Abstract

Background: Chronic kidney disease patients undergoing regular hemodialysis are at risk of developing cardiovascular complications, one of which is intradialytic hypertension. Blood flow velocity, or Quick of Blood (QB), during hemodialysis is thought to be related to fluctuations in patient blood pressure. Objective: This study aimed to analyze the relationship between blood flow velocity (Quick of Blood/QB) and the incidence of intradialytic hypertension in chronic kidney disease patients undergoing hemodialysis. Method: This was an analytical observational study with a cross-sectional approach. The study sample consisted of 74 respondents selected using a total sampling technique, namely patients who had undergone hemodialysis for more than one year. QB data were obtained from the hemodialysis machine monitor, while blood pressure was measured using a sphygmomanometer before and during hemodialysis. The relationship between the two variables was analyzed using the Kendall Tau correlation test. Results: The majority of respondents had a standard QB (200–300 ml/minute), totaling 63 respondents (85.1%), with a mean of 227.3 ml/minute. Most respondents experienced intradialytic hypertension, totaling 66 respondents (89.2%), with a mean increase in systolic blood pressure of 16.22 mmHg. Conclusion: The statistical test results showed a p-value of 0.053 (>0.05), indicating no significant relationship between QB and the incidence of intradialytic hypertension in chronic kidney disease patients. Hemodialysis nurses should continue to monitor patients' blood pressure regularly, even if QB is within the standard range, as intradialytic hypertension is thought to be more influenced by other factors such as age, duration of hemodialysis, and the patient's hemodynamic condition.

Abstract

Latar Belakang: Osteoarthritis genu merupakan gangguan degeneratif pada sendi lutut yang ditandai oleh kerusakan kartilago disertai perubahan pada struktur sendi lainnya. Kondisi ini dapat menimbulkan nyeri, keterbatasan lingkup gerak sendi, penurunan kekuatan otot, serta gangguan aktivitas fungsional. Ultrasound yang dipadukan dengan terapi latihan merupakan salah satu intervensi fisioterapi yang digunakan untuk mengatasi gangguan tersebut. Metode: Desain penelitian yang digunakan adalah studi kasus pada seorang pasien dengan osteoarthritis genu dextra. Pasien menjalani empat sesi intervensi fisioterapi berupa ultrasound dan terapi latihan. Evaluasi dilakukan menggunakan Visual Analog Scale (VAS), goniometer, Manual Muscle Testing (MMT), dan Skala Jette. Hasil: Setelah empat sesi intervensi, terjadi penurunan nyeri diam dari skor 5 menjadi 2, nyeri tekan dari 6 menjadi 4, dan nyeri gerak dari 8 menjadi 5. Kekuatan otot hamstring meningkat dari nilai 3 menjadi 4, sedangkan kekuatan otot quadriceps meningkat dari nilai 4 menjadi 5. Lingkup gerak sendi aktif bertambah dari S 0°–0°–120° menjadi S 0°–0°–130°, sedangkan lingkup gerak pasif meningkat dari S 0°–0°–130° menjadi S 0°–0°–135°. Perubahan tersebut diikuti oleh kemampuan aktivitas fungsional yang mengalami perbaikan berdasarkan hasil pengukuran Skala Jette, terutama pada aktivitas jongkok ke berdiri dan tingkat kesulitan saat naik dan turun tangga. Kesimpulan: Pemberian ultrasound yang dipadukan dengan terapi latihan memberikan perubahan klinis berupa penurunan nyeri, peningkatan kekuatan otot, bertambahnya lingkup gerak sendi, dan perbaikan kemampuan aktivitas fungsional pada pasien dengan osteoarthritis genu dextra.

Abstract

Background: Umbilical cord care is an essential component of neonatal care aimed at preventing infection and promoting the physiological separation of the umbilical cord. Breast milk contains various bioactive components, including lysozyme, lactoferrin, immunoglobulin A (IgA), and growth factors, which possess antimicrobial and anti-inflammatory properties and contribute to tissue healing. Objective: This study aimed to determine the effect of topical breast milk application on umbilical cord separation time in newborns at Wangon I and Jatilawang Community Healthcare Centers. Methods: This quantitative study employed a quasi-experimental approach using a post-test-only non-equivalent control group design. The study included 60 newborns selected through purposive sampling, consisting of 30 infants in the intervention group who received topical breast milk application and 30 infants in the control group who received open umbilical cord care. Univariate analysis was performed to describe the respondents' characteristics, while bivariate analysis was conducted using the Mann–Whitney test. Results: Most respondents were female (33 infants; 55%), and the majority had a birth weight of 3,001–3,500 grams (31 infants; 51.7%). In the intervention group, 17 infants (56.7%) experienced rapid umbilical cord separation, while 13 infants (43.3%) had normal separation. In the control group, 15 infants (50.0%) experienced normal umbilical cord separation, whereas 13 infants (43.3%) experienced delayed separation. The Mann–Whitney test yielded a p-value of 0.001 (p < 0.05), indicating a statistically significant effect of topical breast milk application on umbilical cord separation time. Conclusion: Topical application of breast milk has a significant effect on accelerating umbilical cord separation in newborns.

Abstract

Background: Urinary incontinence is characterized by the involuntary leakage of urine resulting from impaired urinary storage function. This condition is commonly associated with pelvic floor muscle weakness, increased urinary frequency, and reduced quality of life that affects daily activities. The combination of Transcutaneous Electrical Nerve Stimulation (TENS) and Kegel exercises is widely used as a conservative physiotherapy intervention to improve pelvic floor muscle function and alleviate urinary incontinence symptoms. Objective: This study aimed to evaluate the clinical outcomes of physiotherapy using Transcutaneous Electrical Nerve Stimulation (TENS) and Kegel exercises on pelvic floor muscle strength, urinary frequency, and urinary incontinence severity in a patient with urinary incontinence. Methods: A patient with urinary incontinence underwent five physiotherapy sessions consisting of Transcutaneous Electrical Nerve Stimulation (TENS) and Kegel exercises, including bridging, pelvic tilt, and kneeling pelvic thrust. Clinical outcomes were evaluated using Manual Muscle Testing (MMT), a bladder diary, and the International Consultation on Incontinence Questionnaire–Urinary Incontinence Short Form (ICIQ-UI SF). Results: After five physiotherapy sessions, the strength of the pubococcygeus and iliococcygeus muscles remained at an MMT grade of 3/5, urinary frequency persisted at approximately every 1–2 hours, and the ICIQ-UI SF score remained at 10, indicating moderate urinary incontinence. The absence of clinical improvement may be related to the limited duration of the intervention. Conclusion: Five sessions of physiotherapy consisting of Transcutaneous Electrical Nerve Stimulation (TENS) and Kegel exercises did not result in improvements in pelvic floor muscle strength, urinary frequency, or ICIQ-UI SF scores.

Abstract

Background: Hypertension is one of the leading causes of morbidity and mortality worldwide. According to the World Health Organization (WHO), the number of people with hypertension is estimated to reach 1.5 billion by 2025. In Indonesia, hypertension is highly prevalent among older adults, reaching 63.5% in 2023. One non-pharmacological intervention that may help reduce blood pressure is the combination of Slow Deep Breathing (SDB) and isometric handgrip exercise, which promotes relaxation and improves blood circulation. Objective: This study aimed to determine the application of combined Slow Deep Breathing (SDB) and isometric handgrip exercise in helping reduce blood pressure among older adults with hypertension. Methods: A qualitative descriptive study with a case study approach and evidence-based practice implementation was conducted. The participant was selected through purposive sampling and was a 66-year-old male who met the study criteria. The intervention was performed for three consecutive days, with blood pressure measured before and after therapy using a digital sphygmomanometer. Results: Blood pressure decreased from 142/80 mmHg to 122/72 mmHg, from 145/78 mmHg to 134/81 mmHg, and from 145/73 mmHg to 139/76 mmHg. The participant also reported feeling lighter, more relaxed, and no longer experiencing dizziness. Conclusion: The combination of SDB and isometric handgrip exercise may help reduce blood pressure in older adults and may be used as a complementary non-pharmacological therapy.

Abstract

Background: Calcaneal spur is a musculoskeletal condition that may cause heel pain, limited ankle range of motion, decreased muscle strength, and impaired functional activity, particularly during standing, walking, and weight-bearing activities. These impairments may limit daily activities and require physiotherapy interventions to reduce symptoms and improve functional movement. Objective: This study aimed to describe changes in pain, ankle joint range of motion, muscle strength, and functional activity following the application of Ultrasound Therapy and Manual Therapy in a patient with an inferior and posterior calcaneal spur of the left calcaneus. Methods: A case study design was applied to a male patient diagnosed with an inferior and posterior calcaneal spur of the left calcaneus. The intervention was administered over four sessions and consisted of Ultrasound Therapy and Manual Therapy in the form of deep friction massage, joint mobilization, and stretching. Outcomes were evaluated using the Visual Analogue Scale (VAS), goniometer, Manual Muscle Testing (MMT), and Foot and Ankle Disability Index (FADI). Results: Pressure pain decreased from 5.2 to 4.2, while movement pain decreased from 3.4 to 2.8. Active ankle dorsiflexion increased from 15° to 20°, whereas plantarflexion remained at 50°. Muscle strength for dorsiflexion and plantarflexion increased from grade 3 to grade 4. The FADI score increased from 65 (73.9%) to 78 (88.6%), indicating improved functional activity from mild impairment toward near-normal status. Conclusion: Four sessions of Ultrasound Therapy and Manual Therapy were associated with improvements in pain, ankle dorsiflexion range of motion, muscle strength, and functional activity in a patient with a left-sided calcaneal spur.

Abstract

Background: Knee osteoarthritis (OA) is a degenerative joint disorder that may cause pain, reduced muscle strength, and functional limitations. Physiotherapy interventions, including Transcutaneous Electrical Nerve Stimulation (TENS) and exercise therapy, are used to control pain, improve muscle performance, and support functional activity. Objective: This study aimed to describe changes in pain intensity, muscle strength, and functional ability following a physiotherapy program combining Transcutaneous Electrical Nerve Stimulation (TENS) and exercise therapy in a patient with bilateral knee osteoarthritis. Methods: This study used a case study design involving a patient with bilateral knee osteoarthritis. Data were collected through a series of physiotherapy assessments, intervention sessions, and subsequent evaluations. The physiotherapy program consisted of Transcutaneous Electrical Nerve Stimulation (TENS) and exercise therapy delivered over three treatment sessions. Changes in the patient’s condition were assessed through pain intensity, muscle strength using Manual Muscle Testing, and functional ability using the Jette Scale. Results: Changes were observed in several clinical parameters after three physiotherapy sessions. Pressure pain decreased from a score of 3 to 2, while movement-related pain decreased from 4 to 3. Muscle strength improved from grade 4 to grade 5. Functional ability assessed using the Jette Scale also changed, with the score decreasing from 22 at the initial assessment to 18 at the final evaluation. Conclusion: The physiotherapy program combining Transcutaneous Electrical Nerve Stimulation and exercise therapy was accompanied by improvements in pain intensity, muscle strength, and functional ability in a patient with bilateral knee osteoarthritis. After three treatment sessions, reduced pain was accompanied by improved muscle strength and a lower Jette Scale score, indicating better functional ability.

Abstract

Introduction: Rheumatoid arthritis is an autoimmune disease characterized by inflammation of the synovium, which may cause joint pain, stiffness, swelling, joint damage, and deformity, particularly in the hands, wrists, feet, and knees. Objective: This study aimed to determine the effect of the Slow Deep Breathing technique combined with lemon aromatherapy on joint pain among patients with rheumatoid arthritis in the working area of UPTD Puskesmas II Negara. Methods: This quantitative study used a pre-experimental design with a one-group pretest–posttest approach. The sample consisted of 57 respondents selected using convenience sampling. The intervention consisted of the Slow Deep Breathing technique combined with lemon aromatherapy. Pain intensity was measured using the Numeric Rating Scale (NRS). Data were analyzed using the Wilcoxon signed-rank test with a significance level of α = 0.05. Results: The Wilcoxon signed-rank test showed a significant difference in joint pain before and after the intervention (Z = −7.141; p < 0.001), indicating an effect of the Slow Deep Breathing technique combined with lemon aromatherapy on joint pain among patients with rheumatoid arthritis. Conclusion: The Slow Deep Breathing technique combined with lemon aromatherapy significantly reduced joint pain among patients with rheumatoid arthritis in the working area of UPTD Puskesmas II Negara.

Abstract

Background: Proximal one-third left radius and ulna fractures treated with Open Reduction Internal Fixation (ORIF) may be accompanied by pain, limited joint range of motion, decreased muscle strength, and impaired functional activity. In this case, the patient presented with movement pain, tenderness, limited joint range of motion, and reduced functional ability of the left upper extremity. Infrared was applied as a thermotherapy modality to provide heat to superficial tissues, while therapeutic exercises included stretching, active exercise, and hold relax to support joint mobility. Objective: This study aimed to describe changes in pain, joint range of motion, muscle strength, and functional activity following physiotherapy intervention in a patient with post-ORIF proximal one-third left radius and ulna fractures. Methods: A case study design was conducted involving one patient who received four physiotherapy sessions. Pain was assessed using the Visual Analogue Scale, joint range of motion was measured using a goniometer, muscle strength was assessed using Manual Muscle Testing, and functional activity was evaluated using the Wrist and Hand Disability Index. Results: After four therapy sessions, movement pain decreased from 5 to 2, resting pain from 1 to 0, and tenderness from 3 to 2. Joint range of motion at the elbow and wrist improved compared with the initial assessment, accompanied by improved functional activity. Conclusion: The combination of infrared and therapeutic exercise was accompanied by improvements in pain, joint range of motion, and functional activity during four physiotherapy sessions in a patient with post-ORIF proximal one-third left radius and ulna fractures.

Abstract

Background: Nightlife areas in urban communities may influence perceptions of HIV transmission risk. Differences in knowledge, experiences, and social environments may shape how residents understand and respond to HIV-related risks. Objective: This study aimed to explore urban community perceptions of HIV transmission risk in the area surrounding a nightclub. Methods: A qualitative study using a descriptive phenomenological approach was conducted among five residents living around the nightclub area. Data were collected through in-depth interviews using an interview guide and analyzed thematically to identify themes related to community perceptions of HIV transmission risk. Results: Six themes emerged: perceptions of nightlife, understanding of HIV, self-protection efforts, sources of information and the need for HIV education, experiences and learning about HIV, and feelings about living around nightlife areas. Participants had diverse perceptions of nightlife. Some perceived nightlife as potentially increasing the risk of disease, whereas others considered that nightlife did not directly affect health and had both positive and negative aspects. Understanding of HIV varied from recognizing HIV as an infectious disease to having limited knowledge. Participants reported protecting themselves by avoiding risky behaviors. Sources of HIV information included the internet, social media, health workers, and schools, although some participants had never received HIV education. Participants also expressed different feelings about living near nightlife areas, ranging from feeling safe and unafraid to concerns about the risk of HIV infection. Conclusion: Community perceptions of HIV transmission risk around nightlife areas varied in terms of views, knowledge, experiences, and feelings. The six themes illustrate how residents understand and respond to HIV-related risks in the nightlife environment.
