Abstract

Background. Indonesia has the highest prevalence of blindness in Southeast Asia; 81.2% are caused by cataracts. The prevalence of cataract blindness in South Kalimantan was 87.9%. Risk factors for cataracts include age, sex, comorbidities, such as diabetes mellitus and hypertension, steroid use, history of smoking, and exposure to ultraviolet radiation. This study aims to determine the risk factors and incidence of cataracts in the area of Astambul Community Health Center. Method. An observational analytical study with a cross-sectional approach. Primary data from January-February 2023 was collected from health center, posbindu and elderly posyandu visits. The data was analyzed with a chi square test. Results. p-value for age was p=0,046, gender p=0,251, occupation p=0,027, diabetes mellitus p=0,028, and hypertension p=0,023. Conclusion. A significant relationship between cataract incidence and age, diabetes mellitus, hypertension, and occupation. There is no significant relationship between cataract incidence and gender.

Abstract

Background: Malaria is still a major global health concern with a high mortality rate. The clinical spectrums of malaria range from mild to life-threatening. Case: A 25-year-old male with weakness, fever, and anuria. Physical examination showed febris, jaundice, and abdominal tenderness at the right upper quadrant. Laboratory findings revealed Plasmodium falciparum hyperparasitemia with increased total bilirubin and creatinine levels. Diagnosis of severe falciparum malaria with multiple complications (cholecystitis, acute kidney injury, and anemia) was established. Intravenous antimalarial and antibiotics were administered along with hemodialysis. The patient was discharged after 11 days of treatment with the improvement of clinical status and laboratory parameters. Conclusion: A comprehensive approach to the diagnosis of malaria is necessary to detect complications and appropriate management.

Abstract

Hydrops fetalis (HF) is an abnormal fluid collection in two or more fetal compartments. HF is categorized as immune HF (IHF) and non-immune HF (NIHF). IHF is due to rhesus (Rh) incompatibility and NIHF is due to various pathologies. Management is according to underlying etiologies. Prognosis is determined by early diagnosis with ultrasonography (USG), medical intervention, and appropriate delivery time.

Abstract

Introduction: Tihi-tihi and Selangan Islands, floating village areas in Bontang, East Kalimantan, are remote areas in Indonesia, which can only be accessed by sea route. The research was conducted to describe skin diseases in those islands. Method: Retrospective observational cross-sectional and descriptive analysis study on floating communities in Tihi-tihi and Selangan Island, Bontang Lestari Village, South Bontang District, Bontang City, East Kalimantan, Indonesia. Results: The study was conducted on 95 subjects, mostly female (53%), and the largest age group was 26-45 years (29%), followed by the 5-11 years group (24%). Skin infection diseases (58.96%) were dominated by scabies (67.9% of the skin infection disease group). The second highest was allergic contact dermatitis (41%) from the non-infection group. There is an association between age and disease group (p<0.05), with an odds ratio of 3.164. Conclusion: Skin diseases in Tihi-tihi and Selangan Islands are dominated by scabies, followed by allergic contact dermatitis. The pediatric age group is at a three-fold greater risk of developing skin infections than non-infection skin diseases.

Sodium-glucose Co-transporter 2 inhibitor (SGLT2i) is one of the antidiabetic classes recommended for patients with heart and kidney problems. Evidence-based studies show the benefits of SGLT2i extend to heart, kidney, or other metabolic diseases in patients without diabetes mellitus.

Abstract

Introduction: Tenofovir disoproxil fumarate (TDF), an antiviral nucleoside analog reverse transcriptase inhibitor, has been used for the treatment of HIV and HBV in the past decades. TDF nephrotoxicity has been reported and could lead to renal failure and Fanconi Syndrome (FS). Case: A 58-year-old female HBsAg carrier with TDF treatment since 2013 presented with osteomalacia. She had glucosuria, albuminuria, vitamin D insufficiency, hypophosphatemia, and occasional hypokalemia episodes. Laboratory results showed amino aciduria, which indicates renal tubulopathy. The diagnosis of Fanconi syndrome was confirmed. Withdrawal of TDF was followed by improvement. Conclusion: The acquired FS associated with TDF is reversible. The use of TDF-containing antiviral regimens should be followed by screening recommendations to detect early Fanconi syndrome.

Abstract

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are life-threatening mucocutaneous reactions with severe skin involvement; may affect many internal organ systems with significant psychosocial complications. Several studies have documented physical complications associated with Stevens-Johnson syndrome and toxic epidermal necrolysis (TEN). SJS/TEN patients require multidisciplinary care to deal with psychological complications and reduced quality of life.

Abstract

Hair evaluation methods are grouped into three main categories: non-invasive methods (for example: questionnaire, daily hair counts, standardized wash test, 60-s hair count, global photographs, dermoscopy, hair weight, contrasting felt examination, phototrichogram, TrichoScan, and polarizing and surface electron microscopy), semi-invasive methods (for example: trichogram and unit area trichogram), and invasive methods (for example: scalp biopsy). This article explores supporting examinations for the diagnosis of hair disorders.

Abstract

Background: Malaria remains a major health problem in Indonesia; P. falciparum is responsible for the majority of severe cases and malaria-related deaths. This report presents a case of severe tropical malaria complicated with acute kidney injury treated in a remote area with limited resources. Case: A 25-year-old male with a 9-day history of fever presented to the emergency department with severe headache, nausea, jaundice, abdominal pain, and hemoglobinuria. Microscopic examination of the blood revealed P. falciparum parasites. Laboratory results also showed anemia and thrombocytopenia, bilirubinuria, proteinuria, elevated liver enzymes, and pre-renal azotemia. The patient received antimalarial treatment (dihidroartemisinin/piperaquine once daily for 3 days and a single dose of primaquine) and supportive therapy. He was discharged in a stable condition on the fifth day of hospitalization. Conclusion: Early detection and management is crucial for the patient’s prognosis.

Abstract

Cellulite is a topographic change of the skin commonly found in the buttocks and thighs in the majority of post-pubertal women. Cellulite in males is usually accompanied by hormonal disorders. Histological changes were found in the dermis, adipose tissue, and septa. The pathophysiology of cellulite is yet to be determined, several factors are suspected.
