Abstract

Introduction: Cutaneous squamous cell carcinoma (SCC) is the second most prevalent skin cancer in the world. The correlation between smoking and cutaneous SCC is still inconclusive. Methods: Further investigation through literature search was conducted in 3 databases, which were PubMed, Scopus, and Cochrane. The investigators also conducted hand searching by examining references in each acquired article. Selection of articles is based on established inclusion and exclusion criteria. Critical appraisal used instruments from the Oxford Centre for Evidence-Based Medicine and the CASP checklist. Results: All four selected articles show that smokers have a higher risk of cutaneous SCC with OR 1.08 to 2.96. Conclusion: Smoking increases the risk of cutaneous SCC.

Abstract

Introduction: This retrospective study evaluates the impact of triamcinolone-assisted vitrectomy on intraocular pressure (IOP) in patients undergoing vitreoretinal surgery, with particular focus on comparing outcomes in patients with and without lens surgery. Methods: Medical records of patients treated at R.D. Kandou Hospital from May 2019 to May 2024 were reviewed, and pre- and postoperative IOP values were analyzed. A total of 35 patients met the inclusion criteria. Results: The results show a significant reduction in IOP at all postoperative intervals, with mean differences of 11.12 mmHg at 1 day, 8.60 mmHg at 1 week, 13.23 mmHg at 1 month, and 16.91 mmHg at 3 months postoperatively. Analysis revealed no significant difference in IOP outcomes between phakic and pseudophakic patients. Conclusion: These findings indicate that triamcinolone-assisted vitrectomy effectively lowers IOP, regardless of lens status.

Abstract

Trauma is the biggest cause of death in young people. Among these traumas, 80% are blunt traumas, mostly to the thorax and abdomen. Blunt trauma can cause bleeding, which increases mortality. Prompt intervention can reduce mortality. eFAST (extended focused assessment with sonography for trauma) is a bedside ultrasound examination indicated for abdominal and thoracic trauma patients, which is limited to 4 examination windows in the abdomen to evaluate for hemoperitoneum, hemothorax, and pericardial effusion. The eFAST examination uses ultrasound equipment that is widely available in the emergency department and can be performed bedside, making it less time-consuming. eFAST has a high sensitivity and specificity, which is helpful in its role as a screening modality. The eFAST examination has many advantages, such as ease of use, being noninvasive, being fast, being easy to find, and not using radiation or contrast material, making it safe for children and pregnant women, and being inexpensive. The eFAST examination is important for the diagnosis of thoracoabdominal trauma, as it enables rapid screening.

Abstract

Introduction: Color blindness, or color vision deficiency, is an eye condition that can reduce quality of life. Tinted contact lenses have been researched as a potential tool for correcting color blindness. Methods: This review aims to identify the effectiveness of color blindness aids and devices for patients with color vision deficiency as an alternative to enhance their quality of life. Article searches were conducted through electronic databases Google Scholar and PubMed using the keywords “color vision deficiency device,” “color blindness device,” and “color vision deficiency management.” Inclusion criteria included articles published between 2018-2023, full texts, writing in English, alignment of titles with abstracts, and validity of research methods. Results: The literature search yielded 98 articles; 8 articles met the inclusion criteria. Analysis of these 8 articles showed that color blindness devices and aids provide subjective new color perception and interpretation for color-blind individuals, but the objective effectiveness of these tools was not significant. Conclusion: The importance of public awareness of color blindness and the importance of developing more effective color-blind tools and devices. Further research is needed to develop color blindness devices and aids.

Abstract

Introduction: Distance or online learning conducted during the COVID-19 pandemic causes a lack of interaction among students which affects the level of empathy of these students. Online learning also causes stress in medical students which can lead to burnout. The purpose of this study was to determine the relationship between the level of empathy and the level of burnout among students of the Tanjungpura University Medical Study Program class of 2019. Methods: The method used in this research is an observational analytic study with a cross-sectional approach. The research population was students of the Tanjungpura University Medical Study Program class of 2019 who were taken using a sampling technique in the form of total sampling with a minimum sample size of 76 people. Bivariate analysis using Spearman correlation test. Results: The results obtained from the Spearman correlation test based on SPSS 24.0 analysis obtained a significance value of 0.768 (p>0.05) for the relationship between the level of empathy and the level of burnout. Conclusion: There is no relationship between the level of empathy and the level of burnout in students of the Medical Study Program at Tanjungpura University class 2019.

Abstract

Introduction: Maternal mental health is an important yet often overlooked issue, both globally and nationally. About 10% of pregnant women experience mental health problems during pregnancy, with prevalence rates tending to be higher in developing countries. Methods: This descriptive study involved 100 pregnant women who attended routine antenatal care at UPTD Puskesmas Manggar between September and October 2023. The study aimed todetermine the prevalence and characteristics of maternal mental health status. Data were collected through interviews and the Self Report Questionnaire-20 (SRQ-20). Results: A total of 24% pregnant women were found to have common mental disorders, with fatigue being the most frequently reported symptom. The majority of those affected was multiparous (58.3%), in the third trimester of pregnancy (58.3%) and had no history of abortion (91.7%). Conclusion: The relatively high prevalence of common mental disorders during pregnancy underscores the importance of implementing routine mental health screening as an integral part of antenatal care at the primary healthcare level.

Abstract

Introduction: Malaria is an infection transmitted through mosquitoes infected by the Plasmodium parasite, with symptoms such as fever, hepatosplenomegaly, and anemia. Based on the data from Indonesian Basic National Health Survey (RISKESDAS) 2018, malaria prevalence in East Nusa Tenggara Province (ENTP) is the top 3 with a 2% positivity rate with blood tests. Alor district, as a part of ENTP, also an endemic area for malaria and was categorized as a mid-endemic area based on Annual Report of the Indonesian Ministry of Health in 2022. We conducted this study to describe the characteristics of malaria patients, especially in children in Moru Community Health Center (CHC), Alor District, Indonesia. Methods: This descriptive study used data reported on the Malaria Surveillance Information System (SISMAL) from 2019 and 2022. Parameters evaluated in sex, age, type of infection, blood test type, case finding, monthly cases, and area of transmission. Results: There were 33 cases (19,5%) in 2019 and 136 cases (80,5%) in 2022, 46 cases were infected by P. falciparum, 78 by P. vivax, and 45 cases were mixed infections from both plasmodium. Conclusion: Malaria cases in this study increased in April, May, and June, the type of infection was dominated by P. vivax, and most of the infected area was in the Fanating subdistrict.

Abstract

Hypoalbuminemia is a condition of low blood albumin concentration (<3.5 g/dL). Hypoalbuminemia reflects more on the level of physiological stress caused by one or more diseases than lack of nutrition. Hypoalbuminemia is found mostly in inpatient, critically-ill, and geriatric patients. Hypoalbuminemia causes poor functional state, longer hospitalization, and higher morbidity and mortality. The three most common causes of hypoalbuminemia are: (1) Decreased production of albumin, (2) Increased loss of albumin via the kidneys, gastrointestinal tract, skin, or extravascular space, and (3) Increased catabolism of albumin. Management of hypoalbuminemia is to find the underlying disease, and oral or parenteral supplementation of albumin. Parenteral albumin administration is recommended in specific conditions, such as patients with cirrhosis undergoing large-volume paracentesis (ascites >5 liters), hepatorenal syndrome, spontaneous bacterial peritonitis, and burns involving more than 30% of total body surface area after the first 24 hours. In patients with sepsis, acute respiratory failure, major non-cardiac surgery, or cardiac surgery, parenteral albumin may be considered when hemodynamic stability cannot be achieved with crystalloids. Conversely, parenteral albumin administration is not recommended as maintenance fluid or initial resuscitation in patients with acute traumatic brain injury.

Abstract

Introduction: Fatal ventricular tachyarrhythmias such as ventricular tachycardia (VT) or ventricular fibrillation (VF) are the most frequent cause of sudden cardiac death. In-hospital mortality of VT in STEMI was a 4- to 5-fold increase compared to STEMI without ventricular tachyarrhythmia; VT and VF were also initially thought to be the most common causes of out-of-hospital cardiac arrest. Late VT (occurs beyond 24-48 hours of STEMI onset) is a predictor for worse prognosis. Case: A 49-year-old female presented with continuous chest pain for 5 days and a rapid heartbeat (palpitation) for 4 days before hospital admission. Initial examination revealed VT and ST elevation on ECG, leading to a diagnosis of anteroseptal STEMI with VT. Treatment included aspirin, clopidogrel, heparin, isosorbide dinitrate, bisoprolol, and simvastatin. The VT resolved about 2 hours after the initial treatment, and the evolution of STEMI (on ECG) was observed. Conclusion: Early recognition and prompt treatment for VT patients can prevent hemodynamic instability and reduce mortality.

Abstract

Introduction: Hypoxic spells are serious and potentially fatal complications of cyanotic congenital heart disease. These events typically occur in children and are extremely rare in adults. They are caused by a sudden reduction in pulmonary blood flow, leading to severe hypoxia, cyanosis, seizures, and even death. Although rare, hypoxic spells should be considered as a differential diagnosis in adult patients presenting with hypercyanotic crises, particularly those without a known history of congenital heart disease. Case: A 20-year-old woman presented with worsening shortness of breath and cyanosis over the past two hours. She had a lifelong history of recurrent cyanosis, a habit of squatting during episodes of breathlessness, and limited physical activity but had never been diagnosed with congenital heart disease. Electrocardiography revealed right axis deviation and right ventricular hypertrophy, while a chest X-ray showed cardiomegaly with a boot-shaped heart silhouette. A hypoxia test supported the suspicion that her hypoxia was cardiac in origin. Initial management included the knee-chest position, highflow oxygen therapy, intravenous fluids, morphine, and norepinephrine to increase systemic vascular resistance. Subsequent echocardiography confirmed pulmonary atresia with ventricular septal defect and major aortopulmonary collateral arteries (PA + VSD + MAPCAs), a rare variant of tetralogy of Fallot. Conclusion: Hypoxic spells in adults are extremely rare and often underrecognized, especially in resource-limited settings. Rapid diagnosis and early intervention are crucial to improving outcomes. Referral to a specialized cardiac center is essential for comprehensive evaluation and definitive treatment planning.
