Abstract

Type 2 Diabetes Mellitus (DM) is a chronic metabolic disease characterized by elevated blood glucose levels due to impaired insulin secretion and insulin resistance. The increasing prevalence of Type 2 DM is influenced by unhealthy lifestyles, lack of physical activity, and changes in people's dietary patterns. One effort to control blood sugar levels is the use of local herbal plants as complementary therapy. This study aims to analyze the effect of providing herbal drinks based on local plants on glycemic control in patients with Type 2 DM in Tanah Merah Village, Badar District, Southeast Aceh Regency. This study used a quantitative method with a quasi-experimental design through a one-group pretest-posttest design. A sample of 60 respondents was selected using a purposive sampling technique. Data collection was carried out by examining blood glucose levels before and after administering the herbal drink based on local plants using a glucometer. Data analysis used a paired t-test with a significance level of 0.05.α= 0.05. The results of the study showed that the average blood glucose level before giving herbal drinks was 238.6 mg/dL and after the intervention decreased to 187.3 mg/dL. The results of the statistical test obtained a p-value = 0.000 (p < 0.05), indicating that there was a significant effect of giving local plant-based herbal drinks on glycemic control in Type 2 DM patients. A decrease in blood glucose levels of 51.3 mg/dL indicates that local plant-based herbal drinks have the potential to help control blood sugar levels in Type 2 Diabetes Mellitus patients. The results of this study are expected to be a source of information for the public and health workers regarding the use of local herbal plants as complementary therapy in controlling Type 2 Diabetes Mellitus.

Abstract

Gallstone disease is one of the most common gastrointestinal conditions requiring surgical intervention worldwide. Over the past few decades, laparoscopic cholecystectomy (LC) has replaced open cholecystectomy (OC) as the gold standard in gallbladder surgical management. Although its clinical advantages have been widely recognized, a systematic evaluation of its impact on hospital operational efficiency and resource optimization through length of stay is still needed. This study aims to evaluate the effectiveness of laparoscopic cholecystectomy in improving healthcare efficiency and shortening patient length of stay compared with open surgical procedures. This study used a Systematic Literature Review (SLR) method through a literature search in various scientific databases, such as PubMed, Scopus, and Google Scholar. The literature analyzed included meta-analyses, randomized clinical trials, and cohort studies comparing the outcomes of laparoscopic cholecystectomy and open cholecystectomy. A total of eight key journals were selected for further analysis, focusing on mortality, postoperative complications, length of stay, and healthcare cost-efficiency. The study showed that laparoscopic cholecystectomy significantly reduced mortality by 84% (RR 0.16) and reduced the risk of postoperative complications by 54% compared to open surgical procedures. In terms of service efficiency, this procedure was also shown to shorten the average length of stay by 4.08 days compared to open cholecystectomy. In several healthcare centers, the average length of stay for patients undergoing laparoscopic cholecystectomy was only around 2.1 days, compared to 5.6 days for open procedures. Furthermore, the implementation of ambulatory laparoscopic cholecystectomy has been proven safe, provides significant hospital cost savings, and improves patient satisfaction through a faster recovery process. Based on the study results, it can be concluded that laparoscopic cholecystectomy is more effective than open cholecystectomy in improving healthcare efficiency. The significant reduction in length of stay and the potential for outpatient procedures make this method an important strategy in optimizing hospital bed utilization and reducing healthcare system costs. Therefore, investment in infrastructure and development of laparoscopic services is highly recommended to increase accessibility of more efficient, safe, and high-quality surgical services.

Abstract

The pituitary gland (hypophysis) is a very small, pea-sized gland located inside the skull, specifically beneath the brain and above the back of the nasal cavity. However, if the gland grows beyond its normal size (tumor), it can compress and damage the brain and nerves, leading to hormonal disorders and even blindness. Most pituitary tumors are benign. Their symptoms vary depending on the type of tumor and the areas around the pituitary gland that are affected. Many patients present with visual disturbances or even blindness. Although benign, progressive growth can cause symptoms that significantly affect the patient's quality of life. Management of pituitary tumors may include radiotherapy, medication, and surgery. Surgery is the primary treatment for pituitary tumors. Currently, the Endoscopic Skull Base Surgery (ESBS), Transsphenoidal Approach (TSA), has become one of the techniques that offers advantages such as shorter stays in the ICU and hospital ward, reduced risks and minimal bleeding during surgery, faster recovery, and no facial incisions (better cosmetic outcome).

Abstract

Service quality is a critical determinant of patient satisfaction and loyalty in healthcare settings, as it reflects the extent to which services meet patient expectations. This study aimed to examine the effects of trust and patient satisfaction on outpatient loyalty at Sundari Hospital in 2025. A quantitative descriptive approach was employed, involving 99 outpatient respondents who completed structured questionnaires. Data were analyzed using univariate and bivariate techniques, with the chi-square test applied to assess the associations between variables. The results indicated that 63.6% of respondents reported high levels of trust, 60.6% reported high satisfaction, and 56.6% demonstrated loyalty toward the hospital. Chi-square analysis revealed that both trust (p = 0.003) and satisfaction (p = 0.012) significantly influenced outpatient loyalty. The findings confirm that enhancing patient trust and satisfaction can strengthen loyalty, which is essential for sustaining patient engagement and promoting repeat visits. These results provide practical insights for healthcare managers seeking to improve service quality and patient-centered strategies, highlighting the importance of fostering trust and satisfaction to achieve better patient retention and long-term institutional performance.

Abstract

Boesenbergia rotunda is a plant belonging to the Zingiberaceae family. This plant is known to contain various bioactive compounds with potential as natural antibacterial agents. This study aims to determine the microscopic anatomical characteristics, secondary metabolite content, and antibacterial activity of Boesenbergia rotunda rhizome extract against Staphylococcus aureus. The research methods used included microscopic observation, phytochemical screening, and antibacterial activity testing using the disk diffusion method. Anatomical observations revealed that the Boesenbergia rotunda rhizome consists of parenchyma tissue with starch granules scattered throughout the ground tissue. Phytochemical screening results indicated the presence of flavonoids, alkaloids, terpenoids, saponins, and tannins. Antibacterial activity testing showed that the Boesenbergia rotunda rhizome extract was able to inhibit the growth of Staphylococcus aureus, producing an inhibition zone with a diameter of ±17 mm. This inhibition zone diameter falls into the category of strong inhibition. This strong antibacterial activity is believed to result from the synergistic action of secondary metabolites that work through mechanisms of cell membrane damage, disruption of membrane permeability, and inhibition of bacterial cell wall synthesis. Based on the research results, the rhizome of Boesenbergia rotunda has the potential to be developed as a natural antibacterial source derived from herbal materials.

Abstract

Background: Medical education aims to produce physicians with strong clinical competencies, including clinical reasoning skills. Visual-based learning strategies, such as images, videos, virtual simulations, and virtual patients, are considered effective in enhancing these skills through more concrete, contextual, and interactive learning experiences. However, the effectiveness of different visual learning methods in improving clinical reasoning abilities among medical students remains an area that requires further investigation. Methods: This study employed a literature review design using a systematic search approach based on the PICOS framework. Relevant articles were identified through Google Scholar, PubMed, Garuda, and ScienceDirect databases using keywords related to visual learning, clinical reasoning, and medical students. The review included studies published between 2021 and 2025 that examined the effectiveness of visual-based learning strategies among medical profession students. Data were analyzed using descriptive and narrative synthesis methods. Results: The findings indicate that visual-based learning strategies are generally effective in enhancing clinical reasoning skills among medical students. Interactive approaches, particularly virtual simulations and virtual patient-based learning, demonstrate greater effectiveness compared to passive visual methods such as educational videos. In addition to improving clinical reasoning, visual-based learning strategies also contribute to the development of clinical observation skills, critical thinking, communication abilities, and empathy toward patients. Conclusion: Visual-based learning strategies are effective in improving clinical reasoning skills among medical profession students, especially when integrated with active and interactive learning approaches. The effectiveness of these strategies is influenced by the type of visual media used and the instructional design implemented. Therefore, the integration of interactive visual learning methods is recommended to support the development of clinical competencies in medical education.

Abstract

Electronic prescribing (e-prescribing) is an information technology–based innovation in healthcare systems aimed at improving patient safety and quality of care, particularly in the medication prescribing process. The implementation of e-prescribing in hospitals has been shown to reduce medication errors; However, its success is influenced by various complex and multidimensional factors. This study aims to identify and analyze the factors affecting the successful implementation of e-prescribing in hospitals using a literature review approach. The results indicate that the success of e-prescribing implementation is influenced by individual factors (knowledge, attitudes, and user skills), technological factors (system quality, user interface design, and clinical decision support systems), organizational factors (management support, infrastructure readiness, and workflow), and policy factors (regulations and interoperability standards). Additionally, several barriers were identified, including limited infrastructure, user resistance, alert fatigue, and technology-induced errors that may affect system effectiveness. Therefore, a comprehensive and integrated approach is required to optimize the implementation of e-prescribing in improving patient safety and healthcare service quality in hospitals.

Abstract

Exposure to ultraviolet (UV) radiation can cause oxidative stress, premature aging, and even skin cancer, so it is necessary to develop sunscreens based on natural ingredients that are safe and effective. Secang wood (Caesalpinia sappan L.) is known to contain phenolic compounds and brazilin which have antioxidant activity and have the potential as photoprotective agents. This study aims to develop a sunscreen gel formulation from ethanol extract of secang wood and evaluate its antioxidant activity, Sun Protection Factor (SPF) value, physical characteristics, and stability. Extraction was carried out using the maceration method using 96% ethanol. Antioxidant activity was determined using the DPPH method, while sunscreen activity was analyzed in vitro using UV-Vis spectrophotometry with the Mansur method. The gel was formulated in four formulas with varying extract concentrations (0.85% and 1.7%) and Carbopol (0.75% and 1%). Evaluation of the preparation included organoleptic tests, homogeneity, pH, viscosity, syneresis, spreadability, adhesion, and accelerated stability. The results showed that the ethanol extract of sappanwood has strong antioxidant activity with an IC50 value of 58.503 µg/mL. The SPF values ​​of the extract at concentrations of 500, 850, and 1000 ppm were 49.59; 46.64; and 45.86, respectively, which are included in the high protection category. The SPF value of the gel preparation ranged from 15.11 to 22.36, with formula F2 (0.85% extract and 1% Carbopol) producing the highest SPF value of 22.355 ± 0.085. All formulas met the physical quality requirements of topical preparations with a pH range of 4.59-4.91, viscosity of 122.8-283.7 P, spreadability of 4.40-5.38 cm, and adhesiveness of 4.3-4.6 seconds. The results of the study indicate that the ethanol extract of sappanwood has the potential to be developed as an active ingredient in natural sunscreen in the form of a stable, safe, and effective gel preparation.

Abstract

This study aims to systematically review the relationship between spirituality and suicide occurrence in adolescents. A systematic literature search was conducted on PubMed, ScienceDirect, and Google Scholar for articles published between 2013 and 2023. After screening 287 records, 16 observational studies (cross-sectional, cohort, case-control) involving 58,674 adolescents from various countries, including Indonesia, met the inclusion criteria. The majority of studies (13/16) demonstrated a significant negative association between spirituality and suicidal ideation, attempts, and completed suicide. Dimensions of meaning in life and positive religious coping emerged as the strongest protective factors. Spirituality reduced suicide risk by up to 55% through mechanisms of enhanced resilience, hope, moral objections, emotion regulation, and faith-based social support. These protective effects were stronger in highly religious sociocultural contexts. The findings underscore the importance of incorporating spiritual assessment and spiritually integrated interventions into adolescent suicide prevention programs, particularly in settings where religious values are salient. This review provides a comprehensive synthesis of current evidence and highlights the multidimensional role of spirituality as a protective factor against youth suicide.

Abstract

Background: Pain is one of the most common complaints experienced by patients after surgery and can negatively affect sleep quality during the recovery period. Sleep disturbances caused by pain may delay the healing process, increase the risk of complications, and reduce patient comfort. Objective: This study aimed to analyze the relationship between pain intensity and sleep quality among postoperative patients in the surgical ward of Dr. Tadjuddin Chalid General Hospital, Makassar. Methods: This study employed a quantitative research design with a cross-sectional approach. A total of 74 postoperative patients were selected using the Slovin formula. Pain intensity was measured using the Numeric Rating Scale (NRS), while sleep quality was assessed using the Pittsburgh Sleep Quality Index (PSQI). The data were analyzed using univariate and bivariate analyzes with the Chi-square test. Results: The majority of respondents experienced moderate pain (45.9%) and poor sleep quality (66.2%). The Chi-square test revealed a significant relationship between pain intensity and sleep quality among postoperative patients (p = 0.001). All respondents with severe pain had poor sleep quality, whereas most respondents with mild pain had good sleep quality. Conclusion: There was a significant relationship between pain intensity and sleep quality among postoperative patients. The higher the pain intensity experienced, the poorer the sleep quality. Optimal pain management is an essential component of nursing care to improve sleep quality and support faster recovery in postoperative patients.
