Abstract

Background: Concentration in learning is a crucial factor in the academic success of elementary school students. Concentration is influenced by various factors, including breakfast habits and nutritional status. Breakfast provides the brain with the energy it needs for cognitive function, while good nutritional status supports optimal brain growth and development. However, some students still have poor breakfast habits and nutritional problems that can affect their ability to concentrate. Purpose: To analyze the relationship between breakfast habits and nutritional status and concentration in students. Method: This quantitative, observational, and analytical study used a cross-sectional approach. The sample consisted of 84 students at Doro O'o Public Elementary School, Langgudu District, Bima Regency, West Nusa Tenggara Province, selected using a probability sampling technique using simple random sampling. Data on breakfast habits and concentration in learning were collected using a questionnaire, while nutritional status was measured using Body Mass Index for Age (BMI/Age). Data were analyzed using the Chi-Square test with a significance level of p < 0.05. Results: Most respondents had good breakfast habits (53.6%), good nutritional status (69.0%), and low levels of learning concentration (46.4%). Bivariate analysis showed a significant relationship between breakfast habits and learning concentration (p = 0.000). Furthermore, there was a significant relationship between nutritional status and learning concentration (p = 0.000). Conclusion: Breakfast habits and nutritional status are significantly related to learning concentration in elementary school students. Establishing healthy breakfast habits and maintaining optimal nutritional status are important to support cognitive function and improve students' learning concentration. Suggestion: Further research is recommended to develop studies using research designs that can explain causal relationships, such as cohort or experimental, so that the influence of breakfast and nutritional status on learning concentration can be understood more deeply.   Keywords: Breakfast; Learning Concentration; Nutritional Status.

Abstract

Background: Prolonged mechanical ventilation (PMV) is a complication after coronary artery bypass graft (CABG) surgery that increases morbidity, mortality, and length of stay in the ICU. Surgical duration is often considered a factor influencing the incidence of PMV, but previous studies have shown inconsistencies. Purpose: To analyze the relationship between surgical duration and PMV incidence in post-CABG patients in the ICU. Method: This study used a retrospective cohort design with a quantitative approach. The study sample consisted of 237 medical records of post-CABG patients in the ICU of Dr. Hasan Sadikin General Hospital, Bandung. The sample was selected using consecutive sampling techniques according to inclusion and exclusion criteria. Data were collected using a checklist developed by the researchers. Data were analyzed univariately and bivariately using Pearson's correlation test. Results: The mean duration of mechanical ventilation in patients with PMV was 1619.66 ± 836.38 minutes (±27 hours), with a range of 730–3850 minutes. The average surgical duration was 252.52 ± 47.22 minutes. The analysis showed no significant association between surgical duration and the incidence of PMV (r = 0.059; p = 0.367). Conclusion: Surgical duration was not significantly associated with the incidence of PMV in post-CABG patients. This indicates that PMV is a multifactorial condition influenced not only by surgical duration but also by other clinical and perioperative factors. Therefore, a comprehensive clinical approach is needed to prevent and manage PMV. Suggestion: ICU nurses need to conduct more comprehensive monitoring of post-CABG patients, taking into account other clinical factors that may influence the ventilator weaning process. Furthermore, the development of integrated clinical protocols for mechanical ventilation management is needed to support accelerated patient recovery. Abstract

Background: The elderly population undergoes an aging process that renders them vulnerable to various physical, mental, and social health issues. As the number of elderly individuals in Indonesia rises, the need for affordable and sustainable healthcare services becomes increasingly critical. One government initiative aimed at improving the health status of the elderly is the implementation of integrated service posts for the elderly. However, the utilization rate of these services remains suboptimal in many areas, including the working area of ​​the Gemeh Community Health Center in Talaud Regency. Purpose: To analyze the relationship between family support, the role of health workers, and attitudes toward the utilization of integrated service posts for the elderly. Method: This study employed a quantitative, cross-sectional design and was conducted in the working area of ​​the Gemeh Community Health Center, Talaud Regency, between February and April 2026. The study population consisted of 600 elderly individuals aged 60–70 years. A sample of 200 respondents was determined using Slovin's formula and selected via simple random sampling. Data collection was carried out using a questionnaire that had undergone validity and reliability testing. Data analysis included univariate analysis, bivariate analysis using the Chi-Square test, and multivariate analysis using multiple logistic regression. Results: This study reveals that 63.3% of respondents utilized the integrated service post for the elderly. The majority of respondents received family support (92%), rated the role of health workers as "good" (58.5%), and held an attitude categorized as "low" regarding the utilization of the integrated service post for the elderly (54.5%). Bivariate analysis showed a significant association between family support (p = 0.038), the role of health workers (p = 0.030), and attitude (p = 0.010) with the utilization of the integrated service post for the elderly. Furthermore, multivariate analysis revealed that attitude was the most dominant factor associated with the utilization of the integrated service post for the elderly (p = 0.010; OR = 0.453; 95% CI = 0.249–0.826) compared to the other variables. Conclusion: Family support, the role of healthcare workers, and attitudes are significantly associated with the utilization of integrated service posts for the elderly. However, attitude is the most dominant factor related to the use of these services. Suggestion: Efforts are needed to enhance health education, strengthen the role of families, and optimize the role of healthcare workers in providing counseling and support to the elderly in order to increase the utilization of integrated service posts for the elderly.   Keywords: Attitude; Elderly; Family Support; Health Service Utilization; Integrated Health Post; Role of Health Workers.

Abstract

Background: Advances in information technology have brought significant changes to healthcare services, particularly in the interactions between hospitals, medical personnel, and patients. Implementing a digital marketing mix (e-marketing mix) is one strategy that can support improving the quality of healthcare services in the digital era. Purpose: To evaluate the implementation of the digital marketing mix and the quality of healthcare services. Method: This study used a quantitative approach with a survey method involving 97 respondents selected using a random sampling technique. The population in this study were patients of the Bima Regency General Hospital who met the inclusion criteria, namely patients who had previously used services at the hospital. Data were collected through questionnaires and analyzed using descriptive analysis and reliability testing with the help of SPSS software. Results: The core elements of the digital marketing mix, including product, place, price, personalization, privacy, customer service, community, and website, received positive reviews from the majority of respondents, with excellent reliability (Cronbach's Alpha = 0.899). Meanwhile, the supporting elements of promotion, security, and sales promotion received relatively lower ratings, although they still demonstrated good reliability (Cronbach's alpha = 0.793). Furthermore, the majority of respondents expressed satisfaction with the quality of healthcare services provided. Conclusion: Implementing core e-marketing mix elements and good service quality can shape positive patient perceptions of hospital services. Abstract

Background: The high rate of readmission among patients with ischemic stroke indicates that discharge planning implementation has not fully addressed patients’ needs during the transition from hospital to home care. This condition may affect the readiness of patients and families to continue care independently after discharge. Therefore, evaluation of discharge planning based on patients’ experiences is needed to identify gaps and improve the quality of transitional care. Purpose: To explore the experiences of patients with ischemic stroke regarding the implementation of discharge planning during hospitalization. Method: This study employed a qualitative design using a phenomenological approach. The participants consisted of nine patients with ischemic stroke who were hospitalized in a Type B Regional General Hospital in Tanjungpinang, Riau Islands Province, Indonesia. Participants were selected using purposive sampling based on predetermined inclusion criteria. Data were collected through semi-structured interviews lasting 20–36 minutes and analyzed using Colaizzi’s phenomenological method. Results: The characteristics of participants showed that 66.7% were male and 33.3% were female. Most participants had completed senior high school education (77.8%), while 22.2% had higher education. The analysis identified three main themes: (1) information related to discharge planning, (2) discharge planning needs, and (3) expectations regarding discharge planning. Participants reported receiving education about medication management, rehabilitation, and follow-up schedules. However, they still required additional information regarding dietary management, home environment modification, fall prevention, recognition of recurrence signs, and continued care at home. Conclusion: The implementation of discharge planning for patients with ischemic stroke has not fully met patients’ comprehensive needs. These findings highlight the importance of developing a more structured, comprehensive, and patient-centered discharge planning approach to improve discharge readiness, support continuity of care at home, and reduce the risk of readmission.   Keywords: Discharge Planning; Ischemic Stroke; Patient Experience.

Abstract

Background: Spinal anesthesia for cesarean section often causes significant hemodynamic changes due to sympathetic blockade. Mean arterial pressure (MAP) and pulse pressure (PP) are important parameters in assessing perfusion, but their sensitivity in detecting intraoperative changes is not fully understood. Purpose: To analyze the variability of MAP and PP at various time points during a cesarean section procedure under spinal anesthesia. Method: This study used an observational design with a longitudinal approach in 90 cesarean section patients. The study was conducted in the operating room of a referral hospital (e.g., a regional general hospital in East Nusa Tenggara). Hemodynamic parameters were measured preoperatively, at baseline (0 minutes), and at 5, 10, 15, 20, 25, and 30 minutes intraoperatively. Data were analyzed using the Friedman test to assess differences between time points, followed by the Wilcoxon signed-rank test with Bonferroni correction for post hoc analysis. Results: Significant hemodynamic changes occurred throughout the intraoperative period. MAP showed a progressive decline starting at 5 minutes and reaching its lowest point at 20 minutes before gradually recovering. In contrast, PP showed less variation and a relatively stable pattern of change. Inferential analysis revealed significant differences in MAP and PP over time (p < 0.001), with greater variation in MAP. Post hoc analysis identified that the decline in MAP began significantly at 5 minutes and continued until its lowest point, while PP changes tended to be more fluctuating and less consistent. Conclusion: Spinal anesthesia for cesarean section causes significant hemodynamic changes, with a dominant decrease in MAP in the early intraoperative phase. MAP proved to be more sensitive than PP in detecting changes in systemic perfusion, making it more relevant as a primary parameter in intraoperative monitoring. These findings emphasize the importance of close monitoring and early intervention to prevent hypotension and maintain hemodynamic stability.   Keywords: Arterial Pressure; Cesarean Section; Intraoperative Hemodynamics; Pulse Pressure; Spinal Anesthesia.

Abstract

Background: Sinta Pertiwi Klaten Limited Liability Company is a garment manufacturing company with a majority of female workers. Based on data from a preliminary survey of 30 tailors, it was found that the most frequently complained about occupational health and safety issue was fatigue due to high workloads. Environmental factors, such as temperatures above 30°C, can exacerbate fatigue and increase the risk of workplace accidents. Purpose: To analyze the relationship between workload and environmental temperature with fatigue in garment workers. Method: This study was conducted at Sinta Pertiwi Klaten Limited Liability Company using an analytical observational method with a cross-sectional design. Simple random sampling was used, with a population of 259 tailors using the Slovin formula, resulting in 157 female workers. NASA-TLX and IFRC questionnaires were distributed for data collection, and an Area Heat Stress Monitor was used to measure room temperature. Analysis used the Spearman Rank and Mann-Whitney tests with a 95% confidence level. Results: Most workers had a high workload (131 respondents) (83.4%), a work environment that met the NAB (Natural Temperature Limit) of <30°C (129 respondents) (82.2%), and a high level of work fatigue (91 respondents) (58.0%). Bivariate analysis showed a correlation between workload and fatigue (p = 0.000) and no correlation between work environment and work-related fatigue (p = 0.090). Conclusion: Workload due to high work target demands significantly influenced work fatigue compared to workplace temperature due to the respondents' young and productive age and short tenure. Suggestion: Workers are advised to stretch their muscles during breaks to help reduce work fatigue, improve blood circulation, and relax tense muscles due to repetitive work. Abstract

Background: The garment industry is a crucial manufacturing sector, with the majority of female workers performing static and repetitive sewing activities. Unergonomic work postures have the potential to cause musculoskeletal disorders (MSDs). Initial observations revealed that 30 female sewing operators sat with hunched backs, raised shoulders, and feet continuously pressing on the machine pedals. This condition requires immediate intervention, delay in action can lead to permanent disability and directly disrupt factory operational stability and productivity. Purpose: To analyze risk factors for musculoskeletal disorders in female sewing operators. Method: This is an observational analytical study using a cross-sectional approach. The sample was selected using a simple random sampling technique. The study population consisted of 259 female sewing operators at Sinta Pertiwi Klaten Limited Liability Company. The Slovin formula was used for sampling and a 5% margin of error, resulting in 157 samples. This study was conducted in the fourth week of May 2026. The Rapid Entire Body Assessment (REBA) method was used to assess work posture, and MSD complaints were measured using the Nordic Body Map (NBM) questionnaire. The data were then analyzed using the non-parametric Spearman's rank sum test. Results: The majority of respondents (97.5%) had risky work postures (153 respondents), and 66.9% (105 respondents) experienced painful MSDs. Bivariate analysis showed no significant association between work posture and musculoskeletal disorders (MSDs), with a p-value of 0.156 (P > 0.05). Conclusion: Work posture is not a significant factor in the incidence of musculoskeletal disorders (MSDs). Individual factors, such as young age, normal body mass index (BMI), less than 5 years of service, not smoking, and exercise habits or overall lifestyle, were more dominant influences on MSD complaints.   Keywords: Female Tailors; Musculoskeletal Disorders; Work Posture.

Abstract

Background: Diabetic foot ulcers (DFUs) are one of the most serious chronic complications of diabetes mellitus, leading to infection, amputation, reduced quality of life, and increased healthcare costs. Inadequate foot care practices and limited family involvement contribute to the high incidence of diabetic foot ulcers. FaCE is a family-based educational intervention designed to improve patients' knowledge, skills, and preventive behaviors through active family participation. Purpose: To determine the effect of FaCE on diabetic foot ulcer prevention in patients with diabetes mellitus. Method: This was a quasi-experimental study with a single-group pretest-posttest design. The study was conducted at a Community Health Center in Lhokseumawe City, involving 40 patients with type 2 diabetes mellitus selected using purposive sampling from a population of 80 patients. The FaCE intervention consisted of four educational sessions conducted over two weeks. Data were collected using a diabetic foot ulcer prevention behavior questionnaire developed based on the International Working Group on the Diabetic Foot (IWGDF) guidelines. Data were analyzed using the Wilcoxon signed-rank test with a significance level of p < 0.05. Results: The median score for diabetic foot ulcer prevention behaviors increased from 19 at pre-test to 28 at post-test. Improvements were observed in all components of foot care, including daily foot examination, foot hygiene, skin moisturizing, nail care, protection against foot trauma, and appropriate footwear selection. The Wilcoxon signed-rank test demonstrated a statistically significant increase in diabetic foot ulcer prevention behaviors after the intervention (Z = −5.521; p < 0.001). Conclusion: FaCE has been shown to be effective in improving diabetic foot ulcer prevention behaviors in patients with diabetes mellitus. This family-based educational approach can be an effective health promotion and prevention strategy to reduce the risk of diabetic foot ulcers in primary care settings. Suggestion: The FaCE program can be integrated into chronic disease management programs or routine education programs at community health centers to increase family involvement in preventing diabetes complications. Future research is recommended to use a control group design, a larger sample size, and a longer follow-up period to evaluate the sustainability of behavioral changes and their impact on reducing the incidence of diabetic foot ulcers.   Keywords: Diabetic Foot Ulcers; Diabetes Mellitus; Family Class Education.

Abstract

Background: Stunting is a nutritional problem that still occurs frequently in toddlers and can have long-term impacts on physical growth, cognitive development, and future productivity. Maternal knowledge about nutrition and appropriate supplementary feeding practices play a crucial role in meeting children's nutritional needs and preventing growth disorders. However, low maternal knowledge and inappropriate feeding practices remain contributing factors to stunting in toddlers. Purpose: To analyze the relationship between maternal knowledge about nutrition and supplementary feeding practices and the incidence of stunting in toddlers aged 12–36 months. Method: Using an observational analytical design with a cross-sectional approach, the study was conducted among mothers of toddlers aged 12–36 months in the Sori Utu Community Health Center working area. A total of 114 respondents were selected using a simple random sampling technique. Data were collected using a structured questionnaire to measure maternal knowledge about nutrition and supplementary feeding practices provision practices, while stunting status was determined through anthropometric measurements based on the height-for-age index. Data analysis was performed using the Chi-Square test with a significance level of p<0.05. Results: Most respondents had adequate nutritional knowledge (40.4%) and good supplementary feeding practices (54.4%). The prevalence of stunting in toddlers was 37.7%. Bivariate analysis showed a significant association between maternal knowledge of nutrition and the incidence of stunting (p<0.001). A significant association was also found between supplementary feeding practices and stunting (p<0.001). Toddlers whose mothers had poor nutritional knowledge and poor supplementary feeding practices were at higher risk of stunting. Conclusion: This study shows that maternal knowledge of nutrition and supplementary feeding practices are significantly associated with the incidence of stunting in toddlers aged 12–36 months. Suggestion: Further research is recommended to examine other factors associated with stunting, such as history of infectious diseases, environmental sanitation, household food security, family socioeconomic status, birth weight, and maternal nutritional status during pregnancy.   Keywords: Maternal Knowledge; Nutrition; Supplementary Feeding; Stunting; Toddlers.
