation. Method: A systematic literature review based on the PICCO framework was conducted. ework. Articles reviewed included studies on HPV DNA testing and IVA/VIA/visual inspection within a single screening algorithm, either as co-testing, concurrent testing, HPV primary screening followed by VIA triage, or screen–triage–treat. Study quality was assessed using JBI Critical Appraisal Tools, and data were synthesized narratively based on individual, social, healthcare service, economic, and cultural factors. Results: Ten primary studies were analyzed. Dominant determinants included knowledge, risk perception, acceptance of screening methods, stigma, partner/family support, transportation costs, service distance, informal employment, geographic access, follow-up systems, number of visits, HPV DNA result turnaround time, and availability of IVA/VIA triage on the same day. HPV self-sampling, single-visit services, community-based approaches, and active recall systems have the potential to increase uptake and reduce loss to follow-up. Conclusion: Behavioral determinants of early cervical cancer detection in the context of co-testing HPV DNA and IVA are multidimensional. Abstract

Background: Introduction: Treatment-seeking patterns for schizophrenia patients in developing countries often involve indirect access to formal health services. Traditional and spiritual healing remain common initial pathways, particularly in strong cultural and religious contexts. Purpose: To describe treatment-seeking patterns and analyze factors associated with the use of traditional medicine as first aid in male patients with schizophrenia. Method: This quantitative, cross-sectional study was conducted from August to December 2025 at the Aceh Provincial Mental Hospital. The sampling technique used was purposive sampling, with a sample size of 155 respondents. The independent variables in this study were age, age of psychosis onset, education level, employment status, marital status, residence, distance to mental health services, internalized stigma as measured using the Internalized Stigma of Mental Illness Scale (ISMI-10), social support as measured using the Multidimensional Social Support Scale (MSPSS), and symptom severity as assessed using the Positive and Negative Syndrome Scale – Joy Component (PANSS-EC). The dependent variable was the use of traditional medicine. Bivariate analysis used the Chi-square test, Fisher's exact test, independent t-test, or Mann–Whitney U test. Results: A total of 57.4% of respondents used traditional medicine as first aid before accessing formal health services. Bivariate analysis showed no significant association between the use of traditional medicine and age (p=0.389), age of onset (p=0.997), education level (p=0.680), marital status (p=0.500), employment status (p=0.784), or domicile (p=0.333). Multivariate analysis also showed no factors significantly associated with the use of traditional medicine. Conclusion: Traditional medicine remains the dominant initial pathway for seeking help among male patients with schizophrenia in Aceh. Abstract

Background: Hypertension in the elderly is a significant health concern because it can lead to serious cardiovascular complications. Although antihypertensive medications are effective, long-term use often leads to side effects and the risk of polypharmacy. Therefore, non-pharmacological interventions are an option worth considering as supportive therapy. Various methods, such as progressive muscle relaxation, exercise for the elderly, and dietary adjustments, have been extensively studied to help lower blood pressure in the elderly. Purpose: To examine the effectiveness of non-pharmacological interventions in lowering systolic and diastolic blood pressure in the elderly with hypertension. Method: This study used a systematic review design following PRISMA guidelines. A literature search was conducted in PubMed, Google Scholar, and ScienceDirect for articles published between 2020 and 2026. Inclusion criteria included randomized controlled trials and quasi-experimental studies with participants aged 60 years and older. A total of 14 articles meeting the criteria were analyzed descriptively to determine the effects of the interventions on blood pressure. Results: Progressive muscle relaxation, exercise for the elderly, and the DASH diet have positive effects on lowering blood pressure. The average reduction achieved was 10–15 mmHg for systolic blood pressure and 5–10 mmHg for diastolic blood pressure after the intervention. These findings confirm that non-pharmacological approaches play a crucial role in supporting the management of hypertension in the elderly. Conclusion: Non-pharmacological interventions have been shown to be effective in helping lower blood pressure in the elderly with hypertension. The implementation of this approach needs to be strengthened in community nursing and eldercare services as a safer, more holistic, and sustainable hypertension management strategy.   Keywords: Blood Pressure; Elderly; Hypertension; Non-Pharmacological Interventions.
